1.A Study on the Changes of Protein C and Fibrinolytic System in Patients with Cerebrovasculer Disease and Intervened Treatment

Objective To investigate the changes of protein C and fibrinolytic system in patients with acute cerebrovasculer disease and the influence of intervened treatments on acute cerebral infarction(ACI).Methods Determinations of protein C(PC),protein S (PS),thrombomodulin(TM),Tissue-type plasminogen activater(t-PA),plasminogen activator inhabitor(PAI) were performed in patiens with acute cerebrovasculer disease and normal control(NC),then redeterminated these items after treatments in patients with ACI.Results PC,PS:The levels in patients with ACI were significantly lower than that in other groups.After dreatments,the levels in patients with ACI increased.There were no significantly discrepancy in acute cerebral hemorrhage(ACH) and lacunar cerebral infarct(LCI) compared with NC;TM:The levels in patients with ACH,ACI were significantly higher than that with LCI and NC.After dreatments,the levels in patients with ACI decreased;t-PA:The levels in patients with ACI were lower than NC,whereas the levels in patients with ACH increased.After dreatments,the levels in patients with ACI increased;PAI:The levels in patients with ACI were significantly higher than NC,and also found that the levels in patients with ACH and LCH were higher compared with NC.The levels in patients with ACI decreased after dreatments.Conclusions There are obviously abnormalities in plasma coagulative and fibrinlytic parameters in acute cerebrovasculer disease;To check the levels of PC,PS,TM,t-PA,PAI are important to predict the type of stroke,the seriousness of disease,the effect of treatment,prognosis of the cases and suggest that earlier treatments be important.
2.Effect of Peginterferon Alpha-2 a in the Treatment of Chronic Hepatitis C

Objective:To investigate the efficacy and safety of peginterferon alpha-2a plus ribavirin in the treatment of chronic hepatitis C. Methods:Totally 85 patients with chronic hepatitis C were randomly divided into the observation group and the control group. Each patient in the observation group was injected subcutaneously peginterferon alpha-2a with the dose of 180μg once a week plus ribavirin 900-1 200 mg/d, po, and each patient in the control was treated subcutaneously with 5 MU standard interferon alpha-2b three times a week plus the same used ribavirin. After the 48-week treatment, 24-week fellow-up was carried out. HCV RNA and ALT were detected at the baseline, 4th week, 12th week and 48th week during the treatment, and 24th week after the treatment, respective-ly. The rate of rapid virological response ( RVR) , early virological response ( EVR) , end of treatment virological response ( ETVR) , sustained virological response (SVR), ALT normalization and adverse reactions were respectively assessed. Results: The rates of EVR,ETVR and SVR in the observation group were 76. 7%(33/43), 86. 0%(37/43) and 79. 1%(34/43), respectively, which were significantly higher than those in the control group [54. 8%(23/43), 66. 7%(28/43) and 57. 1%(24/43),respectively] (P<0. 05). The ALT normalization rate at 12th and 48th week after the treatment in the observation group was 81. 4% and 90. 7%, re-spectively, which were higher than those in the control group [64. 3% and 71. 4%, respectively](P<0. 05). The rate of white cell counts and platelet underlying (58. 1% and 39. 5%) in the observation group were also higher than those in the control group (35. 7%and 19.0%)(P<0.05). The decreased rate of neutrophil (58.1%) in the observation group was higher than that in the control group(19. 0%)(P<0. 01). The other adverse reactions in the two groups were similar, and there was no new or unique adverse event related to peginterferon. Conclusion:Peginterferon alpha-2a has better effect than the standard interferon alpha-2b in the treatment of patients with chronic hepatitis C with promising tolerance.
3.Relationship between expression of the apoptosic genes Bcl-2 and Fas of thymus and clinic in patients with myasthenia gravis

4.Thearpeutic effect observation of intra-arterial thrombolysis for treatment of acute cerebral infarction

Objective To investigate the therapeutic effect of intra-arterial thrombolysis (IATT) in acute cerebral infarction(ACI). Methods 15 patients with ACI were treated with IATT using Urokinase(UK). All patients were assessed by DSA before and after IATT. The stroke scale(ESS and NIHSS) and Barthel Index(BI) were used before and 2 h,48 h,15 d ,30 d after treatment for appraising thearpeutic effect . 5.Depression and anxiety symptoms among manufacturing workers in a district of Beijing Municipality

