1.Management of polypoid lesions of the gallbladder: an analysis of 748 patients

Objective To investigate the risk factors of gallbladder neoplastic polyps,to determine the proper surgical indications,and to understand more about the clinical characteristics of gallbladder adenoma.Methods The clinical data of 748 patients diagnosed to have polypoid lesion of gallbladder (PLG) and underwent cholecystectomy from January 1998 to December 2012 in the Second Affiliated Hospital of Zhejiang University School of Medicine were retrospectively reviewed.Results Among 748 patients,340 had abdominal symptoms.Postoperative histopathology showed non-neoplastic polyps (n =659),gallbladder adenoma (n =68),gallbladder cancer (n =15) and no polyps (n =6).The mean diameters of the nonneoplastic lesions and the neoplastic polyps were (9.38 ± 3.44) mm and (14.55 ± 5.71) mm,respectively (P < 0.01).The average age of the patients with non-neoplastic lesions was (44.14 ± 11.42) years and (47.39 ± 12.82) years in those with neoplastic polyps (P < 0.05).The proportion of solitary PLG was 41.4% (253/611) and 59.59% (47/79) for the non-neoplastic lesions and the neoplastic polyps respec tively (P < 0.01).The size of PLG (13.34 ± 4.18 mm vs.20.07 ± 8.19 mm,P < 0.05) and the age of the patient (45.78 ± 11.66 years vs.54.13 ± 15.82 years,P <0.05) between the gallbladder adenoma and gallbladder cancer groups were significantly different.Gallbladder adenoma or dysplasia was identified in 66.7% (10/15) of gal1bladder cancer specimens.Conclusions Patients with PLG and with abdominal symptoms,large size (≥ 10 mm),old age (≥50 y) and solitary polyp are indications for cholecystectomy.Gallbladder adenoma may develop to gallbladder cancer within 10 years.
2.Development of recruits training burnout self-reported questionnaires

Objective To develop recruits training burnout self-reported questionnaires, and test the reliability and validity.Methods 780 recruits(360 for first test ,420 for second test) in Xinjiang were randomly assesscd with training burnout self-reported questionnaires.Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were applied to the data, and tested the reliability and validity of recruits training burnout self-reported questionnaires.Results The questionnaires consisted of 13 items and 3 dimensions.One of the dimensions (physically and psychologically exhausted situation, training-alienation, low accomplishment) , explained 61.599% of the total variance.Cronbach's α coefficient of the whole questionnaire was 0.837, Cronbach's α coefficient of each partial scale were between 0.775 and 0.807.Split-half reliability of the whole scale reached to 0.817, split-half reliability of each partial were between 0.758 and 0.793.The correlation coefficient among each dimensions were 0.267 to 0.569 (P<0.01) ,and the coefficient between each dimensions and the total score were between 0.671 and 0.857(P<0.01).Confirmatory factor analysis showed that three factor model was optimal (GFI =0.935, AGFI =0.902, NFI =0.907, CFI =0.936, IFI =0.935, RMSEA =0.050).The three dimensions and the total questionnaire had significant positive correlations with mental stress (r=-0.215--0.313, P< 0.01) and significant negative correlations with depression(r=0.319-0.602, P<0.01).Conclusion The recruits training burnout self-reported questionnaires meets the requirements of psychological surveying, and could be used to text recruits training burnout.
3.Effect and safety of celiac continued circulatory hyperthermia perfusion with chemotherapy in the treat-ment of cancer with ascites Meta-analysis

Objective To evaluate the clinical efficacy and safety of celiac continued circulatory hyper -thermia perfusion with chemotherapy in the treatment of cancer with ascites .Methods Databases including the Cochrane Library MEDLINE,PubMed,Embase,CBM,VIP,CNKI and WanFang Data,and the other sources as supplying .The literatures were screened according to the inclusion criteria ,extracted data and assessed the meth-odological quality ,and then meta -analysis was performed using RevMan 5.0.2 software.Results Totally 14 RCTs with 867 patients were included .Meta-analysis showed that compared intraperitoneal chemotherapy ,with celiac continued circulatory hyperthermia perfusion had a significant difference in CR ( OR=3.18,95%CI:2.18~4.64,P<0.00001),PR(OR=2.05,95%CI:1.51~2.77,P<0.00001),overall effective rate(OR=4.88, 95%CI:3.59~6.46,P<0.00001).The incidence of adverse reactions was no statistically significant in the two groups.Conclusion Celiac continued circulatory hyperthermia perfusion with chemotherapy in the treatment of cancer with ascites demonstrates better efficacy and safety ,but it still needs to verify the above conclusion .
4.Electron microscopic observation on neurodegeneration induced by brain injury newborn rats

