To establish an LC-MS/MS method for simultaneous determination of peimine and peiminine in rat plasma after oral and intravenous administration of Fritillaria thunbergii Miq. extract, the pharmacokinetic parameters were calculated as well. Peimine, peiminine and internal standard carbamazepine were extracted from plasma with liquid-liquid extraction by ethyl acetate, then separated on a Luna C18 column by using acetonitrile-water containing 10 mmol x L(-1) ammonium formate (35:65), as mobile phase. The electrospray ionization (ESI) source was applied and operated in positive ion mode. Peimine was detected at m/z 432.4 --> 414.4, peiminine at m/z 430.4 --> 412.4 and carbamazepine (IS) at 237.1 --> 194.2. The linear calibration curves were obtained at the concentration range of 0.8-800 ng x mL(-1) for peimine and peiminine. The extraction recoveries were 94.1%-105.3% and 85.8%-98.6%, respectively. The precisions, accuracy and stability of the analytes meet the requirements. The method was shown to be effective, convenient, and suitable for simultaneous pharmacokinetic study of peimine and peiminine in rat.

Objective To study the effect of family intervention on treatment compliance and social function for patients with schizophrenia. Methods 76 patients with schizophrenia were randomly divided into the intervention group and the control group with 38 cases in each group. The patients in both groups were treated with antipsychotics, the control group was only given general hygiene education, while the intervention group was given targeted systematic family intervention for 1 year. The Nurse-Brief Psychiatric Rating Scale (N-BPRS),Social Disability Screening scale (SDSS),Insight and Treatment Attitudes Questionnaire (ITAQ),medication adherence questionnaire were used before and after the intervention and then the recurrence rate within 1 year was calculated. Results At the end of 1 year the total scores of NBPRS,SDSS,medication adherence questionnaire significantly decreased in the intervention group and were signif-icantly lower than those of the control group at the same period, while the total score of ITAQ significantly increased in the intervention group and was significantly higher than those of the control group at the same period. The recurrence rate of the intervention group was significantly lower than the control group. Conclusions The family interventions contribute to promote insight recovery of the patients with schizophrenia ef-fectively and improve their treatment compliance, reduce recurrence and to promote the recovery of its so-cial function.

Objective To assess the status and severity of bone disease in patients with multiple myeloma (MM) by using chest computerized tomography (CT) and the relationship between clinical prognostic parameters and bone disease. Methods All 46 newly diagnosed MM in-patients received both imaging tests of chest CT and plain X ray.An experienced radiologist reviewed all the imaging data.Clinical laboratory parameters, stages of Durie-Salmon (DS) and International Staging System (ISS) were evaluated.Five cytogenetic abnormalities of bone marrow myeloma cells were tested by fluorescence in situ hybridization (FISH).Results The sensitivity of CT and X ray to determine pathological fractures was comparable, the positive rates of which were 41.3% (19/46) and 30.4% (14/46) respectively (P =0.29).Nevertheless,the positive rate of osteolytic lesions ascertained by CT was significantly higher than that by X ray (P < 0.001),60.9% (28/46) vs 13.0% (6/46) with diameter 5-10 mm and 50.0% (23/46) vs 10.9% (5/ 46) with diameter more than 10 mm.Osteolytic lesion numbers found by CT were more than those by X ray [5(0-21) vs0(0-4) lesions with diameter5-10 mm (P<0.001),2(0-14) vs0(0-2) lesions with diameter more than 10 mm (P < 0.001),respectively].Patients with positive osteolytic lesions had higher percentage of RB1 gene deletion[46.7% (14/30) vs 18.8% (3/16),P <0.001],D13s319 deletion [43.3% (13/30) vs 18.8% (3/16),P <0.001] and high risk cytogenetic abnormalities[50.0% (15/30) vs 25.0% (4/16),P < 0.001].Conclusions Chest CT is more sensitive than plain X ray in detecting osteolytic myeloma bone disease.Osteolysis determined by CT is relevant to clinical DS stages and risk stratification of cytogenetic abnormalities.

OBJECTIVETo optimize the preparation technology of effective fraction of Catharsius molossus, and investigate the feasibility of process control by the physical and chemical characterization of extracts.
METHODUsed single-factor test method, choosed the main effective components of peptides and amino acids as indexes, combined with theology, chemistry, electricity, and other characterization, the study researched the prepared technology of effective fraction of C. molossus including extraction, concentration, separation, purification, drying and so on.
RESULTThe optimal preparation technology of effective fraction of C. molossus was that soaked an amount of crude drugs with three times of 85% ethanol for 48 h, added 10 times of 85% ethanol, percolated in 4 mL x min(-1) x kg(-1), collected percolation liquid, concentrated to 1:1 at 50-55 degrees C, removed fat by frozen, adopted DA201-C macroporous resin, used 1 BV of water and 4 BV of 70% ethanol as eluting agent, collected eluant respectively. The water part was concentrated and dried, then washed twice with 85% ethanol, collected washing liquid and mixed with 70% ethanol eluant. The product was obtained by concentrating and dring. At the same time, the liquid-phase system of each link was characterized in preparation of effective fraction of C. molossus, which showed that the surface tension related to polypeptide was essentially unchanged, and the conductivity related to salt decreased by about 90% with ineffective substances closely related to salt The results showed that the preparation technology maximumly retained the effective information, and removed the invalid information.
CONCLUSIONThe preparation technology of effective fraction of C. molossus is stable and reliable, and the process control in physico-chemical characterization of extracts is feasible.
