1.Analysis of clinical characteristics and prognostic factors in 92 cases of endometrial cancers

Objective To study the dependablity of clinical characteristics and prognosis in patients with endometrial cancer,so as to apply the basis for the treatment. Methods The clinical and pathological data of 92 patients with endometrial cancer were analyzed retrospectively, 5-year survival rate was followed and observed. Using Cox proportional hazard regression analysis to evaluate the factors related to overall survival. Results The overall 5-year survival rate was 72.41%. Using univariate analysis, the survival time was correlated with histologic type, pathological staging, histologic grade, myometrium invasion,cervical invasion and lymph node metastasis (P<0.05).Using multivariate analysis, the survival time was correlated with histologic type,histologic grade and myometrium invasion. Conclusions Histologic type, histologic grade and myometrium invasion are independent prognostic factors for survival time of patients. In order to improve the prognosis of the patients, they should be treated by individual therapies.
2.Clinical analysis of magnesium sulfate combined with nifedipine in treatment of pregnancyinduced hypertension

ObjectiveTo investigate the clinical eflicacy of magnesium sulfate combined with nifedipine in treatment of pregnancy-induced hypertension.MethodsOne hundred and fifty pregnancyinduced hypertension patients were divided into two groups by random digits table with 75 cases each.The observation group was treated with magnesium sulfate combined with nifedipine.The control group was treated with magnesium sulfate.Then the blood pressure before and after treatment and the pregnancy outcome were observed.ResultsThe systolic blood pressure,diastolic blood pressure after treatment in observation group were lower than those in control group [ (133.2 ± 7.9) mm Hg (1 mm Hg =0.133 kPa) vs.(142.5 ± 8.0)mm Hg,(80.6 ± 6.1 ) mm Hg vs. (86.6 ± 6.1 )mm Hg],and there were significant differences between two groups (P < 0.05).The spontaneous labor rate in observation group was higher than that in control group [ 65.3% (49/75) vs.41.3% (31/75 )],and there was significant difference between two groups (P < 0.05 ).The incidence of placental abruption,hypoproteinemia,postpartum hemorrhage,fetal growth retardation,fetal distress,premature delivery,asphyxia neonatorum,perinatal mortality in observation group were lower than those in control group [ 1.3%(1/75) vs.12.0%(9/75),13.3%(10/75) vs.33.3%(25/75),8.0%(6/75)vs.16.0%(12/75),2.7%(2/75) vs.9.3%(7/75),2.7%(2/75) vs.10.7%(8/75),2.7%(2/75) vs.17.3%(13/75),1.3% (1/75) vs.5.3% (4/75),0 vs.5.3% (4/75)],and there were significant differences between two groups(P <0.05). Conclusion Magnesium sulfate combined with nifedipine in treatment of pregnancy-induced hypertension can effectively reduce the adverse pregnancy outcome,and reduce the asphyxia neonatorum rate and caesarean section rate.
3.Clinical analysis of methotrexate combined with mifepristone in the treatment of unruptured ectopic pregnancy

ObjectiveTo observe the clinical effect of methotrexate combined with mifepristone in the treatment of unruptured ectopic pregnancy.Methods96 cases of unruptured ectopic pregnancy patients were randomly classified into two groups,observation group with 48 cases was treated with methotrexate combined with mifepristone and control group with 48 cases was only treated with methotrexate.The treatment effect and adverse reaction between two groups were compared.ResultsThe time of blood β-HCG recovery to normal and hospitalization time in observation group were significantly shorter than those in control group( P < 0.05 ),and mass diameter was significantly smaller than that in control group( P < 0.05 ).The adverse reaction incidence in observation group was 10.4% ( 5/48 )while 6.3% (3/48) in control group.ConclusionMethotrexate combined with mifepristone in the treatment of unruptured ectopic pregnancy could enhance clinical effect,shorten treatment time,decrease adverse reaction,which could be applied in clinic.
4.Establishment and evaluation of employee satisfaction indicator system for thirteen general hospitals

