Journal of International Oncology 2011;38(9):703-707
Colorectal cancer is the third high incidence of tumor in the world, and liver is one of the most common sites of distant metastasis.Liver metastasis accounts for 50%-75% in distant metastasis.Only about 10% -15% of patients are candidates for resection, and the recurrence is easy.Selective internal radiation therapy is a new type of radiotherapy. It is partial and liver-targeted.For unresectable colorectal liver metastasis, selective internal radiation therapy alone or in combination with other therapies can reduce cancer focus, prolong the survival, and maybe improve overall survival.

Background Retinal vascular recanalization is key to the treatment of retinal vascular occlusive disease.Studies confirmed that urokinase by intravitreal injection inhibits the expression of occludin protein at tight junction complexes among retinal capillary endothelial cells. Objective This study was to observe the effects of urokinase via eye local injection on the outward permeability of blood-retinal barrier by detecting the concentration of intravitreal Evans blue (EB).Methods Sixty healthy Sprague-Dawley (SD) rats were randomly assigned to four groups, and the right eyes of the rats were used as experimental eyes.Urokinase of 4 μl (350 U) and the equal volume of PBS (0.01 mol/L) was intravitreally injected separately in the intravitreal urokinase group and the intravitreal PBS group, and 10 μl urokinase (1000U) and the equal volume of PBS was injected via retrobulbar tissue respectively as the retrobulbar urokinase group and the retrobulbar PBS group.Twenty-four hours after injection of drugs,0.5% EB 4 μl was intravitreally injected.Four hours later, the rats were sacrificed and the right eyeballs were excised for the extraction and drying.EB was extracted from dried vitreous by formamide. Then, the concentration of EB in formamide was determined by a formamide extraction-ultraviolet spectrophotometry method to calculate the concentration of EB in vitreous. The use and care of experimental animals followed the Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission (2011 version). Results The rat vitreous body showed the light blue color in intravitreal urokinase group and the retinal vessels were visible under the microscope, and that in the retrobulbar urokinase group presented blue color. However,in the intravitreal and retrobulbar PBS group, rat vitreous exhibited the deeper blue color and retinas were invisible.Absorbance of EB in formamide was 0.181 ±0.008,0.450±0.017,0.330±0.009 and 0.436±0.012 in the intravitreal urokinase group, intravitreal PBS group, retrobulbar urokinase group and retrobulbar PBS group, respectively. The intravitreal EB concentrations in the intravitreal urokinase group were (0.266±0.014)g/L,which was lower than (0.667±0.026) g/L,(0.496±0.015) g/L and (0.657±0.017) g/L of the intravitreal PBS group, retrobulbar urokinase group and retrobulbar PBS group, showing significant different among the four groups (F =100.406,P<0.01),and the intravitreal urokinase group showed the lowest value in comparison with other three groups (all at P<0.01).Conclusions Local application of urokinase around eye can augment the outward permeability of blood-retinal barrier in rats.Intravitreal assay of EB after intravitreal injection is a feasible approach to the determination of outward permeability of blood-retinal barrier.

BACKGROUND: Posttraumatic kyphosis and neurologic compromise secondary to osteoporotic fractures, which needs surgery treatment. However, the choice of approach method is an argument. OBJECTIVE: To explore the therapeutic effect of transpedicular decompression and pedicle fixation for the treatment of obsolete thoracolumbar vertebral fractures combined with kyphosis deformity.METHODS: A total of 23 cases with obsolete thoracolumbar fractures combined with kyphosis admitted at the Department of Spinal Surgery, First Affiliated Hospital, University of South China were selected, including 14 males and 9 females, aged 18-60 years, mean aged 36 years; The JOA scores of patients were (11.02±1.24) points prior to operation, with (37.43±3.76)° Cobbangel. All patients were treated by transpedicular decompression and pedicle fixation. The functional recovery of lumbar vertebra, bone healing, as well as the complications was compared before and after operation.RESULTS AND CONCLUSION: All patients were followed-up for 6-28 months with an average of 12.5 months. The lumbar vertebra function and kyphosis deformity was notably improved postoperatively. After treatment, the JOA score and Cobb angle was (14.04±2.12) points and (11.02±3.58)°, which was obviously different than that of preoperative (P<0.05). The surfaces of cut bone were healed at 6 months after operation, without complications of pseudoarticulation formation or correction loss. The results suggested that transpedicular decompression and pedicle fixation can achieve a satisfactory result in deformity correction and neurological decompression, which can precipitate the functional recovery.

