1.Clinical manifestation and outcome of severe ulcerative colitis

Objective To evaluate the clinical outcome of severe ulcerative colitis(UC)and to find the factors related to treatment and outcome.Methods Forty one hospitalized patients with UC during 1988-2004 were retrospectively reviewed.Data were recorded including the onset,symptoms,signs, laboratory,endoscopic,radiologic and pathologic findings,as well as the processes of clinical treatment. The patients who undergone surgery were also analysed.Results Forty one of 144(28.5%)hospital ized patients were suffered from severe UC,and among them 92.7%(38/41)had pancolitis.The patients who had first onset,chronic persistent,chronic recurrent type were account for 36.9%(15/41),36.9%(15/41) and 26.8%(11/41),respectively.The steroids treatment played the main role in the inducing remission of severe UC(61.0%).Thirty one cases(75.6%)could be relieved by drug therapy.Seven cases(17.1%) were progressed to have operation.The age of early onset,pancolitis,low hemoglobin and serum albumin levels and need of intravenous steroids treatment were associated with the need of surgery.Conclusions Most of the severe UC patients respond well to the medical therapy,but for some non-responding or steroids depending individuals,after a reasonable duration of treatment,surgery should be considered.
2.A retrospective analysis of azathioprine in the treatment of 24 patients with refractory ulcerative colitis

Objective To evaluate the efficacy and safety of azathioprine (AZA) in the treatment of refractory ulcerative colitis (UC).Methods Retrospective analysis of the clinical improvement,endoscopic improvement and mucosal healing rate,inflammation marker improvement after AZA administration and its safety in 24 refractory UC patients were performed,who were recruited between January 2007 and December 2011 in West China Hospital,Sichuan University,China.Results Twenty-four patients were enrolled,with a median age of 36 years old and a median course of 4 years.Among them,14 cases were moderate UC and 10 cases were severe UC.The patients were treated with AZA in a dose of (1.23 ±0.34) mg· kg-1 · d-1 from 7 weeks to 42 months.Efficacy was judged by Mayo disease activity index.At 3 months,6 months and 1 year after treatment,the effective rates were 73.9% ( 17/23),81.8%(18/22) and 14/16 respectively,and the remission rates were 17.4% (4/23),54.5% (12/22) and 12/16respectively.Both ESR and C reactive protein level after treatment for 6 months and 1 year were significantly lower than those before treatment [ (9.3 ±8.9) mrn/1h,(10.9 ±7.3) mm/1h vs (22.3 ± 10.7) mm/1h;2.5(1.0-22.3) mg/L,2.3(1.0-28.0) mg/L vs 18.4(3.6-137.0) mg/L; all P <0.05].Corticosteroid withdrawal rates at 3 months and 1 year after AZA treatment were 16/18 and 15/16,respectively.At 6 months and 1 year after AZA treatment,the endoscopic improvement rates were 85.7% ( 18/21 ) and 13/15 respectively; the cndoscopic remission rates were 61.9% ( 13/21 ) and 11/15 respectively; and the mucosal healing rates were 61.9% ( 13/21 ) and 11/15 respectively.Adverse effects were occurred in 8 patients.Leukopenia was the most common adverse effect,followed by liver function injury,alopecia and epigastric discomfort.Conclusions AZA is effective in the treatment of refractory UC patients with a low dose of ( 1.23 ± 0.34) mg· kg - 1 · d - 1,especially in the steroid withdrawing,maintaining remission and mucosal healing without severe adverse effects.
3.Diagnostic value of HNF-1βin the diagnosis of ovarian clear cell carcinoma

Purpose To study the expression of HNF-1β in different type ovarian carcinoma and to explore the diagnostic value of HNF-1β in the diagnosis of ovarian clear cell carcinoma. Methods Immunohistochemical EnVision method was used to detect the ex-pression of HNF-1βin 27 clear cell carcinomas, 35 high-grade serous carcinomas, 21 endometrioid adenocarcinoma, 10 mucious carci-nomas, 13 metastatic Krukenberg tumors, and 2 transitional carcinomas. Results All of ovarian clear cell carcinomas variably ex-pressed HNF-1β and the positive rate of HNF-1β in ovarian cleat cell carcinoma was 85. 2%. Of 21 ovarian serous carcinomas, the nuclear positive of HNF-1β was 4 and HNF-1βpositive rate was 2. 9%. There was 5 HNF-1βexpression in endometrioid adenocarci-noma and the positive rate of HNF-1β was 23. 8%. The positive rate of HNF-1β in mucious carcinoma and metastatic Krukenberg tumor was 60. 0% and 53. 8%, respectively. No HNF-1βexpression was seen in transtional cell carcinoma. The sensitivity and speci-ficity of HNF-1β in the diagnosis of ovarian clear cell carcinoma was 85. 2% and 76. 5%. Conclusions HNF-1β may be a higher sensitive marker for the diagnosis of ovarian clear cell carcinoma. The diffuse and strong positive of HNF-1βmay have higher specific for diagnosis of ovarian clear cell carcinoma.
4.Analysis and comparison of reference intervals of complete blood cell count with the published standards

