1.Modified Cantwell-Ransley procedure for epispadias repair in 37 patients

Objective To present the outcomes of modified Cantwell-Ransley procedure in 37 patients with isolated epispadias and exstrophy complex.Methods A total of 37 patients underwent modified Cantwell-Ransley epispadias repair.The median age at surgery was 6.6 years (range 1-32).Of the 37 patients,13 patients had isolated penopubic epispadias and 24 had bladder exstrophy with no previous attempts at epispadias repair.Nine patients with exstmphy complex underwent complete primary repair,while other 15 cases received urinary diversion.Results The patients were followed up for 6 months to 3 years (mean,12 months).All patients had normal conical symmetric glans/corpora and an orthotopic meatus.The complications included glans/corporal ischemia (1 case),urethral fistula (3 cases),and foreskin infection (1 case).Conclusions Modified Cantwell-Ransley procedure is safe and effective for epispadias repair.Cosmetic appearance and functional outcomes of the penis are encouraging.
2.Diagnostic Value of the Direct Low-field MR Arthrography in the Meniscus Degeneration and Tear of Knee

Objective To evaluate the diagnostic value of the direct MR arthrography in the meniscus degeneration and tear of knee.Methods Thirty subjects undefined by MR imaging having meniscus degeneration and tear of knee underwent MR arthrography.The diagnostic accurate rates in meniscus degeneration and tear by using MR imaging and MR arthrography were evaluated according to the arthroscopy results.Results The diagnostic accurate rates of meniscus degeneration and tear were 80% and 96.7% by conventional MRI scan and MR arthrography respectively.Conclusion The direct low-feild MR arthrography can improve the diagnostic accuracy of meniscus degeneration and tear of knee.
3.The relationship between the degree of intracranial and extracranial arterial stenosis and the plasma level of homocysteine in patients with ischemic cerebrovascular disease

Objective To study the relationship between the degree of intracranial and extracranial arterial stenosis and the plasma level of homocysteine ( Hcy ) in patients with ischemic cerebrovascular disease .Methods 96 patients with ischemic cerebrovascular disease were checked by DSA .The distribution of stenosis in the studied cerebral arteries was analyzed .According to the degree of stenosis ,all 96 patients were divided into some groups ,at the same time Hcy level was detected ,and the correlation between Hcy level and intracranial and extracranial artefial stenosis were analyzed .Results 35 of the 96 patients had normal findings in angiography ,and 61 cases had occlusive lesions of different degrees .The mean plasma Hcy concentration in patients with intracranial and extracranial arterial stenosis was 19.26 (14.79~26.13)μmol/L,which was significantly higher than 11.60(8.8~15.11)μmol/L in the patients with no arterial stenosis (P<0.05).There was a significant positive correiation between Hcy level and cerebral artery stenosis degree(r=4.126,P<0.05).Conclusion The plasma Hcy level was positively correlated with intracranial and extracranial arterial stenosis in patients with ischemic cerebrovascular disease ,incidence and nar-row-degree of cerebral artery stenosis are all along with the increasing of Hcy level .
4.Distribution and Antibiotic Resistance of Escherichia coli in Hospital

OBJECTIVE To understand the distribution and antibiotic resistance of E.coli isolated from our hospital and offer suggestions for clinic.METHODS The antibiotic susceptibility to 21 kinds of antibiotics were tested by K-B method and analyzed by Whonet software.RESULTS The antibiotic resistance of 301 E.coli strains to ampicillin,quinolones,piperacillin,gentamicin,cefazolin,cefuroxime,ceftriaxone,ceftazidime,aztreonam,cefepime, ampicillin/sulbactam were 83.39%,69.44%,62.79%,44.19%,42.86%,36.88%,35.22%,31.23%,29.90%,29.57% and 26.91%,respectively.CONCLUSIONS E.coli isolated from our hospital show high resistance to several kinds of antibiotics.The drugs should be choosed reasonably according to their antibiotic suscepitibity results.
5.Cell suspension and level of IL-17 in bronchoalveolar lavage fluid (BALF) of children with labor pneumonia combined with mycoplasma pneumonia and its clinical significance

Objective To explore cell suspension and level of IL-17 in bronchoalveolar lavage fluid (BALF) of children with labor pneumonia combined with mycoplasma pneumonia and its clinical significance. Methods 87 children with labor pneumonia who had been done BAL were divided into CPIS ≥ 6 group and CPIS < 6 group ac-cording to the clinical pneumonia infection score (CPIS). Meanwhile, according to the results of MP-DNA, we grouped the children above into combined with mycoplasma pneumonia infection group and no mycoplasma pneumo-nia infection group, then combined with mycoplasma pneumonia infection group into group of MP-DNA:103~106 and group of MP-DNA: 106~. ELISA method was used to detect IL-17 level, HE staining and microscopy to sort cells, PCR detect MP-DNA. Results The percentage of neutrophils and lymphocytes in BALF are not related to that in blood ( > 0.05). The percentage of columnar epithelial cells in CPIS < 6 group were higher than CPIS ≥ 6, how-ever, macrophagocyte was lower ( < 0.05). The percentage of neutrophils are related with both MP-DNA ( = 0.48,= 0.000) and IL-17 ( =0.49, =0.000). Conclusions The composition of BALF is more appropriate to react the circumstance of lobar pneumonia. When lobar pneumonia occurs, columnar epithelial cells are damaged, macrophagocyte gathered to participate in the inflammatory response. In Mycoplasma pneumonia infection group, IL-17 raises neutrophils to resist infection, NE%in BALF can indirectly reflect the mycoplasma infection degree.
6.Research and Application of MRI Correction in Stereotactic Radiotherapy

