2.Construction of recombinant infectious clones of HCoV-OC43 expressing green fluorescent protein and viral rescue

Objective To investigate the feasibility of using recombinant infectious clones of hu-man coronavirus OC43 (HCoV-OC43) as a vector for the expression of exogenous genes and to analyze the insertion sites. Methods Based upon pBAC-OC43FL, a full-length cDNA infectious clone of HCoV-OC43, three recombinant expression plasmids ( pBAC-OC43-GFPΔNS2, pBAC-OC43-GFPΔNS12. 9 and pBAC-OC43-N-GFP) were respectively constructed by replacing NS2 and NS12. 9 genes with the reporter gene en-coding the green fluorescent protein ( GFP ) and inserting the reporter gene after the N gene by using the overlapping-PCR and in vitro ligation. Reverse genetics techniques were used for viral rescue. All of the res-cued virus strains were characterized by immunofluorescence assay ( IFA) and Western blot ( WB) assay af-ter transfecting BHK-21 cells with the recombinant viruses. Results Two recombinant viruses, OC43-GFPΔNS2 and OC43-GFPΔNS12. 9, could be successfully rescued by transfection the BHK-21 cells with pBAC-OC43-GFPΔNS2 and pBAC-OC43-GFPΔNS12. 9 plasmids. The expressed GFP was observed in BHK-21 cells transfected with pBAC-OC43-GFPΔNS2 or pBAC-OC43-GFPΔNS12. 9 plasmids, but not in the cells transfected with the pBAC-OC43-N-GFP plasmid. An efficient and stable expression of GFP was observed in the pBAC-OC43-GFPΔNS2 plasmid-transfected cells. The 10th generation of OC43-GFPΔNS2 virus was ob-tained after repeated freezing and thawing. The expression of GFP and N protein were detected in cells infec-ted with the OC43-GFPΔNS2 virus after 10 passages. Conclusion The NS2 gene of HCoV-OC43 could be used as a promising insertion site of the pBAC-OC43 FL infectious clone for the expression of exogenous genes. This study might provide a platform for further researches on the replication of HCoV-OC43 and the development of human coronavirus-based vectors.
3.Clinical effect and safety of miconazole nitrate 1200 mg in treating vulvovaginal candidiasis

Objective To investigate the clinical effect and safety of miconazole nitrate 1200 mg in treating vulvovaginal candidiasis(VVC).Methods An open,multicentre,non case control clinical trial was conducted in 568 patients suffering from VVC from Jul 1,2006 to Nov 30,2006.Routine examination,score of clinical symptoms and physical signs,mycetology test and safety evaluation were done in all patients before treatment,7-14 days after treatment and 30 days after treatment.Results Seven to fourteen days after treatment,563 patients could be followed and 323 patients(57.30k)were cured.The overall effective rate was 90.2%.The mycologic cure rate was 91.3%(514).Thirty days after treatment,480 patients could be followed and 411 patients(85.6%)were cured.The total effective rate was 96.0%.Mycologic cure rate was 92.3%(443/480).Adverse effect rate was 2.7%(15/563)and they were relieved without any treatment in one or two days.Conclusions Miconazole nitrate 1200 mg is effective in the treatment of WC,with good compliance and few adverse effects.Moreover,it can be accepted easily.
4.Effect of Modified Liu Junzitang and Erchentang on Hcy, NO, hs-CRP in Patients with Coronary Heart Disease

Objective:To observe the relationship between serum homocysteine (Hcy), nitric oxide (NO), high-sensitivity C-reactive protein (hs-CRP) as well as the number and degree of coronary lesions, and the effect of Liu Junzitang combined with Erchentang on Hcy, NO, hs-CRP in patients with coronary heart disease (CHD), so as to explore the protector effect of Liu Junzitang combined with Erchentang on CHD patients. Method:A total of 76 inpatients with phlegm turbidity and internal resistance (CHD) from the Cardiovascular Department of Jiangxi University of Traditional Chinese Medicine(TCM) from November 2016 to April 2019 were selected to analyze the relationship between Hcy, NO, hs-CRP as well as the number and degree of coronary lesions. By lottery, the 76 patients were randomly divided into observation group and control group, with 38 patients in each group. Patients in the control group were given conventional therapy, while patients in the observation group were given Liu Junzitang combined with Erchentang in addition to conventional therapy. OBJECTIVETo explore the correlation between physical and chemical parameters of oil-bearing water bodies and ultrafiltration flux in the four simulative system of traditional Chinese medicine ( TCM) volatile oil.
METHODFour simulative systems of TCM oil-bearing water bodies such as Tsao-ko Amomum Fruit were selected as the experimental subjects. The membrane separating under the best conditions was carried out, membrane flux, physical and chemical parameters of stock solution and permeate were collected, and SPSS computer software was used for data processing.
RESULTThe membrane separation significantly changed the physical and chemical properties of oil-bearing water bodies, resulted in lower electrical conductivity and turbidity and pH value increased; Physical and chemical parameters also affected the membrane process significantly, and the flux varied with the electrical conductivity, pH, turbidity and viscosity.
CONCLUSIONThere is significant correlation between physical and chemical parameters of oil-bearing water bodies and their ultrafiltration flux in the four simulative systems of TCM volatile oil.

