1.Effect and Action Mechanism of Group Tutoring in Adaptation Education for College Freshmen

Objective:To explore the effect of group tutoring in the adaptation education for college freshmen.Method:All freshmen that attended Wenzhou medical college in 2008 are enrolled in group tutoring to help them better adapt to the new life in college,and the tutoring effect is appraised by a self-designed questionnaire.Result:The freshmen show a general progress and satisfaction after receiving group tutoring,and regard it quite necessary to conduct activities like group tutoring at the beginning of college life to help make freshmen better adaptive.Conclusion:Group tutoring is very effective in the adaptation education for college freshmen,and has a universal applicability for all college students.
2.Pedigree and clinical characteristics in a Chinese family with lattice corneal dystrophy

Background Lattice corneal dystrophy (LCD) is a progressive disease,whose clinical features are varied in different stages.It is rarely be reported that clinical findings of different stages and factors of promoting the occurrence and development on LCD in a family.Objective The aim of this study was to identify the characteristics of the pedigree and clinical features of different stages in a LCD family,and further to discuss its influence factors.Methods A cross-sectional study was performed in this study.A Chinese family with LCD was enrolled in Shenzhen Eye Hospital from 2015 to 2016.Questionnaires for disease-related history,visual acuity measurement,ocular anterior segment examination and color photography were carried out for all the members of the family.In addition,anterior segment OCT (AS-OCT),laser scanning confocal microscope and corneal endothelium microscope were used to observe the morphology of corneal stroma and changes of corneal endothelial cells.The pedigree chart was drawn by Cyrillic2.1 software and analyzed based on Mendel law.Results This family included 5 generations of 73 members.Patients with LCD were found in each generation with similar morbidity in different gender,which followed the law of autosomal dominant inheritance.Eleven patients were found in 49 members related with Ⅲ1 of this family with the prevalence rate of 22.45% and onset age at 21-50 years old,and the course of disease was 3-34 years.All of the members had no systemic disease except for two patients (Ⅲ 1 and Ⅲ 5) with hypertension.In the early stage of LCD,some bifurcate striolae appeared in the patients' corneal stroma without symptoms for many years.In the progressive stage,there was corneal irritation symptom accompanying with vision's decrease in the eyes with LCD.The bifurcate striolae were increased,widened and interwoven into lattice lines that the boundaries gradually became fuzzy,then corneal macula was formed because of recurrent corneal infiltration,and eventually resulted in corneal leucoma.High reflection corresponding to the pathologic region was showed by laser scanning confocal microscope and AS-OCT.No significant differences were found in corneal endothelial cell density and the percentage of hexagonal cells between LCD patients and normal phenotype families (t =1.887,P=0.075;t=-0.719,P =0.481).Penetrating keratoplasty was performed in a patient with corneal opacity and serious corneal opacity occurred near the surgical incision one year after the surgery.One patient was diagnosed as LCD 2 years after laser assisted in-situ keratomileusis.One patient was a welder.Conclusions LCD is autosomal dominant inheritance in the family.The clinical manifestations of LCD in the early,progressive and late stage can be seen in the pedigree,which offers a reference for ophthahnologists.Corneal surgery and lesion may induce the onset or aggravation of LCD.
3.Endoscopic retrograde cholangiopancreatography after Billroth Ⅱ gastrectomy

Objective To evaluate the success rate, safety and efficacy of endoscopic retrograde cholangiopancreatography (ERCP) after Billroth Ⅱ gastrectomy. Methods Data of 75 patients with biliary disease after Billroth Ⅱ gastrectomy, who underwent ERCP from January 2007 to November 2009, were retrospectively analyzed. Results In 75 patients, afferent loop intubation was achieved in 69 (92%) and selective cannulation of bile duct were successful in 68 (68/69, 98. 5%). Diagnostic procedures were carried out in 3 patients, and therapeutic in 65 others, which included EST plus stone removal and ENBD in 16, ERBD in 19, EMBE in 18 and EBD plus stone removal and ENBD in 12. Afferent loop perforation occurred in 1 patient (1.3%) and was treated surgically, and 2 acute pancreatitis (2. 6%) were treated conservatively.There was no complication of bleeding. Conclusion ERCP after Billroth Ⅱ gastrectomy is safe and efficiency for biliary disease.
4.Experimental study on effect of immune response mediated by ICOS signal in renal fibrosis of SHR

