1.Preparation and Quality Control of Shuansaitong Capsules

OBJECTIVE:To prepare Shuansaitong capsules and establish a method for its quality control.METHODS: Distillation method was applied to extract volatile oil from volatile oil-containing medical material and water decoction method was used to extract physic liquor from other medical material. Suitable quantity of excipients was added to prepare Shuansaitong capsules.Qualitative identifications of Ligusticum chuanxiong,Panax quinquefolium,Salvia miltiorrhiza,Angelica sisensis were performed by TLC,and the content of Gastrodin was determined by HPLC.RESULTS:The quality of Shuansaitong capsules was completely in conformity with the related requirements of China Pharmacopeia.The TLC spots were clear,well-separated,specific and free from interferonce of negative sample.The linear range of gastrodin was 0.1~0.5 ?g with a correlation coefficient of 0.999 5.The average recovery was 96.59% with RSD=0.47% (n=6).CONCLUSION:The preparation technology of Shuansaitong capsules is simple and the determination result was accurate,thus suitable for the preparation and quality control of Shuansaitong capsules.
2.The Clinical Study on Management of Side effects of Endocrine Therapy (Tamoxifen) on Breast Cancer Patients in Post Surgery with TCM Treating Principle of Regulating Liver Qi and Tonifying Kidney Yin

Objective By previous overview of patients treated in our hospital, this study highlights on the intervention effects of TCM treating principle of regulating liver Qi and tonifying kidney Yin on the adverse reaction of TAM on breast cancer patients in post surgery. Methods Perspective study with the measures of randomization, and paralleling and synchronization control was conducted in the research. Results symptoms such as flush, insomnia, night sweat, palpitation, depression and heat sensation in the chest, palms and soles were improved significantly; while manifestations such as nausea, vomiting, irregular menstruation, bitter taste and dry mouth showed no significant changes. QOL changed significantly. Conclusion The application of treating principle of regulating liver Qi and tonifying kidney Yin is effective in reducing side effects caused by TAM. This result further improved the theory of TCM that the nature of liver is active, although the liver belongs to Yin in physique and but Yang in function.
3.The effect of TCM method of regulating liver Qi and tonifying kidney Yin on plasma concentration of E2, P and FAS of post-surgery breast-cancer patients received endocrine therapy (Tamoxifen)

Objective The prospective study was trying to explore the altering tendency of plasma concentration of E2, P and PAS after TCM intervention for post-surgery breast cancer patient who received endocrine therapy (tamoxifen).Methods By randomization, parallel, coincidence measures and the lab. technologies of AXSYM registered at Abbott lab. in the USA, plasma concentration of E2, P and FAS were tested before and after treatment. Results The results of biochemical test of plasma E2, P and FAS concentration of the two arms showed no significance difference before and after the intervention,indicating the side effects relating to TAM had nothing to do with plasma E2, P and FAS level. Conclusion It could be predicable that there must be some other by-pass ways initiating breast cancer rather than the way directly altering the level of plasma concentration of E2, P and FAS.

5.Study on the analgesic effect of ropivacaine combined with sufentanil in epidural anesthesia on walk on painless labor

Objective To explore the analgesic effect of ropivacaine combined with sufentanil in epidural anesthesia on walk on painless labor.Methods 134 cases of maternal pregnancy were selected and randomly divided into two groups.61 cases were treated with conventional delivery,73 cases in the experiment group were treated with walk on painless labor.The visual analogue scale and improved Bromage classification score were compared at the first stage of labor,the second stage of labor,the third stage of labor.Results Compared with the control group,the rate of cesarean section was higher(P<0.05),The visual analogue scale was higher at the first stage of labor,the second stage of labor,the third stage of labor,the duration of labor were lower at the first stage of labor,the second stage of labor,the third stage of labor(P<0.05),and there was no significant difference between the two groups in the motor nerve block.Conclusion Ambulatory labor analgesia on maternal effect significantly,can significantly reduce the pain degree of maternal birth process and deliverry time,postpartum less complications.
6.Value of adenosine triphosphate stress and rest nuclide myocardial perfusion imaging in diagnosis of female patients with coronary heart disease

Objective:To evaluate the value of adenosine triphophate (ATP)stress/rest nuclide myocardial perfusion imaging (MPI)in the diagnosis of female patients with coronary heart disease (CHD).Methods:The clinical materials of 47 female suspected CHD patients were retrospectively analyzed,aged from 39 to 74 years,and the average age was (53.7±6.3)years old.All patients were hospitalized and underwent two-day ATP stress and rest nuclide MPI and coronary angiography (CAG)in two weeks. The results and images of MPI and CAG were evaluated by more than 2 attending physicians. Using CAG as the “gold standard”, the diagnostic efficiency (sensitivity, specificity and accuracy) of MPI for CHD was evaluated. Results:Compared with CAG, the sensitivity,specificity and accuracy of ATP stress MPI in diagnosing the female CHD patients were 81.3% (13/16),77.4% (24/31)and 78.7% (37/47)individually;the positive predictive value and negative predictive value were 65.0% (13/20)and 88.9% (24/27).There were no severe adverse effects in the ATP stress test and the incidence of adverse effects was 85.1%.Conclusion:There is a highly diagnostic efficiency of ATP stress MPI in the CHD patients.It can be the first choice of examination methods for screening without injury and diagnosing the myocardial ischemia in the female patients.
8.Correlative analysis between contrast-enhanced ultrasonography and 99mTc-DTPA renography in evaluation of renal insufficiency

