1.Use of Antibacterials in Inpatients of Our Hospital and the Rate of Patients Using Antibacterials

OBJECTIVE:To study the correlation between the rate of inpatients using antibacterials and the cross-sectional rate of inpatients using antibacterials.METHODS:A cross sectional study was conducted on the use of antibacterials in 1040 inpatients between May 22 and May 23 in 2007.Besides,the discharge case histories of 100 cases were collected randomly for an analysis.RESULTS:Of the total 1 040 inpatients,510(49.04%)used antimicrobials.The rate of inpatients using antibacterials in comprehensive ICU took the lead at 100%,followed by pediatric department(86.05%)and the department of nephrology(84.62 %).The average rate of inpatients using antibacterials in surgery department was significantly higher than in the department of internal medicine(63.52% vs.33.94%).81% of the discharged cases used antibacterials.The irrational use of antibacterials manifested as improper timing of medication,prolonged use of antibacterials for prophylactic purpose etc.CONCLUSION:The proportion of the patients using antibacterials in descending order were inpatients,cross-sectional inpatients and outpatients.It is necessary to tighten control on the rational use of antimicrobials.
2.Literature Study on Point-selection Rules in Acupuncture-moxibustion for Gastroptosis Based on Data Mining

Objective Based on the Traditional Chinese Medicine Inheritance Support System (TCMISS) (V2.0), to analyze the point-selection rules in acupuncture-moxibustion prescriptions for gastroptosis indexed by China National Knowledge Infrastructure (CNKI), Wanfang and Vip databases, and to obtain novel prescriptions, for providing clinical references. Method Acupuncture-moxibustion prescriptions for gastroptosis indexed by CNKI, Wanfang and Vip databases were collected and filtered, and then input into the TCMISS. The prescriptions were analyzed by using data mining method. Result The frequency and core combination of the commonly-used acupoints were determined out of the 76 eligible prescriptions for gastroptosis, and 3 novel prescriptions were obtained. Conclusion The TCMISS is an important tool in mining and analyzing the point-selection rules in acupuncture-moxibustion treatment, and the new acupoint prescriptions generated by this system provide references to the treatment of gastroptosis.
3.Effect of penehyclidine hydrochloride pretreatment on NF-κB mRNA expression and SOD activity in lung tissues in rats with acute lung injury induced by LPS

Objective To investigate the effects of penehyclidine hydrochloride (PHCD) pretreatment on NF-κB mRNA expression and SOD activity in lung tissues in rats with acute lung injury (ALI) induced by LPS.Methods Thirty-two male SD rats, 2 months old, weighing 230-280 g, were randomly divided into 4 groups (n=8 each): control group (group C), ALI group, low dose PHCD group (group LP) and high dose PHCD group (group HP). ALI was induced by intravenous LPS 5 mg/kg via tail vein. Group LP and HP received intraperitoneal PHCD 0. 3 and 1 mg/kg respectively 30 min before LPS administration. The rats were killed at 6 h after LPS administration. The lungs were removed immediately for determination of W/D lung weight ratio, lung water content, NF-κB mRNA expression, TNF-α and MDA content, and SOD activity and microscopic examination. Results NF-κB mRNA expression, TNF-α and MDA content, W/D lung weight ratio and lung water content were significantly higher, while SOD activity was significantly lower in group ALI, LP and HP than in group C (P < 0.05). NF-κB mRNA expression, TNF-α and MDA content, W/D lung weight ratio and lung water content were significantly lower, while SOD activity was significantly higher in group LP and HP than in group ALI and HP than in group LP (P < 0.05). The LPS-induced changes were mitigated by pretreatment with low and high doses of PHCD in group LP and HP.Conclusion Pretreatment with PHCD attenuates LPS-induced ALI by downregulating NF-κB mRNA expression, decreasing local inflammatory response and enhancing anti-oxidant activity.
4.Correlation between reduction quality of femoral neck fracture and femoral head necrosis evaluated by three-dimensional measurement method

