1.Preliminary study on the relationship between thrombocytosis and gastric cancer

Objective To explore the relationship between thrombocytosis and gastric cancer at the points of clinicopathological stage and prognosis.Methods The clinical data of 156 gastric cancer patients from January 2005 to January 2007 were retrospectively analyzed,and the status of patients was obtained through telephone calls.The frequency of patients with thrombocytosis in different clinicopathological stages was analyzed,and comparison of survival time was made between normal PLT groups and thrombocytosis groups.Results 20.5% patients with gastric cancer were accompanied with thrombocytosis in 156 cases,and the frequency of patients with thrombocytosis in Ⅰ,Ⅱ,Ⅲ,Ⅳ stage were 4.8%,21.7%,24.4%,30.4%,and the difference was statistically significante (x2 =8.768,P =0.003).Patients with thrombocytosis had a shorter survival time than those with normal platelet counts (one year survival rate:59.4% vs 81.5%,x2 =6.984,P =0.008 ; three years survival rate:34.4% vs 63.7%,x2 =8.968,P =0.003).Conclusion In gastric cancer,thrombocytosis is associated with TNM stage.The gastric cancer patients with thrombocytosis have poorer prognosis than those without thrombocytosis.
2.UTILIZATION OF PROTEIN IN VEGETARIAN AND MIXED DIET BY HUMAN ADULTS

Vegetarian and mixed diets, supplying about 3000 Cal. and 8,5 gms. of protein daily, were given alternately to 6 adult men of 22-34 years old. In the vegetarian diet, composed of cereals, legumes and vegetables, about 20% of the total protein were supplied by the legumes. In the mixed diet, the legumes and part of the cereals of the above were replaced by meats, eggs and milk, furnishing about 50% of the total protein. The experiment consisted of 5 consecutive periods of 10 days each in the first three periods and 5 days each in the last two.Urinary nitrogen excretion of the experimental subjects during the period on vegetarian diet was lower than that on mixed diet. When the diet was changed from vegetarian to the mixed ration, urinary nitrogen remained low in the first two days and increased from the third day. However, when the change of diet was reversed, there was an immediat'e decrease of nitrogen excretion. The nitrogen balance was in favour of the vegetarian diet.The essential amino acid contents of the vegetarian diet were lower than that of the mixed diet. Therefore, better utilization of the protein in the vegetarian diet cannot be satisfactorily explained by essential amino acid index. With adequate caloric intake, 85 gms. of protein supplied daily either by vegetarian or by mixed diet may maintain nitrogen balance and protein storage in adult man.
3.A dosimetric comparison of volumetric modulated arc therapy with intensity modulated radiation therapy for head and neck cancer

Objective:To compare the dosimetric differences between volumetric modulated arc therapy (VMAT) and intensity modulated radiation therapy (IMRT) for head and neck cancer.Methods:From January 2019 to December 2019, 46 cases of patients with head and neck cancer confirmed by pathology in the First Affiliated Hospital of Anhui Medical University were selected. All patients underwent CT-simulation in supine position. Planning target volumes (PTVs) were delineated for two dose levels of PTV70 and PTV54 in 35 equal fraction. Simultaneous integrated boost plan was generated for all patients and optimized with both techniques, VMAT and IMRT with similar planning objectives. The dosimetry differences between the two plans were compared.Results:The conformity index (CI)95% of PTV70 in VMAT plan was higher than that in IMRT plan (0.91±0.02 vs. 0.86±0.06), and the difference was statistically significant ( t=4.933, P=0.004). The homogeneity index (HI)95% of PTV54 in VMAT plan was superior to that in IMRT (0.09±0.04 vs. 0.26±0.02), and the difference was statistically significant ( t=4.548, P=0.038). The spinal cord D 1% of VMAT was lower than that in IMRT (37.62±4.34 vs. 40.93±7.45), and the difference was statistically significant ( t=2.615, P=0.045). The dose of left parotid gland in VMAT was (21.28±8.13) Gy and the dose of right parotid gland was (22.39±7.42) Gy, which were slightly lower than those in IMRT [(22.73±11.42) Gy and (24.25±7.91) Gy], but the differences were not statistically significant ( t=0.703, P=0.322; t=1.134, P=0.315). Compared with IMRT, VMAT could significantly reduce the number of monitor units (521±112 vs. 2 129±564), and could significantly shorten the treatment time of patients [(2.12±0.39) min vs. (9.18±2.62) min], and the differences were statistically significant ( t=18.957, P<0.001; t=18.213, P<0.001). Conclusion:The dose distribution of VMAT technology is better than IMRT, the dose of organ at risk is reduced, the monitor unit is lesser, and the treatment time is shorter.
4.Acupuncture for Chronic Stable Angina:a Systematic Review and Meta Analysis

