1.A Study of Clinical Feasibility of Acceptable Noise Level Test

Objective To observe the clinical feasibility of the acceptable noise level (ANL) test ,analyze the effect of different test instructions to the result of Mandarin acceptable noise level test .Methods Twelve young peo-ple (24 ears) with normal pure-tone test and acoustic immittance test (threshold≤15 dB HL) were included in this study .They were randomly divided into two groups with 12 ears in each group .Group A received the same test in-struction firstly and then received different instructions depended on the different translation of English instruction by tester .Group B received the reverse test order .The acceptable noise level(ANL) ,most comfortable level(MCL) and back groud noise level(BNL) were analyzed .Results The average value of ANL was 7 .5 ± 6 .61 and 8 .29 ± 6 .54 dB SPL ;the average value of most comfortable level (MCL) was 83 .58 ± 8 .57 and 85 .41 ± 8 .89 dB SPL ;the average value of background noise level (BNL) were 78 .92 ± 10 .56 and 77 .13 ± 7 .91 dB SPL respectivoly .The difference of MCL ,BNL ,ANL in these two groups were not statistically significant (P>0 .05) .Test method 1 re-sulted in great individual differences in ANL ,which it ranged from -6 .4 to 20 .12 dB S/N .While ANL from test method 2 ranged from 2 .50~20 .12 dB S/N .Conclusion Different test instruction do not correlated to the results of acceptable noise level ,testers can translate the English instruction by different ways so that it is convenient to the application and clinical research of ANL test .
2.Protective effects of EPA and DHA on lipopolysaccharide-stimulated rat mesangial cells

AIM: To investigate the effects of EPA and DHA on oxidative stress of lipopolysaccharide-stimulated rat mesangial cells. METHODS: The glomerular mesangial cells (GMCs) were stimulated by lipopolysaccharide (LPS) and incubated with EPA (10 μmol/L or 100 μmol/L) and DHA (10 μmol/L or 100 μmol/L) for 24 h, 48 h and 72 h. The activity of SOD, GSH-Px and the level of MDA was measured. The protein and mRNA expressions of MCP-1 and TGF-β_1 were detected by immunocytochemistry and real-time PCR method, respectively. RESULTS: The activities of SOD and GSH-Px were decreased and the concentration of MDA was increased when stimulated with LPS. EPA and DHA increased the activities of SOD and GSH-Px and decreased the concentration of MDA significantly. Meanwhile, the protein and mRNA expressions of MCP-1 and TGF-β_1 stimulated by LPS were decreased. DHA was more effective than EPA at the same concentration. CONCLUSION: EPA and DHA enhance the activities of antioxidant enzymes, decrease the concentration of MDA and inhibit the expression of TGF-β_1 and MCP-1, suggesting that the protective effect of EPA and DHA on kidney is related to the antioxidation and the inhibition of TGF-β_1 and MCP-1 expression.

4.Clinical monitoring of myocardial injury in neonates with intrauterine distress

Objective To investigate whether no asphyxia neonates with intrauterine distress are complicated with myocardial injury and determine the sensitive biochemical diagnostic parameters. Methods A total of 89 neonates born in the First Affiliated Hospital of Sun Yat-sen University from July 2009 to December 2009 were enrolled. Fifty-three fetal distress cases with Apgar score > 7 at 1 and 5 minites were enrolled in the study group; while the rest 36 healthy neonates, whose Apgar score = 10 at 1 and 5 minites, were the control group. Umbilical artery blood samples of all cases were collected for blood gas analysis and biochemical measurement. Results(1)pH(7.23±0.07) and BE [(-4.8±3.0)mmol/L] in the study group were significantly lower than pH (7.31 ±0.03) and BE [(-2.1±1.5)mmol/L] in the control group (P<0.05).The lactic acid of study group [(5.2±2.3)mmol/L] was higher than that of the control group [(2.3±1.1)mmol/L], and the difference was significant (P<0.01). However, there was no significant difference between the two groups in PaO2[(16.2±7.9)mm Hg(1 mm Hg=0.133 kPa) vs. (17.5±6.7)mm Hg] and PaCO2[(54.0±11.2)mm Hg vs. (48.5±5.4) mm Hg; P>0. 05]. (2) The level of CK-MB in neonates with fetal distress[(48 ±59) U/L] was significantly higher than that of healthy neonates [(36±27)U/L]. However, no significant difference was found in CK [(194±73)U/L vs. (162±95) U/L]and BNP levels[(519±309)ng/L vs.(481±216)ng/L;P > 0.05]. (3) Spearman rank correlation analysis showed that CK-MB level was negatively correlated with pH(r=-0.296, P<0.05) and BE (r=-0.318,P<0.05) of umbilical artery blood,while BNP level was positively correlated with umbilical lactic acid (r=0.278, P<0.05). No correlation was found between other parameters (P>0.05).Conclusions Intrauterine distress without neonatal asphyxia had effect on fetal myocardial injury. CK-MB can be used as a sensitive parameter for monitoring the development of myocardial injury. The severity of myocardial injury was related to fetal acidosis.
5.Relationship between the personality and occupation tension of anesthesiologists

