1.Biological effects of hypoxia on mesenchymal stem cell
International Journal of Biomedical Engineering 2013;36(3):184-188
Mesenchymal stem cell (MSC) resides in a relatively hypoxia niche (volume fraction 1%~7%)compared to normoxia (20%).MSC cultured in hypoxia condition shows different biological features from those in normoxia condition.Hypoxia inducible factor-1 α (HIF-1 α),as one of main transcription factors in hypoxia, participates in the regulation of cell proliferation, differentiation, apoptosis, glycolysis and angiogenesis.Recent researches show that HIF-lα also plays an important role in regulating the survival, differentiation and migration of MSC.It turns out that hypoxia can improve survival and migration of MSC,enhance chondrogenesis but inhibit adipogenesis through HIF-lα and its downstream genes. However, whether hypoxia promotes or impedes osteogenesis remains controversial.Such differences can be caused by variances of tissue origination, oxygen concentration and culture condition. The comparison of different researches should be based on the same experiment condition. This review focuses on the influence of hypoxia on proliferation, apoptosis, differentiation and migration of MSC.

Objective To compare the effectiveness and safety of glucocorticoid versus intravenous injection of human immunoglobulin (IVIG) in treatment of with acute primary immune thrombocytopenia (ITP) in children. Methods PubMed, the Cochrane Database of Systematic Reviews, the Cochrane Central Register of Controlled Trials, Chinese biomedical literature database (CBM), Chinese Journal Full Text Database (CNKI) and Wanfang database were searched. The bibliography was screened according to the inclusion and exclusion criteria, and the target literatures were selected. The data were extracted and the quality of included literatures was evaluated. Revman 5 . 3 software was used to make meta-analysis. Results In 1500 papers searched, 8 papers met the inclusion criteria. Meta-analysis showed that there was a statistical signiifcance in platelet count (PLT,>20 × 109/L) after being treated for 48 h between glucocorticoid group and IVIG group (RR=0 . 77 , 95%CI:0 . 67~0 . 89 ). In subgroup analysis, there were statistical signiifcance in methylprednisolone (MP) 30 mg/kg versus IVIG 1 g/(kg·d)× 2 d, and metacortandratin (PDN) 4 mg/kg versus IVIG 1 g/(kg·d)× 2 d (RR=0 . 66 , 95%CI:0 . 47-0 . 91;RR=0 . 79 , 95%CI:0 . 66-0 . 95 ). After treatment for 24 h and 72 h, the platelet count>20 × 109/L were signiifcantly different in glucocorticoid group and in IVIG group (RR=0 . 69 , 95%CI:0 . 53-0 . 91;RR=0 . 82 , 95%CI:0 . 74-0 . 90 ). Moreover, after treatment for 24 h, 48 h and 72 h, the platelet count>50 × 109/L were signiifcantly different between glucocorticoid group and IVIG group (RR=0 . 38 ,95%CI:0 . 21-0 . 69;RR=0 . 53 , 95%CI:0 . 41-0 . 69;RR=0 . 80 , 95%CI:0 . 70-0 . 93 ). There was no difference in the incidence of splenic resection between two groups (RR=5 . 41 , 95%CI:0 . 95-30 . 74 , P=0 . 06 ). Conclusion The probability of platelet count to reach>20 × 109/L in the initial treatment with glucocorticoid of acute ITP patients were 32%lower than that with IVIG. With the initial therapeutic target being platelet count>50 × 109/L in 3 days, the effect of IVIG was better.

Objective To analyze the CT and clinical features of acute pancreatitis(AP) patients with perirenal space(PRS) invasion. Methods CT images and renal function tests(serum urea, creatine) changes of 64 AP patients were retrospectively studied. PRS invasion by inflammation on CT scan and the relation between PRS invasion and renal function changes were analyzed. Results 81% patients had PRS invasion in which 52% were grade B, 25% were grade C. CT features of the PRS invasion varied from mild inflammatory changes to fluid collection or phlegmonous. The PRS invasion was detected in 35 of 52 patients with mild pancreatitis and all severe pancreatitis. Renal function abnormity was 47%, only 3% patients had abnormal renal function in patients without PRS invasion compared to 44% patients with PRS invasion. PRS invasion in patients with abnormal renal function attenuated on CT scan after the pancreatitis was controlled. Conclusions The CT clearly reveals the features of PRS in acute pancreatitis patients. The attenuation of PRS invasion in acute pancreatitis patients on CT parallels renal function recover.