Objective:
To investigate the symptoms of depression and anxiety and their influencing factors among manufacturing workers in a district of Beijing Municipality, so as to provide insights into prevention and intervention of depression and anxiety among workers in this industry.
Methods:
Frontline workers from 15 manufacturing enterprises including large, medium, and small/micro sizes were selected using the stratified random sampling method. Demographic and occupational information were investigated using the Chinese National Occupational Health Literacy Monitoring Questionnaire among Key Populations. The symptoms of depression and anxiety were assessed by the 9-item Patient Health Questionnaire and the Generalized Anxiety Disorder Scale, respectively. The influencing factors for depression and anxiety symptoms were analyzed using a multivariable logistic regression model.
Results:
A total of 759 questionnaires were allocated and 748 valid questionnaires were recovered, with an effective recovery rate of 98.55%. The respondents included 372 people (49.73%) from 3 large enterprises, 167 people (22.33%) from 3 medium enterprises, and 209 people (27.94%) from 9 small/micro enterprises. There were 584 males, accounting for 78.07%. The median age was 39 (interquartile range, 11.00) years, and the median duration of employment was 8.50 (interquartile range, 11.00) years. Depression and anxiety symptoms were detected in 175 and 68 cases, with the detection rates were 23.40% and 9.09%, respectively. Multivariable logistic regression analysis showed that the educational level (junior high school, OR=0.305, 95%CI: 0.129-0.723), weekly working duration (≥55 h, OR=1.727, 95%CI: 1.026-2.906) and sleep disorders (OR=3.062, 95%CI: 2.127-4.407) were influencing factors for depression symptoms; educational level (junior high school, OR=0.196, 95%CI: 0.074-0.523; high school/vocational high school/technical secondary school, OR=0.171, 95%CI: 0.064-0.452; junior college and above, OR=0.187, 95%CI: 0.066-0.527), work shift (night shift, OR=2.369, 95%CI: 1.344-4.177) and sleep disorders (OR=5.411, 95%CI: 3.076-9.519) were influencing factors for anxiety symptoms.
Conclusion
The symptoms of depression and anxiety among manufacturing workers are mainly affected by educational level, weekly working duration, work shift and sleep disorders.

Reversible cerebral vasoconstriction syndrome (RCVS) is characterized by thunderclap headache and
reversible cerebral vasoconstriction, with other neurologic signs and symptoms. To the best of our
knowledge, there were only a few cases of RCVS presenting both as both convexity subarachnoid
hemorrhage (cSAH) and posterior reversible encephalopathy syndrome (PRES). Herein, we report a
case of a 32-year-old woman with RCVS who presented with recurrent thunderclap headaches that
occurred 50 days after delivery, with cSAH and PRES on magnetic resonance imaging (MRI). She
had significant clinical and radiological recovery on 3 months’ follow-up. The clinical coexistence of
cSAH and PRES in our case with RCVS is quite rare. This case illustrates the importance of awareness
of the diagnosis of RCVS among clinicians even when initial brain and vascular imaging are normal.
Early diagnosis and treatment are crucial for better prognosis.
7.Optimizing siRNA transfection by nanoparticle cationic polymer

BACKGROUND: Transfection is the most important beginning component of research about gene function. It is a problem to find a transfection agent with high efficiency and safety. Nanosized materials have high surface activity, are easy to be modified and easier to pass the biomembrane. Researchers are studying how to use nanosized materials as a transfection agent.OBJECTIVE: To study the transfection efficiency of different basic polymers with different molecular mass and degree of substitute, and to find the optimized transfection agent.DESIGN: Controlled study.SETTING: Institute of Genetic Engineering, Southern Medical University.METHODS: This study was performed in the Institute of Genetic Engineering, Southern Medical University from March 2006 to June 2007. Using the lipofectamine reagent as the positive control, we transfected siRNA (0.2 nmol/L), FITC-labeling targeting bcl-2, by nine nanometers (polylactic acid-polyglycolic acid, chitosan-poly-caprolactone, polyethyleneimin-macrogol) into leukemic cell K562 cultured without serum. Six hours post-transfection, 20% FBS serum was added. The cell proliferation was measured at 24, 48 and 72 hours by using the MTT method. After transfecting for 48 hours, the cells were collected to detect transfection ratio by using fluorescent microscopy, apoptosis ratio and expression of K562 and bcl-2 protein by flow cytometer (FCM). RESULTS: ① Fluorescence microscope detection showed there were significant differences of transfection efficiency between different materials (P < 0.05). Moreover there were statistical differences between different degrees of substitute, although they were the same material (P < 0.05). ② MTT method indicated the cell proliferation ratio was positively related to transfection ratio. ③ Flow cytometry results showed the suppression of expression of targeting gene and apoptosis ratio were positive correlated with transfection ratio. CONCLUSION:The nanometer poly-ethylene glycol combined with poly-ethylene imine (PEG-PEI), whose molecular mass is 1 800/2 000 and degree of substitute is 29%, has a high efficiency and low toxicity.
8.Clinical significance of detection of plasma PCT,NT-pro-BNP and cTnT levels in patients with sepsis

