1.The role of three-dimensional digital subtraction angiography in the diagnosis of cerebral vascular diseases

Objective To evaluate the utility of three-dimensional digital subtraction angiography(3D-DSA) in the diagnosis of cerebral vascular diseases.Methods Seventy-one patients suspected and diagnosed with cerebral vascular diseases underwent conventional DSA and 3D-DSA. Three-dimensional (3-D) reconstructed images were obtained at a separate advantage 4.0 workstation after the rotational images were transferred. The available visualization techniques included maximum intensity projection (MIP), shaded surface display (SSD), and virtual angioscopy (VA). Results Sixty-four aneurysms were found in 44 cases. Nineteen cases were diagnosed as cerebral arteriovenous malformation (AVM) and 8 cases of cerebral ischemia were due to cerebral vascular stenosis (internal carotid artery in 6 cases and occlusion of anterior cerebral artery in 2 cases).Conclusions 3D-DSA is reliable, fast and safe for diagnosis of cerebral vascular diseases, especially involving intracanal areurysms AVM and vascular stenosis.
2.Analysis in nosocomial infection in elderly patients with intra-aortic balloon pump after coronary artery bypass grafting and nursing

Objective To summarize the nursing experience on nosocomial infection in elderly patients with intra-aortic balloon pump (IABP) after coronary artery bypass grafting. Methods The clinical data of elderly patients who underwent coronary artery bypass grafting with application of IABP from April 2006 to February 2009 were retrospectively collected and analyzed. Results 29 patients suffered nosocomial infection in all 111 cases. The hospital mortality was much higher among patients with nosocomial infection than in patients without nosocomial infection (19 cases vs 11 cases). 87 positive strains were separated from 29 patients with nosocomial infection, which included 47 strains of gram-negative bacterial, 23 strains of gram-positive bacterial and 17 strains of fungi. Conclusions The nosocomial infection was one of the risk factors for hospital mortality in elderly patients with intra-aortic balloon pump after coronary artery bypass grafting. It is important to take effective nursing strategies to prevent the postoperative nosocomial infection in this population.
3.CT-guided radiofrequency ablation of isolated pulmonary metastases from colorectal carcinoma

Objective To evaluate the safety,local efficacy and survival rates of isolated pulmonary metastases from colorectal carcinoma treated with CT-guided radiofrequency ablation.Methods A total of 28 patients with isolated pulmonary metastases from colorectal carcinoma were performed with CT-guided radiofrequency ablation (radiofrequency ablation group).The size of the lesion was 1.2-5.0 (2.9 ± 1.0) cm.During the same time,9 patients,who did not take any treatment for isolated pulmonary metastases from colorectal carcinoma,were collected as control group.The size of the lesion was 2.0-5.0 (3.6 ± 1.0) cm.The local progression-free rates and overall survival rates were compared.Results All the cases were able to complete the radiofrequency ablation in radiofrequency ablation group.During and after the procedure,no major complications occurred.The 1-,2-,and 3-year local progression-free rates and overall survival rates in radiofrequency ablation group were significantly higher than those in control group.The local progression-free rates were 89.3% vs.76.2%,78.6% vs.33.9%,70.7% vs.16.9%.The overall survival rates were 96.4% vs.77.8%,85.7% vs.44.4%,56.7% vs.22.2%.The differences between two groups had statistical significance (P < 0.01).Conclusion CT-guided radiofrequency ablation for isolated pulmonary metastasis from colorectal carcinoma is a safe,effective,minimally invasive treatment,and can significantly improve the local progression-free rates and overall survival rates.
4.Investigation on Properties of Collagen Nanowires Quasiepitaxially Grown on Mica Lattice Plane

Collagen fibrils and hydroxyapatite might recognize each other at the mesoscale by multiple cooperative interactions due to their intrinsically repetitive structured surfaces, and thus effectively directing the biomineralization, a biological process involving regulating the growth of bones, teeth and other organs. In this work, we developed a simple technique to prepare nanowire arrays of biological macromolecules by reversely using the biomineralization mechanism, with results similar to the hot wall epitaxy, a molecular beam deposition technique under vacuum. With this technique, we successfully cultured 5-10 μg/mL rat tail type I collagen monomer solutions into collagen nanowire arrays on the mica (001) lattice plane along one unique direction across the whole cleavage surface. The atomic force microscope experiments indicated that the nanowires in the arrays became more crowded with higher monomer concentration, but their width and height remained unchanged, about 60.0 nm and 1.5 nm, respectively. The collagen nanowire coating enhanced the hydrophilicity of the mica surface, reducing the contact angle from 25.8° to 9.5°. Based on the characterization results of electron back scattering diffraction and transmission electron microscope, the collagen nanowires were most likely to be oriented along the mica [110] direction, which validated the quasiepitaxial growth mechanism in more details.
5.Effects of Docetaxel and Epirubicin Combined with Cyclophosphamide on Clinical Efficacy and Tumor Markers of Breast Cancer Patients with Different Molecular Types