Objective To analyse the relationship between the personality and occupation tension of anesthesiologists.Methods One hundred anesthesiologists working at different hospitals in Shenzhen volunteered to complete the questionnaire.Ninety-one valid questionaires were recovered.The questionnaire survey included the Eysenck Personality Questionnaire and Job Content Questionnaire and anesthesia events.The standard scores of internal and external factor,stubborn factor,emotional stability factor,social support,sense of personal achievement and work demand/control ratio (D/C) were calculated.Results Internal and external factor (E) and stubborn factor (P) were negatively correlated with demand/control ratio (D/C).The correlation coefficients were-0.7805and-0.2375.There was no correlation between emotional stability factor (N) and D/C.Social support and sense of personal achievement were negatively correlated with D/C.The correlation coefficients were-0.225 and -0.264.More than 50% of the study subjects suffered from mental tension during op.eration and fatique after night shift,overtime work and emergency operation.Conclusion Occupation tension is lower in extroverted and higher in introverted anesthesiologists and is also relatively low in those with stubborn personality.Strong social support and deep sense of personal achievement can lower occupation tension.
6.Expression of miR-218 in cervical cancer tissues and bioinformatics analysis of predicted target genes

Objective To investigate the expression of miR-218 in cervical cancer tissues and bioinformatically analyze the target genes of miR-218 to provide theoretical basis on further studies of miR-218 functions in cervical cancer.Methods miRNA array was applied to detect miRNA expression profile in cervical cancer tissues,and real-time RT-PCR was used for validation.The bioinformatic analysis of the target genes of miR-218 involved gene ontology and signal transduction pathway enrichment was performed.Results miR-218 expression significantly decreased in cervical cancer tissues compared with that of normal cervical tissues.The functions of predicted target genes of miR-218 were enriched in biological regulation,adhesion and motion,proteometabolism,cellular differentiation,protein binding and other biological processes and molecular functions.According KEGG pathway database,the predicted target genes involved in pathway in cancer,adherent junction,focal adhesion,prostate cancer,chronic myeloid leukemia,melanoma,and other signal transduction pathways.Conclusions miR-218 expression significantly decreases in cervical cancer tissues.Some of the predicted target genes of miR-218 are significantly enriched in tumor related with signaling pathways.
7.Investigation of blood transfusion in elderly patients in third-level general hospitals

Objective To investigate the blood transfusion paths in elderly patients in order to ensure the blood transfusion safety in the elderly.Methods Clinical data of elderly patients (aged 60 and over) receiving blood transfusion were selected from January 2008 to December 2013.Data of blood transfusion in different clinical department were analyzed.Results From 2008 to 2013,the number of elderly patients receiving blood transfusion and transfusion volume showed the rising trends.Patients in Department of Cardiothoracic Surgery had the most blood transfusion volume among all surgical departments (9.3U/person),and plasma transfusion accounted for the most with plasma and red blood cells in a 3 ∶ 1 ratio.Patients in Department of Internal Medicine had the most blood transfusion volume among all non surgical departments (15.9U/person),and platelets transfusion accounted for the most with platelets and red blood cells in a 3 ∶ 1 ratio.The oldest patients on average receiving blood transfusion in the top 10 was from Department of Emergency (aged 77 years),and the most common cause was gastrointestinal hemorrhage.The youngest patients receiving blood transfusion in the top 10 was from Department of Cardiothoracic Surgery and Department of Gynecology (aged 69 years),and the most common cause was huge pleural mass and ovarian cancer.Conclusions Blood transfusion for elderly patients should be restrict the indications for transfusion strictly,choose the reasonable transfusion time,blood components and volume.Physiological and functional changes and clinical features of elderly patients should be paid attention to at the same time in order to reduce unnecessary transfusions,particularly the dependence on plasma transfusion and unnecessary collocated blood transfusion,to avoid the overload and adverse transfusion reactions.
8.Preparation of Micronized Shuanghuangpo Hydrogel Patch and Its Transdermal Penetration In Vitro

Objective To study preparation of micronized Shuanghuangpo hydrogel patch and its characteristics of transdermal penetration in vitro. Methods Micronized Shuanghuangpo hydrogel patch was prepared with some macromolecular water-soluble materials as gel base.The content of berberine was determined by HPLC method.Its transdermal penetration in vitro was determined according to the method of Chinese Pharmacopoeia 2010 edition. The rat skin penetration test in vitro was performed by modified Franz diffusion cell method. Results The hydrogel patch had constant content of berberine. Its release property in vitro conformed to Higuchi equation. The penetration of berberine in the hydrogel patch through the rat skin followed zero-order dynamics in 12 h.Its release rate was 7.934μg??(cm2)-1??h1/2 and percutaneous rate was 0.571μg??(cm2)-1??h-1. Conclusion The micronized Shuanghuangpo hydrogel patch is a new transdermal agent with sustained release property.
9.Anti-proliferation effects of dammar-24-ene-3β-acetate-20S-ol on gastric cancer cells