Objective To evaluate whether there is a correlation among plasma levels of procalcitonin (PCT),N-terminal pro-brain natriuretic peptide (NT-pro-BNP)and cardiac troponin T (cTnT)in patients with sepsis,as well as significance to prognosis of patients.Methods 48 patients with sepsis who were admitted to the intensive care unit of a hospital between September 2014 and March 2015 were chosen for study,patients were divided into severe and mild sepsis groups according to the disease condition,plasma levels of PCT,NT-pro-BNP,and cTnT were de-tected,mortality of patients were analyzed statistically,relation between plasma levels of PCT,NT-pro-BNP,cT-nT and patients’death were compared.Results The plasma levels of PCT,NT-pro-BNP and cTnT in severe sepsis group were significantly higher than those in mild sepsis group (all P <0.05);mortality of mild sepsis group was significantly lower than that of severe sepsis group (10.53% vs 41.38%,P <0.05);Levels of PCT,NT-pro-BNP and cTnT levels in died patients were all higher than surviving patients (all P <0.05);levels of PCT and NT-pro-BNP,NT-pro-BNP and cTnT were positively correlated respectively (rs = 0.337,P <0.05;rs =0.456,P =0.001, respectively ),while PCT was not significantly correlated with cTnT.Plasma levels of PCT,NT-pro-BNP,and cTnT were all correlated with the prognosis of patients (P <0.05),and is helpful for judging the prognosis of pa-tients,combination of three indexes had better prognostic value for the prognosis.Conclusion Combination detec-tion of plasmid levels of PCT,NP-pro-BNP,and cTnT can assess the severity of infection in patients with sepsis, and preliminarily judge the prognosis of patients with sepsis.
9.Genetic study of a case with abnormal hemoglobin Ta-li combined with hereditary persistence of fetal hemoglobin

ObjectiveTo investigate the genotype of a case with abnormal hemoglobin combined with hereditary persistence of fetal hemoglobin (HPFH).Methods Male patient,26 years old,were suspected abnormal hemoglobin combined with HPFH after receiving medical examination including hematology exmination,hemoglobin electrophoresis,erythrocyte osmotic fragility analysis in Guangzhou Kingmed Diagnostics in September 2010.Routine examination of anemia and hemoglobin electrophoresis at alkaine pH on agarose gels were applied to analyze the phenotype.Direct sequencing of the complete HBB gene was utilized to identify the hemoglobin variant.Multiplex ligation-dependent probe amplification (MLPA) assay was used to identify the presence of β-globin gene cluster deletion.Gap polymerase chain reaction (gap-PCR) was used to amplify the HBB gene fragment across the breakpoint,and the deletion breakpoint was characterized by direct sequencing the gap-PCR product and comparing the sequencing result with the reference sequence NC_000011.9.ResultsBy direct sequencing of the complete HBB gene,the patient in this study was found to carry a hemoglobin Ta-Li (HBB:c.250G > T) mutation.By combining use of MLPA and gap-PCR with gene sequencing,we found that it had a gross deletion in the β-globin gene cluster,the deletion region was NC_000011.9:g.5222878_5250288del.Therefore,the genotype of this subject was SEA-HPFH combined with abnormal hemoglobin Ta-li.ConclusionCombining application of MLPA and gap-PCR with gene sequencing can help to make sure the genotype.