Objective To analyze the status of reference intervals (RIs) of complete blood cell count (CBC) in China and compare them with the published standards.Methods The RIs information of CBC (containing the items of WBC,RBC,Hb,HCT,MCV,MCH,MCHC and PLT) were collected by the External Quality Assessment (EQA)software from laboratories participated in the Second Complete Blood Cell Count EQA Program in 2013,which including the sources of RIs,the lower and upper limits of RIs and the validation information of RIs.The sources and validation information of RIs were analyzed by Microsoft Excel 2007 using all data except those from maternal and child care service center,children's hospital and corporations.The outliers of limits of RIs were excluded according to the International Federation of Clinical Chemistry(IFCC)guidelines,then data were analyzed by SPSS 13.0.The mean of the upper and lower limits of the RIs were compared with the published standards by the t-test.Results Totally 299 (57.72%) of 518 laboratories reported were using RIs according to National Clinical Laboratory Operation Rules,while 112(26.62%)labs were using RIs from instrument/reagent manufacturer instructions,411 (80.00%)labs did not validate the RIs they used,of which 258 (50.00%) did not know how to validate RIs.The deviation of the maximum and the minimum ranged from 16% to 70%.More than one third laboratories did not separate RIs of RBC,Hb and HCT according to the gender.The upper and lower limits of WBC and MCV of most laboratories were higher,while those of RBC(male/female),PLT and Hb(male) were lower than that from the published standard.The RIs of MCH was narrower than that from the published standard.Meanwhile,HCT,MCHC (male/female) and Hb (female) were near the upper and lower limits of published standards.Conclusions At present,the RIs of CBC used in laboratories is chaotic,which is significantly different from the published standard.On the basis of standardization,the RIs of published standard result from a multi-center study in Chinese people should be preferred.
5.Experimental study on cross-resistance of Culex pipiens pallens to 3 kinds of chemical pesticides

Objective To understand the cross-resistance of Culex pipiens pallens to common pesticides,so as to provide the evidence for improving the application of chemical pesticides. Methods The IV instar larvae of DDVP-resistant,propoxur-resistant and cypermethrin-resistant strains as well as the sensitive strain of Culex pipiens pallens were collected to detect the re-sistance to DDVP,propoxur and cypermethrin based on the WHO bioassay method. Results The resistance coefficients of DDVP-resistant strain to DDVP,propoxur and cypermethrin were 14.47,8.96 and 207.27 respectively. The resistance coeffi-cients of propoxur-resistant strain to DDVP,propoxur and cypermethrin were 3.27,6.93 and 8.65 respectively. The resistance coefficients of cypermethrin-resistant strain to DDVP,propoxur and cypermethrin were 2.93,1.61 and 501.11 respectively. Con-clusion The resistance and cross-resistance could be generated during the long-term application of a single kind of chemical pesticide,and we should pay more attention to the varieties and dosages of them.
6.Preparation of PTD4-Cu,Zn-SOD fusion protein

Objective To prepare PTD4-Cu,Zn-SOD fusion protein.Methods The recombinant plasmid of pET 1 6b-Cu,Zn-SOD and pET16b-PTD4-Cu,Zn-SOD was transformed into Escherichia coli BL21 (DE3).Isopropyl β-D-1-thiogalactopyranoside was then added at a final concentration of 0.84 mmol/L,and the cells were incubated for 4 h to induce the expression of Cu,Zn-SOD and PTD4-Cu,Zn-SOD fusion protein.Lysozyme and ultrasound were used to lyse the bacteria,the supernatant was collected for 15% SDS-PAGE to analyze the expression of the target protein.Ni-NTA His bind resin was used to purify Cu,Zn-SOD protein and PTD4-Cu,Zn-SOD fusion protein under natural conditions.Western blot was used to identify the target protein.Results The results of Western blot showed that the purity of the target protein was about 90%,and the Cu,Zn-SOD protein with a molecular weight about 19 kDa and the PTD4-Cu,Zn-SOD fusion protein with a molecular weight about 20 kDa were found.Conclusion PTD4-Cu,Zn-SOD fusion protein is prepared successfully.
7.The influence of humanistic management atmosphere on nurses′ intention to stay

Objective:To explore the influence of humanistic management atmosphere on nurses′ intention to stay in hospital, so as to provide reference for stabilizing nursing team.Methods:The convenience sampling method was used to investigate 1 044 clinical nurses in 5 tertiary hospitals in Shandong province by using the general information questionnaire, nurses′ perception of humanistic management questionnaire and nurses′ intention to stay questionnaire in May 2019.Four dimensions of demographic factors and nurses′ perception of hospital humanistic management were used as independent variables, and the total score of nurses′ intention to stay was used as dependent variable for stepwise regression analysis.Results:The total scores of humanistic management perception and intention to stay were 166.70±29.42 and 23.30±4.29 respectively. There was a positive correlation between humanistic management perception and intention to stay( P<0.05). The three dimensions of human management perception, emotional management perception and cultural management perception, as well as the professional title, working years and employment form could explain 27.5% of variation. Conclusions:The nurses′ intention to stay is in the middle and upper level, which is related to the hospital′s humanistic management. The perception of humanistic management of nurses has a positive predictive effect on the intention to stay, especially in talent management, emotional management and cultural management. Hospital managers should improve the humanistic management mode, enhance nurses′ perception of humanistic management in order to stabilize the nursing team and improve nurses′ intention to stay.