Objective Establishing an employee satisfaction indicator system for tertiary hospitals in Beijing.Methods Evaluation indicators are built on theory research,and questionnaires are designed for survey.Such methods as principal component factor analysis,and verification factor analysis are called into play to test the construct validity and establish in the end the employee satisfaction indicators system.Results Such indicators comprise five grade-1 dimension and 34 grade-Ⅱ dimension,such as hospital management satisfaction indicator.Conclusion This self-designed scale ideally mirrors the employee satisfaction of tertiary hospitals in Beijing,proving the indicators to be objective and reasonable.
5.Quality Evaluation of Levofloxacin Tablets Produced by Six Different Pharmaceutical Factories

OBJECTIVE:To evaluate the quality of levofloxacin tablets produced by 6 different factories.METHODS:To investigate the quality of 17 batches of levofloxacin tablets produced by 6 factories according to related standards and to evaluate the drug dissolubilities using paddle method.RESULTS:17 products from six factories were all proved qualified.Significant differences of dissolution parameters and treatment costs were found among the different products.CONCLUSION:There are difference in quality of levofloxacin tablets.The tablets produced by the factory holding the patent right are more expensive,however the qualiity is more stable.
6.Correlation between stroke volume variation and blood volume during hypovolemia

Objective To investigate the correlation between stroke volume variation (SVV) and blood volume during hypovolemia.Methods Twenty ASA Ⅰ or Ⅱ patients,aged 20-64 years,with body mass index (BMI) of 20-30 kg/m2,scheduled for elective orthopedic operation were enrolled in this study.Anesthesia was induced with dexamethasone,midazolam,propofol,fentanyl and cisatracurium,and maintained with sevoflurane,fentanyl and cisatracurium.Then the patients received endotracheal intubation and mechanical ventilation.Heart rate (HR),mean arterial blood pressure (MAP),central venous pressure (CVP),arterial pressure-based cardiac output (APCO),SW,systemic vascular resistance (SVR) and cardiac index (Cl) were recorded 5 minutes after endotracheal intubation.Blood was taken from the central vein at a rate of 30-50 ml/min and the volume was 5% of the whole blood volume,and then haemodynamic parameters mentioned above were recorded after the haemodynamics were kept stable for 5 minutes.Blood was taken again with the method mentioned above and the haemodynamic parameters were recorded.Then 6% hydroxyethyl starch (HES) 130/0.4 was infused at 50-70 ml/min via the right internal jugular vein,and the volume was equal to 5% of the whole blood volume,and then haemodynamic parameters were recorded after the haemodynamics was kept stable for 5 minutes.Fluid replacement was performed again using the method mentioned above and the haemodynamic parameters were recorded.Linear correlation of the changes in blood volume (difference between the blood volume at each time point and the baseline value) with dSVV (difference between the value monitored at each time point and the baseline value) was analyzed.Results Significant changes were found in SW,APCO and Cl after each change in blood volume (P < 0.05 or 0.01),while no significant changes were found in HR,MAP,CVP and SVR after each change in blood volume.The change in blood volume was negatively correlated with dSVV (r =-0.875,P < 0.01).Conclusion There is high correlation between SVV and blood volume during hypovolemia.And SVV can reflect the changes in blood volume accurately and can be used for volume therapy during hypovolemia.

8.Expression and immunization assessment of HSV-2 glycoprotein D in baculovirus expression vector system

Objective:To express the glycoprotein D of herpes simplex virus type 2 (gD2) in the insect cells,and to determine its immunogenicity.Methods:HSV-2 genome was used as the template for amplification of gD2 extracellular domain fragment gene by PCR.The PCR product was inserted into the vector Bacmind,and the constructed recombinant plasmid gD2-Bacmind was transfected into the sf9 cells to package the recombinant baculovirus.The Sf9 cells were infected by recombinant baculovirus seed derived from the forth passage(P4),the titer of P4 recombinant baculovirus was detected by a plaque assay and the expression of recombinant protein gD2 was determined by Western blotting method.The supernatant of infected cells was collected and purified by Ni-NTA affinity chromatography to obtain the target protein gD2,the purified gD2 protein was used to immunize the BALB/c mice in 0, 2, 4 weeks (gD2 group),and PBS was used as negative control(PBS group);the titers of gD2 specific IgG in serum were detected by ELISA assay.Results: The PCR analysis and sequencing results proved that gD2-Bacmind was constructed correctly.The titer of recombinant baculovirus was 2.0×109 pfu·mL-1,the purified gD2 was about 37 000 with expectation,the percentage of gD2 in total protein was 90%.The average value of Log10 of titer of gD2 specific IgG in serum detected by ELISA assay in gD2 group at the sixth week was 4.34,and there was significant difference compared with PBS group(P<0.01).Conclusion: The gD2 expressed by insect-baculovirus expression vector system has the immunogenicity and can be selected as candidate protein for HSV-2 vaccine.
9.Effects of abdominal breathing exercises at different periods on the gastrointestinal symptoms in patients with gastroesophageal reflux disease

