1.Application of new techniques of medical hyperbaric oxygen cabin in the reformation of old cabin

Objectives:According to the present structural characteristics, the safety and the functional states of the old hyperbaric oxygen cabin used in our hospital for about 30 years, new techniques were rationally used in the reformation of the old cabin. And to keep the cabin in the first level in the country for its advancement, preeminence and technical contents in the safety and utility. Methods:Correctly understand the present national standards and regulations, carefully design the reformation plan after a thorough investigation, research and proof. And the reformation construction unit was chosen by public bidding after approved by the competent authorities of the local state. The local competent authorities and the specialized personnel in the hospital were invited during the reformation construction to exam and supervise on the spot. After the accomplishment of the reformation construction, the provincial and city competent authorities and the corresponding experts in this specialized field were invited to check and accept. Results:The check and acceptance report showed that the reformation construction was meticulous in the design and construction, the technological level was high, and several new advanced techniques had been used. The hyperbaric oxygen cabin after the reformation had a high safety and comfort. Some of the indexes had exceeded the present national standards, and achieved the national advanced standards for large medical hyperbaric oxygen cabins. Conclusions:Old cabins can be fully reformed into first level cabins in technical advancement and safety.
2.Effects of EGF and bFGF on expression of microtubule-associated protein tau and MAP-2 mRNA of mononuclear cells derived from human umbilical cord blood

AIM: To explore the regulatory effects of cytokines such as EGF, bFGF on expression of neural-specific molecules in mononuclear cells (MNCs) cultured in H-DMEM medium. METHODS: The umbilical cord blood samples were collected from health puerperal natural delivery. The mononuclear cells were isolated by centrifugation over Lymphoprep and planted in T-75 flasks containing H-DMEM medium with or without addition of EGF, bFGF or EGF plus bFGF at a final concentration of 20 ?g/L, respectively. Phenotypic changes were monitored by inverse phase-contrast microscopy. Tau and MAP2 mRNA were determined by reverse-transcript polymerase chain reaction (RT-PCR). Tau and MAP2-positive cell were determined by immunocytochemistry. RESULTS: The expression of tau protein mRNA was negative in uncultured cells, but MAP2 mRNA was positive. In cultured cells, tau protein mRNA expressed positively, MAP2 mRNA expression was upregulated by EGF+bFGF, EGF or bFGF compared with control group (no cytokines). The upregulatory capability of EGF+bFGF to MAP2 mRNA expression was stronger than that of EGF or bFGF alone. The same upregulatory tendency was noted in tau mRNA expression. In the group of control, bFGF, EGF, EGF+bFGF, the rate of MAP2-positive cells was 14.4%, 19.6%, 25.6%, 33.5%, respectively. Tau protein-positive cells were 13.5%, 15.3%, 21.4%, 29.8%, respectively. Under inverse microscopy, the freshly isolated MNCs were small and round, after culturing, the cells became larger with some big, long cytodenrites in the EGF+bFGF group, with 1 or 2 threadlike cytodenrites in the EGF group, or with some short multi-dendron like-astrocyte in the bFGF group and control group, but the number of astrocyte-like cells in the control group was less than that in bFGF group. CONCLUSION: MNCs derived from human umbilical cord blood cells express some neural specific molecules and are upregulated by cytokines, especially EGF and bFGF, which have the synergetic action. [
3.Learning curve of Tianji orthopaedic robot in assisting percutaneous kyphoplasty for osteoporotic vertebral compression fractures

Objective To explore the learning curve of the Tianji orthopedic robot in assisting percutaneous kyphoplasty (PKP) for osteoporotic vertebral compression fractures (OVCF).Methods A retrospective study was conducted of the consecutive 44 OVCF patients (71 fractured vertebrae) who had been treated with PKP assisted by the Tianji orthopedic robot from May 2018 to March 2019 at Department of Ortbopedics,The First Hospital of China Medical University.They were 2 men and 42 women,aged from 55 to 89 years (average,68.6 years).Their operations were performed by the same surgical team.The patients were divided into 4 even groups (n =11) according to the date of operation.The accuracy of robot-assisted puncture,total operation time,robot operation time,PKP operation time and complications were statistically analyzed and compared between the 4 groups.Natural logarithm regression analysis of total operation time,robot operation time and PKP operation time was conducted to determine the association between operation time and operation number [y =aln(x) + b,x indicating operation number and y operation time].Results The 4 groups were comparable due to insignificant differences in age,gender or operative segment between them (P > 0.05).The total rate of accurate puncture was 95.8% (68/71).The rates of accurate puncture were 92.9% (13/14),94.4% (17/18),94.7% (18/19) and 100% (20/20) for groups 1,2,3,and4,respectively,with no significant differences between groups (P > 0.05).The total operation time for the 4 groups was respectively 56.6 ± 6.6 min,46.4 ± 8.5 min,40.2 ± 4.7 min and 40.4 ± 5.7 min and the robot operation time for the 4 groups was respectively 32.1 ±4.9 min,23.8 ±5.1 min,18.9 ±2.7 min and 19.6 ± 5.3 min,showing significant differences between the 4 groups (P > 0.05) and a significant difference between groups 1 and 2 versus groups 3 and 4 (P > 0.05) but no significant difference between group 3 and 4 (P > 0.05).The natural logarithm regression analysis showed that the total operation time [y =-7.2861n(x) + 66.616,R2 =0.4889,P < 0.001] and robot operation time [y =-5.6621n(x) +39.763,R2 =0.5196,P < 0.001] decreased significantly with the increase of operation number but became stable in groups 3 and 4.There was no significant difference in PKP operation time among the 4 groups (P > 0.05).The main intraoperative complication was leakage of bone cement in the anterior vertebral space and intervertebral space,with a total incidence of 8.5% (6/71),showing no significant difference among the 4 groups (P > 0.05).Conclusions The puncture accuracy of the Tianji orthopedic robot in assisting PKP for OVCF may not change with the increased operation number.The total operation time and robotic operation time decrease with the increased operation number.The learning curve of the Tianji orthopedic robot becomes stable very fast,indicating the surgical technique is easy to learn.