6.Predictors of in-hospital heart failure in patients undergoing successful primary percutaneous coronary intervention for acute myocardial infarction

Objectives We sought to determine the factors that predicted in-hospital heart failure(HF)in patients undergoing successful primary percutaneous coronary intervention(PCI)for ST-segment elevation myocardial infarction(STEMI). Methods The clinical and angiographic data were retrospectively reviewed in patients undergoing successful primary PCI for their ifrst STEMI. According to the occurrence of in-hospital HF, patients were divided into HF group and non-HF group. The incidence and predictors of in-hospital HF and its impact on prognosis were determined. Results A total of 834 patients were included, among them 94 patients (11.3%) were in the HF group and 740 patients(88.7%) were in the non-HF group. The mean age was (62.9±12.9) years and 662 patients (79.4%) were male. All-cause mortality at 30 days was signiifcantly higher in the HF group than in the non-HF group (24.5%vs. 1.5%, P<0.001). In Cox regression analysis, left anterior descending artery (LAD) as the culprit vessel (HR 2.173, 95% CI 1.12~4.212, P=0.022), ln 24 h NT-proBNP (HR 1.904, 95%CI 1.479~2.452, P<0.001), 24 h hsCRP≥11.0 mg/L (median) (HR 2.901, 95%CI 1.309~6.430, P=0.009) and baseline serum glucose (HR 1.022, 95%CI 1.000 ~ 1.044, P=0.046) were independent predictors of in-hospital HF. Receiver operator characteristic analysis identiifed 24 h NT-proBNP ≥ 1171 pg/ml (c=0.883, P < 0.001) and 24 h hsCRP ≥ 13.5 mg/L (c=0.829, P < 0.001) were the best cut-off values in discriminating in-hospital HF with a sensitivity and speciifcity of 92.5%and 76.8%for 24 h NT-proBNP, 86.0%and 77.0%for 24 h hsCRP, respectively. Even among patients with LAD as the culprit vessel, the incidence of in-hospital HF was only 0.4%in patients whose 24 h NT-proBNP was<1171 pg/ml and 24 h hsCRP was<13.5 mg/L;while the incidence of in-hospital HF was up to 60.9%in patients whose 24 h NT-proBNP≥1171 pg/ml and 24 h hsCRP≥13.5 mg/L (P<0.001). Conclusions The incidence of in-hospital HF was still high in STEMI patients even after successful primary PCI. Patients with in-hospital HF had poor prognosis. LAD as the culprit vessel, hsCRP, NT-proBNP and baseline serum glucose were independent predictors of in-hospital HF. Assessment and combined use of different serum biomarkers were effective methods to estimate the risk of in-hospital HF in STEMI patients undergoing primary PCI.

Influenza poses a serious threat to global public health and causes serious economic los-ses. Vaccination is the most effective measure to prevent influenza. However, the mismatch between the vac-cine strain and the epidemic strain, which results from antigenic drift and antigen conversion of influenza vi-rus, often invalidates the conventional vaccine stockpiles. mRNA vaccine is a relatively safe platform com-posed of nucleic acid. Improvements in genetic engineering technology and novel delivery systems have great-ly promoted the research and development of mRNA vaccines against influenza. mRNA vaccines are promis-ing candidates for the rapid and efficient prevention of seasonal influenza epidemics and influenza pandemics.

