1.Secondhand Smoke Exposure among Restaurant Workers and its Impact on Health in Shanghai

Objective To estimate the status of secondhand smoking (SHS) among restaurant workers in Shanghai and explore its impact on health. Methods A random sample of 242 workers from 33 restaurants was selected to participate in the face to face interview. Results There were 90.7% restaurant workers suffered SHS during working with mean exposed time of 23.9?18.6 hours per week. The exposure time each week was positively associated with the symptom of dyspnea and irritated eyes, and the odds ratios were 1.03 (95%CI: 1.01~1.06) and 1.02 (95%CI: 1.01~1.03), respectively. There were 70.7% surveyed participants supported the concept of smoke-free restaurants. Conclusion The restaurant workers in Shanghai suffered from SHS seriously which result in impairment of respiratory system.
2.Secondhand Smoke Exposure among Customers in Restaurants in Shanghai

Objective To estimate the status of secondhand smoke (SHS) among restaurant customers in Shanghai and evaluate their support to smoke-free restaurants. Methods A random sample of 284 customers from 27 restaurants participated in the face to face interview. Results There were 55.8% smoking customers often smoke in the restaurants and 84.4% nonsmoking customers had suffered from secondhand smoke in restaurants. Among the customers, 73.9% supported the smoke-free law in restaurants and 49.6% of them expressed that they would more likely to go and eat in the restaurant after the smoke-free law implemented. Elder age, nonsmoking as well as college and above education background were the supporting factors for the smoke-free restaurants and the odds ratios were 1.03 (95%CI: 1.01~1.06), 6.40 (3.06~13.88) and 2.71 (1.44~5.11), respectively. Conclusion Customers have seriously suffered from SHS and most of them support the smoke-free policy. In general, the smoke-free restaurant policy will increase the intention of eating in the restaurants among customers.
3.Monitoring of Human Cytomegalovirus(HCMV) Infection in Patients with Diabetes and Its Clinical Significance

OBJECTIVE To monitor human cytomegalovirus(HCMV) infection in patients with diabetes and its clinical significance.METHODS HCMV pp65-mRNA and anti-HCMV pp65-IgM were simultaneously tested by RT-PCR using the primer sequences from HCMV pp65 genome and ELISA method was used in 727 patients with diabetes and control group.RESULTS The positive rates of HCMV pp65-IgM and HCMV pp65-mRNA in 727 patients with diabetes were 11.14% and 16.64%,respectively.There was a significant difference compared with control groups(HCMV pp65-IgM,0.87% and HCMV pp65-mRNA,2.17%)(P
4.Effects of continuous quality improvement on dietary and nutrition management in patients with chronic kidney disease

Objective To evaluate the effects of continuous quality improvement (CQI) management on nutritional status,renal function progression,and compliance of low protein diet in patients with chronic kidney disease (CKD).Methods Totally 115 CKD patients who were regularly followed up in CKD clinic services were recruited in this study.Plan,Do,Check,and Act (PDCA) method was adopted to manage the dietary of these patients for 12 months.The clinical indicators and diet compliance before and after receiving CQI management were compared.Results After receiving the CQI management,the nutritional status of patients was well maintained;meanwhile,the average hand strength and the hemoglobin,serum albumin,total cholesterol,and triglyceride levels showed no significant changes (all P > 0.05).Subjective feelings of patients were improved.The modified Subjective Global Assessment of Nutrition (mSGA) score was decreased from 7.0 (7.0,8.0) to 7.0 (7.0,7.0) (P =0.000).The estimated glomerular filtration rate (eGFR) calculated by formula of modified MDRD was decreased from (40.74 ± 14.49) to (37.94 ± 16.86) ml/(min · 1.73 m2) (P =0.000),and the average descended speed was (2.81 ±7.42) ml/(min · 1.73 m2) per year.The creatinine clearance rate had no statistical difference between pre-and post management (P =0.910),and the average descended speed was (0.19 ± 17.01) ml/min per year.The daily protein intake (DPI) and protein equivalent of nitrogen appearance rate (PNA) were both significandy descended:DPI/kg decreased from (0.79 ± 0.27) to (0.64 ± 0.15) g/ (24 h · kg) (P =0.000),and PNA/kg dropped from (1.02 ± 0.32) to (0.82 ± 0.24) g/ (24 h · kg) (P =0.000).The scores of awareness and compliance of patients on low protein diet were significantly increased after CQI management (P =0.000).Conculsion Applying CQI on dietary and nutrition management in CKD patients can maintain the good nutritional status and improve the compliance of low protein diet.
5.Impact of visit-to-visit variability in blood pressure on renal function of stage 3-4 chronic kidney diseases patients with hypertension

