1.Endoscopic release of distal holdfast fibers of the flexor retinaculum for carpal tunnel syndrome

Objective To investigate the necessity of releasing the distal holdfast fibers of the flexor retinaculum(DHFFR) during endoscopic carpal tunnel release(ECTR).Methods The Experiment Group included 16 cases.The operation was conducted under brachial plexus anesthesia without the use of tourniquet.A 1 cm skin incision was made.The USE system(Universal Subcutaneous Endoscope System) was employed.Both flexor retinaculum(FR) and distal holdfast fibers of the flexor retinaculum were cut off.Postoperative outcomes were compared with another 16 cases of flexor retinaculum release only(Control Group).Results Follow-up evaluation was carried out at 6 postoperative months.According to the Kelly's criteria,there were 13 cases of excellent results and 3 cases of good results in the Experiment Group,and 8 cases of excellent,5 cases of good,and 3 cases of fair results in the Control Group.Significant difference was obser red in flameda Ⅱ or Ⅲ grade patients between the two groups in carative effects(?~2=6.278,P=0.043).No serious complications or postoperative recurrence occurred.Conclusions Flexor retinaculum is not the only structure existing in the carpal canal to be released.More attention should be paid to complete decompression of both flexor retinaculum and distal holdfast fibers of the flexor retinaculum,especially in those who have serious symptoms.
2.Clinical observation of ganciclovir treatment for the young child viral encephalitis

Objective To observe effect of ganciclovir on the treatment of the young child viral encephalitis. Methods 68 cases of viral encephalitis were randomly divided into treatment group(45 cases) and the control group (23 cases), the two groups of children with clinical manifestations and treatment effects were observed. Results Headache, vomiting, convulsions symptoms and the resumption of the time was obviously shorter in treatment group than that of the control group, fever symptoms persisted despite the shortened time, but the difference was not statis- tically significant. Conclusion Ganciclovir in children with viral encephalitis have significant effect.
3.Endoscopic tendon sheath release for stenosing tenovaginitis: A preliminary report

Objective To evaluate the endoscopic tendon sheath release for stenosing tenovaginitis (trigger finger). Methods Five patients with stenosing tenovaginitis (2 in thumbs, 1 in middle finger, and 2 in ring fingers) underwent operations by Smith & Nephew Endoscopic Trigger Finger Release system. After two 3.0mm transverse incisions were made, the window cannula assembly was inserted subcutaneously along the sheath from the proximal portal and advanced until it passed through the distal portal. Then a 2.7 mm endoscope was passed into from the proximal portal and a retrograde knife was introduced into the operative site from the distal portal. Finally the entire length of sheath was sectioned under direct endoscopic vision. Results All operations were successfully completed. Finger's flexion and extension function recovered immediately after the operations. All the patients restarted their employment one week postoperatively. There were no complications such as distinct pain or delayed wound healing in these patients. Conclusions This method has the advantages of minimal invasion, safety, effectiveness and quick recovery, especially suited to diabetic patients or multiple trigger fingers.
4.Surgical treatment of the lumbar degenerative de novo scoliosis combined with vertebral stenosis

[Objective]To explore the surgical plan of lumbar degenerative de novo scoliosis combined with vertebral stenosis(LDDS) and evaluate the surgical efficiency.[Method]From July 2003 to Jan.2007,32 LDDS patients were diagnosed and operated in the author’s hospital.The X-ray,computed tomography and MRI radiological data were measured carefully preoperately which included the Cobb’s angle,vertebral rotational and lateral subluxation degree.All the patients were treated by posterior decompression and pedicle screw internal fixation,and the surgery,fluoroscopic time and bleeding amount were recorded.Patients were administered pre- and postoperatively with clinical outcome surveys of Visual Analogue Pain Score(VAS),Low Back Outcome Score(LBOS).Pre- and postoperative measurements of the Cobb's angle and lumbar lordosis of the involved segments were analyzed.[Result]The surgery time was 4?0.5 hours,the average bleeding amount was 400?30 ml,the fluoroscopic time was 30?5 min.All the patients were followed from 8 months to 4 years,and radiography was proceeded on 8 and 16 months respectively post surgery.Twenty-four cases got good bone fusion,and mean Cobb's angle decreased from 37.2 to 18.4 degrees,mean focal lordosis increased from 13.1 to 16.4 degrees,mean vertebral rotation angle decreased from 14.2 to 9.8 degrees,mean preoperative measures of VAS and LBOS improved from 5.3?2.2 and 24.8?15.6,respectively to 2.2?2.1 and 44.8?18.0 at last follow-up,the Cobb’s angle,lumbar lordosis and the degree of vertebral rotation had significantly difference pre and post surgery(t test,P

Objective To explore the effect of Schisandrae Sphenantherae on proliferation and differentiation of rat osteoblast in vitro. Methods Using the method of serum pharmacology, osteoblast was isolated from calvaria of newborn SD rats by means of modified sequential collagenase digestion and incubated in RPMI 1640 medium. The cell morphology was observed under a phase contrast inverted microscope. MTT assay, ALP activity, mineral node count were detected to determine the status and activities of proliferation and differentiation. 6.Proximal tibial shape and its effect on intramedullary tibial nailing

