To establish an optimized determination method for the alkaloids peimine and peiminine in three varieties of Fritillaria. Methods:The samples were refluxed in a mixed solution, the pH value of the mixed solution was adjusted, and then the solution was extracted by ether to obtain the test solution. A Shim-pack VP-ODS column (4. 6 mm × 250 mm,5 μm) was used, the mobile phase consisted of acetonitrile-0. 02% diethylamine(75∶25), and the flow rate was 1. 0 ml·min-1. An Alltech 2000ES ELSD detector was used, the drift tube temperature and the flow rate of nitrogen carrier gas in the ELSD was set at 93℃ and 2. 4 L·min-1 , respectively. The regression equation was calculated between the logarithm values of the reference solution concentration and the corre-sponding peak area. Results:The method could effectively extract the alkaloids from the samples. Peimine and peiminine were well separated by the method, and showed good linearity within the respectively given ranges. The average recovery of the alkaloids in the three varieties of Fritillaria was within the range of 95. 22%-103. 51%. Conclusion: The method is simple, convenient and universal with easy operation, which provides references for the determination of alkaloids in the other varieties of Fritillaria.

Objective To investigate the tracing value of drug-loaded nano-active carbon for lymphatic visualization and its potential application in the lymphadenectomy of gastric cancer. Methods Sixty-three patients with advanced gastric carcinoma were randomized into 2 groups:control group and tracing group. For the tracing group, 5-Fu-loaded nano-active carbon (NP-5-FU) suspension was intratumorally administered by preoperative gastroscope-guided or intra-operative injection. No tracing reagents was given in the control group. The numbers of removed lymph nodes and metastasis-involved lymph nodes as well as the lymph node staining were recorded and compared between the two groups. The adverse effects was monitored. Results The resected lymph nodes in the tracing group were much more than that of control group (43.73 ± 9. 25 vs. 25. 36 ± 4. 73, t = 3.82,P < 0. 05). For the N1, N2 and N3 lymph nodes, 27. 17 ± 5. 82,13.73 ± 4. 61 and 2. 83 ± 0. 15 per case of the tracing group were desected, while they were 14. 45 ± 2. 75,8. 27 ± 1.89, and 2. 64 ± 0. 11 per case in the control group, respectively (P < 0. 05 ). 26. 2% (211/842) of the lymph nodes in the tracing group showed black intensity, higher than that of unstain lymph nodes (4. 5% ,21/470,χ2 =95. 108,P <0. 05) and that in control group (16.9%, 142/837)( χ2 =7.485 ,P < 0.05, ). No local and systemic adverse reactions were observed for the local injection of nano-active carbon. No extension of operating time and increased complications occurred. Conclusion Local injection of nano-active carbon which was loaded with 5-FU around the tumor is a very useful measure to trace the lymph nodes and facilitate the lymphedetectomy and chemotherapy.

Indoleamine 2,3-dioxygenase (IDO) is a kind of immune regulation enzyme.IDO is the only rate-limiting enzyme that can catalyze the oxidative clearage of indole ring in tryptophan and its catabolism along the kynurenine pathway.IDO1 is one of the important mechanisms in maternal immune tolerance, which participates in the regulation of maternal-fetal immune relationship. The newly discovered IDO2 also plays an important role in this relationship. The abnormal expression of IDO2 is associated with the male infertility.

Objective To explore the diagnostic value of dilated intercellular space detected by light microscope for non-erosive reflux disease (NERD) and erosive esophagitis (RE). Methods A total of 104 subjects were divided into normal control group (n = 20), NERD group (n = 30) and RE group (n = 54).Biopsies were taken at 2-3 cm above the dentate line and were examined by light microscope to calculate the intercellular space and compared between different groups. Results The mean values of intercellular space in RE ( 1.40 ±0. 17 μm) and NERD ( 1.11 ± 0. 14 μm) were significantly higher than that in control group (0.66±0. 18 μm, x2 = 154. 170, P =0.000). But no significant difference was noted between RE and NERD groups ( t = 0. 044, P = 0. 834). The cut-off value of mean intercellular space with light microscope was 0. 89 μm, with sensitivity and specificity at 95.2% and 95.0%, respectively. Conclusion Dilated intercellular space under light microscope can be a sensitive, specific and objective indicator of NERD.

