Objective Discuss elderly hypertension of chronic heart failure among the edema of lower limbs and the N end of b-type natriuretic peptide(NT-proBNP) formerly relationship and its value in differential diagnosis.Methods Hospitalized for high blood pressure and breathing difficulties in 301 cases of elderly patients,with 220ng/L for the NT-proBNP exception standard defines value,and comparing the edema of lower limbs and the NT-proBNP levels and heart failure.Results In the elderly hypertension of chronic heart failure population of double lower limbs swelling predict heart failure with NT-proBNP similar value.Conclusion Elevated levels of double lower limbs swelling in the NT-proBNP positive predictive value was 90.3%,negative predictive value was 42.1%,sensitivity 74.4%,specificity 80.0% ;On heart failure of sensitivity 86.4%,specificity 78%.The edema of lower limbs were significantly associated with NT-proBNP and heart failure.
2.The impact of serum uric acid level to CSFP

ObjectiveTo investigate on the impact of the serum uric acid (UA) level coronary slow flow phenomenon(CSFP).MethodsSelected 30 cases of coronary slow flow patients as CSFP group,30 cases of patients with coronary heart disease,30 cases of patients with angiographically normal as the control group.The serum UA level was measured in three groups.The CSFP group and the control group were measured left anterior descending,circumflex and right coronary artery TIMI frame count(TFC) value.To analyze the mean TFC value of CSFP patients and the relationship between the serum UA level.ResultsCompared with the control group,the left anterior descending,circumflex and right coronary TFC results were significantly higher(all P < 0.05) in the CSFP group.The mean serum UA level of CSFP group ,control group and CHD group were(340.79 ± 79.40)μmol/L, (220.45 ± 82.34)μmol/L,(42625 ± 87.02) μmol/L, the difference was statistically significant (all P < 0.05).The mean serum UA level and TFC value of CSFP group was positively correlated(r = 0.943, P = 0.007).ConclusionElevated level of serum UA is closely related to the occurrence and development of the CSFP.

OBJECTIVE: To discuss the feasibility of the General Regulations for the Processing of Crude Drugs in the supplement in China Pharmacopeia. METHODS: The problems existing in the General Regulations for the Processing of Crude Drugs in the supplement in China Pharmacopeia were analyzed, aimed at which, some constructive suggestions were presented. RESULTS & CONCLUSION: It's urgent to improve the feasibility of the General Regulations for the Processing of Crude Drugs in the supplement in China Pharmacopeia and standardize the production of Chinese cut crude drugs.
4.Relationship between cognitive performance and location and number of infarcts

ObjectiveTo explore the relationship between the impairment cognitive function and the location and number of infarcts.MethodsThe composites the scores for memory,processing speed and executive function were detected using the neuropsychological measuring scales and determined the cortical,subcortical and cerebral infarcts by magnetic resonance imaging( MRI),then analyzed the relationship between the impairment cognitive function and the location and number of infarcts.ResultsCompared to the control group,the patients with infarcts in multiple locations had poorer memory(the score of AVLT-5 was 6.14 ± 1.89 and 7.26 ± 1.82,respectively,P < 0.01 ).Compared to the patients without infarcts,a combination of cortical and subcortical infarcts was associated with slow processing speed.The patients with multiple infarcts in multiple locations had significantly lower composites scores for memory(the score of LMT-delay was 10.86 ± 3.48 and 12.69 ± 2.60,respectively,P< 0.01 ),processing speed ( the score of SDMT was 27.08 ± 11.13 and 34.23 ± 7.44,respectively,P < 0.01 ) and executive function( the score of TMT-B was 1.34 ± 0.65 and 0.40 ± 0.35,respectively,P < 0.01 ) compared to control group.ConclusionThe multiple infarcts in multiple locations is associated with the impairment cognitive function in patients with cerebral infarcts.Suggesting that both the location and the number of infarcts jointly contribute to cognition impairment.

OBJECTIVEThe color difference meter and electronic nose fingerprint analyzer were introduced to discriminate processed betel nut's different huohou and quantize empirical index during the processing of Chinese medicine.
METHODBy collecting the color's and the smell's characteristic parameters of betel nut and its processed products to set up huohou's math discrimination modules and 90% reference value scale.
RESULTWe set up a huohou's math discrimination module and 90% reference value range base on L a*b*, which discrimination coincidence was 100% and 90% reference value range respectively was Shengpin: L* (50.07-55.03), a* (4.136-6.906), b* (6.65-8.82); Chaopin: L* (43.874-47.998), a* (3.816-6.732), b* (3.786-6.558); Jiaopin: L* (38.744-40.616), a* (1.11-3.01), b* (-1.434-0.538). At the same time, we set up a huohou's math discrimination module base on smell's characteristic parameters too.
CONCLUSIONThe betel nut and its processed products' color and smell measurement based on the color difference meter and electronic nose fingerprint analyzer may be feasible, and provides a reference to better study on traditional Chinese medicine theory.

