Moyamoya disease is a chronic progressive cerebrovascular disease which is characterized by progressive stenosis or occlusion at bilateral terminal portions of the internal carotid artery and its large branch vessels,and with an abnormal formation of new vascular network in skull base.As a noninvasive method,magnetic resonance imaging (MRI) plays an important role in the diagnosis and evaluation of moyamoya disease.This article reviews the application of MRI in the identification,diagnosis and treatment evaluation of the stroke subtypes in patients with moyamoya disease.

3.Effects of Recreational Therapy on Social Communication of Stroke Patients

ObjectiveTo investigate the effect of the recreational therapy on the ability of social communication in stroke patients. Methods58 cases of stroke patients were divided into recreational group (28 cases) and control group (30 cases). All of them received routine rehabilitation therapy, while the recreational group received the recreational therapy in addition. All patients were evaluated with Functional Independence Measure (FIM) before and after treatment. ResultsThe scores of FIM significantly improved after treatment (P<0.05) in both groups, and the scores in social communication improved more (P<0.01) in recreational group than in control group. ConclusionRecreational therapy may improve the ability of social communication more effectively for stroke patients.
4.Development and Evaluation of an Information Gathering Soft-ware Platform for Spleen System Inquiry in Traditional Chinese Medicine

Objective To develop an information gathering soft-ware platform for spleen system inquiry of TCM, and to discuss the practicability and feasibility of applying computer technology to standardizing management of TCM inquiry data. Methods Based on the TCM Spleen System Inquiry Scale, combined with clinical practice of TCM diagnosis and treatment, an inquiry information gathering soft-ware platform for TCM spleen system was designed. The platform was interfaced with B/S structure, and in the use of computer programming languages, the platform possessed the good functions of data collection, storage, query, export and print, etc. In addition to this, by calling the correct algorithm, it contents users with analysis and statistics on the target data set. Results The platform designed has been initially equipped with the functions of collecting, storing, inquiry, deleting, modifying, exporting and printing the inquiry-based four diagnostic data for TCM spleen system diseases. And to some extent, it also achieves standardization and objectification of the collection of TCM Spleen System Inquiry Scale. Conclusion It is practicable and feasible to applying computer technology into standardizing management of TCM inquiry informations. Besides, it could act as the direction for the research of TCM four diagnostic data in the future.
5.Clinical efficacy observation of stereotactic radiation therapy combined with gemcitabine for advanced pancreatic carcinoma

Objective To seek the curative effect of stereotactic radiation therapy (SRT) combined with gemcitabine for unresectable advanced pancreatic carcinoma.Methods 24 patients were treated by SRT of 6MV X-ray.Patients were fixed with the rack of stereotactic localization and heated plastics mould.The CT scanning results were put into the treatment planning system.According to the target area of tumor,sensitive organs and moving error drew GTV,CTV and PTV.The best plan was selected by the dose-volume histogram (DVH).5 to 7 beams of non-coplanar radiation ray were chosen.PTV was surrounded by ≥95 % isodose curves.Conventional fraction,5 fractions per week,1.8-2 Gy per fraction was used.All patients received a total dose of 50-60 Gy.Gemcitabine was performed 1000 mg/m2,once a week,iv gtt,in the 1st,2nd and 4th,5 th weeks.Results In chemo-radiotherapy,50.0 % (12/24) patients showed light nausea,41.7 % (10/24) patients showed leucopenia or thrombocytopenia of grade 1 or 2,after symptomatic treatment,all patients completed the planned treatment.In a period of one month to three months,after SRT combined with chemotherapy,appetite improvement was 83.3 % (20/24),jaundice disappeared in 6 of 6 patients (100.0 %),abdominal pain was relieved in 21 of 24 patients (87.5 %),3 patients were relieved completely among them.The complete remission (CR) rate was 16.7 % (4/24) and partial remission (PR) rate was 66.7 % (16/24),with CR+PR rate of 83.4 % (20/24).One-year survival rate was 70.8 % (17/24).As a consequence of cachexia,intestinal obstruction or bleeding,7 patients died within one year.Nobody survived more than 2 years.Conclusions SRT combined with gemcitabine for advanced pancreatic carcinoma may relieve symptoms.It is an effective approach to improve life quality and prolong survival time for advanced pancreatic carcinoma,especially lod and weak patients are more suitable to select SRT combined with gemcitabine.
6.Analysis of multiple factors correlated with the six months prognosis of comprehensive function in patients with stroke

