1.A Study of the Evaluation of the Prognosis of Acute Stroke by APACHEⅢ Scoring System and Its Correlating Factors

Objective To evaluate the prognosis of acute stroke by APACHEⅢ scoring system on and analyze some correlating factors.Methods 136 patients with acute stroke received APACHEⅢ scoring every day in the first week of their admission of hospital.All patients were divided into different groups respectively: death group and alive group according to the outcome of disease,midline group and non-midline group according to the location of illness,large-focus group and small-focus group in terms of volume of cerebral hemorrhage or cerebral infarction.Results APACHEⅢ scores of death group,midline group and large-focus group show significant differences from those of the other groups(alive group,non-midline group and small-focus group).The former is much higher than that of the latter.The highest accuracy can be observed when predicted mortality is 0.4.Conclusion The APACHEⅢ scoring system is a valuable reference for evaluating the severity and prognosis of acute stroke.

4.Clinical value of pre-operative embolization of maxillary artery for nasopharyngeal angiofibroma

Objective To assess the clinical value of pre-operative embolization for nasopharyngeal angiofibroma. Methods 13 patients with nasopharyngeal angiofibroma confirmed by surgery and biopsy were retrospectively analysed. Bilateral carotid artery angiograpy was perfomed for demonstration of the arterial supply of tumor. According to the size and different caliber of arteries, embolization were seperately undertaken by different types of coil via 4.1or 5F Head-Hunter catheter. The embolization efficacy, embolization was evaluated by amount of blood loss. Results After coil embolization , no complication happened ranging 1~4 days (mean 2 days) in all 13 patients and then all the tumor masses were totally resected with mean blood loss of (584.6?379.4) ml (range 250~1500 ml). Conclusions Preoperative coils embolization is safe, reliable with less complication especially for reducing intraoperative blood loss and promoting the prognosis.

The standardization of pediatric Tuina is beneficial to pediatric Tuina practitioners in a norm practices. The paper collects the content from teaching textbooks, TCM ancient books and database literature, and tries to develop the technical specifications of pediatric Tuina by four rounds Delphi surveys and expert consensus. This specification covers the manipulation of pediatric Tuina, the position of acupoints, the effects of acupoints and the diagnosis and treatment of pediatric Tuina, including indications, contraindications, cautious use, operation steps and methods.

To assess the clinical application of video head impulse test (vHIT) for vestibular function in vestibular neuritis (VN) and benign paroxysmal positional vertigo (BPPV) patients.Thirty-three patients with VN and 43 patients with BPPV were enrolled from Sir Run Run Shaw Hospital and Ningbo Second Hospital from March 15 to September 10, 2015; and 50 healthy controls were also enrolled in the study. vHIT was used to quantitatively test the vestibulo-ocular reflex (VOR) gains of a pair of horizontal semicircular canals. VOR gains two pairs of vertical semicircular canals, and the corresponding asymmetrical value of three VOR gains. The saccades information was also recorded.Compared with the healthy control group and BPPV patients, the affected horizontal and vertical VOR gains were declined and the corresponding asymmetries were increased in VN patients (all<0.01). BPPV group also showed higher vertical VOR gain asymmetries compared with the healthy control group (all<0.01), but no significant difference was observed in VOR gains and horizontal VOR gain asymmetry (all>0.05). The sensibility of vHIT in diagnosis of VN was 87.9%. Among 33 VN patients, 22 were diagnosed with superior vestibular nerve dysfunction, 7 were found with inferior vestibular nerve dysfunction and 3 were with both dysfunction; and 1 case was not distinguished.Video head impulse test can quantitatively evaluate the vestibular dysfunction of VN and can help early diagnosis of VN, which may be widely used in clinic.