Objective To evaluate the efficacy and safety of midodrine hydrochloride in the treatment of intradialysis hypotension (IDH)in maintenance hemodialysis (MHD)patients.Methods One hundred and tburteen MHD patients from 8 dialysis centers with IDH were enrolled in the study.These patients took orMly midodfine for 4~6 weeks.Midodrine(2.5~10 mg)was given 15~30 minutes after the beginning of hemodialysis,and another 2.5~10 mg was used during hemodialysis if systolic blood pressure(SBP)increased less than 20 mm Hg.The total usage of each dialysis session was not more than 20 mg.The pre-,intra-,post-hemodialysis blood pressure and heart rate,the pre-and post-hemodialysis body weight,the uhrafiitrated volume of each dialysis,the pre-and post-treatment liver and renal function and electrocardiogram were measured and recorded.The symptoms of IDH were observed. Results Compared to those before treatment with midodrine hydrochloride,the minimum intradialysis SBP and heart rate at that time,the post-dialysis SBP and heart rate,and total uhrafiitrated volume changed significanlly (P<0.01).The total effective rate was 84.2%.And the symptoms of IDH were improved significantly (P<0.01).The side effects were observed in only 2 patients.Conclusion Midodrine iS safe and effective for the treatment of IDH.
9.Analysis of risk factors for acute thrombosis of arteriovenous fistulae in patients undergoing mainte-nance hemodialysis and the construction of a Nomogram prediction model

Objective To explore the risk factors for acute thrombosis of arteriovenous fistulae(AF)in maintenance hemodialysis(MHD)patients and the construction of a Nomogram prediction model.Methods A total of 418 patients who underwent MHD treatment in the outpatient clinic of the Third Affiliated Hospital of Xinxiang Medical University from December 2020 to December 2022 were selected for the study.The patients were divided into the acute thrombosis group(n=32)and non-acute thrombosis group(n=386)according to whether acute thrombosis of AF was formed or not.The influencing factors affecting acute thrombosis of AF in MHD patients were analyzed using univariate and multivariate analysis.A Nomogram predic-tion model was established based on their independent risk factors,and Bootstrap was used to validate the efficacy of the Nomo-gram model.Results The complicated diabetes,complicated hypotension,puncture failure on dialysis,calcium-phosphorus product,hypersensitive C-reactive protein(hs-CRP),total cholesterol(TC),and low density lipoprotein-cholesterol(LDL-C)levels in patients in the acute thrombosis group were significantly higher than those in the non-acute thrombosis group(P<0.05);logistic regression analysis showed that diabetes,hypotension,puncture failure on dialysis,calcium-phosphorus product elevation,high hs-CRP and high LDL-C level were the independent risk factors affecting acute thrombosis of AF in patients undergoing MHD(P<0.05);the Nomogram model was constructed based on the 6 independent risk factors,and the consistency index(C-index)of the model was 0.893(95%confidence interval:0.833-0.928);in addition,its calibration curve and the standard curve were well fitted,the area under the curve was 0.918.Conclusion Diabetes,hypotension,puncture failure during dialysis,calcium-phosphorus product elevation,and high levels of hs-CRP and LDL-C are the risk factors for acute thrombosis of AF in patients undergoing MHD,and the Nomogram model constructed based on the independent risk factors has excellent predictive ability in predicting the occurrence of acute thrombosis of AF in patients undergoing MHD,which can help in the early screening of patients with high risk of clinical acute thrombosis.
10.A qualitative study on the participation of patients with rheumatoid arthritis in shared medical appointments experience

Objective To explore the experience of patients with rheumatoid arthritis participating in the shared medical appointments(SMA),so as to provide references for medical staff to formulate targeted intervention strategies.Methods By the descriptive phenomenology research method in qualitative research,14 patients with rheumatoid arthritis who participated in the SMA in a tertiary A integrated traditional Chinese and western medicine hospital in Shanghai from August to October 2023 were selected for the in-depth interviews,and the data were analyzed and themes were extracted by Colaizzi 7-step analysis method.Results The experience of patients with rheumatoid arthritis participating in SMA can be summarized into 3 themes:①Driving factors of participation in SMA(seeking clarity and resolution,seeking support and hope);②Perceived benefits of participation in SMA(feelings of efficiency and convenience,reduced isolation,increased security,reduced stigma,increased self-efficacy,insights and experiences,lifestyle changes);(3)Barriers to participation in shared outpatient procedures(obscure medical terminology,inapplicability to some special groups,single content and form of sharing,worries and concerns about personal privacy disclosure,transportation inconvenience and time conflicts).Conclusion Medical staff should pay attention to the experience of patients with rheumatoid arthritis participating in the SMA,improve the training skills of medical staff,solve the obstacles in the process of participation in the SMA,and promote the continuous improvement and development of the SMA.