objective To investigate the multiple early variables that influenced the 6th months prognosis of comprehensive function in patients with stroke.Methods Two hundreds and eleven patients of primary cerebral infarction and primary cerebral hemorrhage in Shanghai were divided into treated group and controlled group randomly.Patients in the treated group were given standardized tertiary rehabilitation,while those in the controlled group received normal internal medicine treatments but without standardized tertiary rehabilitation.Twenty seven early variables were collected with regard to the patients'medical history,physical examination,scores with CNFD,S-FMA and MBI at the time of enrollment.The patients'FCA scores at the ends of the 6th months after stroke were used as the comprehensive functional outcome.Stepwise multiple regression analysis applied to analyze the data.Results Stepwise multiple regression analysis revealed that therapeutic regimen,CNFD scores,age,diabetes mellitus,SFMA scores,smoking and labour intensity predicted comprehensive functional outcome at the ends of the 6th months after stroke.Conclusion Certain early variables predict the long-term comprehensive functional prognosis of stroke patients.Therapeutic regimen,CNFD scores and age were significant predictors.

Objective To explore the expression of transforming growth factor ?1(TGF-?1)in the mucosa of guinea pigs with otitis media with effusion(OME)and the role of TGF-?1 in the development of OME.Methods 70 guinea pigs were randomly devided into 7 groups,each with 10 animals.OME was produced by injecting deactivated streptococcus pneumoniae into the typmpanic cavity of guinea pigs.The expression of TGF-?1 was examined by means of immunohistochemistry 6 h,1 d,3 d,7 d,14 d and 30 d after injection.Results TGF-?1 expression could be detected in the middle ear mucosa at 7 d after injection and reached maximum at 30 d.Conclusion TGF-?1 is expressed in the middle ear mucosa of the guinea pigs with OME induced by deactivated streptococcus pneumoniae,and this indicates that TGF-?1 may play a role in the chronic protraction of OME.
8.Applying manofluorography in the evaluation of dysphagia after brainstem injury

Objective To evaluate the swallowing function of patients with dysphagia after brainstem injury using manofluorography (MFG),analyzing the abnormal biomechanical and kinematic parameters as well as any correlation between changes in the pharynx and the upper esophageal sphincter (UES) measured manometrically and changes in the kinematics of the hyoid bone.Methods Thirteen patients with dysphagia after brainstem injury (the patient group) and 13 healthy participants (the control group) underwent manofluorography.Kinematics and biomechanical changes during swallowing were compared between the two groups and the correlations between the observations were analyzed.Results The patient group showed significantly lower maximum pressure and rate of pressure change at the base of the tongue and in the hypopharynx,as well as less hyoid anterior displacement,smaller and briefer UES opening,but significantly higher minimum pressure at UES relaxation.The duration of tongue root elevation and hypopharynx pressure was also shorter than in the control group,on average.There was a negative correlation between hyoid anterior displacement and the minimum pressure on UES relaxation in the control group,and a positive correlation between hyoid anterior displacement and the maximum pressure at the base of the tongue and in the hypopharynx in the patient group.Conclusions The concurrent use of manometry and video-fluorography for evaluating dysphagia can be an objective and effective diagnostic tool for the comprehensive evaluation of swallowing function.

Objective To investigate the relationship between obstructive sleep apnea-hypopnea syndrome(OSAHS)and plasma homocysteine(Hcy)levels in patients with ischemie cerebrovascular disease(ICVD).Methods Seventy-six patients with ICVD were monitored with polysomnography(PSG) for 7-8 hours during sleeping.The levels of fibrinogen(FBG),high density lipoprotein cholesterol(HDLC),low-density lipoprotein cholesterol(LDL-C),triglyceride(TG)and plasma Hey were assessed.Results Of the 76 ICVD patients,57 showed apnea symptom during sleeping,of which 53(69.7%)were diagnosed with OSAHS and 4 with central sleep apnea,According to the mean apnea-hypopnea index (AHI),the severity of apnea was classified as no apnea symptom(19 cases with mean AHI of 2.38±0.96 and Hey level of(8.78 ±2.01)μmol/L),mild apnea(21 cages with mean AHI of 14.14 ±4.37 and Hcy level of(12.91 ±3.00)μmol/L),moderate apnea(24 cases with mean AHI of 29.62±5.81 and Hcy level of(14.85 ±4.15)μmol/L)and severe apnea(8 cases with mean AHI of 46.75±2.82 and Hcy level of(19.30±4.82)μmol/L).The level of Hcy was statistical significant among these 4 groups(F=40.32,P<0.01)and correlated with the mean AHI(r=0.598,P<0.01).Conclusion Patients with ICVD have a high morbidity of OSAHS,mainly suffering from mild to moderate apnea;The plasma Hcy level elevates in the ICVD patients with OSAHS and is correlated with the severity of apnea.
