Objective To observe some current dilemmas in the treatment of extremely preterm infants and to explore the method to solve the problems.Methods To analyze the problems with Bioethics.Results To advance some methods for solving the bioethical problems associated with medical staffs,parents,and the society.Conclusions Medical staffs should keep studying professional knowledge,understand the advances in the treatment of extremely preterm infants,improve the ability and skills to inform parents,diagnose and treat according to related guidelines,respect the rights and decision of the parents,and provide scientific guidance.The parents should be actively involved in medical activities and informed the rights,responsibilities in the treatment of their infants.Society should also take into consideration the interests of this special group in the field of insurance with the development of economy.
2.Clinical Observation of Herbal Foot-bath plus Acupoint Application in Improving Sleep Quality Prior to Stomach Cancer Surgery

Objective To investigate the effect of herbal foot-bath plus acupoint application on sleep quality of stomach cancer patients before surgery. Method Eighty eligible patients who were going to receive stomach cancer surgery were randomized into an observation group and a control group, 40 in each group. The control group was given psychological comfort plus switching off lights on time, while the observation group was additionally given herbal foot-bath plus acupoint application before the surgery. The sleep quality on the night before surgery was observed. Result There were significant differences in comparing sleep latency, night wakening, total sleep duration, total sleep quality, daily body function and total score (P<0.05). Conclusion Herbal foot-bath plus acupoint application can enhance the effective sleep duration and sleep quality of patients before stomach cancer surgery.
3.The Specific High Expression of Apoptosis-Inducing BAX Gene Driven by Human Cyclooxygenase-2 Promoter in Ovarian Cancer Cell Line

Objective: To verify that COX-2 promoter can drive its downstream genes specifically in COX-2-positive o-varian cancer cells; Moreover, comparing with CMV promoter, analyze the transcript efficiency of COX-2 promoter. Methods: Contacting the recombinant plasmids named COX-2-BAX and CMV-Luc. After transient transfection liposome-mediated with the plasmids COX-2-Luc and CMV-Luc, respectively, the expression of Luciferase reporter gene was measured in COX-2-positive ovarian cancer cell line-SKOV3 and COX-2-negative colon cancer cell line-SW480. SKOV-3 and SW480 were transfected with COX-2-BAX and CMV-BAX in the same way, respectively. The apoptosis rates were measured through flow cytometry. Results: The recombinant plasmids named COX-2-BAX and CMV-Luc were constructed successfully. The expression efficiency of reporter gene was 1554 ? 86. 5 in SKOV3 and 53. 7 ? 10.9 in SW480 after 24 hours transfected with phPES2, 9851. 7 ? 129. 5 in SKOV3 and 8831. 0 ? 167. 3 in SW480 after 24 hours transfected with CMV-Luc in the same way. The apoptosis rate was 10.4% in SKOV3 and 3.7% in SW480 after transfected with COX-2-BAX, 21.7%in SKOV3 and 25. 6% in SW480 after 36 hours transfected with pcDNA3-BAX in the same way. Conclusions: COX-2 promoter can drive its downstream genes specifically in COX-2-positive ovarian cancer cell lines, but its expression efficiency wasmarkedly lower than CMV promoters. With proper modification, COX-2 promoter is expected to be useful in gene therapy of ovarian cancers.
4.Reproducibility and influencing factors of 31p MR spectroscopy in rabbit liver with two-dimensional chemical shift imaging

