3.Role of Dectin-1 in the production of IL-10 and TNF-α by rat tracheal epithelial cells stimulated with heat-treated Candida glabrata

Objective To investigate the effect of Dectin-1 on the release of inflammatory factors interleukin-10 (IL-10) and tumor necrosis factor-α (TNF-α) in rat tracheal epithelial cells (RTECs) stimulated with heat-treated Candida glabrata (C.glabrata).Methods RTECs cultivated in vitro were randomly divided into three groups,including control group(RTECs + sterile normal saline),fungal stimulation group(RTECs + heat-treated C.glabrata),and inhibitor intervention group(RTECs + laminarin + heat-treated C.glabrata),cells were harvested after incubation for 0,2,4,6 hours respectively,cell survival rate was determined by MTT method,expression of Dectin-1 was analyzed by Western Blot,expression of IL-10 and TNF-α were detected by enzyme-linked immunosorbent assay (ELISA).Results Heat-treated C.glabrata destroyed cell structure and reduced cell survival rate.At the beginning of the culture (0 h),cell survival rate,expressions of Dectin-1,IL-10 and TNF-α among three groups were all not significantly different(all P>0.05).After incubation for 2,4,6 hours,expressions of Dectin-1,IL-10 and TNF-α in fungal stimulation group and inhibitor intervention group were both significantly higher than control group;expressions of Dectin-1,IL-10,and TNF-α in inhibitor intervention group was lower than fungal stimulation group(all P<0.05).The expressions of IL-10 in inhibitor intervention group at 0 h and 2 h was not significantly different,expressions of Dectin-1,IL-10,and TNF-α in fungal stimulation group and inhibitor intervention group at different incubation periods were significantly different(all P<0.05).Conclusion Dectin-1 is an important receptor for RTECs to recognize the heat-treated C.glabrata,it induces the release of IL-10 and TNF-α,and mediate the occurrence of inflammation.
4.Application of team based learning in teaching of dental endodontics

Objective To apply team based learning (TBL) in the teaching of dental endodontics for undergraduates in order to expand and deepen the TBL teaching in stomatology education.Methods TBL was used among 2008 grade oral undergraduates in Chongqing Medical University.Average test scores of 2008 grade undergraduates before and intra class were calculated and questionnaire was designed.At the same time,final examination scores between 2007 grade and 2008 grade under-graduates were compared.Results There was no significant correlation in average test scores before and intra class (r =0.027,P > 0.05).Final examination scores were higher in 2008 grade than in 2007 grade.Based on the questionnaire survey,learning interests,sense of teamwork and classroom knowledge grasp of 2008 grade undergraduates were obviously elevated.Conclusions TBL teaching significantly improve students' learning effect and it can be promoted in stomotology education.
5.Methodological evaluation on enzymatic method for detecting HbA1c and influence of sample pre-processing on detection result

Objective To evaluate the methodological performance of the new enzymatic method for detecting glycated hemo-globin(HbAIc)and its influencing factors.Methods HbAIc was detected by the enzymatic method.The precision,anti-interfer-ence,recovery rate,accuracy and the influence of pre-processing(anti-coagulation,preservation,centrifugation)on the detection re-sults were evaluated,its correlation with HPLC and the bias degree were analyzed.Results The within-run coefficients of variation (CVs)for high,middle and low value QC samples in the enzymatic assay were 1.04%,1.26% and 1.37% respectively and the be-tween-run CVs were 1.83%,2.24% and 2.64%,respectively;the enzymatic method showed the linear correlation with HPLC(r=0.996,P <0.01);the HbA1c target value concentrations were 5.20%,6.40%,7.60%,8.80%,10.00% and 11.20% respectively, the recovery rates were 100.15%,98.91%,98.84%,98.20%,103.62% and 99.82% respectively;the interference test showed that this method had no significant interference on the detection results when glucose <15.50 mmol/L,UA<516.00 μmol/L,bili-rubin <217.00 μmol/L,triglyceride<10.20 mmol/L,urea<11.50 mmol/L,albumin<50 g/L and globulin <50 g/L.The HbA1c detection results in the samples with anti-coagulation by heparin sodium,EDTA-2K and sodium citrate stored for 3 d under -20-20 ℃ had no obvious change (P >0.05);the sample was centrifuged at 500,1 000 r/min(R=15 cm)for different time(1,2,5,10 min)and at 2 000 r/min for 1 min,their detection results had statistical differences compared with the sample centrifuged=3 000 r/min for 5 min (P <0.05).Conclusion The precision,anti-interference,accuracy and linearity range of the enzymatic method all conform to the clinical requirement.Compared with the conventional method,its correlation is good with small deviation,which can entirely satisfy the demand of the HbAIc detection in clinic.
6.Risk factors for nonunion or malunion after internal fixation of femoral neck fracture

