1.Investigation and analysis of the status of using intravenous transfusion instrument in grade three hospitals of Guangzhou

Objective To comprehend the application of intravenous transfusion instrument in grade three hospitals of GuangZhou,and promote the use of trochar.Methods Using the questionnaire survey,we investigated the application of intravenous transfusion instrument in 12 grade three hospitals within 24 hours on October 10th to 11th in 2012,and then collected all data for analysis.All personnel participated in the investigation were given training to ensure the accuracy of the data collection.Results 9 732 patients needed to receive intravenous therapy,among whom 3 866 patients chose scalp acupuncture and 4 195 patients chose vein detained needle.We divided all the patients who had scalp needle and vein detained needle by age.The utilization of vein detained needle was higher than scalp acupuncture in the patients of <19 years old and >70 years old,but the utilization of scalp acupuncture was higher than vein detained needle in the patients of 19~70 years old.The difference of choosing intravenous transfusion instrument among different age groups had statistical significance.Conclusions Patients of 19~70 years old are in young and maturity phase,middle age,old age.The compliance of them is lower than other patients.They think the vein detained needle influence their daily activities and individual image,and also worry about infection.So they do not actively use vein detained needle,except when having poor situation in peripheral venous.Nurses are lack of health education,and fail to allow patients actively choose vein detained needle.

Parkinson disease (PD) is one of the common neurological conditions with chronically progressed dyskinesis. 35%~70% of them are perplexed by micturition disorder, which frequently occurs in advanced stage. Urodynamics shows detrusor's overactivity and reflexic incontinence complete voiding. Treament at present includes mainly anticholinergic agents, infusing agents in bladder and/or Botulinum toxin multipoint injections to detrusor. Its genesis and effect is related to deterioration of dopaminergic neurons, hovever, and the mechanism is needed to be elucidated.
3.Effect of PCB118 on cell adhesion of human hepatocellular carcinoma cells BEL-7402

OBJECTIVE To investigate the effects and mechanism of PCB118 on cell-matrix and cel-cel adhesion in human hepatocel ular carcinoma cel s. METHODS Human hepatocel ular carcinoma cel s BEL-7402 were treated with PCB118 0.1,1.0 and 10.0 nmol · L-1 for 4 or 6 d,respectively. Then the cell-matrix adhesion assay and cell aggregation experiments were conducted to study the effect of PCB118 on cell-matrix adhesion and cell-cell adhesion in BEL-7402 cells. Quantitative real-time PCR and Western blotting methods were employed to assess the expression of key cytokines CD29,N-cadherin and E-cadherin. RESULTS The results showed that the cell-matrix adhesion ability of human hepato?cellular carcinoma cells BEL-7402 were significantly increased(P<0.05)after treatment with PCB118 0.1,1.0 and 10.0 nmol·L-1 for 6 d,whereas the cell-cell adhesion ability was significantly reduced(P<0.05). Exposure to PCB118(0.1,1.0 and 10.0 nmol·L-1)for 6 d induced significant upregulation of the mRNA expression levels of CD29 and N-cadherin along with the downregulation of E-cadherin(P<0.05). Western blotting analysis revealed that PCB118 exposure significantly increased protein expressions of CD29 and N-cadherin but reduced E-cadherin protein level(P<0.01). CONCLUSION PCB118 exposure affects the expression of CD29,N-cadherin and E-cadherin, which may be involved in PCB118-induced alteration of cell adhesion of hepatocellular carcinoma cell line BEL-7402.
4.Clinical effect of high dose of calcitriol on end-stage renal disease patients complicated with secondary hyperparathyroidism

Objective To investigate the clinical effect of high dose of calcitriol on end-stage renal disease (ESRD) in patients with secondary hyperparathyroidism (SHPT). Methods The clinical data of patients with ESRDcomplicated with SHPT were retrospectively analyzed. According to the treatment method were divided into control group and observation group, the control group were given conventional dose of calcitriol treatment, the observation group were treated with high dose of calcitriol. The differences of calcium and phosphorus levels, parathyroid hormone (PTH) level and quality of life were observed between the two groups before and after treatment. Results The serum phosphorus, calcium and calcium-phosphorus product in two groups of patients before treatment had no significant difference. After treatment, the above indicators decreased in two groups of patients compared with those before treatment, and the observation group was significantly lower (P<0.05). Before treatment, there was no difference in the levels of PTH between the two groups. After treatment, the level of PTH in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in quality of life between the two groups before treatment. After treatment, the quality of life in the two groups were higher than before treatment, and the observation group was significantly higher (P<0.05). Conclusion High dose of calcitriol has good therapeutic effect for ESRD patients with SHPT and can significantly improve the patient's calcium and phosphorus and PTH levels, improve the quality of life of patients.
5.Videourodynamic Study in Neurogenic Bladder after Operation for Lumbar Intervertebral Disk Hernia