Objective To investigate the reproducibility and influencing factors of relative quantification of phosphorus metabolites with two-dimensional chemical shift imaging (2D CSI) in rabbit liver. Methods Using 2D CSI MRS, 500 ml phosphate (NaH2PO4) solution phantom with 0. 05 mol/E concentration and one healthy rabbit were scanned 30 times respectively in one day and rescanned 30 times in the next day, and the stability of MR scanner and reproducibility of within-run and between-days in the same individual were analyzed. Each of thirty rabbits was scanned and rescanned one time respectively in different days, and the reproducibility of between-days in one group was analyzed. The data were statistically analyzed with t tests. Results (1) Phosphate solution phantom had a good reproducibility of within-run with the coefficient variation (CV) of 4. 92% and 5. 12% respectively in different two days. No significant change of phosphorus metabolites was detected in between-days, which was 16. 68 ± 0. 82 and 16. 56 ± 0. 85 respectively(t = 0. 665, P > 0. 05 ). (2) The CV of metabolites in one healthy rabbit ranged from 8. 04% to 34. 13%. Among the metabolites, β-ATP had the best reproducibility with the CV less than 10%. PME was 0. 88 ± 0. 28 and 0. 88 ± 0. 30, PDE was 4. 35 ± 0. 66 and 4. 35 ± 0, 66, Pi was 0. 95 ± 0.30 and 0.97±0.28, α-ATP was 5.58±0.60 and 5.61±0.61, β-ATP was 2.70±0.22 and 2.71± 0. 22, γ-ATP was 2. 20±0. 63 and 2. 18±0.44 respectively, no significant changes of metabolites were detected in between-days( P >0. 05 ). (3) The CV of metabolites in 30 healthy rabbits ranged from 8.48% to 36. 21%. Among the metabelites, β-ATP had the best reproducibility with CV less than 10%. PME was 0. 84 ± 0. 30 and 0. 79 ± 0. 28, PDE was 4. 29 ± 0.72 and 3.94 ± 0. 84, Pi was 0. 91 ± 0. 28 and 0. 92 ± 0. 31, α-ATP was 5.65±0. 66 and 5. 36±0. 60, β-ATP was 2. 71±0. 23 and 2. 66±0. 25, γ-ATP was 2. 07±0. 29 and 1.99±0. 37 respectively, no significant changes of metabolites were detected in between-days (P > 0. 05). Conclusions The relative quantification of hepatic β-ATP may be most reliable among the phosphorus metabolites for rabbit liver because of its good reproducibility and small CV. The quantification of phosphorus metabolites by 31p MRS with 2D CSI in rabbit liver is affected by many factors.
5.Vascular disruption effects of HepG2 tumor by microbubble desruption-re-desruption

Objective To investigate the change of HepG2 liver tumor perfusion after microbubble enhanced ultrasound cavitation treatment and observe the related pathological injury.Methods Twenty eight Balb/c(nu/nu) nude mice transplanted subcutaneous HepG2 tumor were divided into three groups randomly,including the microbubble enhanced ultrasound cavitation group,the ultrasound group and the sham group.Microbubble enhanced ultrasound cavitation treatment was performed by 0.1 ml microbubbles intravenous injection combined with pulse ultrasound emission in experimental group,while in control groups only ultrasound exposure or microbubble injection were applied.The perfusion of tumors was imaged using contrast-enhanced ultrasonography before and after treatments.Time-intensity curve and peak intensity were analyzed.The tumors were then harvested for histological examination.Results The perfusion of HepG2 tumors almost vanished immediately after treatment in experimental group,with the peak intensity reduced from (26.9 ± 10.9)% to(8.2 ± 5.8)% (P <0.05).There was no significant changes before and after treatments (P > 0.05) in the two control groups.Histological findings were disruption of the endothelia,significant hemorrhage and increased intercellular fluid.Conclusions Microbubble-enhanced ultrasound cavitation can significantly reduce tumor blood perfusion and disrupt tumor vascularture.This new ultrasound therapy can potentially become a new physical anti-angiogenetic therapy for liver tumor.
7.Study on extracting process of Fumai Decoction

