BACKGROUND: Bone marrow necrosis has unspecific clinical features, which is often misdiagnosed or missed due to a lack of the knowledge of the disease. OBJECTIVE: To improve the awareness and vigilance to bone marrow necrosis, and to further explore the clinical manifestations, hematological characteristics, pathogenesis and treatment of bone marrow necrosis. METHODS: The bone marrow necrosis, hematologic neoplasms, solid tumor, bone marrow puncture, bone marrow pathology in Chinese and English served as the search terms to search articles related to bone marrow necrosis in PubMed and Wanfang databases, published from 1941 to 2016. Totally 43 articles were selected for review. RESULTS AND CONCLUSION: Bone marrow necrosis is a rare complication caused by various diseases, clinically characterized by bone pain, fever, anemia, and nucleated red cells and immature neutrophilic leukocytes in the blood smear. Bone marrow aspiration and/or bone marrow biopsy show(s) necrotic features. Its pathogenesis is complex, and it is still poorly understood and needs further research. There is no good treatment for bone marrow necrosis, and the prognosis is poor. Early correct diagnosis and etiological treatment are crucial for the prognosis of bone marrow necrosis. With the improvement of disease awareness, bone marrow cytology, genetics, MRI and hematopoietic stem cell transplantation, bone marrow necrosis is expected to get a better prognosis.

Objective :
To construct an appropriate technology catalogue for general practice based on Delphi method, and to provide reference for promoting appropriate techniques and training general practitioners.
Methods :
The catalogue was briefly constructed based on literature reviews and the data of the top twenty diseases diagnosed in out-patient department of Sir Run Run Shaw Hospital and two community health service centers in Hangzhou. Two-round Delphi consultation was conducted by fourteen general practitioners coming from upper first-class hospitals and community hospitals. The final catalogue was established according to evaluation of the degree of involvement, authority coefficient and Kendall's W values of the fourteen specialists.
consultations :
Results Of the fourteen specialists, nine had bachelor's degrees and five had master's degrees or above;twelve were deputy chief physicians or chief physicians and two were physicians. They were engaged in general practice for 17.21 years in average. The positive coefficients of the two rounds of were both 100.00%. The authority coefficient of the specialists was 0.891. After the first round of consultation, the W values for importance and feasibility of 6 first-class indicators were 0.170 and 0.244,and the ones of 56 second-class indicators were 0.236 and 0.250(all P<0.05). Six of second-class indicators were excluded because their coefficent of variation(CV)for importance and feasibility were more than 0.25. After the second round of consultation, the W values for importance and feasibility of 6 first-class indicators were 0.245 and 0.247,and the ones of 50 second-class indicators were 0.355 and 0.370(all P<0.05). The CV for each indicator was less than 0.25. Finally,an instructional catalogue was defined, consisting of 6 first-class indicators(basic diagnosis and treatment skills, first aid skills, symptomatic diagnosis and treatment skills, chronic disease management skills, common disease diagnosis and treatment skills, and common manipulative skills)and 50 second-class indicators.
Conclusion
The degrees of involvement, authority and consistency of the specialists were relatively high. The catalogue of appropriate technology for general practitice had good reliability and feasibility, which was worthy of promotion and application.

Chinese Journal of Neurology 2008;41(9):607-609
Objective To observe the clinical characteristics of progressive stroke (PS) patients with vascular stenosis and the relationship between PS and cerebral blood flow (CBF). Methods Fifteen patients of PS with anterior circulation vascular stenosis were chosen, their clinical documents and CBF were analyzed with Xenon-CT when they were in progression. Results These patients mostly presented hemiparalysis and language dysfunction at the beginning ( 13/15 ) before the disease developed rapidly into a serious state. The infarction usually happens in the periventricular area (10/15). Upper limbs paralyzed more severely than low limbs(11/15). Cerebral hypoperfusion areas around the infarction in 11 patients ( 10 ml·100 g-1·min-1 < CBF < 20 ml·100 g-1·min-1 ) were found. Conclusions Patients of PS with vascular stenosis present typically clinical characteristics, Low CBF caused by cerebral artery stenosis may be one of the most important factors leading to PS.
8.Effect of lipopolysaccharide and transforming growth factor-beta 1 on expression and signal pathway of Toll like receptor 4 in dental pulp cells.
West China Journal of Stomatology 2012;30(1):77-86
OBJECTIVETo explore the expression of Toll like receptor 4 (TLR4) on the pulp cells and the change of related signaling molecules under the condition of concomitant lipopolysaccharide (LPS) and transforming growth factor-beta 1 (TGF-beta 1) during the course of pulpitis.
METHODSAfter treated by LPS and TGF-beta 1, the expression of TLR4 on pulp cells was detected by flow cytometry (FCM). The expressions of signaling molecules evolutionarily conserved signaling intermediate in Toll pathways (ECSIT) and nuclear factor-kappaB (NF-kappaB) were detected by real-time polymerase chain reaction (real-time PCR) and Western blot. The secretion of interleukin-6 (IL-6) was detected by enzyme-linked immunosorbent assay (ELISA). Furthermore, the change of corresponding targets in inflamed pulp cells from clinical samples were detected by real-time PCR.
RESULTSAfter treated by LPS and TGF-beta 1 in vitro, there was no change in the expression of TLR4 on pulp cells, but the secretion of proinflammatory cytokines IL-6 increased. LPS and TGF-beta 1 could also increase the expression of signal downstream ECSIT and actived NF-kappaB. Furthermore, the expression of TLR4 mRNA had no increase in inflamed pulp cells from clinical samples, while the expression of TGF-beta 1, ECSIT and IL-6 mRNA increased through real-time PCR.
CONCLUSIONDuring the course of pulpitis, although the expression of TLR4 on pulp cells was inhibited by increased expression of TGF-beta 1, the TLR4 pathway was still activated. This effect could be caused through activation of ECSIT mediated by LPS, which might inhibit the TGF-beta 1 pathway.