OBJECTIVE:To investigate the effects of neoadjunctive chemotherapy(NAC)of docetaxel and epirubicin com-bined with cyclophosphamide on clinical efficacy and tumor markers of breast cancer patients with different molecular types. METH-ODS:A total of 88 female patients with locally advanced breast cancer collected from our hospital during Jan. 2014-Jan. 2016 were divided into Luminal A type(23 cases),Luminal B type(21 cases),basal-like type(11 cases),HER2-over expressing type(18 cases)and normal breast-like type(15 cases)according to molecular type. All patients were given Docetaxel injection+Epirubicin hydrochloride injection+Cyclophosphamide for injection for consecutive 6 cycles(21 d as a cycle). Total response rates and patho-logical complete remission(pCR)rates were compared among breast cancer patients with different molecular types. The expression of serum tumor markers [CEA,CA125,CA153] were compared before and after treatment,and the occurrence of ADR was record-ed. RESULTS:Total response rate of 88 patients was 63.64%,among which that of basal-like breast cancer patients was 72.73%, significantly higher than other molecular types,with statistical significance(P<0.05). There was no statistical significance in total response rates of pairwise molecular type comparison(P>0.05). The pCR rate of 88 patients was 27.27%,and that of basal-like breast cancer patients was the highest(45.45%). There was statistical significance in pCR rates of pairwise molecular type compari-son(P<0.05),except there was no significant difference in pCR rate between HER2-over expressing type and normal breast-like type(P>0.05). Before treatment,there was no statistical significance in the expression of CEA,CA125 and CA153 in breast can-cer patients with different molecular types(P>0.05). After treatment,the expression of CEA,CA125 and CA153 in different mo-lecular types were decreased significantly,with statistical significance(P<0.05). There was no statistical significance in the expres-sion of above markers among different molecular types(P>0.05). There was no statistical significance in the incidence of ADR among different molecular types(P>0.05). CONCLUSIONS:NAC plan of docetaxel and epirubicin combined with cyclophospha-mide can reduce the expression of tumor markers and shows certain therapeutic efficacy for breast cancer patients with different mo-lecular types. Total response rate and pCR rate of basal-like type are better than those of other molecular types,so NAC plan is the preferred treatment for basal-like type breast cancer.
6.Photodynamic therapy combined with cell therapies for advanced esophageal cancer

Objective To explore the clinical efficacy and immunological effects of photodynamic therapy combined with cell therapies for advanced esophageal cancer. Methods Ninety patients with advanced esophageal cancer were collected and divided into three groups by a non-randomized controlled trial according to treatment intention. Group A (30 patients) received photodynamic therapy (PDT) alone; group B (30 patients), PDT received PDT plus cytokine-induced killer (CIK) cell therapies; and group C (30 patients) received CIK cell therapy aloen. In all the patients, the efficacy was assessed, the quality of life was documented, the immune function was detected, and the 6-month and 1-year death tolls were counted. Results The total clinical effectiveness rate was much higher in groups B and A than in group C (90.0% and 86.7% vs. 63.3%, P < 0.05), and there was no a statistical difference between group B and group A (P > 0.05). The rate of an increase in quality of life was significantly higher in group B than in groups A and C (86.7%vs. 60.0%and 33.3%, P<0.05), and it was higher in group A than in group C (60.0% vs. 33.3%, P < 0.05). As compared groups A and C, the percentages of CD3+ and CD3+ CD56+ were significantly improved in B group (P < 0.05), there was no a statistical difference between group A and group C. The 6-month survival rate did not differ statistically among the three groups (P > 0.05), while the 1-year survival rate was much higher in group B than in groups A and C (73.9% vs. 55.6% and 29.4%, P < 0.05). Conclusion Photodynamic therapy combined with cell therapies has a synergistic effect, and it enhances the overall immune function, significantly improves the quality of life and prolongs the survival period, showing a better clinical prospect.
7.Prophylactic recombinant human thrombopoietin treatment alleviates chemotherapy- induced thrombocytopenia in tumor patients.