Objective:To explore the risk factors for nonunion or malunion after internal fixation of femoral neck fracture.Methods:A retrospective analysis was done of the 140 patients with femoral neck fracture who had been admitted to Department of Traumatology and Orthopedics, Weifang People's Hospital from January 2013 through June 2019. They were 61 males and 79 females, aged from 18 to 65 years (average, 51.3 years). All patients received "inverted triangle" internal fixation with 3 hollow nails after reduction. The univariate analysis was conducted of the 9 factors: age, gender, fracture side, Garden classification, injury-to-operation time, reduction quality, postoperative ambulation with crutches but no weight-bearing, preoperative bone density, and preoperative 25-hydroxyvitamin D3 concentration. Multivariate logistic regression analysis was used for the factors with P<0.05 to identify risk factors. P<0.05 was considered statistically significant. Results:This cohort of 140 patients obtained follow-up for 6 to 12 months (average, 10.8 months). Nonunion or malunion occurred in 25 cases of them (17.86%). The multivariate logistic regression analysis showed that Garden types Ⅲ&Ⅳ ( OR=2.873, 95% CI: 1.027~8.039, P=0.044), non-anatomic reduction ( OR=4.678, 95% CI: 1.133~19.310, P=0.033), postoperative ambulation time with crutches but no weight-bearing <3 months ( OR=0.230, 95% CI: 0.072~0.737, P=0.013), and preoperative 25-hydroxyvitamin D3 concentration<30 ng/mL ( OR=3.718, 95% CI: 1.337~10.341, P=0.012) were the risk factors for nonunion or malunion after internal fixation of femoral neck fracture. Conclusion:The risk factors for nonunion or malunion after internal fixation of femoral neck fracture may be the Garden types Ⅲ&Ⅳ, non-anatomic reduction, postoperative ambulation time with crutches but no weight-bearing <3 months, and preoperative 25-hydroxyvitamin D3 concentration < 30 ng/mL.
7.The effect of osteoking on postoperative fracture healing of femoral neck fracture

Objective:To analysis the effect of osteoking on fracture healing after internal fixation in patients with femoral neck fracture.Methods:A total of 120 patients with femoral neck fracture treated in Weifang people's Hospital from January 2017 to April 2020 were analyzed retrospectively. All patients underwent inverted triangular fixation with 3 cannulated screws after reduction. According to the condition whether they took osteoking after operation, they were divided into two groups, with 60 in each groups. The control group was treated with traction and rotation closed reduction. If closed reduction failed, open anatomical reduction was performed. The treatment group took osteoking on the basis of the control group. Both groups were observed 3 months and followed up to 9 months after operation. Bone osteotylus growth and fracture healing were evaluated by X-ray or CT. Lumbar vertebral bone density was detected by dual-energy X-ray bone density instrument. Serum amino terminal peptide of type I procollagen (PINP) and serum carboxy-terminal peptide β (β-CTX) of type I collagen were detected by double-antibody sandwich ELISA and competitive ELISA.Results:The fracture healing time of the treatment group was (13.06±2.35) weeks, and that of the control group was (17.75±3.56) weeks, and the difference between the two groups was statistically significant ( t=8.52, P<0.01). During the follow-up period, the fracture healing rate was 93.3% (56/60) in the treatment group and 75.0% (45/60) in the control group, and the difference was statistically significant ( χ2=7.57, P=0.006). The rate of nonunion was 1.7% (1/60) in the treatment group and 5.0% (3/60) in the control group, and there was no significant difference between the two groups ( χ2=1.03, P=0.309). The rate of malunion was 5.0% (3/60) in the treatment group and 20.0% (12/60) in the control group, and the difference was statistically significant (χ 2=6.17, P=0.013). The BMD of the treatment group was significantly higher than that of the same group before operation at 9 months after follow-up [(0.76±0.12) g/cm2 vs. (0.71±0.06) g/cm2; t=2.89, P<0.05]. The level of serum β-CTX at the 3rd month after operation [(186.76 ± 26.23) ng/L vs. (286.05 ± 23.18) ng/L, t=21.97] in the treatment group was significantly lower than that of the control group ( P<0.05), at the 6th month [(252.34 ± 21.58) ng/L vs. (302.52 ± 16.87) ng/L, t=14.19] in the treatment group was significantly lower than that of the control group ( P<0.05). The PINP level at the 3rd month [(37.52 ± 7.59) μg/L vs. (27.59 ± 5.36) μg/L, t=3.56] in the treatment group was significantly higher than that of the control group ( P<0.05), at the 6th month [(30.54 ± 5.63) μg/L vs. (25.63 ± 4.98) μg/L, t=2.36] in the treatment group was significantly higher than that of the control group ( P<0.05). Conclusion:Osteoking can regulate the bone metabolism balance of patients with femoral neck fracture after internal fixation, shorten the fracture healing time and promote fracture healing.
8.Characteristic analysis of inhibitory control and cognitive flexibility in hearing-impaired children