Objective To clarify the clinical significance of visit-to-visit variability in blood pressure (BP) of stage 3-4 chronic kidney disease (CKD) patients with hypertension.Methods One hundred and fifty-two cases of stage 3-4 CKD patients with hypertension were enrolled in the study.Variability in BP was defined as the standard deviation (SD) in BP.For each patient,SD and mean BP from BP measurements were calculated at all the visits.Correlations between the decline in estimated glomerular filtration rate (eGFR) and SD in BP were analyzed by multivariable regression.Results Visit-to-visit variability in BP was significantly associated with renal function decline (P < 0.05),in addition,baseline eGFR,baseline albuminuria and mean SBP during follow-up were significantly associated with renal function decline as well (all P < 0.05).The percentage of CCBs used in low SD of the SBP group was higher than that in high SD of the SBP (76.1% vs 58.2%,P < 0.05).Conclusion Visit-to-visit variability in BP is significantly associated with renal function decline.Drugs which can decrease the variability of blood pressure should be the first choice in the treatment of hypertension.
6.The preoperative design optimization and clinical application of the anterolateral thigh flap

Objective To prospectively summary the piercing-out position,direction,length and piercing-in position of perforator,and investigate the feasibility of preoperative design and optimization of the anterolateral thigh flap and its clinical application.Methods All 58 cases of anterolateral thigh flaps were designed and taken from the lateral thigh area from January,2014 to January,2016.Portable Doppler ultrasound was used before an operation to detect the piercing-out position (point P) of perforators.The direction and length (lower subcutaneous segment of perforators) of perforators after leaving piercing-out position were observed during the operation.And the piercing-in positions (point P') on superficial fascia and the dermis were observed.Based on this,we added line B (anterior superior spine-lateral femoral epicondyle) and line C (anterior superior spine-the middle point of superior border of patella) in the lateral and anterior side of original ilium-patella line in the thigh (line A),respectively.Results All perforators found in 58 cases before and during the operations were located on line A or between line A and line B.No perforators were found between line A and line C.Perforators walked toward the anterior medial side after leaving the muscle membrane.The perforator vascular subcutaneous segment (distance between point P and point P') was (2.02±0.23) cm.There was rectus muscle branch in the descending branch of lateral femoral circumflex artery,while no rectus muscle cutaneous branch was seen.20 cases were designed by one-line method,12 cases were designed by two-line method,while 26 cases were designed by three-line method.Conclusion Advanced three-line method is beneficial to detect of the perforators on the anterior thigh lateral region and to reduce the intraoperative injury perforator vessels at the puncture point.Clinical application of the anterior lateral thigh flap is simple and reliable.
7.Effect of acute cardiovascular disease on the healthcare costs associated with chronic kidney disease