Objective:To explore the effect of abdominal breathing exercises at different periods on the gastrointestinal symptoms, quality of life and proton pump inhibitor dependency in patients with gastroesophageal reflux disease (GERD). To provide reference for patients to choose the best time for abdominal breathing exercises.Methods:This was a prospective study. From March 2020 to December 2021, totally 108 GERD patients were collected in digestive outpatient clinic of Yibin Hospital of Chinese Medicine Hospital by convenient sampling method, they were randomly divided into pre meal group, 1 h postprandial group and 2 h postprandial group with 36 cases in each group. All patients in the three groups were given abdominal breathing training for 8 weeks on the basis of conventional acid suppression drug treatment and nursing. The training time of pre meal group, 1 h postprandial group and 2 h postprandial group was at 30 min before meal, 1 h after meal, 2 h after meal, respectively. Before and after 8 weeks of intervention, the difference of gastrointestinal symptoms and quality of life were assessed by Reflux Disease Ouestionnaire (RDQ) and the MOS 36 Item Short Form Health Survey (SF-36). Patients followed up for 12 weeks after drug withdrawal, the medication of proton pump inhibitor (PPI) between three groups were compared.Results:There was no significant difference in RDQ score and SF-36 score among the three groups before intervention ( P>0.05). After intervention, the symptom scores were (7.89 ± 1.86) in the 1 h postprandial group, lower than in the pre meal group (10.38 ± 1.81) and in the 2 h postprandial group (9.64 ± 1.65), the difference was statistically significant ( t = 5.83, 4.06, both P<0.01). The scores of each demensions in SF-36 were higher in the 1 h postprandial group compared to the pre meal group and the 2 h postprandial group, the differences were statistically significant ( t values were 2.04-3.70, all P<0.05). After followed up for 12 weeks, the PPI discontinuation rate was 71.43% in the 1 h postprandial group, higher than in the pre meal group 44.12% and in the 2 h postprandial group 45.45%, the differences were statistically significant ( χ2 = 5.28, 4.73, both P<0.05). Conclusions:Abdominal breathing exercises at 1 hour after meal can effectively alleviate gastrointestinal symptoms, promote quality of life and decrease the proton pump inhibitor dependency of GERD patients.
10.Optimization of streptozotocin dosing for establishing tumor-bearing diabetic mouse models.

OBJECTIVETo determine the optimal dosing of streptozotocin (STZ) for establishing lymphoma-bearing diabetic mouse models.
METHODSA total of 200 healthy male Balb/c mice were randomized into 4 groups (n=50) for intraperitoneal injection of a single dose of vehicle solution (control) or 75, 150, or 200 mg/kg STZ. The changes of body weight and blood glucose were observed regularly, and the success rate of modeling, mortality rate, and survival of the mice were recorded after the injections. The mice with successfully induced diabetes received subcutaneous or tail vein injection of A20 lymphoma cells, and the rate of tumorigenesis, mortality rate, and survival time were observed at 1 month and 3 months after tumor cell injection.
RESULTSCompared with the control group, the mice receiving STZ injection at 150 and 200 mg/kg showed significantly decreased body weight and increased blood glucose (P<0.05), while STZ at 75 mg/kg did not produced such obvious changes. STZ injection at 200 mg/kg resulted in a significantly higher mortality rate and shorter survival time than STZ at 150 mg/kg (P<0.05). In the control group and 150 and 200 mg/kg STZ groups, the rate of tumorigenesis or mortality rate showed no significant differences after subcutaneous injection of A20 lymphoma cells (P>0.05), but differed significantly at 3 months after tail vein injection of the tumor cells (P<0.05).
CONCLUSIONIntraperitoneal injection of STZ at 150 mg/kg is associated with a low mortality rate, a high successful modeling rate of diabetes and a long survival time in mice, and is therefore optimal for establishing diabetic mouse models bearing transplanted tumors.