Bovine adenovirus type 3 (BAdV3) is being used in the development of potential vehicles for gene therapy and vectored vaccine. To that end, a more comprehensive description of BAdV3 biology is essential. In this study, we focused on the role of pIX in BAdV3 virion rescue after full-length BAdV3 genome transfection. Initially, pIX deletion or initiation codon mutation abolished the production of progeny virions, which suggested that pIX was essential for the rescue of BAdV3 containing a full-length genome. Moreover, through transfection of a panel of pIX mutant BAdV3 genomes, we observed that the conserved N-terminus and the putative leucine zipper element (PLZP) were essential for virion rescue, whereas the C-terminus following the coiled-coil domain was non-essential. In addition, swap of the PLZP element and its following region of BAdV3 pIX to corresponding domains of human adenovirus type 5 (HAdV5) did not affect virion production, whereas swap of the entire pIX abolished production of progeny virions. We suggest that failure of the full-length BAdV3 pIX swap might be due to species specificity of its N-terminus region before the PLZP element.

9.Expression of cysteine-rich secretory protein 2 in patients with asthenozoospermia and its clinical significance.

OBJEVTIVETo investigate the expression of cysteine-rich secretory protein 2 (CRISP2) in spermatozoa of patients with asthenozoospermia and explore its clinical significance.
METHOSSemen samples were collected from 24 normal volunteers and 24 patients with asthenozoospermia for detecting CRISP2 mRNA and protein expressions using qRT-PCR and Western blotting, respectively. The correlation of CRISP2 expressions with sperm morphology, progressive motility and fertility prognosis were analyzed in patients with asthenozoospermia.
RESULTSCRISP2 protein expression was obviously lowered in the ejaculated spermatozoa of patients with asthenozoospermia as compared to the normal volunteers, but no significant difference in CRISP2 mRNA expression was found between the two groups. Correlation analysis showed that CRISP2 protein expression was positively correlated with normal sperm morphology (r=0.6182, P=0.0037) and progressive motility (r=0.6309, P=0.0029). Follow-up study of the patients revealed a higher fertility rate in patients with a relatively high CRISP2 protein expression than in those with low CRISP2 protein expression (80.0% vs 20.0%, P=0.0230).
CONCLUSIONThe expression level of CRISP2 protein is positively correlated with normal sperm morphology and progressive motility. A reduced CRISP2 protein expression indicates poor fertility prognosis of patients with asthenozoospermia, suggesting the potential value of CRISP2 as a novel therapeutic target for treating asthenozoospermia.

10.Practice of Management Mode of Long-term Medication Safety Assessment for Chronic Airway Disease Patients Established by Pharmacists Team of Medical Consortium

OBJECTIVE:To provide reference for pharmacists to participate in the management of chronic disease. METHODS:A total of 259 patients with chronic airway disease [included asthma and chronic obstructive pulmonary disease (COPD)] met the inclusion criteria were selected from our hospital and 5 community health care centers of medical consortium. These patients received medication safety assessment management,which was led by clinical pharmacists of our hospital with the participation of community pharmacists,including medication safety comprehensive evaluation and risk classification management, follow-up and medication guidance, integrated prescriptions checking, establishment of shared database. 1 years after the implementation,the effectiveness were evaluated by score the relatived indicators in related groups. RESULTS:After a year of the management mode practice,compared with before intervention,the patients'safety medication cognitive ability score in high-risk and low-risk group increased from(4.49±1.26)and(7.31±1.01)to(5.40±1.56)and(7.44±0.91);medication adherence score increased from(4.96±1.21)and(7.08±1.24)to(6.66±1.08)and(7.38±0.98);ACT score from asthma patients increased from (16.15±2.58)and(21.15±1.03)to(16.80±2.57)and(21.64±1.55);CAT score from COPD patients decreased from(25.51± 4.07) and (14.90 ± 3.95) to (24.20 ± 3.96) and (13.80 ± 4.08);the rate of irrational prescription effective identification and intervention by pharmacists increased from 3.6% and 1.4% to 9.4% and 7.6%,respectively. All the differences above were statistically significant (P<0.05). CONCLUSIONS:The participation of pharmacists in long-term medication safety assessment management for chronic airway disease patients can improve patients'safety medication cognitive ability,medication adherence, disease control and the pharmacists'ability of irrational drug use identification and intervention.
