1.Research progress on components in human milk and their mechanism for preventing necrotizing enterocolitis

Necrotizing enterocolitis (NEC) is one of the common intestinal critical disease in newborns especially in premature infants.However,the etiology and pathogenesis of NEC are not fully clear.There were a lot of research which had shown that breast-feeding could significantly reduce the incidence of NEC.The composition in human milk,such as human milk oligosaccharides,growth factors,glutamine,lactoferrin,has effects on anti-infection,maintaining intestinal mucosal integrity and immune regulation,and the proportion of nutrients,and osmotic pressure in human milk is especially suitable for the needs of gastrointestinal physiological of premature infants,which can effectively prevent the happening of the NEC.Now,the research progress on components in human milk and their mechanism for preventing necrotizing enterocolitis was reviewed.
2.Valproic acid inhibits the cardiomyocyte hypertrophy induced by angiotensin Ⅱ in rats

Objective To observe the inhibitory role of valproic acid (VPA) in hypertrophic cardiomycytes induced by angiotensin Ⅱ (Ang Ⅱ)in rats. Methods The cultured rat cardiomyocytes were divided randomly into control group, hypertrophic group and VPA group. They were treated with Ang Ⅱ to induce hypertrophy and then given with VPA. The morphologic changes of cardiomyocytes were observed under the contrast phase microscope and electron microscope. The mRNA levels ofβ-MHC were exmained by reverse transcriptase polymerase chain reaction (RT-PCR) method. The protein expression of C-FOS was exmanined by immunohistochemistry method. Results Stimulated by Ang Ⅱ, the cardiomyocytes were enlarged under the contrast phase microscope and the ultrastructure also changed. After stimulated by Ang Ⅱ, the mRNA level of β-MHC and the protein expression of C-FOS increased in the hypertrophic cardiocytes. Given by VPA, these data decreased accordingly. Conclusion VPA may inhibit the cardiomyocyte hypertrophy induced by angiotensin Ⅱ in some degree.

4.Comparison of retinopathy of prematurity incidences between single and multiple premature infants and analysis of related factors

Background Retinopathy of prematurity (ROP) is one of major causes of childhood blindness.Early screening and treatment of ROP is necessary.Objective This study was to analyze the incidence of ROP in single and multiple premature infants and related factors.Methods This study was approved by Ethic Committee of Suzhou Municipal Hospital,and oral informed consent was obtained from the parents of the infants before any medical examinations.Five hundred and four fetus with the gestational age of ≤34 weeks or birth weight of ≤ 2 000 g were collected in Suzhou Municipal Hospital,of whom 357 infants were single fetus birth and 147 infants were multiple fetus,including 26 twins and 21 three fetus birth.The fundus were examined by RetCam Ⅱ equipment and ROP was determined based on the fundus findings and history of birth.ROP incidences between the single fetus and multiple fetus were compared,and the inducing factors were analyzed.Results Sixty-six infants were ROP in all 504 fetus,with the incidence of 13.10%.The incidence of ROP in the single fetal group was 10.92% (39/357),which was significantly higher than 18.37% (27/147) in the multifetal group (x2=5.069,P=0.024),and risk of ROP in the multifetal group was higher than that in the single fetal group (OR =1.835).The oxygen inhalation rate was 42.02% (150/357) and the oxygen inhalation duration was 5.71 days in the single fetal group,and that in the multifetal group was 58.50% (86/147) and 9.42 days,respectively.Conclusions Incidence and risk of ROP in the multifetal infants are higher than those in the single fetal infants,and low-birth-weight,less gestational weeks and excess oxygen inhalation are influence factors.
5.Rapid identification and drug susceptibility assay of mycobacterin in HIV patients

Objective To establish the rapid pathogen identification method for HIV and Mycobac-terium tuberculosis (Mtb)co-infection and the assay for the drug susceptibility. Methods Geneprobe and 16S rDNA sequencing were used to differentiate mycobacterium species and modified microscopic observation drug susceptibility (MODS) was used for the drug susceptibility test. The above assays were compared with acid-fast smear, L-J culture and proportional drug susceptibility tests. Results (1) Thirty-four mycobacte-rial isolates were obtained from 112 samples collected from 68 HIV patients. Among these isolates, the strain species were determined by Geneprobe and 16S rDNA sequencing as the followings: 21 Mtb complex, 10 NTM including 5 M.avium complex, 2 M.gordonae, 2 M.kansasii, 1 M.colombiense, and 3 co-infection. (2) The sensitivity of Mtb to rifampicin, ethambutol, isoniazid and streptomycin were 100%, 100%, 76.2%, 90.5% respectively, while the sensitivity of NTM to rifampicin, ethambutol, isoniazid and strepto-mycin were 40%, 60%, 0%, 30% respectively. There is no significant statistic difference between the two methods, MODS and the reference standard, for the drug susceptibility test. (3) Six to eight weeks are nee-ded for the identification of the species of mycobacteria and the drug susceptibility test by using traditional method. In this study, 5-14 d, 6-15 d and 10-14 d are needed for Geneprobe, 16S rDNA sequencing, and MODS respectively. The time for the testing has been dramatically shortened. Conclusion The identifica-tion of mycobacterial species and the drug susceptibility test using clinical samples could be completed within 15 days by using combined Geneprobe, 16S rDNA sequencing and modified MODS. This combined method can be used for the pathogen identification and drug resistant test in HIV patients who are co-infected by my-cobacteria.
6.MULTIPLE ENDOCRME NEOPLASIA-AN ANALYSIS OF 68 CASES