6.Research progress on T cell glycolytic metabolism in oral lichen planus

Oral lichen planus (OLP) is a chronic inflammatory disease of the oral mucosa. The pathogenesis of OLP is still unclear. Immune abnormalities mediated by T cells and related cytokines play a crucial role in the pathogenesis of OLP. In recent years, glycolytic metabolism-related transporters, enzymes and regulators, such as glucose transporter-1 (Glut1), glyceraldehyde 3-phosphate dehydrogenase (GAPDH), lactate dehydrogenase A (LDHA), mammalian target of rapamycin (mTOR) and hypoxia inducible factor-1α (HIF-1a), have attracted an increasing amount of attention in OLP by regulating the proliferation and differentiation of T cells and the secretion of inflammatory factors. It has been shown that 2-deoxy-D-glucose (2-DG) or rapamycin (RAPA) inhibits the glycolytic metabolism of T cells and then inhibits OLP. This article reviews the research progress of glycolytic metabolism-related transporters, enzymes and regulatory factors in OLP in recent years.
7.Coexisting gene mutations of FLT3-ITD mutation in acute myeloid leukemia

Objective:To explore the coexisting gene mutations of FLT3-ITD mutation and its association with partial clinical parameters in acute myeloid leukemia (AML).Methods:The clinical data of 236 newly diagnosed AML outpatients and hospitalized patients of Changzhou No.2 People's Hospital and the Second People's Hospital of Wuxi between December 2012 and August 2019 were retrospectively analyzed. Genome DNA-polymerase chain reaction (PCR) combined with Sanger sequencing was used to detect FLT3-ITD mutations, and 51 tumor target gene mutations in patients with FLT3-ITD mutations were detected by using high-throughput DNA sequencing combined with Sanger sequencing.Results:Among 236 AML patients, FLT3-ITD mutations were found in 71 cases (30.1%). About 97.2% (69/71) patients with FLT3-ITD mutations were accompanied by additional mutations, of which 19 patients harbored double coexisting genes mutations, 24 patients harbored 3 coexisting genes mutations and 26 patients harbored ≥ 4 coexisting genes mutations. The most common coexisting genes mutations were NPM1 (55 cases, 77.5%), followed by DNMT3A (36 cases, 50.7%), TET2 (9 cases, 12.7%), CEBPA (5 cases, 7.0%), IDH1 (4 cases, 5.6%) and NRAS (4 cases, 5.6%). In FLT3-ITD mutation group, the hemoglobin level of patients with DNMT3A mutation type was lower than that of those with DNMT3A wild type ( t = -2.37, P = 0.020); the hemoglobin level of patients with NPM1 mutation type was higher than that of those with NPM1 wild type ( t = 2.04, P = 0.045). The platelet in patients with 3 mutations and ≥ 4 mutations was higher than that in those with double mutations ( χ2 = 7.72, P = 0.021). After chemotherapy in 71 patients, the curative effect of 66 cases was evaluable, and the white blood count of 18 patients who did not reach complete remission was higher than that of 48 patients who reached complete remission ( Z = -2.74, P = 0.006). Conclusions:Most FLT3-ITD mutated patients with AML commonly show coexisting gene mutations, and the mutation types of coexisting genes are correlated with the clinical features of patients.

Objective To analyze the effect of endoscopic middle turbinate angioplasty on postoperative nasal function and olfaction in patients with chronic sinusitis and nasal polyp.Methods Clinical materials of 80 patients with chronic sinusitis and nasal polyps were analyzed retrospectively,and they were divided into control group and research group,40 cases in each group.Control group was treated with endoscopic partial middle turbinate resection,while research group was treated with endoscopic middle turbinate angioplasty.Clinical efficacy,nasal function,olfactory function,SNOT-20CV score,and Lund-Kennedy scoring and complications were compared between two groups.Results Total effective rate of the research group was significantly higher than the control group (P <0.05).Before operation,nasal airway resistance and minimal nasal cross-section to anterior nostril distance (DCAN),nasal minimum cross-sectional area (NMCA),nasal volume (NCV),SNOT-20 CV,nasal endoscopic Lund-Kennedy scores showed no significant differences between two groups (P > 0.05).After operation,nasal airway resistance and DCAN and SNOT-20 CV,nasal endoscopic Lund-Kennedy scores decreased significantly in both groups,and these indexes in research group were significantly lower than the control group (P < O.05).The NMCA and NCV increased significantly in both groups,and these indexes in research group were significantly higher than control group (P < 0.05).There was no significant difference in incidence rate of postoperative complications between the two groups (P > 0.05).Conclusion Endoscopic middle turbinate angioplasty is effective in treating chronic sinusitis patients with nasal polyp,which can effectively relieve nasal obstruction and promote nasal function and the sense of smell.

10.Evaluation of accessibility and quality of diagnosis and treatment services of Internet hospitals in China

Objective:To evaluate the healthcare accessibility and quality of diagnosis and treatment services of Internet hospitals in China.Methods:One hundred and eighty Internet hospitals in 60 cities were seleted based on the sampling of development levels in the eastern, central and western regions of China. From April to May 2023, standardized patients methodology was applied to evaluate the accessibility(including the number of Internet hospitals, functional settings, online doctor status, the doctor′s attending rate and consultation fees) and diagnosis and treatment service quality(including the diagnosis and treatment services quality, response speed and patient′s evaluation) of Internet hospitals.Results:The average opening rate of Internet hospitals in China was 52.9% (560/1 058), the average online rate of doctors was 64.2% (1 099/1 713), the average doctor′s attending rate was 33.6% (112/333), the average consultation fee was 4.85 yuan, the average score of consultation was 1.92 out of 9, the average score of diagnosis and treatment was 1.12 out of 4, the average score of the response speed was 1.70 out of 3, and patient satisfaction was 2.73 out of 3.Conclusions:The Internet hospital accessibility in China is unevenly developed, and the overall quality of diagnosis and treatment is low. It is recommended to accurately position and optimize the function of Internet hospital, establish the incentive mechanism for online consultation doctors, construct and improve the regulatory system of Internet hospital diagnosis and treatment, so as to improve the accessibility and quality of diagnosis and treatment of Internet hospitals.
