Singapore medical journal 2016;57(1):45-50
A 31-year-old male motorcyclist presented with prepatellar swelling of the left knee after a collision with a car. Magnetic resonance imaging of the knee showed no bony or ligamentous injury to the knee. Instead, a well-defined, thin-walled, T2-weighted hyperintense fluid collection with internal septations was identified in a prefascial location overlying the left patella and patellar tendon. The findings were in keeping with those of a Morel-Lavallée lesion, a closed internal degloving injury. Morel-Lavallée lesions are occasionally encountered after a blunt soft-tissue trauma. The presentation and imaging features are discussed.

2.Influence of rice and added sugar intakes on fasting plasma glucose and triacylglycerol levels amongst a population sample of Malaysian adults

International e-Journal of Science, Medicine and Education 2015;9(1):26-31
Introduction: A recently published meta-analysis
showed that each additional serving of rice increased
risk of type 2 diabetes mellitus (DM) by an alarming
11%. We investigated whether this phenomenon is seen
in the Malaysian population by studying the effect of rice
intake and added sugar consumption on fasting plasma
glucose (FPG) and fasting triacylglycerol (TAG).
Methods: Ninety subjects (60 females, 30 males, aged
30-70 years), adequate to detect a weak-to-moderate
Pearson correlation of r=0.26 at a=0.05 and power=
0.80, were recruited by convenience sampling from six
communities in the Klang Valley, Malaysia. Fasting blood
samples were collected by finger-prick and analysed for
FPG (AccuCek, Roche) and TAG (Accutrend, Roche).
Macronutrient intakes, including rice, were obtained
by a single interview using a previously-evaluated food
frequency questionnaire (FFQ) and quantitated as grams
by the DietPLUS V2 programme. Added sugar intakes
by subjects were estimated using an Added Sugar Intake
excel programme.
Results: Rice contributed to 85% of dietary
carbohydrates, accounting for 41.8 % kcal of the average
1750- kcal diet. Rice intakes or added sugar consumption
did not have a significant correlation (p>0.05) with
FPG nor fasting TAG. Added sugar consumption, which
averaged 44g/person/day (5% kcal) was markedly lower
than the 137g/person/day reported elsewhere for the
Malaysian population.
Conclusion: High consumption of rice as a risk factor of
type 2 DM was not indicated in the present study. Since
white rice consumption varied 10-fold in the present
subjects, the reduction in daily intake of this staple food
represents a feasible option for cutting back on calorie
intake for overweight or obese individuals.

INTRODUCTION:
We aimed to describe the clinical characteristics, diagnostic challenges, treatment patterns and outcomes of uveal melanoma (UM) in a tertiary care centre.
METHODS:
This is a retrospective case series of 11 consecutive patients with UM who were managed in a tertiary referral centre between 2002 and 2017. Epidemiological, clinical, pathological and radiological characteristics were reviewed. Classification of choroidal melanoma as small, medium or large was based on the criteria established by the Collaborative Ocular Melanoma Study.
RESULTS:
Mean age at presentation was 42.9 (range 27‒67) years. In 7 (64%) patients, a definitive diagnosis of UM was made after a mean follow-up period of 6.4 (range 1‒17) months. There were one, six and four patients with small-, medium- and large-sized choroidal melanomas, respectively. Treatment was enucleation in 5 (45.5%) patients, plaque brachytherapy in 4 (36.4%) patients, transpupillary thermotherapy in 1 (9.1%) patient, and observation in 1 (9.1%) patient. Median follow-up was 29 months. Metastatic disease developed in 5 (45.5%) patients at the mean age of 46.6 (range 38‒56) years, with median overall survival of 20 months. Genetic mutations in three patients were monosomy 3 (n = 2), and gain of 3q and 8q (n = 1).
CONCLUSION
Our study supports the finding that UM in Chinese and Asian Indian patients presents at a younger age than in Caucasians. Although it is rare, ophthalmologists should remain mindful of this life-threatening disease. We propose establishing a national and regional registry for ocular tumours with genetic information to characterise the disease spectrum in Southeast Asia.

The Health Promotion Board (HPB) has developed the Clinical Practice Guidelines (CPG) on Falls Prevention among Older Adults Living in the Community to provide health professionals in Singapore with recommendations for evidence-based assessments and interventions for falls prevention. This article reproduces the introduction and executive summary of the key recommendations from the HPB-MOH CPG on Falls Prevention among Older Adults Living in the Community for the information of SMJ readers. The chapters and page numbers mentioned in the reproduced extract refer to the full text of the guidelines, which are available from the Health Promotion Board website: http://www.hpb.gov. sg/cpg-falls-prevention. The recommendations should be used with reference to the full text of the guidelines. Following this article are multiple choice questions based on the full text of the guidelines.