BACKGROUND:Reduction quality is essential for the healing and function of limb. Traditional X-ray and CT scans have some limitationsin quantitatively and visualy observing the reduction quality of the remnant fracture in three-dimensional displacements. Mimics have advantages in bone rapid modeling and three-dimensional measurement, which offers the chance in quantifying the reduction quality of the displacement in three-dimensional methods.
OBJECTIVE:To measure residual displacement in femoral neck fracture patients with satisfactory reduction by using three-dimensional measurement method, and to evaluate the association between residual displacement and femoral head necrosis.
METHODS:We enroled 141 patients with femoral neck fracture, who were treated in the Third Department of Joint, Affiliated Hospital of Logistics University of People’s Armed Police Force from January 2012 to December 2013. Al patients were divided into undisplaced fracture group (n=44; Garden1&2) and displaced fracture group (n=97; Garden3&4). Based on DICOM data of CT files, three-dimensional models were constructed in each patient. Three parameters were measured: displacement of femoral head’s center, fossa capitis femoris and rotational displacement. Femoral head necrosis was folowed up. The correlation between residual displacement and femoral head necrosis was explored.
RESULTS AND CONCLUSION:(1) The duration of folow-up in those patients ranged from 25 to 43 months. Femoral head necrosis occurred in 7 cases from the undisplaced fracture group and 21 cases from the displaced fracture group. (2) The three parameters (displacement of femoral head’s center, fossa capitis femoris and rotational displacement) were respectively (7.44±2.84) mm, (11.75±4.12) mm and (17.68±8.99)° in the undisplaced group, and (6.51±3.75) mm, (9.34±5.27) mm and (14.67±7.43)° in the displaced group. Except displacement of femoral head’s center, significant differences in the other two parameters were detected between the two groups (P< 0.05). (3) These findings suggest that poor reduction quality is the main factor causing femoral head necrosis. X ray is insufficient to describe the spatial shift and the angle, and has limitations in assessing the reductionquality of femoral neck fracture. Three-dimensional measurement has certain advantages to assess quality of fracture reduction.

6.Comparative study of laparoscopic and transabdominal radical hysterectomy's impact on the cellular immunity

Objective To compare the difference of impact on the cellular immunity between laparoscopic and transabdominal radical hysterectomy. Methods 60 patients with early cervical cancer (Ⅰa2~Ⅱa1), half of them were assigned to do abdominal radical hysterectomy (ARH) and the other half for laparoscopic radical hysterectomy (LRH). Adopt flow cytometry (FCM) detect peripheral blood T lymphocyte subsets, NK cells, CIK cells and T lymphocyte ac-tivation function on one day before surgery, one day, five days, and 28 days after the surgery separately. Compare the changes of immune status. Results After one day, the number of T lymphocyte subsets declined compared with preoperative one day(P <0.05). After five days, each index in LRH group was increased compared with postoperative one day, and the degree of decline is less than the ARH group ( P< 0.05), and recovered faster. After one day, the number of HLA-DR+CD3+in LRH group compared with the preoperative one day declined and HLA-DR+CD8+in-creased, and the degree of change is less than the ARH group, and recovered faster. Conclusions Immune function in patients after laparoscopic group was less changed, and recovered quickly, while the immune function were less inhibited, it may protect patients' immune function better.
7.Purification and bioassay of a new type of human interferon α 1c

Aim To express and purify a new sub-type of interferon α , α 1c in E.coli. Methods To establish a brief protocol for expression and purification of IFN-α 1c. In vitro assay for the IFN activity is required for the measurement of antiviral, antiproliferative and immunoregulatory properties. Results Expression of IFN-α 1c was verified by its reactivity to IFN-α 1-specific neutralizing antibodies by ELISA. Western blot also detected a band with relative molecular mass(Mr) of 19 000. IFN-α 1c possessed antiviral biological activity(3.2× 107) higher than IFN-α 1b(1.0× 107~ 1.8× 107) did on VSV challenged WISH cells. IFN-α 1c also inhibited growth rate of A549 comparable to IFN-α 1b. IFN-α 1c as an immunologic effector also augmented the cytotoxic activity of NK cells. Conclusion These results demonstrate that IFN-α 1c can be successfully expressed in E.coli. This interferon with high bioactivity may be a good candidate for clinical use.
9.125I stent implantation in treatment of malignant tracheal stenosis with help of ventilation catheter

