1.A follow-up study of left ventricular function in ischemic mitral regurgitation patients after percutaneous ;coronary intervention

Objective To analyze the relation between ischemic mitral regurgitation (IMR)and the changes of left ventricular systolic synchrony and volume parameters before and after percutaneous coronary intervention (PCI)in patients with subacute myocardial infarction and to evaluate the impact of PCI on IMR by measuring such changes with three-dimensional echocardiography volume quantitation technology. Methods Eighty-seven patients were enrolled and echo data were acquired before and 6 months after PCI. Patients were classified into alleviation group (group Ⅰ)and non-alleviation group (group Ⅱ)according to the change of mitral regurgitation volume (MRvol).The systolic synchrony indexes (Tmsv 1 6-SD%,Tmsv 1 6-Dif%)and left ventricle volume parameters (LVEDVI and LVESVI)before and after PCI for patients of two groups were measured and contrasted.Results Systolic synchrony indexes of all patients of two groups were improved after PCI.Left ventricular volume parameters were decreased after PCI in group Ⅰ,and the remodeling was reduced.Correlation analysis of the parameters variations showed that,there was significant positive correlation between variations of left ventricular volume and MRvol in all patients and no significant correlation between variation of systolic synchrony index and MRvol.Conclusions PCI can improve left ventricular systolic dyssynchrony for submyocardial infarct patients.Left ventricular volume index that reflects the remodeling process is closely related to IMR.
2.Website information disclosure by 80 public hospitals in China and the analysis

Objective Studying information disclosure at China' s public hospitals and recommending on information disclosure policies.Methods Collecting and analyzing the websites of 80 public hospitals in five main Chinese cities (Beijing,Shanghai,etc.).Results Information disclosure by these hospitals is found to be identical,with a slight difference in information publicity.By percentage,the information disclosure of hospital profile,department navigation,specialists characteristics,hospital news,health education,and doctor-seeking guidance,is 100.0%,98.3%,95.0%,93.3%,90.0%and 88.3%respectively.The disclosure is found to be lack of general planning,objectivity,timeliness and sufficiency.ConclusionEstablishing the information disclosure system based on mandatory information disclosure,stipulating the content,approach and timing for public hospitals information disclosure.
3.Progress of diagnosis and treatment in acute undifferentiated leukemia

Acute undifferentiated leukemia (AUL) is a clinically rare subtype of acute leukemia (AL). AUL lacks the serial specific antigen expressions, and the blasts have no morphological features of myeloid differentiation. Due to the rarity of AUL in clinic, there is no unified understanding of clinical and biological characteristics. It′s difficult to diagnose and choose the optimal treatment for AUL patients. And the prognosis of patients with AUL is poorer compared with other types of AL. Thanks to the big development of biotechnology in recent years, the related researchers have a better understanding of this rare disease. This article reviews the progress of diagnosis and treatment in AUL.
4.The utility of intraoperative real-time three-dimensional transesophageal echocardiography in cardiac valvular diseases

Objective To evaluate the usefulness of real-time three-dimensional transesophageal echocardiography(RT-3D-TEE)in cardiac valvular diseases.Methods Preoperative and postoperative RT-3D-TEE studies were performed in 32 patients who had undertaken cardiac valve repair or valvular replacement using Philips iE33 with X7-2 probe,and quantitative analysed the mitral structure in cases of mitral valve prolapse by using online QLAB7.0 quantitative analysis software.Results RT-3D-TEE can demonstrate the anatomicsl structure clearly,evaluate the function of cardiac valves precisely,and reveal details of type,position and extent of lesions vividly in surgical view.RT-3D-TEE helped correct misdiagnose in 2 cases,revised surgical plans in 3 cases,and guided implementation of remedial surgery in 1 case.Conclusions RT-3D-TEE can present images lively,and it can provide adequate information for preoperative diagnosis,aid the formulation of surgical plan and evaluate surgical effect.
5.Application of transesophageal echocardiography on minimally invasive surgical closure of atrial and ventricular septal defects

Objective To evaluate the clinical value of transesophageal echocardiography (TEE) on minimally invasive surgical closure of heart septal defects. Methods Thirty-four patients with atrial septal defect (ASD) and 38 patients with ventricular septal defect(VSD) were selected by transthoracic echocardiography (TTE) prepared for minimally invasive surgical closure. TEE was performed to choose appropriate occluder and guide occluder release during the procedure. The immediate closure effect also evaluated by using TEE. A week follow-up was done by using TTE. Results Successful occlusion was in 32 patients with ASD and 29 patients with VSD. On 1 week follow-up,neither displacement for the occluders nor residual shunt except minimal residual shunt in 3 patients. The ventrieular remodeling was improved, the valvular regurgitation and pulmonary arterial pressure decreased. Conclusions TEE is important in minimally invasive surgical closure of atrial, ventricular septal defects, especially in choosing the candidate for the procedure and occluder, guiding occluder released and evaluating the procedure.
6.Analysis of the surgical treatment of the tumors in infratemporal fossa by mandibular swing approach

Objective:To analyse the effects of mandibular swing approach in the treatment of the tumors in infratemporal fossa. Methods:11 patients with tumors in infratemporal fossa treated by the surgical operation with mandibular swing approach. The mandi-ble cut was at the front of foramina mentale in 5 cases, at the front of mandible angle in 4 cases and at the middle of chin in 2 cases re-spectively. Results:According to the nature, location, size and the relationship of the tumors with peripheral nerve and blood vessels, flexible selection of the position of mandibular cut could fully expose and completely remove the infratemporal fossa tumor. Conclusion:Surgical treatment of the tumors in infratemporal fossa by mandibular swing approach is safe and effective.
7.Clinical study on intermaxillary traction screws used in internal fixation in the treatment of jaw fracture