[Objective]To evaluate the effects of clinical application of B-twin and cage for treatment of lumbar degenetative disease. [Methods]From June 2004 to December 2007,a total of 112 patients with lumbar degenetative disease who underwent posterior lumbar interbody fusion(PLIF) with cage and 79 patients who underwent PLIF with B-twin were enrolled. Sixty patients were selected and randomly divided into two groups(30 in each group). The clinical curative effect, the rate of bone graft fusion, the intervertebral space height and complications in both groups were analyzed and compared.[Results]In the B-twin group, the treatment was a minimally invasive technique, which offered a satisfactory effect with good bone graft fusion, fewer complications and sooner recovery, but the cost was higher than that in cage group. The cage treatment was an open sugery for fenestration operation. It had more complications but more sufficient decompression and lower cost. The rates of bone graft fusion between the 2 groups were not different in statistics.[Conclusion]The B-twin and cage techniques are two treatmens for interbody fusion of lumbar degenetative disease. Both have advantages and clinical application value. It should be choosen according to different specific conditions in the clinical practice.

Objective To investigate the bacterial pathogenesis in chronic prostatitis. Methods The prostatic fluid of 132 patients was studied with "five glass"segmented lower urinary tract localization culture test and measurement of IgA and IgG levels. Some bacteria positive cases were treated with levofloxacin. Results 74 patients were bacteria positive in their prostates,32 had Staphylococcus aureus,17 with Staphylococcus epidermidis,10 cases had Escherichia coli and 15 with other bacteria IgG and IgA levels were significantly higher ( P =0.031 and P =0.036) in bacteria positive prostatic fluid than in bacteria negatives ones. Conclusions The positive rate of bacteria culture in the prosatic fluid of patients with chronic prostatitis was high (56.1%) and Gram positive bacterias were more common. These pathogens may come from the partners reproductive tract. The levels of IgA and IgG in prostatic fluid were correlated to the results of bacteria culture.

Radiation-induced brain injury is a neurological injury caused by radiation therapy of head and neck tumors. The pathogenesis is still unclear. It is hypothesized that immune cells play an important role in radiation-induced brain injury. The excessive activation of microglia in the central nervous system and the migration of peripheral immune cells into the brain collectively promote the incidence and development of radiation-induced brain injury. In this article, the immunological mechanism underlying the radiation-induced brain injury was reviewed.

AIM: To investigate the effect of mfn2 on mitochondrial function in steatosis hepatocytes. METHODS: Plasmid pEGFP-mfn2 was transfected into hepatocyte strain L02 by Lipofectamine 2000 in vitro, then the steatosis model of hepatocytes was establish by oleic acid induction. RT-PCR was used to evaluate mRNA expression and Western blotting was use to detect the protein expression. ATP level was determined by firefly luciferase bioluminescent. ROS production was measured by fluorescence probe DCFH-DA. Chondrosome transmembrane potential of L02 was observed by labeling of JC-1 and FCM. RESULTS: The stable expression of ectogenesis mitofusin2 in L02 cells was confirmed by RT-PCR and Western blotting. In the model of oleic acids induced lipid formation, Mfn2 obviously inhibited the descent of chondrosome transmembrane potential and ATP level, and increased ROS production in L02 cells. CONCLUSION: Up-regulated expression of mfn2 attenuates mitochondria dysfunction caused by oleic acids induced lipid formation.

Replenishing the insulin-producing β cell mass is considered to be a hot topic in the field of diabetes treatment. It has long been known that pancreatic β cells are generated primarily by self-duplication in adults. However, insulin resistance can induce dramatic compensatory β cell mass expansion in the pancreas. Therefore,it is of great value to investigate the etiology and pathogenesis of β cell proliferation under insulin resistance. The recent study has shown that betatrophin,a protein secreted by the liver, modulates β cell growth in response to insulin resistance. We published one article in Diabetes Care titledIncreased circulating levels of betatrophin in newly diagnosed type 2 diabetic patientsto elaborate the relationship between diabetes and betatrophin.