Objective To explore the clinical efficacy of 1251 stent implantation in treatment of malignant tracheal stenosis with the help of ventilation catheter under local anesthesia.Methods Totally 180 consecutive patients with malignant tracheal stenosis underwent 125I airway stent implantation were analyzed retrospectively.The anhelation grade,oxygen saturation and respiratory rate before and after operation were recorded and analyzed,125I stent situation,clinical symptoms and survival situation were followed up after operation.Results The success rate of 125I stent placement was 100% (180/180),and the stent type was tubular stent in 132 cases,Y-shaped stent in 34 cases,and L-shaped stent in 14 cases.Dyspnea was significantly relived in all patients.Oxygen saturation and respiratory rate improved from (80.60±3.87)% and (29.36± 3.20)times/min before operation to (94.31±3.40)% and (19.29±2.19)times/min after operation (t=-30.52,35.09,both P<0.01).Patients were followed up 3-13 mouth after operation,and stent restenosis were occurred in 6 patients.The lifetime was 49-401 days and the average lifetime was (182±94) days.The 60-day survival rate was 0.99 and the 180-day survival rate was 0.65.Conclusion 125I stent implantation in treatment of malignant tracheal stenosis with the help of ventilation catheter under local anesthesia is a safe and effective method.
10.Prognosis of children with septic shock assessed by lactate and lactate clearance rate

Objective To evaluate the relationship between lactate,lactate clearance rate and prognosis of children with septic shock.Methods A total of 42 children with septic shock admitted in the PICU from Oct 2009 to Jun 2011 were enrolled in the study.All the children were given goal-directed therapy early according to the treatment routine of septic shock.The blood lactate levels before resuscitation and at 1h,2h,6 h after resuscitation were detected,the lactate clearance rates at 1 h,2 h,6 h after resuscitation were calculated.All the patients were divided into the survival group ( n =20 ) and the non-survival group ( n =22 ) by prognosis.The relationship between the early lactate level,lactate clearance rate and prognosis of the children with septic shock was evaluated.Results ( 1 ) The lactate levels before resuscitation and at 1 h,2 h,6 h after resuscitation in the survival group were significantly lower than those in the non-survival group [ (3.92 ± 2.58) mmol/L vs (6.91 ± 4.16) mmol/L,( 2.79 ± 1.89 ) mmol/L vs ( 7.93 ± 4.39 ) mmol/L,( 2.20 ±1.83) mmol/L vs (9.20 ±4.97) mmol/L,( 1.32 ±0.51 ) mmol/L vs (9.94 ±5.02) mmol/L,P <0.05].(2) The lactate clearance rates at 1 h,2 h,6 h after resuscitation in the survival group were significantly higher than those in the non-survival group [ (26.31 ± 20.82) % vs ( - 24.28 ± 53.39 ) %,(43.46 ± 17.85 ) % vs (-34.31±58.98)%,(61.04±16.71)% vs ( -45.33 ±83.51)%,P<0.05].(3) The area under curve of the lactate and the lactate clearance rate at 6 h after resuscitation were 99.4% and 96.7% according to the receiver operating characteristic curve.The children had high mortality if the lactate value > 2.20 mmol/L and lactate clearance rate < 18.65% at 6 h after resuscitation.Conclusion It is important to dynamicly monitor the lactate of the children with septic shock,the lactate and the lactate clearance rate at 6 h after resuscitation can be used to evaluate the prognosis.