Objective:To preliminarily study the immune response mediated by inducible costimulation molecule in the role of essential hypertensive renal damage .Methods: The blood pressure of spontaneously hypertensive rats and the wild control group (Wistar-Kyoto rats)was dynamically monitored by noninvasive tail artery blood pressure measuring instrument .The urine protein of the rats in 24 hours was dynamically detected by ELISA .The levels of ICOS protein and its mRNA in the rat′s kidneys were dynamically detected by immunohistochemistry , RT-PCR, respectively.The levels of IL-17A and TGF-β1 in the rat′s plasma and kidneys were dynamically detected by ELISA ,immunohistochemistry ,respectively.The renal pathological changes of the rats were detected by HE and MASSON staining.Results:The blood pressure and urine protein in 24 h of SHRs were significantly higher than that of group WKY from 6 weeks.The expression of ICOS protein and its mRNA in SHRs were significantly higher than that of WKY rats from 6 weeks,and there are significantly positive correlations between the dynamic change of ICOS protein and its mRNA and renal fibrosis score of SHRs (rA=0.813,PA<0.05;rB=0.753,PB<0.05).The expression of IL-17A and TGF-β1 in SHR′s plasma and kidneys were significantly higher than that of WKY rats at the weeks 10 and 23.HE and Masson staining showed that the degree of renal fibrosis of SHRs was sig-nificantly higher than that of WKY rats at 23 weeks.Conclusion:The immune response mediated by ICOS plays an important role in hypertensive renal damage .
5.Pathological Characteristics of Multiple Site Small Intestinal Biopsies in Adults With Celiac Disease

Background:Celiac disease is an autoimmune enteropathy which can present with patchy mucosal lesions.Therefore,the diagnosis of the disease requires histological evaluation of multiple site biopsies.Aims:To analyze the pathological characteristics of multiple site small intestinal biopsies in adult patients with celiac disease and provide reference for early identification and diagnosis of celiac disease.Methods:The pathological data of 22 adult patients who were newly diagnosed as having celiac disease at the People's Hospital of Xinjiang Uygur Autonomous Region from August 2019 to April 2022 were collected retrospectively.All patients were positive for serum anti-tissue transglutaminase antibody IgA,and biopsies of duodenal bulb,descending part of the duodenum and terminal ileum were obtained under endoscopy.Histological examination was performed by experienced pathologists according to the modified Marsh grading system.Results:The most common pathological grade of duodenal bulb(50.0%)and descending part of the duodenum(45.5%)was Marsh Ⅲc,while those of terminal ileum was Marsh Ⅲa(63.6%).All of the bulb biopsies,95.5%of the descending part and 72.7%of the terminal ileum biopsies showed characteristic histological changes of celiac disease.Mucosal pathology was patchy in 7 patients,of which one patient was duodenal bulb and terminal ileum involved,and 6 were duodenum involved only.Fifteen patients had diffuse small intestinal mucosal pathology involving duodenal bulb,descending part and terminal ileum,of which 4 patients showed concordant histology(the same Marsh grade in duodenal bulb,descending part and terminal ileum)and 11 patients showed discordant histology.In 18 patients(81.8%),duodenum was the only affected site or duodenum showed more serious mucosal lesions compared with terminal ileum.Conclusions:Adult celiac disease may affect the whole small intestine,and the mucosal involvement may be patchy,which highlights the importance of taking small intestinal biopsies from multiple sites repeatedly in the diagnostic work-up of celiac disease.
6.Study on mechanism of the anti-tumor activity of Acanthopanax gracilistylus.