Object To optimize the extraction conditions of Fumai Decoction. Methods Uniform design test was used, and the contents of glycyrrhizinic acid and ginsenoside Rb 1 in extracted solvent were determined by HPLC. Results The ethanol volume and the extracting temperature in the uniform test impacted the test remakably. The best extraction process of Fumai Decoction as follows: the raw drugs were extracted by 10 times of 50% ethanol at 60 ℃ for 4 hours. Conclusion Glycyrrhetinic acid and ginsenoside Rb 1 contents in Fumai Decoction are higher according to the above extraction process.
8.Measurement of Urinary S100B Protein Concentrations for the Early Evaluation of Brain Damage in Preterm Infants

Objective: To investigate the changes of urinary S100B protein concentrations and their relationship with brain damage in preterm infants there of . Methods: The urinary S100B protein of 84 preterm infants and 26 full term infants, which were used as control, were measured at 24 h and 120 h after birth. At the same time, routine clinical observations, neurologic patterns and ultrasound screens were recorded. The value of urinary S100B protein and brain damage were evaluated in preterm infants with different gestational age. Results: The differences of urinary S100B protein were statistical significance between the different gestations. The levels of urinary S100B protein were higher in preterm infants, whose gestations were lower than 32 W, than those of other groups. The levels of S100B protein were significantly higher in samples of 27 peri-intraventricular hemorrhage (PIVH) and 3 peri-ventricular leukomalacia(PVL) than those in samples without brain damage( P < 0.05). The S100B levels were significantly higher in urine of 10 preterm infants with polycethemia than those in infants without brain damages. In addition, the S100B levels were different in urine of preterm infants with different prognosis. The S100B levels were significantly higher in urine of infants who died or deteriorated than those of others(P < 0.05). Conclusion: There is an evident trend of decrease in urinary S100B protein concentration with increasing gestational age. It will be helpful to identify preterm infants with PIVH,PVL and high risk of brain damages by measurement of S100B protein in urine early after birth, which indicates further inspection, provides protective treatment and enhances follow up.
9.The clinical application of the Fresnei press-on prism to children ocular torticollis

Objective To evaluate the clinical application and the therapeutic effect of the Fresnel press-on prism for children ocular torticollis.Design Retrospective case series.Participants 64 patients with ocular torticollis.Methods All the patients with ocular torticollis wore Fresnel press-on prism on glasses for 6-12 months to correct small angle vertical paralytic heterotropia.Main Outcome Measures Changes of abnormal head position and stereoscopic vision when naked and glasses-wearing.Results Fresnel press-on prism could provide an improvement in the correction of symptoms in children ocular torticollis.58 patients(90.6%) had improvement of abnormal head position and three-grade binocular vision.All 55 cases(100.0%)matching examination after wearing prism showed simultaneous perception,52 eases(94.5%)had fusion,and 50 cases(90.9%)had stereoscopic vision.Chi square test showed significant difference from pre-wearing prism(P

T cell immunoglobulin and mucin domain 3 (Tim-3) is well known to negatively regulate T cells responses, but its role in burn-induced T cells immune suppression remains unclear. In the present study, in order to identify the relationship between Tim-3 expression and post-burn T cells immune suppression, C57BL/6 mice were subjected to burn injury or sham injury, and the liver and spleen were harvested at the day 1 after operation. The expression level of Tim-3 on hepatic or splenic T cells and the functional properties of Tim-3(+) T cells were evaluated. It was found burn injury induced dramatically elevated Tim-3 expression on both hepatic and splenic CD4(+) and CD8(+) T cells in contrast with the post-burn depletion of T cells. Furthermore, Tim-3 expression was correlated with the suppressive phenotype of T cells following burn injury, including increased expression of anti-inflammatory cytokine IL-10, decreased expression of pro-inflammatory cytokines IFN-γ and TNF-α, reduced T cell proliferation and elevated co-expression of Tim-3 and PD-1. Moreover, Tim-3(+) T cells subsets were more prone to spontaneous apoptosis than Tim-3(-) T cells subsets. Our findings reinforce the idea that the up-regulated expression of Tim-3 on T cells after burn injury plays an important role in the development and maintenance of burn-induced T cell immune suppression.