Radiation-induced brain injury is a neurological injury caused by radiation therapy of head and neck tumors. The pathogenesis is still unclear. It is hypothesized that immune cells play an important role in radiation-induced brain injury. The excessive activation of microglia in the central nervous system and the migration of peripheral immune cells into the brain collectively promote the incidence and development of radiation-induced brain injury. In this article, the immunological mechanism underlying the radiation-induced brain injury was reviewed.

Objective To research the resilience and related factors of the rescue soldiers. Methods Resilience Scale for Adults( RSA ), Self-report Symptom Checklist 90 (SCL-90), Eysenck Personality Questionnaire(EPQ) and Coping Style Questionnaire were applied in this survey to 1401 rescue soldiers. Results ①The soldiers' resilience score was (2.49±0.57) ,significantly lower than college students( t= 16.619, P=0. 000).The rescue soldiers with the year in army above 3 were significantly higher than that below 2 years in social competence( t=2.581, P=0.01 ) ,social resources( t=2. 158, P=0.031 ) ,structured style( t=3.254, P=0.001 ) and RSA scores(t=2.455, P=0.014). There exist significantly differences in various education level of the rescue soldiers in perception of self(t=3.732, P=0.024), planned of future ( t = 9.324, P = 0.000 ) , social competence ( t = 8. 838, P = 0.000 ), social resources ( t = 13. 660, P = 0. 000 ) and RSA scores (t=9.805, P=0.000) , and college ≥ senior high school ≥ junior high school. ②Compared to the lower group in the RSA, the higher group scored significantly higher in extraversion (t=16. 204, P=0.000) and positive coping( t = - 18. 171, P=0.000), while lower in SCL-90 (t=8.461, P=0.000) and neuroticism (t=8.833, P=0.000 ).③Resilience have the obvious positive correlation with the education ( r= 0. 116 ), extraversion ( r= 0. 463 ) and positive coping(r=0.500) ,and negative correlation with the neuroticism(r=-0.251 ) and SCL-90( r= -0.260). It can be assumed that positive coping, extraversion and neuroticism had good predictive ability(35.9%) to resilience. Conclusion The rescue soldiers have low resilience than college students, positive coping, extraversion, and neuroticism are important psychological factors for rescue soldiers.

Objective To investigate the early risk factors of the formation of pancreatic pseudocysts after severe acute pancreatitis. Methods One hundred patients with severe acute pancreatitis admitted from Jul. 2005 to Mar. 2007 were included. Clinical and laboratory data within 24 hours of admission and radiological tests of chest, abdominal dynamic contrast-enhanced computed tomography and abdominal ultrasound within 3 days after admission were analyzed and multiple stepwise logistic regression analysis was performed. Results 30 patients developed pancreatic pseudocysts and the incidence of pancreatic pseudocysts in the clinical course of severe acute pancreatitis was 30%. There were significant difference between group A (pancreatic pseudocyst group) and group B (non-pancreatic pseudocyst group) in serum albumin[(33.23±4.810g/L vs (36.07±4.92)g/L], CT severity index (CTSI) (3~6 vs 2~4 points) ,length of hospital stay[(26.83±19.760) day vs (14.51±7.71) days, (P<0.05)]. Meanwhile, there were no significant differences between the two groups in age, gender proportion, body temperature, heart rate, breath rate and mean arterial pressure in admission, urine volume within 24 hours, early defaecation within 24 hours after admission, blood routine, liver function, kidney function, electrolytes, blood cholesterol and triglycerol, PT, APTr, arterial blood gas analysis, blood amylase, C-reaction protein, APACHE Ⅱ, RANSON scoring, early ascites and pleural effusion. But multiple stepwise logistic regression analysis showed that the serum albumin and CTSI were associated with the formation of pancreatic pseudocysts after severe acute pancreatids. Conclusions The serum albumin and CTSI were the independent risk factors of the formation of pancreatic pseudocysts after severe acute pancreatitis.