OBJECTIVETo study the mechanism of anti-tumor activity of Acanthopanax gracilistylus extract (Age).
METHODSThe tumor cells proliferation was detected by using (3H)-TdR incorporation method, and the effects of Age on cell cycle of tumor cells, retinoblastoma (Rb) protein and cyclin-dependent kinases (Cdk) were analyzed by flow cytometry and Western blotting assay, respectively.
RESULTSIt was indicated by cytoactivity test in vitro that Age only had effect in inhibiting the proliferation of tumor cells, it couldn't lead to death of cells. Under action of Age, the proliferation of tumor cells was halted at G0/G1 stage of cell cycle, and showed no direct cytotoxic effect by Age. Age could induce lowering of the expression of Rb, Cdk2 and Cdk4, cause halt of tumor cell proliferation.
CONCLUSIONThe tumor inhibitory effect of Age is realized by way of regulating the activity of cell cycle controlling enzymes to suspend the proliferation of tumor cells.

7.High tibial osteotomy plus arthroscopic surgery for medial gonarthrosis

Objective:To evaluate the clinical efficacy of high tibial osteotomy(HTO) plus arthroscopic surgery in the treatment of medial gonarthrosis.Methods:From January 2017 to May 2018, 40 patients were treated at Department of Joint Orthopaedics, Wujin Hospital Affiliated to Jiangsu University and at Department of Joint Orthopaedics, The First Affiliated Hospital to China Medical University for medial gonarthrosis. They were divided into 2 groups according to their different treatment methods. Group A was treated by HTO plus arthroscopic surgery; there were 20 cases, 8 males and 12 females with an age of 59.7 years ± 5.5 years. Group B was treated by only HTO; there were also 20 cases, 10 males and 10 females with an age of 58.2 years ± 4.3 years. The 2 groups were compared in terms of Hospital for Special Surgery (HSS) knee score, visual analogue scale (VAS), hip knee ankle angle (HKA), medial proximal tibia angle (MPTA) and posterior tibial slope angle (PTSA) at 6, 12 and 24 months postoperatively and at the last follow-up.Results:The 2 groups were comparable because there were no significant differences between them in the preoperative general data ( P>0.05). The HSS and VAS scores at 6 months after operation in group A (82.7±2.4 and 1.7±0.7) were significantly better than those in group B (78.4±2.6 and 2.2±0.8) ( P<0.05); the HSS score at 12 months after operation in group A (88.1±1.8) was significantly better than that in group B (82.9±1.7) ( P<0.05). There were no significant differences between the 2 group in the VAS score at 12 months after operation, or in the HSS or VAS scores at 24 months or at the last follow-up ( P>0.05). There were no significant differences either in the HKA, MPTA or PTSA scores between postoperative 6, 12, 24 months and the last follow-up ( P>0.05). Conclusion:In the treatment of medial gonarthrosis, high tibial osteotomy plus arthroscopic surgery may lead to better short-term outcomes than high tibial osteotomy alone, but the 2 methods may result in similar curative efficacy by 24 months after surgery.
8.Analysis of risk factors associated with functional delayed gastric emptying after ret-roperitoneal tumor resection surgery

Objective:To identify the risk factors for functional delayed gastric emptying(FDGE)after retroperitoneal tumor resection.Meth-ods:Sixty-seven patients with postoperative FDGE after retroperitoneal tumor resection from September 2017 to December 2022 admited in Peking University International Hospital were included in the observation group,and 836 normal patients who underwent ret-roperitoneal tumor resection during the same period were included in the control group.Medical histories and clinical treatment data were obtained for each group and compared to identify the risk factors for FDGE after retroperitoneal tumor resection.Results:The overall incid-ence of postoperative FDGE was 7.42%.Factors that were significantly elevated(P<0.05)in the observation group relative to the control group included adverse mental factors(37.31%),preoperative digestive tract obstruction(41.79%),postoperative abdominal cavity complic-ations(79.10%),diabetes(29.86%),average age of(61.85±6.11)years,and blood loss(1 011.94±507.30)mL.Pre-and post-surgery albumin levels[(38.22±3.75)g/L and(30.22±3.36)g/L,respectively]were significantly reduced(P<0.05)in the observation group compared to the control group.Risk factor analyses revealed that advanced age,diabetes,preoperative digestive tract obstruction,intraoperative blood loss,perioperative albumin level,postoperative abdominal cavity complications,and adverse mental factors were correlated with postoperative FDGE.Conclusions:Diverse factors could impact the risk of FDGE after retroperitoneal tumor resection.Patients should be accurately evalu-ated;reasonable and detailed prevention and treatment plans should be developed.
9.Clinical characteristics of family clustering pediatric and adult cases with severe acute respiratory syndrome coronavirus 2 Omicron variant infection