Objective:To analyse the methord and effects of intermaxillary traction(IMT)screws used in the treatment of jaw frac-ture.Methods:In the treatment of 1 68 cases of jaw fracture IMT screws were used for the restoration of normal and stable occlusion. The screw number,position,traction effect and postoperative complications of the treatment were analysed.Results:705 screws were used in 168 patients,4 screws were used in each of 147 cased (88%),6 in 12(7%),5 in 9(5%).336(47.7%)screws were fixed between the roots of first and second premolar,292(41 .4%)between the roots of canine and first premolar,50 (7.1%)be-tween the roots of second premolar and first molar,27(3.8%)between the other tooth roots.Normal postoperative occlusion relation was achived by the use of screws in 92 cases without traction.Occlusion disharmony or deviation was found in 76 cases by the used of screws and was restored to normlal by 1 4-day traction in 71 cases.The total efficiency of the treatment was 97%.Pain of the mucosa surrounding the screws was observed in 23 cases (1 3.7%).Root damage,traction screw loosening and adjacent tooth dislocation were observed in 1 3(1 .8%),1 1 (1 .6%)and 2(0.3%)cases respectively.Conclusion:For the fixation of IMT screws,the posi-tions between tooth root from canine to first molar were most common and safe.The fixation point should be in 5 ~8mm below the gin-gival margin.Use of 4-6 screws can meet treatment need.
8.Surgical treatment of trigeminal neuralgia of the third division by nerve avulsion inside and outside the mandibular cannel

Objective:To study the effects of the surgical treatment of nerve avulsion inside and outside the mandibular cannel.Methods:34 patients with trigeminal neuralgia of the third division,received neurotomy and avulsion of inferior alveolar nerve in the mandibular cannel above the lingual or through the mental foramen,concurrently with the avulsion of buccal nerve.Patients were fol-lowed up for 2-4 years.Results:All the operations were successfully completed,The wounds were well-healed.All patients felt that pain disappeared in the area innervated by inferior alveolar nerve.During the 2-4 years of follow-up,no recurrence was found.Conclusion:Inferior alveolar nerve neurotomy and avulsion in the mandibular cannel is simple,safe and effective in the treatment of trigeminal neuralgia of the third division.
9.Correlation between plasma oxidized low density lipoprotein and vascular endothelial injury degree in patients with coronary heart disease

Objective: To explore correlation between plasma oxidized low density lipoprotein (ox-LDL) and endothelial injury degree in patients with coronary heart disease (CHD).Methods: A total of 80 CHD patients were enrolled as CHD group, and 21 healthy subjects were selected as healthy control group.Plasma ox-LDL, circulatory endothelial cell count (CEC), nitric oxide (NO) levels were measured and compared between two groups, and their correlation was analyzed.Results: Compared with healthy control group, there were significant rise in plasma ox-LDL level[(350±173) μg/L vs.(687±169) μg/L]and CEC[(2.5±0.5) ×106个/L vs.(9.0±1.7) ×106个/L], and significant reduction in NO level[(77.4±21.1) μmol/L vs.(52.3±16.6) μmol/L]in CHD group, P<0.05 or <0.01.Spearman correlation analysis indicated that ox-LDL was significant positively correlated with CEC (r=0.793, P=0.019), and ox-LDL, CEC were significant negatively correlated with NO level (r=-0.782,-0.952, P=0.013, 0.021).Conclusion:There are significant positively correlation between ox-LDL and circulatory endothelial cell count(Vascular endothelial injury degree) in CHD patients, therefore, its treatment should focus on anti-inflammatory and endothelial cell function protection.
10.Ultrasonic experimental research of systolic left ventricular transmural torsion in different electro-mechanical patterns: a canine model

Objective To evaluate the mechanical characteristics of systolic left ventricular(LV) transmural torsion in different LV electro-mechanical patterns using speckle tracking imaging. Methods Five open-chest canine models were employed for the acquirement of the basal, apical short-axis and four-chamber views of LV during baseline(BASE) and right atrial appendage(RAA), right ventricular apical (RVA), left ventricular lateral wall (LVL) and left ventrieular apical (LVA) pacing. Subendocardial (subend),subepicardial(subepi) and bulk rotation angle(RA) and segmental angle excursion(AE) at basal and apical level were analyzed using a dedicated workstation. LV torsion at different layers and bulk and global LV ejection fraction (EF) were calculated. Results ① There were no significant difference of transmural torsion and RA at basal and apical level between BASE and RAA pacing (P>0.05);② LV torsion of subend, subepi and bulk during RVA pacing were lower than those during RAA pacing(P0.05);LV torsion of subend and bulk during LVA pacing were lower than those during RAA pacing(P0.05);LV RA of subend,subepi and bulk at basal level during RVA and LVA pacing were lower than those during RAA pacing (P<0.05); ③ For normal electro-mechanical pattern, LV torsion of subend were significant higher than that of subepi(P<0.05), there only were a higher tendency for all pacing models (P>0.05); ④AE of segments near the pacing site decreased during different ventricle paeings (P<0.05); ⑤At BASE and during RAA pacing, LV bulk and subepi torsion were positively correlated to EF; RA of subend,subepi and bulk at basal level were positively correlated to EF. Conclusions LV transmual torsion are significantly depressed during RVA and LVA pacing. There is a spatial co-relationship between LV EF and torsion and rotation of bulk and subepi at basal level in normal LV electro-mechanical patterns.