BACKGROUND: As a result of immature brain of children and their imperfect blood brain barrier, improper clinical treatment would affect growth and development of children. It is fully important to perform further investigation on immature brain injury induced neurodegeneration.OBJECTIVE: To observe the ultramicrostructure of neurons in homolateral parietal cortex and hippocampus in newborn 7-day SD rat with contusion of parietal cortex.DESIGN: Completely randomized controlled trial.SETTING: Laboratories of Nerve Morphology and Cytobiology, Medical College of Shanghai Jiaotong University; Electron Microscope Room of Institute for Physiology, Chinese Academy of Science.MATERIALS: The experiment was performed in Teaching and Research Section of Anatomy, Laboratories of Nerve Morphology and Cytobiology ofShanghai Second Medical University (Medical College of Shanghai Jiaotong University; Electron Microscope Room of Institute for Physiology of Chinese Academy of Science from October 2002 to June 2003. A total of 19newborn 7-day SD rats were randomly divided into experimental group, operation control group and normal control group with 15, 2 and 2 in each group respectively.METHODS: In experimental group, free-fall device for brain injury was used for establishing model of contusion of parietal cortex in newborn 7-day SD rat. Anesthesia and scalp incision were conducted, without using free-fall device in operation control group. But above procedures were not carried out in normal control group. The changes of ultramicrostructure were observed under transmission electron microscope after routine treatment of electron microscopic samples.MAIN OUTCOME MEASURES: Ultramicrostructure of neurons in homolateral parietal cortex and hippocampus.RESULTS: All the 19 rats entered results analysis. ① There were two sorts of morphological changes in neurons in experimental group. One was evident swelling of dendrites and bodies of neurons, accompanied with the changes of organelles. In the early stage, expansion of endoplasmic reticulum cisterna could be observed and mitochondria became compact and concentrated. Then, vacuolization of endoplasmic reticulum, progressive swelling and vacuolization of mitochondria, dissociation of polysomes from rough endoplasmic reticulum and scattering of them in cytoplasm could be seen. Changes of nucleolus presented after significant changes of cytoplasm. Nuclear chromatin clustered together under karyotheca and arranged as clockface, which were some masses with irregular contours gathering to the center. Axons were almost normal. The other was concentration of cytoplasm and nucleolus with vacuolizations of unequal size in cytoplasm. ②There was no abnormal change in neurons in homolateral parietal cortex and hippocampus both in operation control group and normal control group.CONCLUSION: Swelling of brain cell and concentration of cytoplasm and nucleolus after brain injury play important roles in brain injury-induced neurodegeneration of immature rats.
5.Rehabilitation outcomes for 51 earthquake victims with spinal cord injury

Objective To analyze the outcomes of clinical management and rehabilitation interventions for earthquake victims with spinal cord injury. Methods Comprehensive rehabilitation intervention was conducted for 3 to 4 months with 51 earthquake victims with spinal cord injuries. The study involved their physical examination and on site interviews during hospitalization, and follow-up by telephone. Etiology, severity, complications and outcomes of rehabilitation and clinical management were analyzed. Results In 9.8% of the patients the spinal cord injury was at C7 or above, 5.9% at C8 to T6 , 68.6% at T7 to L2 , 13.7% at L3 to S2 and 2.0% at S2 or below. There were 22 complete injuries and 29 incomplete. After 4 months of training, 76.5% of the wounded reached wheelchair independence, 78.4% ambulated with an orthosis and 88.2% were partially independent in the activities of daily living.Conclusions The comprehensive rehabilitation intervention allowed most of the victims to resume an independent life. Their long-term care remains a challenge.
6.Circular RNA_0032131 regulates the proliferation,migration and invasion of rheumatoid arthritis synovial fibroblasts by targeting microRNA-145-5p