Objective This study aimed to conduct a systematic review and meta-analysis of randomized controlled trials(RCTs)assessing the clinical efficacy of acupuncture for chronic stable angina(CSA).Methods PubMed,The Cochrane Library,Embase,CNKI,VIP,WanFang Data,CBM databases,ClinicalTrial.gov and Chinese Clinical Trial Registry were electronically searched to collect randomized controlled trials(RCTs)of acupuncture for CSA and only RCTs that included acupuncture as the sole or combination of acupuncture in the treatment of CSA.The retrieval period was from the establishment of the database to May 19,2022.The primary outcome measure was the frequency of angina attacks,and the secondary outcome measures were angina efficacy,total effective rate of ECG improvement,nitroglycerin use after treatment,patient-perceived overall effectiveness,performance on the Six-Minute Walk Test(6-MWT),depression as measured by the anxiety as measured by the Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),adverse effects.Results Twelve studies with a total of 1605 case subjects were included and grade quality of evidence reviews had 1 outcomes as medium quality evidence,5 outcomes as low quality evidence and 4 outcomes as very low quality evidence.Acupuncture treatment resulted in significantly greater reductions in angina attacks,visual analog scale(VAS),6-minute walk test,angina efficacy,electrocardiogram improvement rate,angina efficacy,total clinical response rate.Conclusion The efficacy of acupuncture combined with western medicine in treating CSA is better than that of Western medicine alone,thanks to restricted by the number and quality of included researchs,higher quality studies are required to prove above conclusions.
5.Study on nutritional risk and nutritional support in patients with liver cancer during perioperative period

Objective:To study the nutritional risk and nutritional support in patients with liver cancer during perioperative period.Methods:In Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical, the clinical data of 507 liver cancer patients who underwent surgery College from January 2019 to January 2020 were retrospectively analyzed. The perioperative nutrition was assessed by nutritional risk screening 2002 (NRS 2002), ≥3 scores was diagnosed nutritional risk, and the nutritional support was counted.Results:Among 507 patients, 82 cases (16.2%) had nutritional risk. There was no statistical difference in rate of nutritional risk between male and female: 15.3% (58/379) vs. 18.8% (24/128), χ2 = 0.838, P>0.05. There was no statistical difference in rate of nutritional risk between primary liver cancer patients and secondary liver cancer patients: 18.0% (63/350) vs. 12.1% (19/157), χ2 = 2.781, P>0.05. The rate of nutritional risk in ≥ 60 years old patients was significantly higher than that in <60 years old patients: 25.9% (62/239) vs. 7.5% (20/268), and there was statistical difference ( χ2 = 31.819, P<0.01). The age, incidence of dystrophy and rate of nutritional support before surgery in patients with nutritional risk were significantly higher than those in patients without nutritional risk: (65.3 ± 12.7) years old vs. (55.9 ± 8.9) years old, 13.4% (11/82) vs. 0 and 24.4% (20/82) vs. 2.6% (11/425), and there were statistical differences ( P<0.01); there were no statistical differences in sex composition, tumor origin, rate of nutritional support after surgery and albumin between patients with nutritional risk and patients without nutritional risk ( P>0.05). Among 31 nutritional support patients before surgery, parenteral nutrition (PN) was in 1 case, enteral nutrition (EN) was in 30 cases; among 453 nutritional support patients after surgery, PN was in 297 cases, EN was in 27 cases, and PN + EN was in 129 cases. Conclusions:The incidence of nutritional risk in patients with liver cancer during perioperative period is high, and especially elderly patients should pay attention to nutritional support. NRS 2002 is a powerful tool and should be recommended to use at patients with liver cancer, and provide the evidence of nutritional therapy.
6. Diagnostic value and patient compliance of a pancreas-oriented multidisciplinary clinic: a retrospective analysis from a Chinese pancreatic disease center

Objective:
To evaluate the role of multidisciplinary team (MDT) clinic in the diagnosis of pancreatic diseases and patient compliance with MDT advice in the current medical system.
Methods:
The study included 512 patients that had visited the pancreas-oriented MDT clinic of Zhongshan Hospital between May 2015 and May 2019.The clinical and pathological data were collected and analyzed retrospectively. Diseases were classified in accordance with 2010 WHO classification of tumors of the digestive system and usual clinical practices. Compliance was determined according to whether a patient received corresponding therapies or undergoing further checks or follow-ups.
Results:
Among the 512 patients that had visited the MDT clinic, 121 patients were referred due to undetermined diagnosis. Classified according to the final diagnosis, the rate of undetermined diagnosis in different disease categories from high to low in order was inflammatory diseases of the pancreas (75.0%, 24/32), other lesions of the pancreas (56.1%,23/41), pancreatic cystic lesions (19.1%,17/89), pancreatic carcinomas (18.3%,48/262) and pancreatic neuroendocrine neoplasms (pNEN)(10.2%,9/88). The MDT clinic made diagnosis to 68 patients directly with an accuracy of 89.7%. The rate of compliance in the entire cohort was 63.4%. The rate of compliance of patients from June 2017 to May 2019 (68.4%) was higher than that of patients from May 2015 to May 2017(59.6%). The compliance rate of patients in different disease categories from high to low in order was inflammatory diseases of the pancreas(84.4%, 27/32), pancreatic carcinomas (67.9%, 178/262), pNEN(60.2%,53/88), other lesions of the pancreas (56.1%,23/41), and pancreatic cystic lesions(49.4%, 44/89). The compliance rate of patients with different MDT advice from high to low in order was best supportive care(78.6%,22/28), antitumor approaches beyond surgery(71.6%,159/222), further tests(62.6%, 77/123), surgery(53.7%, 65/121) and follow-up(49.2%, 31/63). In patients suggested for surgery, the compliance rate of patients with carcinomas(67.4%, 33/49) was higher than patients with other kinds of neoplasms.
Conclusions
MDT clinic could facilitate the diagnosis of pancreatic diseases conveniently and inexpensively. The overall compliance rate of MDT clinic patients is rather low, and patients with carcinomas have a relative high rate of compliance with the suggestion of surgery.
