BACKGROUND: Clinically, in patients undergoing hematopoietic stem cell transplantation (HSCT), human cytomegalovirus (HCMV) can be associated with delayed platelet engraftment, phenotypically abnormal peripheral blood leukocytes, and graft rejection, possibly through a direct viral effect on hematopoietic progenitor cells after HCMV infection. OBJECTIVE: To investigate the inhibitory effect of ganciclovir (GCV) on proliferation of colony forming unit (CFU) granulocyte-macrophage (CFU-GM), CFU-erythroid (CFU-E), CFU T-lymphocyte (CFU-TL), CFU-multipotential (CFU-Mix) and CFU-megakaryocyte (CFU-Mk) progenitor cells of cord blood (CB) and the protective effects on them. DESIGN: Contrast observational study.SETTING: Department of Molecular Biology, Affiliated Hospital of Luzhou Medical College.PARTICIPANTS: A total of 20 cord blood (CB) samples (with 10 mL for each sample) from fetal umbilical vein of normal term spontaneous delivery neonates were provided by the Department of Gynaecology and Obstetrics, Affiliated Hospital of Luzhou Medical College. All the patients were informed and agreed with the experiment.METHODS: The experiment was carried out in the Department of Molecular Biology, Affiliated Hospital of Luzhou Medical College from June 2004 to December 2006. Colony forming unit-assay was applied to observe the suppression effect of HCMV-AD169 strain on CFU-GM, CFU-E, CFU-TL, CFU-Mix and CFU-Mk of CB with the presence of GCV. The techniques of polymerase chain reaction (PCR) and fluorescence quantification PCR were used to demonstrate the existence of HCMV-AD169 DNA in the colony cells of cultured CFU-GM, CFU-E, CFU-TL, CFU-Mix and CFU-Mk. Normal progenitor cells culture system was regarded as blank control group; normal progenitor cells culture system with inactivated HCMV fluid as inactivated (IV) control group.MAIN OUTCOME MEASURES: ① The number and maintaining duration of colonies of cultured progenitor cells were counted by using a light inverted phase contrast microscope. ② The techniques of PCR and fluorescence quantification PCR were used to demonstrate the existence of HCMV-AD169 DNA in the colony cells of cultured progenitor cells.RESULTS: ① Number and lasting time of colonies: The numbers of CFU-GM, CFU-E, CFU-TL, CFU-Mix and CFU-Mk colonies in the HCMV infection group were significantly less than those in the blank control group (P < 0.01). The maintaining duration of colonies in the HCMV infection group was significantly shorter than that in the blank control group (P < 0.01). HCMV-DNA copies of colony cells of GCV group decreased significantly by using fluorescence quantification PCR compared with HCMV group (P < 0.01), while negative in blank control and inactivated control in CFU-MK and CFU-Mix. ② CFU Growth rate: The Growth rate of colonies was 37.4%, 74.2%, 40.1%, 67.4% and 38.9% of CFU-GM, CFU-E, CFU-TL, CFU-Mix and FU-MK, respectively. ③ CFU-HCMV-AD169 DNA: Fluorescence quantification PCR showed that nucleonic acid content of progenitor cells after GCV-affected HCMV infection was decreased as compared with that after HCMV infection (P < 0.01).CONCLUSION: The differentiation and proliferation of CFU-GM, CFU-E, CFU-TL, CFU-Mix and CFU-Mk are significantly inhibited after infected with CMV-AD169 strain. The growth of hematopoietic progenitor cell after HCMV-AD169 infection is promoted by GCV, which suggests that GCV has an effect of anti-HCMV in vitro.

Objective To evaluate the clinical application of dynamic contrast-enhanced MRI of the breast. Methods In this study, 56 cases received dynamic contrast-enhanced MRI of the breast, among them 37 cases with a total 45 solid masses were confirmed by pathology. The clinical value of breast MRI was analyzed retrospectively with the combination of surgery and pathology. Results Except for multiple bilateral cysts in one case among the 45 solid masses, 17 masses were benign and 28 were malignant.MRI detected multiple carcinoma in 2 cases, residual carcinoma in one case and intraductal carcinoma in 2 cases. The smallest mass detected was 4 mm in diameter.Tumor detection rate in this series was 100%, with the diagnostic sensitivity, speciality and accuracy being 92%, 92% and 90% respectively. Conclusion Dynamic contrast-enhanced MRI of the breast has high sensitivity and helps to make a accurate diagnosis.

Objective To explore the values of manual anastomosis with two operations out of anus in laparoscopic anal sphincter preserving resection of ultra low rectal cancer. Methods Radical excision of ultra low rectal cancer was performed with ultrasonic scalpel in 12 patients based on the concept of total mesorectal excision (TME) and ultra low coloreclal/anal anastomosis was performed applying manual anastomosis with two operations out of anus. Results All the operations were finished successfully, without conversions to open for surgery. One case had anastomotic leakage, and there were no bleeding and infection of abdominal cavity, anastomotic stenosis and other complications. The operating time was 185-310 (218 ±10) min, the blood loss was 160-450 (232 ±8) ml,the length of hospital stay was 9-14 (11 ±3) d. All patients were followed up 6-36(18 ± 2) months, local recurrence was not found but 1 case had liver metastasis. Conclusions The manual anastomosis with two operations out of anus in laparoscopic anal sphincter preserving resection of ultra low rectal cancer is safe, economical, effective, minimally invasive, and has the benefits of less bleeding during the operation and shorter hospital stay. It should be widely used.

BACKGROUND:The siphon of internal carotid artery is a complex of bending and surrounding structure, which has become a research hot in the field of medical imaging and regional anatomy. There is little research on the hemodynamics of internal carotid artery. Finite element analysis provides the basis for the related dynamics research. OBJECTIVE:To explicit the hemodynamic characteristics of the normal and stenosis internal carotid artery, and to explore the relevant influential factors. METHODS:Finite element model of the siphon of internal carotid artery was built based on 64-slice spiral CT data, and then, three-dimensional models were constructed using Mimics 10.01 software. There were simulations of the normal and post-stenosis blood flows with ANSYS 13.0 CFX software, to observe the changes of hemodynamics, and to analyze their characteristics and differences. RESULTS AND CONCLUSION:The normal blood flow at the siphon of internal carotid artery was in a laminar state, but rotation and turbulent flow formed at the two angle regions. The velocity of external bending zone was slower than that of the internal. The slower was the velocity, the more obvious was the turbulence. The wal shear force decreased at the angle regions, and the wal shear force of external bending zone was smal er than that of the internal. At the region of arterial stenosis, the blood flow was sped, and at the downstream, turbulent flow and low zone of wal shear force were formed. With the increase of stenosis severity, the turbulent flow and low wal shear force area were expanded. Central stenosis showed more obvious effects than the eccentric one. The degree of stenosis and bending at the siphon of internal carotid artery can directly influence the formation of turbulent flow and low wal shear force area, which are more obvious at the external bending zone and central stenosis.

Raman spectroscopy has generated many branches during the development for more than 90 years. Surface enhanced Raman spectroscopy (SERS) improves SNR by using the interaction between tested materials and the surface of rough metal, as to quickly get higher sensitivity and precision spectroscopy without sample pretreatment. This article describes the characteristic and classification of SERS, and updates the theory and clinical application of SERS. It also summarizes the present status and progress of SERS in various disciplines and illustrates the necessity and urgency of its research, which provides rationale for the application for SERS in microbiology.