Objective To discuss the feasibility of stereotactic radiotherapy plan designing based on MRI. Methods Based on the study of the serious geometric distortion of MRI while MRI was used in the condition of position by the experiment, how to deal with geometric distortion of MRI for patient positioning by the means of software was studied deeply. The basic thought of correction arithmetic for patient positioning images, of which the corrected data being used to correct patient images was created by primitive sparse corrected data calculated from design data and sampling data was brought forward. Supported by arithmetic research, the MRIC software system was designed and implemented. At last, the experiment model MRI was dealt with the MRIC system. Results The correction arithmetic was proved valid by the result of the experiment model MRI can be corrected back to normal state with MRIC system, which implemented the correction arithmetic. Conclusion It's possible that MRI would be used for positioning and plan designing in SRT.
7.The effect of recombinant human endostatin combined with 5-Fluorouracil on the growth of xenografted gastric cancer in nude mouse

Objective This experiment was to identify the influence of recombinant human endostatin (rhES) combined with 5-FU on the growth of a xenografted gastric cancer in nude mouse. Methods After the establishment of the model, mice were divided into 4 groups (six in each) receiving respectively, for consecutive 10 days: NS, 5-FU (10 mg/kg) intraperitoneally; rhES (2 mg/kg); and 5-FU + rhES. On 15th day tumor volume was measured. The expression of VEGF, bFGF, VEGF-C, PCNA, bcl-2 and apoptosis index (AI) were examined. 8.Human umbilical cord blood mononuclear cell transplantation for treatment of radioactive premature ovarian failure in nude mice

BACKGROUND:Stem cel transplantation, in recent years, has become a preferred treatment for premature ovarian failure. Umbilical cord blood mononuclear cel s containing a large number of mesenchymal stem cel s, immature stem/progenitor cel s, and endothelial progenitor cel s can be used as an important source of stem cel transplantation.
OBJECTIVE:To study the therapeutic efficacy of human umbilical cord blood mononuclear cel transplantation on radioactive premature ovarian failure in nude mice.
METHODS:120 female BALB/C nude mice were randomly divided into four groups:blank control group without any intervention;model group, intravenous transplantation group, and in situ transplantation group exposed to 60Coγrays, 0.5 Gry per day, for 30 days. After 30 days, premature ovarian failure models were made in the latter three groups. Then, nude mice in the latter three groups were given bilateral ovary injection of 10 μL DMEM, tail vein injection of 10 μL human umbilical cord blood mononuclear cel s (1×1013/L), and bilateral ovary injection of 10 μL human umbilical cord blood mononuclear cel s (1×1013/L), respectively. Thirty days after cel transplantation, serum levels of estradiol, fol icle hormone, luteinizing hormone, inhibin B and vascular endothelial growth factor were detected, cel apoptosis in the ovary tissue and cel survival were observed pathological y.
RESULTS AND CONCLUSION:After modeling, the serum levels of estradiol, inhibin B and vascular endothelial growth factor were significantly reduced (P<0.01), while fol icle hormone and luteinizing hormone levels increased (P<0.01). After transplantation, these indexes were al improved in the in situ transplantation group (P<0.01), and reduced fol icle hormone and luteinizing hormone levels were visible in the intravenous transplantation group (P<0.05, P<0.01). Compared with the model group, in situ transplantation and intravenous injection of human umbilical cord blood mononuclear cel s could effectively reduce cel apoptosis in the ovary tissue (P<0.05), and transplanted cel s were able to survive in the ovary of nude mice. Al these findings show human umbilical cord blood mononuclear cel transplantation do have curative effects on premature ovarian failure in nude mice through the inhibition of apoptosis and the regulation of hormone secretion.
9.Intraoperative combined usage of fibercholedochoscopy and ultrasound for the treatment of intrahepatic cholelithiasis

ObjectiveTo evaluate the intraoperative combined application of fibercholedochoscopy and B-type ultrasound for the treatment of intrahepatic biliary duct stones. Methods In our department 65 cases of intrahepatic biliary duct stones treated by fibercholedochoscopy and followed up for ten years were used as control. Results were compared with that of 78 cases undergoing combined management.ResultsFollow-up at first 5 years showed that patients treated with fiberbiliascopy had a much lower stone recurrent rate than that without, though difference became less significant within second 5 years. Intraoperative combined application of fibercholedochoscopy and B-ultrasound futher decreases the stone recurrence rate. ConclusionsCombining fibercholedochoscopy with B-type ultrasound intraoperatively enables the operator to locate intrahepatic biliary duct stones, reduces retained stone rate and stone recurrence.
10.ImprovingMycoplasma ovipneumoniae culture medium by a comparative transcriptome method

Mycoplasma ovipneumoniae (Mo) is difficult to culture, resulting in many difficulties in related research and application. Since nucleotide metabolism is a basic metabolism affects growth, this study conducted a “point-to-point” comparison of the corresponding growth phases between the Mo NM151 strain and theMycoplasma mycoides subsp. capri (Mmc) PG3 strain.The results showed that the largest difference in nucleotide metabolism was found in the stationary phase. Nucleotide synthesis in PG3 was mostly de novo, while nucleotide synthesis in NM151 was primarily based on salvage synthesis. Compared with PG3, the missing reactions of NM151 referred to the synthesis of deoxythymine monophosphate. We proposed and validated a culture medium with added serine to fill this gap and prolong the stationary phase of NM151. This solved the problem of the fast death of Mo, which is significant for related research and application.