Objective To improve diagnosis effectiveness for patients with multiple endocrine neoplasia(MEN).Methods 68 patients with MEN were reviewed in this paper.The diagnosis,clnical features and principle of treatment were discussed.Results (1)MEN-Ⅰ 22.05%,MEN-Ⅱ 36.86%,MEN-Ⅲ 8.82% MEN-Ⅰ\,Ⅱ 33.72%.(2)The cases in female were more than male and ten years earlier than male.(3)Endocrinophathies involred in order were thyroid,adrenal medulla,parathroid and islet.Conclusion It is important that more than one endocrinopathy be examined in doubtful cases.
7.Rapid molecular assay and strain typing methods of extrapulmonary tuberculosis

[Abstract] Objective To evaluate the effect of Mycobacterium tuberculosis Direct Assay (MTD) for rapid detecting Mycobacterium tuberculosis rRNA and Multi-locus PCR for M.bovis BCG strain typing in patients with suspected extra-pulmonary tuberculosis.Methods From June 2010 to December 2011,47 children and 75 adult patients with suspected extra-pulmonary tuberculosis in Shanghai public health clinical center were recruited.Also 48 non-tuberculosis patients were taken as a negative control.Clinical specimens from these patients were collected.Acid fast stain,solid culture,liquid culture,and MTD were used to detect all clinical specimens simultaneously.Screen tuberculosis strains of the culture isolates by MPT64 antigen assay and use Multi-locus PCR for the BCG strain genotyping of the isolates without MPT64 antigen.SPSS16.0 was used to analyse the results.Results The sensitivity for acid fast stain,solid culture,liquid culture and MTD test was 10.7% (13/122),11.5% (14/122),16.4% (20/122) and 37.7% (46/122),respectively.And the specificity of MTD was 100.0%.Six clinical isolates from children were identified as BCG by Multi-locus PCR typing,the same with chemical tests.Conclusions The MTD assay and the MGIT960 liquid culture are effective and reliable method for diagnosing extra-pulmonary tuberculosis.And Multi-locus PCR can be assisted for the early diagnosis of extra-pulmonary tuberculosis patients with suspected BCG infection.
8.Effect of ginsenoside Rg_2 on cardiac hemodynamics of anesthetized open chest dog

AIM: To study the effect of ginsenoside Rg 2 on cardiac hemodynamics in dog. METHODS: The parameters of cardiac hemodynamics were determined by using anesthetized open chest dog. RESULTS: In dogs treated with ginsenoside Rg 2 in a dose of 0.5、 1.0、2.0mg?kg -1 respectively, the heart rate (HR) slowed, the diastolic blood pressure (DBP)、 left ventricular systolic pressure (LVSP) and maximum decreasing rate ( dp/dtmax) increased; the cardiac output (CO)、 cardiac index (CI) and stroke index (SI) decreased; The coronary vascular resistance (CVR)、 the renal vascular resistance (RVR) and total periphery resistance (TPR) increased. CONCLUSION: Effect of ginsenoside Rg 2 on cardiac hemodynamics in dogs is similar to strophanthin K(SK), and can support the blood circulation of important organs.

AIM: To observe the changes of serum NO、ET、IL-6 of chronic renal failure (CRF) patients with blood stasis symptom and the relation between renal function and the change of traditional Chinese medicine(TCM) symptom,and the curative efficacy of Lei's Danshen Tablets (LDT). METHODS: At first,The levels of serum NO、ET、IL-6 and renal function of 60 non-dialysis patients with CRF of blood stasis symptom were mensurated, meanwhile the syndrome points of TCM were observed.The relativity was analysed after lineal relation of among these points were set up.Secondly,sixty cases were randomly divided into two groups,30 cases in the control group were given conventional treatment,30 cases in the treatment group were given Lei's Danshen Tablets in addition to the conventional treatment.The levels of serum NO、ET、IL-6 and renal function were observed after therapy.