Objective To evaluate the contrast-enhanced ultrasonography (CEUS) in quantitative diagnosis of chronic renal insufficiency. Methods Correlation of CEUS indexes with glomerular filtration rate (GFR) detected by 99mTc-DTPA renography was examined. Thirty-three cases of clinical chronic renal insufficiency were enrolled in the study. They were 15 males and 18 females with average age of (43.33±6.78) years. After intravenous bolus injection of 1 ml SonoVue,CEUS of renal cortex blood perfusion was performed successfully, and a time-intensity curve (TIC)was created with PHILIPS iU22 system's QLAB software. A 148 to 222 MBq dose of 99mTc-DTPA was injected as a bolus from antecubital vein. Renal scintigraphic images were collected immediately and GFR was obtained. Results The significant correlation coefficients between GFR and CEUS quantitative indexes were as follows: rAUC (area under curve)=0.886 (P<0.05), rA (slope rate of ascending curve, A) =0.804(P<0.05). However, rDPI (derived peak intensity, DPI)=0.021 (P>0.05), rTTP (time to peak, TTP) =0.043 (P>0.05), rα (slope rate of descending curve,α)=0.039 (P>0.05). Conclusion CEUS can precisely display the hemodynamic change of chronic renal insufficiency, which is well correlated with GFR by 99mTc-DTPA renography.
9.Comparative study of contrast-enhanced ultrasound and color Doppler flow imaging in quantitative evaluation of chronic kidney dysfunction

Objective To evaluate the value of contrast-enhanced ultrasonography (CEUS) in quantitative diagnosis of chronic kidney dysfunction(CKD) by comparing it with color Doppler imaging (CDFI). Methods Tirty-three cases (15 males and 18 females) of clinical confirmed CKD (stage Ⅲ~Ⅴ)were included. Forty-five healthy volunteers were performed as control group. CEUS and CDFI were performed on each patient. After intravenous bolus injection of 1ml SonoVue each side,CEUS of renal cortex blood perfusion was collected successively,and a time-intensity curve(TIC) was created with Philips iU22 system's QLAB software. Resistance index(RI) and peak systolic velocity(PSV) of renal partial arteries were also tested. Results Compared with normal kidney,CKD patients had delayed perfusion and decreased intensity. Changes of area under curve(AUC), derived peak intensity(DPI), slope rate of ascending curve(A)and time to peak(TTP) were statistically significant ( P <0. 05). Sensitivities of AUC,DPI,A and TTP in diagnosis of CKD (stage Ⅲ~Ⅴ ) were 91.2% ,84. 9% ,90.9% and 85.3%, their specificities were 95.4%,88.9% ,93.3% and 90.9%, their accuracies were 93.6%, 87. 2%, 92.3% and 88. 5%, respectively. The results of CEUS were better than RI in CDFI (sensitivity 70.4%, specificity 37. 8%, accuracy 52.2%).Conclusions CEUS can precisely display the hemodynamic change in CKD ( stage Ⅲ~Ⅴ ), and is more sensitive than CDFI.
10.Primary hepatic lymphoma: ultrasound and pathology

Objective To investigate the relationship between ultrasound features and pathological types of primary hepatic lymphoma (PHL).Methods Ultrasound and pathological features in 26 cases of PHL were analyzed retrospectively.Results The most common presentation of PHL was a solitary lesion,which occurred in about 57.7% (15/26) of cases,followed by multiple lesions in about 26.9% (7/26) of patients,and least commonly as diffuse infiltration patterns in about 15.4% (4/26) cases.The first two types were similar and usually demonstrated as hypoechoic lesions to the surrounding normal liver parenchyma in 86.4% (19/22) of cases on ultrasound.All the cases were B-cell non-Hodgkin lymphoma proven by histological and immunohistochemical examinations.The diffuse pattern demonstrated diffuse hepatomegaly without nodular,which were T-cell non-Hodgkin lymphoma proven by histological and immunohistochemical examinations.Conclusions PHL could be divided into single,multiple and diffuse types on ultrasound.The single and multiple types mainly present as hypoechoic lesions and the diffuse type shows hepatomegaly on ultrasound,which are closely related to histology and T or B-cell phenotype on immunohistochemical examination.
