1.Diagnosis and treatment of primary lactase deficiency in infants

Objective To investigate the clinical characteristics and treatment strategies of primary lactase deficiency in infants.Methods The clinical data of 26 infants with primary lactase deficiency in Shenzhen Chidlren's hospital from October 2009 to February 2011 were analyzed retrospectively and the diagnoses were confirmed by dietary therapy.Results The patients were consisted of 19 males and 7 females,whose ages of onset ranged from 1 to 2 months,persisting 1 to 4 months.Breast feeding,mixed feeding and formula feeding were given in 10 cases,8 cases and 8 cases,respectively.The main clinical manifestations were persistent or chronic diarrhea,with watery and or foamy stools,4 to 8 times per day.Nineteen patients had received antibiotics but without improvement.Lactose free formula and oral probiotics improved the symptoms within 1 week in all 26 cases,but recurrence of symptoms followed reintroduction of dairy foods.Conclusion The clinical manifestations of primary lactase deficiency in infants are persistent or chronic diarrhea.Lactose free formula can be used to confirm the diagnosis and to improve the symptoms.
2.Clinical Application of IL-10 and IL-21 Levels Monitoring in Patients with Psoriasis

Objective To detect the level of IL-10 and IL-21 in serum of patients with psoriasis and explore its significance of judge the extent of serious condition and treatment of psoriasis.Methods According to clinical dermatology edited by Zhao Bian diagnostic criteria,selected 65 cases of patients with psoriasis during the period from March to September in 2014 ad-mitted to Department of Dermatology of out-patient and hospitalization.25 cases were progressive stage (group A),20 pa-tients with stable stage (group B),20 patients were in recovery period (group C),and 25 cases of control group (group D). With ELISA method detect the expression level of IL-10 and IL-21 in serum of each group respectively.PASI score will take to all psoriasis patients meanwhile.Results The expressionlevels of IL-21 in serum of A (127.59 ± 16.09 pg/ml),B (105.74±21.08 pg/ml)two groups were significantly higher than that in D group (85.46±14.25 pg/ml),and the differ-ence were statistically significant (t=5.174,4.863,both P <0.01),while the expression levels of IL-10 in A (10.64±3.23 pg/ml)and B (12.27±2.18 pg/ml)were both lower than D group (20.29±2.51 pg/ml),and the difference were statisti-cally significant (t=2.031,2.027,both P <0.05).The expression of IL-21 (94.03±8.90 pg/ml)in C group was higher than that in D group,and the difference was statistically significant (t=2.033,P <0.05).The level of IL-21 in A group was higher than that in group C,and the difference was statistically significant (t=2.352,P <0.05),while the expression level of IL-10 was lower than that in C group (19.69±1.54 pg/ml),and the difference was statistically significant (t=2.071,P<0.05).The expression level of IL-21 in the serum of patients with psoriasis and PASI score was positively correlated (r=0.508,P =0.027),while the expression level of IL-10 and PASI score was negatively correlated (r=-0.413,P =0.039). Conclusion The progression of psoriasis is related to the increasion of IL-21 or the decreasion of IL-10.The detection of IL-21 and IL-10 was beneficial to judge the severity and efficacy evaluation of psoriasis.
3.Progress of the segmentation methods of magnetic resonance image and its application

Magnetic resonance imaging (MRI) plays a more and more important role in medical image area for its advantages of nonradiative,multiple imaging and high spatial resolution.This review gives a systematic discussion over a couple of MRI segmentation algorithms that are used widely to help people have an entire knowledge of MRI segmentation methods.On the base of classification and summary of MRI segmentation,the MRI segmentation algorithms have been classified into 5 different categories after preliminary investigation and survey.Based on threshold,pattern recognition,active contours,Markov random field (MRF) and graph cut,separately.After further investigation and survey we summarize the characters and field of applications of the 5 different algorithms,then take a few segment experiments among these algorithms on abdomen MRI and present their distinct characteristics.At last,we take a prospect at the future of the MRI segmentation.
5.Clinical Observation on Treatment of Spastic Cerebral Palsy with Tuina plus Music Therapy

Objective: To observe the effect oftuina plus music therapy on range of motion of ankle joints and muscle spasm of lower limbs in children with spastic cerebral palsy. Method:All of 286 cases that conformed to the diagnostic criteria of infantile cerebral palsy were treated with 7 types of tuina manipulations respectively to unblock the Governor Vessel, reinforce the kidney and strengthen the spleen, pinch along the spine, stimulate specific foot-reflex area and different spinal segments, accelerate recovery of muscle strength and increase joint range of motion, 25-30 rain each treatment, once or twice a day, 30 d constitute a course of treatment.After this, the efficacy on femoral medial adduction and dorsiflexion angle and composite spasticity score (CSS) was evaluated. Result: The statistical analysis showed significant differences in dorsiflexion and femoral medial adduction angle and CCS scores (P<0.01) after the treatments. Conclusion: Tuina plus music therapy can lubricate the joints, relax contraction of tendons, alleviate muscle spasm and improve scissors and toe-walking gaits, thereby benefiting the gross motor function of infants in sitting, kneeling, standing and walking.
6.The effect of high intensity focused ultrasound on the immunity of patient with hepatocellular carcinoma