Objective To investigate the effect of circular RNA_003213(hsa_circ_003213)targeting microRNA-145-5p(miR-145-5p)on the proliferation,migration and invasion of rheumatoid arthritis synovialfibroblasts(RA-FLSs).Methods The levels of hsa_circ_0032131 and miR-145-5p in peripheral blood of RA patients and healthy subjects,RA-FLSs as well as normal human synovial fibroblasts were detected.RA-FLSs were divided into control(NC)group,si-NC group,si-hsa_circ_0032131 group,miR-NC group,miR-145-5p group,anti-miR-NC+si-hsa_circ_0032131 group,anti-miR-145-5p+si-hsa_circ_0032131 group,pcDNA group and pcDNA-hsa_circ_0032131 group.CCK-8 and transwell assays were used to detect the viability,migration and invasion of RA-FLSs.The targeting relationship between hsa_circ_0032131 and miR-145-5p was determined through a dual lu-ciferase reporter assay.Results Compared with healthy subjects,the expression of hsa_circ_0032131 was increased,while the expression of miR-145-5p was decreased in the peripheral blood of RA patients,and the difference was statistically significant(P<0.05).Compared with normal hu-man synovial fibroblasts,the expression of hsa_circ 0032131 was increased,while the expression of miR-145-5p was decreased in RA-FLSs,and the difference was statistically significant(P<0.05).Compared with the NC group,the viability,migration and the number of RA-FLSs invasion in the si-hsa_circ_0032131 group were reduced,and the difference was statistically significant(P<0.05).Compared with the miR-NC group,the viability,migration and the number of RA-FLSs in-vasion in the miR-145-5p group were reduced,and the difference was statistically significant(P<0.05).Compared with the anti-miR-NC+si-hsa_circ_0032131 group,the viability,migration and the number of RA-FLSs invasion in the anti-miR-145-5p+si-hsa_circ_0032131 group were increased,and the difference was statistically significant(P<0.05).Compared with the pcDNA group,the lev-el of miR-145-5p in the RA-FLSs of the pcDNA-hsa_circ_0032131 group was reduced(P<0.05).The miR-145-5p was a direct target of hsa_circ_0032131.Conclusion Knockdown of hsa_circ_0032131 inhibits the proliferation,migration,and invasion of RA-FLSs by upregulating miR-145-5p.

Methods:The samples were extracted by ultrasonic in organic solvents followed by the separation on a Waters ACQUITY UPLC BEH C18 column(100 mm × 2. 1 mm, 1. 7 μm) with gradient elution of 0. 1% formic acid in methanol and 0. 1% formic acid solution as the mobile phase and in a mode of atmosphere pressure chemical i-onization multiple reaction monitoring (MRM). Results:The method exhibited a good linear relationship (r≥0. 99) with the detection limit between 0. 9 and 38. 3μg/100 g for the nine fat-soluble vitamins. The recoveries were 78. 5%-114. 9% and the RSDs were 1. 57%-11. 1%(n=6). Conclusion:The analytical method is simple, rapid, accurate and sensitive, and suitable for the determina-tion of fat-soluble vitamins in nutritional supplements.

10.Relationship between mitochondrial DNA content and acute coronary syndrome

Objective To investigate the relationship between peripheral blood mtDNA content and acute coronary syndrome. Methods A total of 244 patients who received coronary angiography(CAG)was enrolled in this study. They were divided into acute coronary syndrome(ACS)group(n=183)and control group(n=61)ac-cording to the CAG results. A quantitative real-time PCR-based method was used to measure relative content of mtDNA in peripheral blood. According to lesion blood vessel counts ,ACS patients were divided into single lesion group,double lesion group and three lesions group. Gensini score was used to quantitatively evaluate the severity of coronary stenotic lesions. Then we analyzed the relationship of mitochondrial DNA content with the lesion counts and the Gensini score. Results The ACS group had a lower mtDNA content ,as compared to controls (P <0.001).The lesion blood vessel count and mtDNA content in the single ,double and three vascular lesions group were all significanlty lower than that of the control group(P<0.05)and that of the double,three vascular lesions group were both significantly lower than that of the single vascular lesion group(P<0.05). According to the Gensi-ni score,all patients were divided into four groups,where with mtDNA content was decreased with the increase of the Gensini score(P < 0.01). The linear regression analysis showed that Gensini score was negatively correlated with mtDNA content(r=-0.644,P<0.001). ROC curve analysis showed that the area under the curve of mtDNA content for ACS diagnosis was 0.855(P<0.001),with the sensitivity and specificity of 0.756 and 0.833,respec-tively. Conclusion MtDNA content is closely associated with ACS and the Gensini score ,the latter indicating the severity of coronary stenotic lesions. MtDNA content detection has its value in the diagnosis of ACS.
