1.Antibiotic bone cement and renovation after artificial joint replacement

BACKGROUND:Antibiotic bone cement is the important method for the prevention and treatment of infection after artificial hip replacement and renovation. OBJECTIVE:To review the research progress of antibiotic bone cement. METHODS:A computer-based online search was performed in PubMed database, CNKI database, Chinese Biomedical Literature database, VIP database and Wanfang database for the literatures from 1978 to 2012. The key words were“bone cement, antibiotic bone cement, infection, joint replacement”in English and Chinese. RESUTLS AND CONCLUSION:A total of 335 literatures were screened out. Final y, 29 literatures were included for in-depth analysis after the primary screen through reading the title, abstract and ful-text. Antibiotic bone cement has been widely used in the treatment of infection after artificial joint replacement and renovation as it can reduce the risk of infection after initial joint replacement and renovation. The material properties and mechanical properties wil not change after bone cement mixed with appropriate amount of antibiotic. Different antibiotics in the bone cement have different release rates, which were closely related with the porosity of bone cement. Adding the additive that used for increasing the porosity of bone cement can increase the antibiotic release.
2.Effects of cancer stem cell surface marker CD44 in gastric cancer invasion and lymph node metastasis

BACKGROUND:Tumor stem cels not only initiate tumorigenesis, but also are involved in the invasion and metastasis of tumor cels. For tumor stem cels, to identify the specific cel surface marker has become a research hotspot. OBJECTIVE:To explore the clinical significance of cancer stem cel surface marker CD44 in gastric cancer invasion and lymph node metastasis. METHODS: CD44 protein expression in specimens of gastric cancer tissue was detected by the immunohistochemical SABC method. The relationship between CD44 protein expression and biological characteristics and prognosis of gastric cancer was detected using Pearsonχ2 test and Cox regression analysis. RESULTS AND CONCLUSION:Among 100 cases of gastric carcinoma, 59 cases (59%) were positive for CD44 protein expression. CD44 protein expression in normal gastric mucosa at above 5 cm from the edge of primary gastric cancer was negative. CD44 protein was widely expressed in tissues of gastric cancer, mainly expressed in the cel membrane, and a smal amount of expression in the cytoplasm. CD44 protein expression in gastric cancer tissue was not correlated with sex of the patients or age (P > 0.05), but was associated with tumor staging and lymph duct tissue infiltration, histological grade, and tumor size (P < 0.05). Deep tumor invasion, high histological grade, big diameter of tumor, and lymph node metastasis could lead to high positive CD44 protein expression. Positive expression of CD44 is an independent prognostic factor affecting postoperative survival (P < 0.05). The results show that the cancer stem cel surface markers CD44 in gastric carcinoma tissues is strongly associated with invasion of gastric carcinoma and lymph node metastasis. High expression of CD44 presents poor prognosis.

Objective To compare the treatment effect of different doses methylprednisolone(MP)combined with monosialotetrahexosyl ganglioside(GM-1)on experimental spinal cord injury.Methods Based on the duplication of compressed spinal cord animal model,36 rats were divided into three groups randomly:the first group with high dose of MP,the second with moderate dose of MP and GM-1,the third with low dose of MP and GM-1.The motor evoked potential(MEP)and the grades of BBB were detected on the 7th day and 14th day after injury.Results The peak latencies and the amplitudes of MEP and the BBB grades changed significantly after injury in the three groups.After treatment by different administration,the recovery of the two combined groups was more obvious than that of the high dose MP group,and the effect with the low dose of MP and GM-1 group was most significant.Conclusion The treatment effect of low dose MP combined with GM-1 is better than the other two therapies on experimental spinal cord injury.
6.Development of a nonlinear 3D solid finite element model for the calculation of bending moments of flexural members

A numerical model is developed in this paper to calculate the bending moments of flexural members through integration in 3D solid finite element analyses according to the nonlinear constitutive model of concrete and the elastoplastic constitutive model of steel, utilizing the stress condition of the cross-section, considering the destruction characteristic of reinforced concrete members, and based on the plane cross-section assumption. The results of this model give good agreement with those of the classical method. Consequently, we can also deduce the corresponding numerical expression for eccentrically loaded members according to the analysis method.

[Objective] To study the protective effect of Xing Nao Jing (XNJ) on cultured cortical neurons in rats. [Methods] Primary cultured cortical neurons were applied to observe the effect of XNJ in counteracting the excitotoxicity ofglutamic acid. [Results] XNJ decreased the release of intracellular lactic dehydrogenase induced by glutamic acid and reduced the histological changes of cultured cortical neurons. [Conclusion] XNJ can counteract the excitotoxicity of glutamic acid and protect cultured cortical neurons in rats.
9.Clinical efficacy of allogeneic cross-linked dermal dressing for the treatment of enteroatmospheric fistula in patients with open abdomen