Objective:To analyze the characteristics of inhibitory control and cognitive flexibility in hearing-impaired children.Methods:From March to April 2023, a convenience sampling method was used to select 33 hearing-impaired children from a special education school in Meizhou City, Guangdong Province, and 35 normal-hearing children from two ordinary schools as participants. Inhibitory control and cognitive flexibility of the participants were assessed by the Flanker task and the dimensional change card sorting (DCCS) task. Statistical analysis was conducted using SPSS 26.0 software, and independent sample t-test was used to compare the differences in reaction time and accuracy rate between two groups of participants. Results:There were no significant differences in the Flanker task reaction time ((558.39±123.65) ms vs (566.11±118.20) ms) and accuracy rate((0.93±0.10) vs (0.96±0.04))between hearing-impaired children and normal-hearing children ( t=-0.295, -1.645, both P>0.05). The hearing-impaired children had significantly longer reaction time ((1 019.60±131.08) ms)than the normal-hearing children ((857.85±129.19) ms) ( t=4.046, P=0.001) in the DCCS task, while there was no statistically significant difference in the accuracy rate between hearing-impaired children (0.62±0.16) and normal-hearing children (0.57±0.15) ( t=-1.602, P>0.05). Conclusion:There is no difference in inhibitory control ability between hearing-impaired children and normal-hearing children, but the hearing-impaired children have a lag in cognitive flexibility.
9.Staged treatment of infectious femoral defects of Cierny-Mader type Ⅳ using bone transport combined with locking plating after En-Bloc resection debridement

Objective:To investigate the efficacy of staged treatment of infectious femoral defects of Cierny-Mader type Ⅳ using bone transport combined with locking plating after En-Bloc resection debridement.Methods:A retrospective analysis was conducted of the 10 patients with distal femoral traumatic bone infection who had been treated at Department of Orthopedics, The People's Hospital of Weifang from January 2020 to January 2023. There were 8 males and 2 females with an age of (48.5±11.4) years. All cases were classified as Cierny-Mader type Ⅳ. At the first stage, En-Bloc resection debridement was performed in all cases to remove previous internal fixation devices and fill the defects with antibiotic bone cement. After infection control, the second stage involved removal of bone cement, re-fixation with internal devices, and external fixation support for bone transport. After the bone segments met, freshening of the bone ends, minor bone grafting, and screw locking of the transported bone segments were performed. Outcomes observed included bone defect length, frame carrying time and index, bone healing time, limb function, and complications.Results:After the first stage of debridement, a bone defect of (9.1±2.1) cm was created in 10 patients. All patients were followed up for (19.8±6.6) months. The duration for carrying external fixation frame was (107.2±25.1) days, and the frame index (11.8±0.5) d/cm. No recurrence was observed postoperatively. Bone union was achieved in 9 patients within 8 months, but in 1 patient only after secondary bone grafting due to poor healing at the meeting ends. All patients returned to their previous life or physical labor with no complications like pain or re-fracture. Three patients experienced varying degrees of knee joint stiffness, but were able to meet needs of daily life; one requested joint release surgery which resulted in satisfactory therapeutic efficacy.Conclusion:Staged treatment of infectious femoral defects of Cierny-Mader type Ⅳ using bone transport combined with locking plating after En-Bloc resection debridement is simple and effective.
10.Wound immersion with weakly alkaline solution after debridement for refractory diabetic foot ulcer

A total of 44 patients with diabetic foot ulcers were treated in the Traumatic Orthopedics Department of Weifang People′s Hospital from January 2019 to December 2022. After debridement of foot ulcers the wounds were soaked in alkaline water of pH 7.5-8.0 (study group, n=22) or covered with vaseline gauze following iodophor disinfection (control group, n=22). The therapeutic effects of the two methods were compared. Four weeks after debridement, the wound area of study group was smaller than that of control group (3.15 (0, 7.60) vs. 6.75 (3.50, 9.32)cm 2, P<0.05), and the proportion of positive wound bacterial culture was lower than that of control group (40.9% (9/22) vs. 72.7% (16/22), P<0.05). At 12 weeks after surgery, there was no statistically significant difference in the wound healing rate between the two groups (72.7% (16/22) vs. 63.6% (14/22)), but the healing time of the study group was significantly shorter than that of the control group ((6.56±2.68) vs. (9.50±3.87) weeks, P<0.05). It is suggested that immersion of weak alkaline solution is helpful to promote wound healing for patients with diabetic foot ulcers after debridement surgery.