Objective:To investigate the clinical characteristics of family clustering pediatric and adult cases with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron variant infection in Shanghai City.Methods:A field investigation among the pediatric cases with Omicron variant infection and their household contacts from April 4 to April 30, 2022 in Children′s Hospital of Fudan University was conducted. The informations on case finding, clinical manifestations and SARS-CoV-2 vaccination status were collected. The epidemiological and clinical characteristics were compared between pediatric cases and adult cases. The independent sample t test or chi-square test was used for statistical analysis, and the relative risk ( RR) and 95% confidence interval (95% CI) were used to evaluate the protective effect of vaccination on the infection of Omicron variant. Results:There were 1 274 family members in 297 families including 370 children and 904 adults of whom 1 110(87.13%) were infected with Omicron variant, with 989(89.10%) symptomatic and 121(10.90%) asymptomatic. There were 355 children infected with Omicron variant, of whom 337(94.93%) were symptomatic, and the main manifestations were fever (96.74%(326/337)) and cough (40.36%(136/337)). Only one pediatric case with Rett syndrome developed critically severe pneumonia. A total of 194 pediatric cases had imaging examination, 64(32.99%) showed pulmonary inflammatory lesions. There were 755 adult cases infected with Omicron variant, of whom 652(86.26%) reported symptoms, and the main manifestations were fever (73.16%(477/652)) and cough (49.85%(325/652)). Among symptomatic cases, fever was more common in pediatric cases than in adult cases, while cough was more common in adult cases than in pediatric cases, and the differences were both statistically significant ( χ2=80.87 and 8.04, respectively, both P<0.01). The fever spike was higher in pediatric cases than in adult cases ((39.3±0.7) ℃ vs (38.6±0.6) ℃), and the difference was statistically significant ( t=9.85, P<0.001). The interval from the onset of symptoms to cycle threshold (Ct) value of the nucleic acid of Omicron variant≥35 was longer in pediatric cases than in adult cases ((13.0±3.1) d vs (10.9±3.6) d), and the difference had statistically significance ( t=2.97, P=0.004). Among 160 children aged 3 to 18 years, 54 (33.75%) received two-dose vaccination. Among the 904 adults, 388 (42.92%) received two-dose vaccination and 293 (32.41%) received a booster dose. In the adult cases, the risk of symptomatic infection was reduced by only 8% ( RR=0.92, 95% CI 0.86 to 0.98, P=0.014) following two-dose vaccination, and the risks of fever and cough following booster vaccination were reduced by 42%( RR=0.58, 95% CI 0.49 to 0.67, P=0.001) and 50% ( RR=0.50, 95% CI 0.34 to 0.78, P=0.001), respectively. Conclusions:Secondary attack rate and symptomatic rate of household infection are high in the context of the Omicron variant outbreak in Shanghai. Symptomatic infection is common in children and adults in household setting. Fever is the most common symptom and fever duration is short. Booster vaccination may provide certain protection against common symptoms caused by Omicron variant infection.
10.Strategies for Improving Compliance of Cancer Patients in Foreign Countries and Their Enlightenment to China

Compliance is directly related to the patient’s understanding and implementation degree of treatment plans, and is closely related to treatment outcomes. But previous studies have shown that the compliance of cancer patients is generally low. Foreign countries have rich practical experience in improving the compliance of cancer patients. Therefore, this paper started with the concept of compliance, expounded the classification of compliance, analyzed the current situation and gaps of compliance of cancer patients at home and abroad, introduced experience of foreign cancer patients’ compliance improvement strategies from the three aspects of policy, doctors and patients, and put forward localized recommendations for improving the compliance of cancer patients in combination with the national conditions.