Objective To investigate the clinical efficacy of allogeneic cross-linked dermal dressing (ACLD) for the treatment of enteroatmospheric fistula (EAF) in patients with open abdomen (OA).Methods The clinical data of 44 patients with experienced trauma who received OA at Nanjing General Hospital of Nanjing Military Command from January 2011 to January 2014 were retrospectively analyzed.All patients received temporary abdominal closure (TAC) by modified sandwich-vacuum package (MSVP).From January 2011 to December 2012,33 patients with OA received vaseline gauze as abdominal cover layer (VG group),and from January 2013 to January 2014,11 patients with OA received allogeneic cross-linked dermal dressing as abdominal cover layer (ACLD group).Patients were followed up via outpatient examination or telephone interview till October 2014.The incidence of EAF,time for skin-grafting,hematologic indexes in 2 weeks after surgery (white blood cell counts,percentage of neutrophil,the mean value of procalcitonin and C reactive protein),the duration of postoperative hospital stay,hospital expenses and survival rates in the 2 groups were analyzed using t-test,repeated measures analysis of variance and Fisher's exact test.Results The incidence of EAF in the VG group and ACLD group was 42.4% (14/33) and 0,respectively,with a significant difference between the 2 groups (P < 0.05).The duration of skin-grafting was (15 ±6) days in the VG group and (11 ±3)days in the ACLD group,with a significant difference between the 2 groups (t =2.10,P < 0.05).The mean values of preoperative procalcitonin and postoperative procalcitonin at day 1,3,7,14 in the VG group were (1.20 ± 0.60)pg,/L and (2.50-± 0.90)pg,/L,(1.70 ± 0.30) pg/L,(1.90 ± 0.40) μg/L and (2.70 ± 0.60) μg/L,which were significantly higher than that of (0.90 ± 0.30) μg/L and (1.80 ± 0.60) μg/L,(1.30 ± 0.50) μg/L,(0.60 ± 0.20) μg/L and (0.30 ± 0.07) μg/L in the ACLD group (F =8.50,P < 0.05).The white blood cell counts and percentage of neutrophil and the percentage of C reactive protein between the 2 groups were changed from (13.8 ±2.4) × 109/L,(12.9 ± 2.1) × 109/L,0.90±0.09,0.88 ±0.06,(81 ± 19)mg/L,(136 ±28)mg/L to (16.2 ±3.3) × 109/L,(7.9 ± 3.0) x 109/L,0.85 ±0.12,0.79 ±0.09,(131 ±30)mg/L,(59 ±22)mg/L at postoperative day 14,showing no significant difference between the 2 groups (F =3.10,0.50,1.20,P > 0.05).Duration of hospital stay and hospital expenses in the VG group and ACLD group were (137 ±32)days,(638 831 ± 113 670) yuan and (82 ± 44)days,(474 839 ± 78 543)yuan,respectively,with a significant difference between the 2 groups (t =3.60,4.43,P < 0.05).The time of follow-up and overall survival rate in the VG group and ACLD group were 29 to 38 months and 81.8% (27/33),and 10 to 20 months and 90.9% (10/11),respectively,with no significant difference (P > 0.05).Conclusion Allogeneic cross-linked dermal dressing as abdominal cover layer can effectively decrease EAF in patients with open abdomen.
10.Clinical Observation of Carboxymethylstarch Sodium Solution in Auxiliary Treatment of Pediatric Bronchi-al Asthma

OBJECTIVE:To observe clinical efficacy and safety of Carboxymethylstarch sodium solution in auxiliary treatment of pediatric bronchial asthma. METHODS:Totally 102 children with bronchial asthma selected from our hospital during Mar. 2015-Mar. 2016 were divided into observation group and control group according to admission order,with 51 cases in each group. Control group was given aerosol inhalation of Budesonide suspension for inhalation 0.5-1 mg,tid+ aerosol inhalation of Ipratropi-um bromide solution for inhalation 1-2 mL+Ambroxol hydrochloride and clenbuterol hydrochloride oral solution 2.5-15 mL,po, bid. Observation group was additionally given Carboxymethylstarch sodium solution 7 mL(1-<4 years old),10 mL(4-12 years old),po,tid. Both groups received treatment for 3 months. Clinical efficacy,the time of cough and wheezing relief,the time of lung wheezing disappearance,hospitalization time and the levels of immune indexes (IgG,IgM,IgA) were observed in 2 groups, and the occurrence of ADR was recorded. RESULTS:Total response rate of observation group(90.2%)was significantly higher than that of control group(70.6%),and the time of cough and wheezing relief,the time of lung wheezing disappearance and hospitaliza-tion time of observation group were significantly shorter than those of control group,with statistical significance (P<0.05). There was no statistical significance in the levels of IgG,IgM and IgA between 2 groups before treatment(P>0.05). After treatment,above immune indexes of observation group were significantly higher than before treatment and control group,with statistical significance (P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Carboxymethylstarch sodium solution in aux-iliary treatment of pediatric bronchial asthma shows good therapeutic efficacy and improves immunity of children with good safety.