Objective To explore the videourodynamic manifestation of neurogenic bladder after operation for lumbar intervertebral disk hernia. Methods A consecutive series of 25 patients with neurogenic bladder dysfunction after operation for lumbar intervertebral disk hernia were analysed retrospectively. They were respectively divided into L4-5 and L5-S1 groups according to the focus, or normal, injured and no-drawn groups according to saddle sensation. Their videourodynamic manifestation were analyzed statistically. Results Videourodynamic manifestation including bladder sensation,bladder compliance, stability, bladder neck's opening extent, bladder capacity and urethral pressures were not significantly different between the group L4-5 and L5-S1 (P>0.05), nor was the saddle sensation (P=0.51). But the bladder sensation and urethral pressures were related with the saddle sensation (P<0.05). Conclusion The videourodynamic changes may not be related with the focus, but the saddle sensation in patients after operation for lumbar intervertebral disk hernia, which need further elucidation.
6.Expression of Notch4 in renal cell carcinoma and its relationship with the microvascular density

Objective To investigate the expression of Notch4 protein and to analyze its correlation with the clinical parameters and the microvessel dentisty in renal cell carcinoma. Methods The expression of Notch4 was examined in 60 cases of renal cell carcinoma and the para-carcinoma tissue by SP immunohistochemical stain-ing ,and CD34 detection was used for counting microvessel density. Statistical analysis was performed to reveal the correlation with clinicopathological parameters ,microvessel density and prognosis. Results The positive rate of Notch4 protein expression was 75%(45/60)in para-carcinoma tissue,and was 43.3%(26/60)in renal cell car-cinoma,with significant difference on tumor grade and Lymph node metastasis(P<0.05). The microvessel densi-ty in Notch4 positive tissues was significant lower than that in the negative samples(P<0.05). The survival time of patients with Notch4 positive expression was significantly longer than that of patients with Notch4 negative expres-sion(P<0.05). Conclusion Notch4 protein plays an important role in the development of renal cell carcinoma. Notch4 expression might both attenuate the malignant biological characteristics and suppress the angiogenesis dur-ing tumor development.

It is a brief review of our research on the role of metallothioneins (MTs) in the occurrence and development of hepatocellular carcinoma (Hcc) in more than a decade,and the mechanism of prevention and treatment for Hcc in the field of TCM.It was found that MTs were required for the malignant proliferation of Hcc cells and their expressions significantly increased under different levels of stress.MTs may exert their effects on promoting proliferation via increasing ribosomal protein gene or other gene expressions.The over-expressions of MTs directly altered the balance of ions in the cell nucleus,and interacted on the genes promoting malignant proliferation.Hepatotoxicity and hepatic damage caused by DEN were reduced and relieved to different extents by the representative formula or constitute,which under the common therapeutic principle,by which MTs expressions were down-regulated as well.In conclusion,both effects above are important in the liver protection,inhibition of hyperplasia and carcinogenesis.
10.Effect of Detrusor Hyperactivity with Impaired Contractile Function on Voiding Function in Benign Prostatic Hyperplasia

Objective To investigate the impact of detrusor hyperactivity with impaired contractile function(DHIC)on voiding function in benign prostatic hyperplasia(BPH).Methods68 patients with BPH accepted the test of urodynamics.The urodynamic parameters between DHIC group and no DHIC group were compared statistically.ResultsThe residual urine in DHIC group was larger and the detrusor pressure at Qmax,the bladder volume were smaller than that in no DHIC group(P<0.01).ConclusionDHIC worsens the voiding function of the patients with BPH,and the test of urodynamic is helpful to diagnose and to treat it.