7.Influence of nasal septum fracture on the effect of nasal bone reduction

OBJECTIVE To investigate the incidence rate of nasal septum fracture and it's influence on the effect of nasal bone reduction. METHODS Retrospective analysis of 212 cases of nasal fracture compared with nasal septum deviation were carried out. Among these cases 112 treated with nasal bone reduction only was called group A; the other cases were treated with nasal reduction concomitantly with septoplasty. Among them 58 cases were diagnosed with nasal septum fracture, it was called group B; 42 cases were diagnosed with nasal septum deviation, it was called group C. The postoperative nasal deformity rate was assessed, and the intraoperative findings of nasal septum contrast with preoperative CT diagnosis was analysed. RESULTS Three months after the operation, 27 cases (24.1%) of nasal deformity were found in group A; 9(9.0%) cases of nasal deformity were found in group B+C; septum fracture was found in 58 cases (100%) in group B and 30(71.8%) cases in group C introperatively. The diagnosis rate of nasal septum fracture by CT was 58.0%, while it's incidence rate was 88.0%. CONCLUSION More nasal septum fractures were observed introperatively than CT findings, and septoplasty concomitantly with nasal bone reduction can obviously reduce the rate of nasal deformity.
8.Experimental research of the olfactory receptor neurons of olfactory dysfunction by allergic rhinitis

OBJECTIVE To set up an intranasal ovalbumin-induced animal model of allergic rhinitis(AR) accompanied with olfactory dysfunction in mice. By observing the olfactory pathway in mice using manganese-enhanced magnetic resonance imaging (MEMRI) and the relatively morphologic structural and immunological changes in olfactory epithelium, the influence of AR on olfactory receptor neurons(ORNs) was studied.METHODS Forty SD mice were randomly divided into two groups, the research group(n=30) and the control group(n=10). The research group was intraperitoneally injected and intranasal application of ovalbumin to establish an AR mice model. The olfactory function of the mice was evaluated by buried food test(BFT). ELISA was performed to measure the level of IgE in serum. MEMRI images were acquired with a 7.0 T micro-MR scanner. HE staining and immunohistochemistry were used to observe the tissues morphology change of olfactory mucosa and OMP expression.RESULTS The olfactory function evaluation of the AR mice model indicated that the incidence of olfactory dysfunction in AR mice was 40.0%. The AR mice with olfactory dysfunction had no signal enhancement in MEMRI. The olfactory epithelium became thinner, layer numbers of ORNs were decreased with disorder arrangement and the OMP expression was decreased in AR mice with olfactory dysfunction compared with that in AR mice without olfactory dysfunction(P=0.018) and the control group(P=0.0141).CONCLUSION An animal model of AR accompanied with olfactory dysfunction in mice was successfully established. The influence of AR on ORNs and thus cause the change of the olfactory pathway is one of the major pathogenesis of olfactory dysfunction in AR.
9.Design, synthesis and antitumor activities of oxazolo[5, 4-d] pyrimidine derivatives

A series of oxazole[5,4-d] pyrimidine derivatives were designed and synthesized to discover novel compounds with antitumor activity.Compounds 8a-8m were synthesized using acetamidine hydrochloride as the start material.The structures of synthesized compounds were confirmed by IR,1H NMR,EI-MS and elemental analysis.The antiangiogenesis activities of the synthesized compounds were determined by MTT in human umbilical vein endothelial cell (HUVEC).The in vitro antitumor activities of the synthesized compounds were determined by MTT assay in A549,HepG2 and U251.Compounds 8c,8d,8g,8i and 8l were found to inhibit the proliferation of all the tested cell lines.Compound 8l exhibited noteworthy activities in A549,HepG2 and U251 cell lines with IC50value lower than the positive reference sunitinib,suggesting that compound 8l might be the promising antitumor agent for further investigation.
10.Levels of Serum Endothelin and Other Inflammatory Factors in Prehypertension and Hypertension Patients

Objective:To observe the levels of serum endothelin(ET),nitric oxide(NO),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in prehypertension and hypertension patients.Methods:Three groups were recruited from 3050 subjects of regular health checkup in Tangshan Railway Hospital from June to September,2007. Prehypertension (PH) group: n=106,Essential hypertension (EH) group: n=91,and Normotensive (NT) control group: n=98.The levels of ET,TNF-α,IL-6 and NO were compared by radioimmunoassay and biochemical tests. Results: ①Compared with NT group,body mass index (BMI) and serum lipids (TC,TG,LDL-C) were significantly increased in PH and EH groups (P<0.05),high-density lipoprotein cholesterol (HDL-C)was decreased in EH groups (1.59±0.15 vs. 1.77±0.19,P<0.05). ②Compared with NT group,ET,IL-6,TNF-α were increased in PH and EH groups (P<0.05).NO was increased in PH group (6.55±0.94 vs. 5.87±0.80,P<0.05) while decreased in EH group (5.28±0.81 vs. 5.87±0.80,P<0.05). ③Logistic analysis indicated that BMI,ET,NO,TNF-α and IL-6 were related to the hypertension. Conclusion: ET,NO,TNF-α,IL-6 were elevated in prehypertension patients and those might be used to predict the risk of hypertension.
