1.Change of Lipopolysaccharide Binding Protein and Its Significance in Rats with Obstructive Jaundice

Objective To study the changes of lipopolysaccharide binding protein (LBP) in the serum of Wistar rats with obstructive jaundice and to investigate its potential mechanism.Methods Eighty male Wistar rats were randomly divided into obstructive jaundice group (OJ group, n =40) and sham operation group (SO group, n =40). Before operation and the 5th, 10th, 15th, 20th day after common bile duct ligation, the levels of LBP, endotoxin, tumor necrosis factor alpha (TNF ?) and interleukin 6 (IL 6) in plasma were detected in all the rats. Results LBP levels in serum increased significantly in OJ group on the 10th day after operation compared with those of SO group. Moreover, LBP levels gradually increased in OJ group with the prolongation of obstructive time. A positive correlation existed between serum LBP and plasma endotoxin, TNF ? and IL 6.Conclusion The study demonstrates that LBP in serum is high and plays an important role in the pathogenesis of multiple organ injury secondary to obstructive jaundice. It may be an appropriate way to treat patients with obstructive jaundice by decreasing LBP levels in serum.
2.Clinical comparative study of breast-conserving surgery and improved surgical in breast cancer patients

Objective To comparative analysze of the applications value of breast-conserving surgery and improved surgical in breast cancer patients.Methods One hundred and sixty-eight female breast cancer patients from February 2005 to October 2010 in the People's Hospital of Rizhaoour hospital were selected.The patients were divided into breast-conserving surgery group (n =80) and improved surgical group (n =88).The patients in two groups were followed-up and investigated for three years.Operative time,blood loss,postoperative drainage,postoperative drainage time and hospital stay,incidence of complications,local recurrence,distant metastasis and survival after 3 year,satisfaction ratio of breast cosmetology,psychological anxiety scores and quality of life scores after 3 years of the patients in two groups were compared and analyzed.Results The complication rate of breast-conserving surgery group was 12.50% (10/80),lower than the improved surgical group,and the difference was significant statistically (29.55% (26/88),x2 =7.23,P<0.05).The operative time ((72.68±5.28) min),blood loss ((39.21 ± 8.59) ml),postoperative drainage ((162.25 ± 28.56) ml),postoperative drainage time((8.16±2.58) d) and the length of hospital stay((11.43±1.98) d) of the patients in breast-conserving surgery group were lower than those of the improved surgical group,and the differences of two groups were significant statistically ((92.48 ± 6.01) min,(68.26 ± 10.23) ml,(343.29 ± 62.18) ml,(16.59±4.96) d,(18.47±2.94) d;t =22.72,19.99,24.61,13.99,18.35;P<0.05).The satisfaction rate after 3 years of breast-conserving surgery group was 92.50%(74/80),higher than that of the improved surgical group,and the difference was significant statistically (81.82% (72/88),x2 =4.20,P< 0.05).The psychological anxiety scores after 2 and 3 years of breast-conserving surgery group were (40.02 ± 2.63) points and (34.26 ± 3.59) points,lower than those of improved surgical group((42.58±2.37) points and (39.85±2.10) points),the quality of life scores after 3 years of breast-conserving surgery group was (67.58± 11.26)points,higher than that of improved surgical group ((50.85 ± 10.42) points),and the differences were significant statistically (t =6.60,12.16,9.96;P < 0.05).Conclusion Breast-conserving surgery could reduce the incidence of complications and psychological anxiety situations,improve clinically relevant indicators and satisfaction ratio of breast cosmetology and quality of life for the breast cancer patients.

4.Clinical value of postoperative radiotherapy for node-positive middle thoracic esophageal squamous cell carcinoma and modification of target volume

Objective To analyze the clinical value of postoperative radiotherapy for node-positive middle thoracic esophageal squamous cell carcinoma ( TESCC ) and to modify the target volume .Methods A total of 286 patients with node-positive middle TESCC underwent radical surgery in Cancer Hospital, Chinese Academy of Medical Sciences, from 2004 to 2009.In addition, 90 of these patients received postoperative intensity-modulated radiotherapy.The Kaplan-Meier method was used to calculate survival rates, and the log-rank test was used for survival difference analysis.The Cox model was used for multivariate prognostic analysis.The chi-square test was used for comparing the recurrence between patients receiving different treatment modalities.Results The 5-year overall survival ( OS) rates of the surgery alone ( S) group and surgery plus postoperative radiotherapy ( S+R) group were 22.9%and 37.8%, respectively, and the median OS times were 23.2 and 34.7 months, respectively ( P=0.003) .For patients with 1 or 2 lymph
node metastases (LNMs), the 5-year OS rates of the S group and S+R group were 27.3%and 44.8%, respectively ( P=0.017);for patients with more than 2 LNMs, the 5-year OS rates of the S group and S+R group were 16.7%and 25.0%, respectively (P=0.043).The peritoneal lymph node metastasis rates of N1 , N2 , and N3 patients in the S group were 2.9%, 10.9%, and 20.0%, respectively ( P=0.024) .The S+R group had a significantly lower mediastinal lymph node metastasis rate than the S group ( for patients with 1 or 2 LMNs:8.0%vs.35.3%, P=0.003;for patients with more than 2 LNMs, 10.0%vs.42.3%, P=0.001) , and had a prolonged recurrence time compared with the S group ( 25.1 vs.10.7 months, P=0.000) .However, for patients with more than 2 LNMs, the S+R group had a significantly higher hematogenous metastasis rate than the S group (46.7%vs.26.1%, P=0.039).Conclusions Patients with node-positive middle TESCC could benefit from postoperative radiotherapy.The target volume can be reduced for patients with 1 or 2 LNMs.Prospective studies are needed to examine whether it is more appropriate to reduce the radiotherapy dose than to reduce the target volume for patients with more than 2 LNMs.A high hematogenous metastasis rate warrants chemotherapy as an additional regimen.
5.Analysis on medication regularity and action mechanism of Tibetan medicine in treatment of spleen and stomach diseases based on data mining and integrated pharmacology.