Objective To evaluate the effect of cardiovascular disease (CVD) on the healthcare costs associated with chronic kidney disease (CKD).Methods A total of 96 patients with stage 3-4 CKD treated at CKD clinic of Beijing Friendship Hospital,Capital Medical University were enrolled in the study.Their mean age was (61.6±9.5) years including 65 male and 31 female patients.A retrospective analysis of clinical material and health claims between January 2010 and October 2010 was conducted.Firstly,patients were grouped according to median CKD-associated healthcare cost and clinical characteristics were compared between two groups.Secondly,patients were stratified into three categories based on CVD prevalence (with acute cardiovascular events,with CVD but no acute events,and without CVD),and CKD-associated healthcare costs were assessed among the groups.Finally,the potential factors influencing CKD-associated healthcare costs were evaluated by optimal scaling regression analysis.Results During January to October in 2010,median CKD-associated healthcare costs was 13960.5 yuan (interquartile range 10226.5,19667.2 yuan).In the group with higher healthcare costs,more females,greater urine albumin-creatinine ratio,more emergency observations and/or hospitalizations caused by acute cardiovascular events,higher diabetes mellitus prevalence and calcium-phosphorus products,and lower eGFR and hemoglobin levels were found (P<0.05,respectively).In contrast,the total prevalence of CVD was not significantly different between the groups (P=0.386).When grouping by CVD prevalence,significant differences of CKD-associated healthcare costs were observed only between patients with acute cardiovascular events and the other two groups (P<0.01,respectively).The median healthcare cost of the former was approximately twice as higher as that of the other two groups,and the maximal cost was also found in the acute-cardiovascular-event group.For the optimal scaling regression analysis,both emergency observations and/or hospitalizations caused by acute cardiovascular events and diabetes mellitus entered the equation,and standardized coefficients were -0.538 and -0.217 respectively (P<0.01 and P<0.05).Conclusions Emergency observations and/or hospitalizations caused by acute cardiovascular events are important factors inducing high CKD-associated healthcare costs in patients with stage 3-4 CKD.Therefore,the prevention of acute cardiovascular events may be favorable to reduce CKD-associated healthcare costs.Larger and longer-time perspective studies are required to confirm it.In addition,diabetes mellitus also influences CKD-associated healthcare costs.
8.Investigation of canine mesenchymal stem cells differentiation to vascular endothelial cell in vitro.

To induce endothelial cell, canine bone marrow-derived mesenchymal stem cells (MSCs) were separated from bone marrow by density gradient centrifugation. The isolated MSCs were induced to form endothelial-like cell in the presence of vascular endothelial growth factor (VEGF), endothelial growth factor (EGF) and so on. These results showed that the cells uniformly took on a cobblestone morphology under the light microscope, and cell nucleolus was in the middle of the cells. The cells displayed Weibel-Palade bodies under the transmission electron microscope. vWF, a specific marker of endothelial cell was positive in the cells. The above results demonstrate that MSCs may be differentiated into endothelial cells in vitro.

9.Management of acetabular comminuted fractures combined with compressive defects

Objective To discuss the therapy for acetabular comminuted fractur e combined with compressive defects. Methods From July 1997 to February 2005, 43 cases of comminuted acetabular fracture combined with compressive defect were t reated. 25 cases were obsolete, 16 fresh, and 2 malformed (90 days after injury) . 34 cases were complicated fractures with defects, and 9 simple fractures with defects. The defect volumes ranged from 3 to 9 cm3, averaging 4.5 cm3. They were treated with ATMFS (acetabular tridimensional memory fixation system) to fixate the comminuted bone fragments tridimensionally. The modified acetabular approac h, reduction of acetabular comminuted articular face, anatomical reconstruction of posterior wall of acetabulum with autogenous ilium, autogenous and artificial bone implantation and bone wax isolation were used. The follow-ups lasted from 5 to 86 months, averaging 15.7 months. Results 31 cases achieved anatomical red uction by filling up the compressive defects. 12 cases were treated by anatomica l reconstruction of posterior wall. On average, 5.3 months after operation, the injured hip joint was as good as the healthy one in 40 cases. Ischemia necrosis of femoral head occurred in 1 case, and 2 cases experienced heterotopic ossifica tion with ischemia necrosis of femoral head which led to osseous fusion of hip j oint. Conclusion The new methods for treatment of acetabular fractures with comp ressive defects elevate the reduction rate of acetabulum and femoral head, and a re effective for the functional recovery of hip joint.
10.Recent advance in interventional embolization for chronic subdural hematoma

Chronic subdural hematoma (cSDH) occurs in acute subdural hemorrhage after head trauma or converts from effusion. Traditional treatment is based on conservative treatment and surgical drainage. The effective rate of conservative treatment is only 3%-18%; even with drilling drainage treatment, the recurrence rate is as high as 33%. Recently, the middle meningeal artery embolization technique based on pathological analysis can greatly reduce the recurrence rate, and the operation is simple and curative effect is exact. This article reviews the pathogenesis of cSDH and progress of interventional therapy.
