1.Effects of Room Temperature and Low Temperature Storage on the Chronergy of Domestic Cisatracurium

OBJECTIVE:To observe the effect of room temperature and low temperature storage on the chronergy of domes-tic cisatracurium. METHODS:90 patients undergoing elective surgery under general anesthesia were randomly divided into low tem-perature group,room temperature A group and room temperature B group,with 30 cases in each groups. Through single supramaxi-mal electrical stimulation of the adductor pollicis muscle,TOF-Watch SX muscle relaxation monitor was used to observe the degree of muscle relaxation. After induction of anesthesia and lost consciousness,the patients was given intravenous injection of cisatracu-rium 0.2 mg/kg;when muscle twitch decreased to the maximum inhibition degree,the patient received endotracheal intubation. The cisatracurium of low temperature group was stored in refrigerator at 2 to 5℃;that of room temperature A group and room tempera-ture B group was stored in incubator for 15 days or 30 days. The onset time,maximum inhibition degree of muscle twitch,clini-cal duration,recovery index and total chronergy were recorded in 3 groups. RESULTS:The maximal inhibition degree of muscle twitch reached 0 under the action of cisatracurium. There was no statistical significance in onset time and clinical duration between room temperature A group,room temperature B group and low temperature group(P>0.05);the recovery index and total chroner-gy of room temperature A group and room temperature B group were or longer than low temperature group,with statistical signifi-cance(P<0.05). CONCLUSIONS:Domestic cisatracurium stored at low temperature shows weaker effect on muscle relaxation.

Objective: To compare with coliform bateria and Escherichia coli testing results in oral drugs. Methods: 24 species Chinese herb medicines and 35 kinds of Chinese traditional patent medicines in 68 batches were examined by using the method of coliform bacteria and escherichis coli test.Results: There were significant differences. Conclusion: The result showed that using the coliform bacterias as the hygienic indicative bacteria was significant.
3.The effect of glucocorticosteroids in prevention of post-ERCP pancreatitis:a meta-analysis

Objective To evaluate the effect and safety of glucocorticosteroids in prevention of post-ERCP pancreatifis(post-endo- scopic retrograde cholangiopancreatography pancreatitis,PEP).Methods The database of Cochrane Library(issue 4,2006),Medline (1966-2007.1),EMBASE(1986-2007.1)and CBM(1978-2007.1)were searched.The cited literature of obtained articles and some core English and Chinese medical journals(July,2006-Jan,2007)relevant to gastro-enterolngy were also searched by hand.All random- ized controlled trials(RCT)comparing glucocorticosteroids to placeboes in prevention of PEP were included.Data collection and literature evaluation were performed by two reviewers independently.RevMan 4.2.8 was used for statistical analysis.Result Six RCTs were includ- ed in meta-analysis which showed no statistical difference for the incidence of overall PEP between the glucocorticosteroids group and place- bo group(RR=1.11,95%CI 0.89-1.39).Stratified by severity of PEP,four trials were included.Meta-analysis showed no statistical difference for the incidence of mild to moderate PEP and severe PEP between the two groups(RR=1.17,95%CI 0.91-1.51 for mild- moderate PEP;RR=1.79,95%CI 0.69-4.65 for severe PEP).Stratified by therapeutic ERCP,three RCTs were included.Meta-analy- sis showed no statistical difference for the incidence of PEP between the two groups(RR=1.24,95%CI 0.91-1.67).3 trials as to inter- im-analysis were excluded by sensitive analysis.Meta-analysis also showed no statistical difference for the incidence of PEP between the two groups(RR=0.73,95%CI 0.44-1.20).Conclusion Based on current evidence,glucocorticosteroids is not beneficial in preventing the occurrence of PEP.Further high quality study is needed to disclose the efficacy of glucocorticosteroids in prevention of PEP.

Objective To analyze and evaluate the precision ,accuracy and linearity of the chemiluminescence method for detec‐ting plasma BNP .Methods According to the experimental schemes of CLSI EP15‐A2 ,EP6‐A files and other relevant documents , the precision ,accuracy and linearity of the Siemens Centaur XP chemiluminescence instrument for detecting BNP ,and the detection results were compared with the performance declared by manufacturer or the quality target formulated by laboratory .Results The imprecision of BNP detected by the chemiluminescence method was less than 1/3 TEa regulated by the Clinical Laboratory Center of Ministry of Health (allowable total error);the variation coefficient index (CVR) of internal quality control data was less than ± 2;the relative bias of the results of external quality control blind samples with the target values were less than Tea regulated by the Ministry of Health ;the linear evaluation results showed that BNP was once linearity in the range of 5 .3‐4696 .7 pg/mL .Conclusion The precision ,accuracy and linearity of the Siemens Centaur XP chemiluminescence instrument for detecting BNP can accord withthe quality objectives requirements and meet the clinical needs .
6.Preparation of Magnetic Imprinted Polymer by Surface Grafting and Its Recognition Performance for Dibutyl Phthalate