To explore the medication regularity of Tibetan medicine in the treatment of spleen and stomach diseases, analyze the potential drug targets and interactions of the prescriptions, and reveal the mechanism of Tibetan medicine in the treatment of spleen and stomach diseases. The prescriptions in Tibetan medicine for treatment of spleen and stomach diseases were collected, and Traditional Chinese Medicine Inheritance Support System (TCMISS) was used to analyze the association rules between the herbs and discover the core herbs and new prescriptions. The integrated pharmacology platform V1.0 software was used to construct "herb-compound-target" network and investigate the interactions between various herbs and related pathways of Tibetan medicine Wuwei Shiliu powder in the treatment of spleen and stomach diseases. Among the 216 prescriptions of Tibetan medicine in the treatment of spleen and stomach diseases, pomegranate seed was used at a highest frequency (118 times), followed by white cardamom (107 times) and comatose (107 times). 12 new prescriptions were evolved by using the association rules (support>=34%, confidence>=0.85). 5 242 related drug targets and 20 related pathways were obtained from classic formula Wuwei Shiliu Powder (FDR<0.01). It was proposed that Tibetan medicine treatment for spleen and stomach diseases was mainly based on proliferation of "stomach fire" and the main drugs were for regulating Qi-flowing for strengthening spleen. The mechanism may be associated with regulation of digestive juice secretion, proton pump, mitochondria, regulation of intestinal digestion and immunity, the body's immunity to microorganisms function and other multiple targets and pathways to achieve the joint intervention.
6.Research on Standardization of Tibetan Medicine Names and Their Relationship With Official Drug Standards Based on Text Association

Objective:To construct the knowledge base of Tibetan medicine prescriptions and explore to standardize the names of Tibetan medicine prescriptions. Method:By using the concept of "man-machine combination",through the construction of Tibetan medicine terminology glossary (data sources: national drug standards,local drug standards,text classics on Tibetan medicines,etc.),the terminology glossary of Tibetan medicine prescriptions was mined. Upon its combination with expert review,the text association between Tibetan medicine prescriptions and various drug standards and dictionaries was constructed,and the standardization methods and techniques of prescription drug names were explored. Result:In this paper,the Tibetan medicine prescriptions approved for marketing in China were taken as the research object,and various inconveniences caused by the inconsistency between the names of prescriptions and the names of medicinal herbs were revealed. This paper also discussed the design ideas on name standardization of Tibetan medicines from three levels: text association,optimization of evaluation methods,and formation of expert decision-making system. We put forward a five-in-one (screening, evaluation, reviewing, fixing, and renewing) research model of Tibetan medicine name standardization. The construction,functions and advantages of the database and thesaurus of Tibetan medicine prescriptions were described in detail, and in combination with the text notes, association between the standard medicinal materials and the prepared prescriptions was then established. Conclusion:The text association method in this paper can accurately reflect the nonstandard names of Tibetan medicine prescriptions. Combined with expert review,it can be, to a certain extent, extended to the standardization of herb names in prescriptions with large scale of or more complex network structures.

The liver has a complex cellular composition and a remarkable regenerative capacity. The primary cell types in the liver are two parenchymal cell populations, hepatocytes and cholangiocytes, that perform most of the functions of the liver and that are helped through interactions with non-parenchymal cell types comprising stellate cells, endothelia and various hemopoietic cell populations. The regulation of the cells in the liver is mediated by an insoluble complex of proteins and carbohydrates, the extracellular matrix, working synergistically with soluble paracrine and systemic signals. In recent years, with the rapid development of genetic sequencing technologies, research on the liver's cellular composition and its regulatory mechanisms during various conditions has been extensively explored. Meanwhile breakthroughs in strategies for cell transplantation are enabling a future in which there can be a rescue of patients with end-stage liver diseases, offering potential solutions to the chronic shortage of livers and alternatives to liver transplantation. This review will focus on the cellular mechanisms of liver homeostasis and how to select ideal sources of cells to be transplanted to achieve liver regeneration and repair. Recent advances are summarized for promoting the treatment of end-stage liver diseases by forms of cell transplantation that now include grafting strategies.