Objective To investigate the changes of immunity in patients with hepatocellular carcinoma (HCC) treated by high intensity focused ultrasound (HIFU). Methods HIFU system was used to treat 21 patients with mid-advanced HCC. The blood tests of cellular and humoral immunity related indexes were made before and on the 3rd, 7th, 14th, 21st day following the sonication. The histological changes of the target lesions were observed by light and electron microscope for the cases undergoing second stage operation. Results Light and electron microscope all revealed that HIFU could lead to irreversible changes in the tumor cells of the target lesions. There were not significant differences between the values of CD 3, CD 4, CD 8, CD 16, IgG, IgA, IgM,etc. before and after the ablation. Conclusions HIFU could destroy the tissues of HCC effectively. HIFU could not change the immunity of the patients with mid-advanced HCC remarkably. It may be ideal to unite immunotherapy and the other methods to get a sound clinical prognosis for HCC patients.
7.Clinical observation on Nie-pinching spine manipulation combined with point therapy for cerebral palsy in children with spleen deficiency

Objective:To observe the clinical effect of Nie-pinching spine coupled with point application and injection for cerebral palsy (CP) in children with spleen deficiency.
Methods:A total of 70 CP children with spleen deficiency were randomly allocated into a treatment group (n=35) and a control group (n=35). Children in the treatment group were treated with comprehensive rehabilitation training, Nie-pinching spine, point application and point injection, whereas children in the control group were treated with comprehensive rehabilitation training and oral administration of multi-enzyme tablets. One course of treatment made up of 20 d. There was a 20-day interval between two courses. The cases were treated for a total of 3 courses. Then the therapeutic efficacies were evaluated using scores of spleen-deficiency symptoms and gross motor function measure (GMFM).
Results:After 3 courses of treatment, the total effective rates for spleen-deficiency symptoms and rehabilitation were 85.7% and 77.1% respectively in the treatment group, versus 51.4% and 57.1% respectively in the control group, showing statistical differences (bothP<0.05).
Conclusion:In addition to comprehensive rehabilitation training, Nie-pinching spine combined with point application and injection obtained better effects than oral administration of multi-enzyme tablets.
8.Clinical observation on acupuncture plus occupation therapy for fine motor functions in children with spastic cerebral palsy

Objective: To observe the clinical effect of acupuncture plus occupation therapy for fine motor functions in children with spastic cerebral palsy. Methods:A total of 80 cerebral palsy kids with fine motor dysfunction were allocated into two groups by envelop, 40 cases in each group. Cases in the control group were treated with occupation therapy. Based on the therapy given to the control group, cases in the treatment group were supplemented with acupuncture at the points of the three yang meridians of hand. The treatment was done every other day and 10 times for a course. There was a 15-day interval between two courses, and 3 courses in total. The therapeutic efficacies were evaluated using Peabody developmental motor scale 2 (PDMS-2) and modified Ashworth scale (MAS) before and after treatment. Results:After treatment, the standard score for grasping (Grs), standard score for visual-motor integration (Vis), fine motor quotient (FMQ) and modified Ashworth scale (MAS) were significantly improved in both groups (P<0.05). The improvement in the treatment group was more obvious than that in the control group (P<0.01). Conclusion:Acupuncture plus occupation therapy can achieve better effect than occupation therapy alone in improving fine motor functions of upper limbs in cerebral palsy kids.
9.Effect of Integrated Traditional Chinese and Western Medicine on Persistent Vegetative State in Children: 7 Cases Report

Objective To observe the effect of Integrated Traditional Chinese and Western Medicine on persistent vegetative state (PVS) in children. Methods 7 children with PVS were treated with Integrated Traditional Chinese and Western Medicine, and were analyzed retrospectively. Results 3 childer were effectual, 3 improved, 1 ineffective. The PVS improved significantly after the treatment. The ones with course less than 3 months, the neuroimage show atrophy, appeared more effective. Conclusion The integrative therapy is effective on PVS in the children. The prognosis may associate with the course of the disease and the pathology of the brain.

Myofibrillogenesis regulator-1 (MR-1) is a novel protein involved in cellular proliferation, migration, inflammatory reaction and signal transduction. However, little information is available on the relationship between MR-1 expression and the progression of atherosclerosis. Here we report atheroprotective effects of silencing MR-1 in a model of Ang II-accelerated atherosclerosis, characterized by suppression focal adhesion kinase (FAK) and nuclear factor kappaB (NF-κB) signaling pathway, and atherosclerotic lesion macrophage content. In this model, administration of the siRNA-MR-1 substantially attenuated Ang II-accelerated atherosclerosis with stabilization of atherosclerotic plaques and inhibited FAK, Akt, mammalian target of rapamycin (mTOR) and NF-kB activation, which was associated with suppression of inflammatory factor and atherogenic gene expression in the artery. In vitro studies demonstrated similar changes in Ang II-treated vascular smooth muscle cells (VSMCs) and macrophages: siRNA-MR-1 inhibited the expression levels of proinflammatory factor. These studies uncover crucial proinflammatory mechanisms of Ang II and highlight actions of silencing MR-1 to inhibit Ang II signaling, which is atheroprotective.