The magnetic molecularly imprinted polymers (MMIPs), based on the surface of magnetic nanoparticles being modified by surface grafting, have been successfully synthesized, with dibutyl phthalate (DBP) as template molecule, ethylene glycol dimethacrylate (EGDMA) as cross-linking agent and azobisisobutyronitrile (AIBN) as initiator. Scanning electron microscopy (SEM), transmission electron microscope (TEM), and elemental analysis were employed to characterize the MMIPs. The structure and magnetic properties of the MMIPs were investigated by means of X-ray diffraction and vibrating sample magnetometer. The BET surface area shows that MMIPs is 380 m2/g and MNIPs is 324 m2/g. A series of static adsorption experiments were conducted to analyze its adsorption performance, which followed pseudo-second-order model by the kinetic analysis with correlation coefficient (R2) 0.9797, and Sips equation with correlation coefficient (R2) 0.999 by the isothermal analysis. The imprinting factors of diallyl phthalate (DAP), DBP and di-2-ethylhexyl phthalate (DEHP) were 1.53, 2.21 and 1.39 respectively, showing that MMIPs had better recognition performance for DBP. The experiment of regeneration recycles with five times showed the regeneration ability of DBP was only reduced by 12.3%.
7.Three-dimensional Finite Element Analysis of Lumbar Ligaments

The lumber ligaments play an important role in spinal bsomechanics. The results of three-dimensional finite element analysis showed that one of functions of lumbar ligaments is transmission of the tensile load between the lumbar vertebrae. The anterior longitudinal ligament is loaded in extension of lumbar spine and the resistance to the tensile load in flexion is provided by other ligaments. These ligaments are subject to much more tension with degsneration of the intervertebral disc so that a series of pathological changes occur. Relevant significance in clinical aspect is also discussed.

9.Umbilical cord blood mononuclear cell transplantation for treatment of decompensated cirrhosis

BACKGROUND:Orthotopic liver transplantation is the most effective therapy for the treatment of end-stage liver diseases, but the lack of donor source, immune rejection, and repeated infections limit its application. Stem celltransplantation technology provides a new idea for the treatment of end-stage liver diseases. A variety of methods have been confirmed to successful y induce umbilical cord blood mesenchymal stem cells converted into liver cells in vitro.
OBJECTIVE:To explore the clinical efficacy and feasibility of human umbilical cord blood mononuclear cells transplantation in the treatment of decompensated cirrhosis.
METHODS:Twenty-three patients with decompensated cirrhosis received al ogeneic human umbilical cord blood mononuclear celltransplantation. Serum alanine aminotransferase, albumin, cholinesterase, total bilirubin and prothrombin time were detected at post-transplantation weeks 2, 4, 8 and 24. Improvement in clinical signs and symptoms as wel as adverse reactions was observed.
RESULTS AND CONCLUSION:Liver function had no changes at 2 weeks after human umbilical cord blood mononuclear celltransplantation (P>0.05). At 4 weeks after celltransplantation, serum alanine aminotransferase was improved significantly (P<0.05), but the other indexes stil had no changes. Until 12 weeks after celltransplantation, there were significant improvements in al the liver function indicators (P<0.05) and the liver stiffness (P<0.05). By the end of 24 weeks, al the test results were improved significantly (P<0.01). Clinical symptoms were al eviated, including fatigue improvement in 20 cases (87%), improved appetite in 21 cases (91%), and relieved ascites in 19 cases (83%). No severe adverse reactions were found during the transplantation and 24-week fol ow-up. These findings suggest that human umbilical cord blood mononuclear cells transplantation is effective and safe for the treatment of decompensated cirrhosis, which can be considered as a clinical therapy for patients with advanced cirrhosis.
10.Cerebral perfusion semi-quantitative imaging for assessing the treatment of delayed encephalopathy from carbon monoxide poisoning

Objective To evaluate the effects of hyperbaric oxygen (HBO) combined with drug therapy on patients with delayed encephalopathy caused by carbon monoxide poisoning ( COP). Methods Twenty to forty sessions of HBO therapy were used to treat 34 COP patients. Assessment relied on 99mTc-ethyl cysteinate dimer (~(99m)Tc-ECD) single photon emission computed tomography (SPECT) imaging of cerebral perfusion before and after treatment. Results After HBO therapy, cerebral perfusion in the COP patients improved significantly. There was a significant difference of the SPECT images before and after treatment. Conclusions SPECT imaging of cerebral perfusion can play an important role in the diagnosis of delayed encephalopathy caused by carbon monoxide poisoning, and it can be used for the therapeutic surveillance of HBO treatment.