Objective To describe and analyze the misdiagnosis and mistreatment status of pulmonary alveolar proteinosis (PAP), especially cases with combination of severe infection. Method The misdiagnosis and mistreatment of 84 PAP patients and 6 cases with combination of severe infection before admitting to PUMCH was analyzed retrospectively. Results 66.7%(56/84) had experienced misdiagnosis before admission. 32.1% (27/84) were misdiagnosed with idiopathic interstitial pneumonia (IIP) and 14.3% (12/84) with pulmonary tuberculosis. 88.1%(74/84) had received anti-infection treatment, 33.3%(28/84) received eortieosteroids, and 19.0%(16/84) received anti-tuberculosis treatment. The percentage of the pa-tients who had taken the bronchoscopic examination outside the PUMCH was 53.6%(45/84). The PAP pa-tients diagnosis was established through bronchoscopic examination (including BAL and TBLB) in 86.9% (73/84) patients. There were 6 patients who had severe infection when PAP established. All of these 6 cases had been misdiagnosed with IIP and received corticosteroids treatment, 2 of whom died. Conclusions The misdiagnosis and mistreatment happens commonly in PAP patients. The misuse of corticosteroids makes some PAP cases get the severe infection. The image feature and the examination of bronchoscopy, BAL and TBLB are the key point in avoiding misdiagnosis. The administration of corticosteroids in uncertain patients should be avoided.

BACKGROUND: Chinese anatomical parameters of antedor atlantoaxial transarticular screw fixation have been rarely reported although the technique is a novel method out of China for patients with C1-C2 instability. OBJECTIVE: To provide Chinese anatomical data for anterior C1-C2 transarticular screw fixation. DESIGN, TIME AND SETTING: A measurement experiment was performed at the Department of Anatomy, Southern Medical University and Department of Orthopedics, Wuhan General Hospital, Guangzhou Command of Chinese PLA between September 2006 and April 2008. MATERIALS: A total of 50 sets of dried Chinese adult human C1 and C2 specimens, without regard to gender and age, but no abnormality and breakage, were measured with an electronic digital caliper (precision 0.01 mm) and a goniometer(precision 0.5°) made in China. METHODS: One proper screw was drilled through the atlantoaxial joint respectively in the direction to middle part of laterosuperior angle in the posterior of C1 lateral mass, and the screw point should not break through the superior facet articularsurface of the C1. In the procedure, the screw drilling point was at the junction of the lateral border of C2 body to 4 mm above the inferior border of C2 anterior arch. border of transverse foramen of C2 body and the median line of C2 body, and the distance between the inserting point and the medial border of transverse foramen of C2 body. RESULTS: The data from all specimens were involved in the result analysis. In the sagittal plane, the minimum lateral angulation of the screw tract was (10.80±2.10)°(left) and (10.76±2.40)°(right) respectively, and the maximum lateral angulation was (25.13±3.12)°(left) and (25.12±2.86)°(right), respectively. In the coronal plane, the minimum posterior angulation was (8.85±2.12)° (left) and (9.28±2.65)° (right) respectively, and the maximum posterior angulation was (26.96±3.09)°(left) and (27.49±2.51)°(right), respectively. The left screw tract length was from (17.48±2.10) mm to (25.41±2.59) ram, and the right was from (17.49±2.23) mm to (25.58±2.42) mm. The left distance between the inserting point and the median line of C2 body was (9.84±0.69) mm, and the right was (9.81±0.66) mm. The left distance between the median line of C2 body and medial border of transverse foramen of C2 body was (14.12±1.28) mm, and the right was (14.60±1.36) mm. The left distance between the inserting point and medial border of transverse foramen of C2 body was (6.28±1.38) mm, and the right was (6.79±1.39) mm. CONCLUSION: It is optimal for the anterior C1-C2 transarticular screw fixation to place the antedor screw with a length of 17 to 25 mm in lateral angulation ranging from 10° to 25° and the posterior angulation ranging from 9° to 27°. During the procedure, the dissecting distance from the middle of C2 body to lateral should not exceed 14 mm.