OBJECTIVETo assess the efficacy of prophylactic treatment with recombinant human thrombopoietin (rhTPO) on chemotherapy-induced thrombocytopenia in tumor patients.
METHODSIn this randomized cross-over self-controlled clinical trial, 24 patients with malignant neoplasms were randomized group A (12 cases) and group B (12 cases). All the patients underwent two identical cycles of chemotherapy. In group A, RhTPO (1.0 µg/kg) was administered subcutaneously on a daily basis 3 days before the second chemotherapy cycle for 7 consecutive days, and in group B, RhTPO was administered daily 6-24 h after the second chemotherapy cycle for 7 days. In both groups, RhTPO was not administered in the first chemotherapy cycle, which served as the control cycle.
RESULTSIn both the groups, platelet count was significantly higher in the second cycle than in the control cycle, and the duration of thrombocytopenia was significantly shortened in the second cycle. Compared with group B, the patients in group A showed a significantly higher platelet count in the second cycle and a significantly shorter duration of thrombocytopenia in the second cycle.
CONCLUSIONProphylactic administration of rhTPO can significantly reduce the severity and duration of thrombocytopenia and promote platelet recovery in patients undergoing chemotherapy for malignant tumors.

8.UHPLC-MS/MS analysis of cAMP and cGMP in rat plasma as potential biomarkers of Yin-Yang disharmony in traditional Chinese medicine

Cyclic 3',5'-adenosine monophosphate (cAMP) and cyclic 3',5'-guanosine monophosphate (cGMP) are considered as potential biomarkers for Yin-Yang disharmony in traditional Chinese medicine.However,phosphodiesterase-mediated ex vivo degradation of these molecules in biological samples may result in their underestimation.In the present study,a ultra-high performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS) method was developed for determination of cAMP and cGMP in rat plasma,with special consideration of their stability ex vivo.Following precipitation of proteins from plasma samples with 0.4 M perchloric acid,the analytes were chromatographed on a Shimadzu Shim-pack-XR-ODS Ⅱ column with 2.5 mM ammonium acetate and methanol in gradient mode.The MS/MS detection was performed using multiple reaction monitoring in the positive electrospray ionization mode.The lower limit of quantification was 0.27 ng/mL for cAMP and 0.37 ng/mL for cGMP.The method was used to determine the plasma cAMP and cGMP levels in normal and Yin deficiency diabetic rats treated with or without Rehmannia glutinosa.The developed method may be useful for evaluating the regulatory effects of Chinese herbal medicine on the levels of cAMP and cGMP in the body.

Objective:To summarize the patient profiles and therapeutic efficacies of ABO-incompatible living-related kidney transplantations at 19 domestic transplant centers and provide rationales for clinical application of ABOi-KT.Methods:Clinical cases of ABO-incompatible/compatible kidney transplantation (ABOi-KT/ABOc-KT) from December 2006 to December 2009 were collected. Then, statistical analyses were conducted from the aspects of tissue matching, perioperative managements, complications and survival rates of renal allograft or recipients.Results:Clinical data of 342 ABOi-KT and 779 ABOc-KT indicated that (1) no inter-group differences existed in age, body mass index (BMI), donor-recipient relationship or waiting time of pre-operative dialysis; (2) ABO blood type: blood type O recipients had the longest waiting list and transplantations from blood type A to blood type O accounted for the largest proportion; (3) HLA matching: no statistical significance existed in mismatch rate or positive rate of PRA I/II between two types of surgery; (4) CD20 should be properly used on the basis of different phrases; (5) hemorrhage was a common complication during an early postoperative period and microthrombosis appeared later; (6) no difference existed in postoperative incidence of complications or survival rate of renal allograft and recipients at 1/3/5/10 years between ABOi-KT and ABOc-KT. The acute rejection rate and serum creatinine levels of ABOi-KT recipients were comparable to those of ABOc-KT recipients within 1 year.Conclusions:ABOi-KT is both safe and effective so that it may be applied at all transplant centers as needed.
10.Effect of poria cocos on gastrointestinal motility in mice

Objective To investigate the regulatory effect of poria cocos on gastrointestinal motility in mice. Methods A total of 130 Kunming mice were randomly divided into negative control group, low-dose and high-dose groups of raw poria cocos powder, low-dose and high-dose groups of cooked poria cocos powder, low-dose and high-dose groups of poria cocos surrogate culture powder, low-dose, medium-dose and high-dose groups of poria cocos water extract, and low-dose, medium-dose and high-dose groups of poria cocos alcohol extract, with 10 mice in each group. The animals were administered by gavage for 7 days, once a day. After the last administration, the intestinal propulsion function test and gastric solid emptying test were conducted to observe the regulating effect of poria cocos on gastrointestinal motility of mice. Results Compared with the negative control group, the small intestine propulsion rate in the low-dose group of poria cocos surrogate culture powder was significantly increased (P<0.01). Except the high-dose group of raw poria cocos powder, the other poria cocos groups had higher gastric residual rate (P<0.05). Conclusion Poria cocos does not promote intestinal propulsion of mice under normal physiological condition, but it can inhibit gastric empting and exert a moderating effect on gastrointestinal function in normal mice.
