OBJECTIVE:To investigate the curative effects of alfacalcidol soft capsules combined with calaium carbonate tablet on secondary hyperparathyroidism(SHPT)associated with chronic renal failure(CRF).METHODS:55patients were assigned to dialysis group(28cases)and non-dialysis group(27cases),both groups had taken oral alfacalcidol soft capsules in combination with calcium carbonate tablet continuously for3months.Then the changes in serum Ca 2+ ,P 3- ,PTH,AKP and Cr levels were observed.RESULTS:After treatment, serum Ca 2+ concentration was significantly increased(P

Objective To study bacteria-blocking effect of surgical gowns with new material. Methods Semi-quantitative and qualitative testing methods were used to detect bacteria-blocking rates of key sites of surgical gowns(chest and forearm), the detected samples included sample A (composite material, unused), sample B (composite material, after washing 100 times), and sample C (monolayer material, unused). Results In semi-quantitative testing, the average bacteria-blocking rates of three samples were 75.47%, 70.78%, and 73.73% respectively. In qualitative testing, three samples could effectively block the penetration of Staphylococcus aureus under wet condition and Bacillus subtilis var.niger spores under dry condition. Conclusion In semi-quantitative testing, all three kinds of samples had bacteria-blocking effect, and the average bacteria-blocking rate was> 70%;in qualitative testing, three samples all meet requirements of bacteria-blocking effect under wet and dry condition.

Objective To study the incidence and clinical significance of the herniation pits of the femoral neck. Methods 600 cases(299 men,301 women,18~82 years)were collected. The incidence, radiologic finding and clinical significance of the herniation pit of the femoral neck were analysed. Results Of 1200 hip joints in 600 cases, there was 58 cases(64 sides)(5.3%)with herniation pits of the femoral neck, including 39 men(68.7%)and 19 women(31.3%),the lesions localized in the left joint in 25 eases(39.1%),in the right joint in 27 cases(42.2%)and in bilateral joints in 6 cases(18.7%).There were a obvious significant differences on both sexes, and no significant differences on age groups. On X-ray film, the lesions appeared as a round radiolucency with thin clear sclerotic rim. Conclusion The incidence of the herniation pit of the femoral neck is 5.3%,which has a typical X-ray feature, and may indicate the femoroacetabular impingement.

Objective To evaluate the efficacy of transfrontal neuroendoscopic surgery in the treatment of chronic subdural hematoma. Methods Analysis of clinical data of 80 cases of chronic subdural hematoma.According to the surgical method,40 cases were divided into traditional external drainage of parietaltuber,40 cases were divided into transfrontal neuroendoscopic small bone window hematoma removal. The treatment efficiency, hematoma recurrence rate, operation time and intraoperative blood loss were compared between the two groups. Results Treatment efficiency of neuroendoscopic group was 95.0%,significantly higher than drainage group 75.0%,hematoma recurrence rate one month after surgery of neuroendoscopic group was 5.0%,significantly lower than that in drainage group 25.0%,operation time of neuroendoscopic group was(54.1±7.5)min,longer than that of drainage group(40.7±9.4)min,the differences were statistically significant(P<0.05).The intraoperative blood loss of neuroendoscopic group was(30.1±4.5)mL,compared with(27.1±6.4)mL in the drainage group, the difference was not statistically significant(P>0.05).Conclusion Transfrontal neuroendoscopic surgery can significantly improve the efficacy of chronic subdural hematoma, and hematoma recurrence rate is extremely low.It is worthy of clinical application.

Objective To observe the clinical curative effect of Tangbining granule in the treatment of diabetic periph-eral neuropathy syndrome of Yang deficiency and blood stasis type and the effect on oxidative stress. Methods selec-tion of diabetic peripheral neuropathy in patients with syndrome of Yang deficiency and blood stasis type, on the basis of the control group adds the clothing Tangbining granule, with symptom scores as the criterion of curative effect ob-servation of sugar,Tangbining granule in the treatment of diabetic peripheral neuropathy (DPN)effectiveness, safety, and observed before and after treatment with indicators of oxidative stress in the body superoxide dismutase enzyme(SOD)and malondialdehyde(MDA)changes. Results The total efficiency of the control group 70.37%,the total effec-tive rate of treatment group was 92.59%,there was significant difference between two groups (P<0.05); the SOD and MDA changes of the control group had no significant difference before and after the treatment, the SOD and MDA changes of the treatment group before and after treatment were statistically differences, and there was significant differ-ence in the change of MDA and SOD after treatment between two groups. There was no adverse reaction in two groups. Conclusion Tangbining granule in the treatment of diabetic peripheralneuropathy with Yang deficiency and blood stasis syndrome has exact curative effect,at the same time can improve oxidative stress in the body, without adverse reaction.

Objective To investigate the effects of protein intake in the early phase and later phase on the outcomes of critically ill patients. Methods A total of 326 critically ill patients admitted in intensive care unit of our hospital from September 2016 to March 2018 were enrolled in this prospective observational study.According to the 28-day prognosis of patients, they were divided into death group and survival group.Early protein target (EPT) was defined as the daily protein intake≥0.8 g/ (kg · d) on days 1-3,and late protein target (LPT) was defined as the daily protein intake≥0.8 g/ (k · d) on days 4-7.Results Daily protein intakes on day 1 and day 3 and cumulative protein intakes on days 1-3 were significantly higher in non-survivors than in the survivors (P<0.05),but daily protein intakes on day 2,4,5,6 and 7 and cumulative protein intakes on days 4-7 and 1-7 showed no significant difference between two groups (P>0.05).Hospital mortality was the lowest in the LPT group, the highest in the EPT,and in the middle in the EPT+LPT group and non-EPT+non-LPT group (P<0.05).The survival curve analysis showed that the survival time of the EPT-only group was significantly lower than that of the LPT-only group (P<0.05).Multivariate analysis showed that age, sex, cumulative protein and caloric intakes on days 1-7 were the independent risk factors for mortality. Conclusion Early low protein intake is benefit for the outcomes of critically ill patients, and combined with adequate intake of protein in the later stage may further improve the outcomes.