Objective Based on the previous research that the ethanolic extract from traditional Chinese medicine fructus forsythiae (Lianqiao) can obviously inhibit cancer cells in vitro, the article aimed to investigate the anti-proliferation effects of dammar-24-ene-3β-acetate-20S-ol (DM) extracted from fructus forsythiae on gastric cancer cells and its mechanism.Methods MTT assay was used to assess the anti-proliferation effects of DM on gastric cancer cells including SGC-7901, BGC-823, and MKN-45 in vitro.There were MKN-45 control group and its low dose and high dose groups, BGC-823 control group and its low dose and high dose groups, SGC-7901 control group and its low dose and high dose groups in the experiment.Flow cytometry was used to analyze the cell apoptosis rate.Cellquest software was used to analyze the results and record the ratio of cells at different cycles.DCFH-DA probe was applied to detect the ROS levels of blank control group, docetaxol group and DM group.The reaction system of microtubule assembly test was set with 10?mol/L docetaxol, 50 or 100 μmol/L DM final density and no medicine in blank control group.The readings of UV spectrophotometer were recorded.Microtubule assembly assay and microtubule immunofluorescence staining were applied to investigate the effects of DM on microtubule system.Results The inhibition ratio of 50 μg/L DM on the proliferation three gastric cell lines were all above 80%, with IC50s of MKN-45 11.72±1.35 μg/mL, BGC-823 17.19±0.82 μg/mL, SGC-7901 7.55±0.79 μg/mL.8 days′ low density culturing at 48 hours after 2 μg/mL DM treatment, compared with control group, the number of cell clones significantly reduced without much change in clone size, while 48 hours after 10 μg/mL DM treatment, besides a few clones of BGC-823, there were just several megascopic clones of SGC-7901 and MKN-45.In comparison with apoptotic cell ratio in MKN-45 control group[(21.1±2.5)%], its low dose group and high dose group resulted in significant rise of apoptotic cell ratio[(25.1±1.3)% and (55.2±2.3)%] (P<0.01).Compared with apoptotic cell ratio in BGC-823 control group[(13.2±2.5)%], its low dose group and high dose group resulted in significant rise of apoptotic cell ratio[(18.2±2.1)% and (41.8±2.1)%] (P<0.01).Compared with apoptotic cell ratio in SGC-7901 control group[(10.5±1.8)%], significant rise was observed in its high dose group[(41.8±2.1)%] (P<0.05) while no statistical significance was found in its low dose group[(12.3±1.6)%] (P>0.05).In comparison with MKN-45 control group, the ratio of cells at S phase decreased in its low dose group[(14.5±2.7)% vs (12.3±3.3)%,P>0.05].In comparison with BGC-823 control group, the ratio of cells at S phase increased in its low dose group[(12.2±5.4)% vs (20.2±2.1)%,P<0.05].In comparison with SGC-7901 control group, the ratio of cells at S phase increased in its low dose group[(21.5±3.8)% vs (31.3±2.6)%,P<0.05].From the detection of intracellular active oxygen after DM treatment, dose-dependent ROS level increased in all three cell lines 48 hours after 10μg/mL and 50μg/mL DM treatment.From the results of microtubule immunofluorescence staining, 48 hours after the treatment of IC50 docetaxol and 10μg/mL DM, the fluorescence signals were in local concentration and disorder.Conclusion Dammar-24-ene-3β-acetate-20S-ol demonstrated anti-proliferation effects due to the apoptosis induced by cell cycle arrest at S phase.
10.Comparative analysis of risk degree of bleeding in patients with different kinds of hematonosis

Objective To retrospectively analyze the platelet count and related factors in bleeding patients with hematonosis,and to calculate the risk of bleeding when the platelet count is at each exposure level.Methods Retrospective analysis of patients from Department of Hematology Inpatients in Nanjing Drum Tower Hospital,Nanjing First Hospital and Nanjing Jiangning Hospital from July 2013 to June 2017 was collected.And the risk of bleeding for different hematonosis was calculated.Results The tolerance of the 5 categories of hematonosis to low platelet counts is compared:AA and ITP can tolerate lower levels of platelet count;MDS and AML(except M3) are more prone to bleeding;ALL is the most susceptible to bleeding.Conclusion When platelet resources are scarce,priority should be given to ALL,MDS and AML patients,in order to ensure the safety of critically ill patients.For patients with AA and ITP,the platelet infusion threshold may be reduced appropriately,in oder to reduce the incidence of platelet transfusion refractoriness.
