2. Thinking and primary practice on discovery of pharmacodynamic material basis of Chinese materia medica assisted by virtual screening method

Given in-depth study on the numerous chemical constituents from commonly used Chinese materia medica (CMM) whose pharmacodynamic material basis have not been completely clear yet, the tested virtual screening method has been used to reveal the pharmacodynamic material basis. It would be an unimaginable thing to experimentally evaluate the activity of every compound on all interrelated biomacromolecule targets one by one. In this study, the attempt of applying high-throughput virtual screening method to discover the therapeutically effective components, which represented corresponding active components in the given CMM, was developed. For the Chinese herb Sang (Morus alba) including Mori Folium, Mori Fructus, Mori Ramulus, and Mori Cortex, a molecular library consisted of their 510 known chemical components was docked with four target models related to antidiabetic and diuretic activity using Molegro software, respectively. It was shown that the numbers of components with theoretical activity on α-glucosidase and carbonic anhydrase XII were far more than those on insulin receptor and mineralocorticoid receptor, and some components were observed to display potent theoretical activity on multiple targets. Besides, the forecast of activity of some components aiming at α-glucosidase was found to be supported by literature experimental data. On the basis of these results, we deduced that the antidiabetic and diuretic activity of Sang could be mainly due to the effects on α-glucosidase and carbonic anhydrase XII, rather than those on insulin receptor and mineralocorticoid receptor. Virtual screening method should help us to build a new open mind for clarifying CMM pharmacodynamic material basis.
3.The evaluation of educational measurement indexes from the Undergraduates Clinical Skills Competition in a medical university

Objective The purpose of this study was to analyze and evaluate the educational measurement indexes from the undergraduates clinical skills competition in a medical university.The rationality of the competition was discussed in the view of education survey.Methods 84 students' results which selected from the China Medical University 2nd Clinical Skills Competition were the research objects in this study.The indexes of the difficulty,discrimination,reliability and criterion-reated validity of the competition were evaluated based on the Education surveying principle.The difficulty was calculated by subjective topic difficulty computing formula,and other indexes were carried out with SPSS 18.0 except the difficulty index.Results The average difficulty of the questions of this university was 0.61,the average discrimination was 0.77 and the overall reliability was 0.926.The OSCE scores of the participating students were taken as the criterion in this study.The calculated criterion-related validity was 0.416.Conclusion The college students' clinical skills competition has good reliability,discrimination and effective criterion-related validity.For the selective competition,the difficulty can be further improved while considering the scope of the topic.
4.Levels of interleukin-8, interleukin-4 and tumor necrosis factor-α of neonatal mouse with systemic inflammatory response syndrome treated by different doses of dexamethasone

Objective To observe the expression levels of IL-8,IL-4,TNF-α of neonatal mouse with systemic inflammatory response syndrome(SIRS) treated by different doses of dexamethasone.Methods A total of 50 neonatal mice were randomly divided into five groups:blank control group,saline control group and treatment group A,B,C.The saline control group and treatment groups were established into SIRS models,treatment group A:a single high-dose of dexamethasone (2 mg/kg),subcutaneous injection (SC) ; treatment group B:two doses of dexamethasone (1 mg/kg,q1 2 h,total 2 mg/kg,SC) ; treatment group C:four doses of dexamethasone (0.5 mg/kg,q 1 2 h,total 2 mg/kg,SC) ;saline control group:saline of the same volume (0.4 ml/kg,SC).All mice were detected IL-4,IL-8,TNF-α by ELISA 72 hours after animal models being completed.Results The levels of IL-4,IL-8,TNF-α in saline control group and treatment group A,B,C were higher than those in blank control group respectively (P < 0.05,respectively).The levels of IL-4,IL-8,TNF-α in treatment group A,B,C were lower compared to those of saline control group respectively (P < 0.05,respectively),levels of TNF-α,IL-8 in treatment group B and C were lower than those of treatment group A(P < 0.05,respectively).There were no significant differences in the level of IL-4 among treatment group A,B,and C (P > 0.05).Conclusion Dexamethasone could lower the expressions of pro-inflammatory cytokines IL-8,TNF-α and anti-inflammatory factor IL-8 of neonatal mouse with SIRS,and the effect of multiple low-doses of dexamethasone on SIRS is significantly better than a single high dose.
5.Transvaginal ultrasound guided methotrexate injection for unruptured early tubal pregnancy

2 000 mIU/ml,combined use of mifepristone(100 mg,twice daily,for 3 days) was employed. Results The treatment succeeded in 28 patients,with a success rate of 87.5%.Acute lower abdominal pain occurred in 4 patients and laparoscopic salpingectomies were carried out.The serum ?-hCG levels decreased by 23%~35% at 3 days after injection,by 38%~62% at 1 week,and to normal levels(≤6.1 mIU/ml) at 2~4 weeks.The mass completed subsided at 30~55 days after injection. Conclusions Transvaginal ultrasound guided methotrexate injection for the management of unruptured early tubal pregnancy is simple,safe, and effective.Further investigations should be carried out in respect of the recovery of tubal anatomy and physiological functions.
6.Detection of Legionella pneumophila rcp Virulence Gene by Polymerase Chain Reaction

Objective To establish a polymerase chain reaction method for the detection of the rcp virulence gene of L. pneumophila. Methods Sixteen strains of Legionella pneumophila, L. dumiffii, L. bozemanii and L. Longbeachae isolated from the cooling towers of the centralized air-conditioning systems in Tianjin form 2005 to 2007 were detected by polymerase chain reaction method using L. pneumophila rcp virulence gene-specific primer according to GenBank published nucleotide sequence. Results The 900 bp rcp genetic fragment was amplified in three strains of L. pneumophila, while others not. Conclusion The rcp virulence gene-based polymerase chain reaction method for the detection of L. pneumophila has been successfully established and is the foundation for the studies of Legionella virulence.
7.Regulation of Gene Expression Profile by Twist in Murine Breast Cancer Cell

Previous studies have revealed Twist,a basic helix-loop-helix transcription factor,plays an important role in breast cancer metastasis.To clarify the molecular mechanism of its involvement in cancer metastasis,Twist was silenced by RNAi in highly metastatic 4T1 cells.Then microarray chips were used to investigate the gene-expression pattern of the Twist-knockdown 4T1 cells and the normal 4T1 cells.The results indicated that silencing of Twist significantly suppressesed lung metastasis of 4T1 cells in vivo.Direct comparison of gene-expression profiles showed that 167 genes in Twist-knockdown cells differed dramatically in expression levels from those in control cells.Among the 167 genes,26 well-known tumor-associated genes,including 15 up-regulated and 11 down-regulated genes were found.These genes appear to be regulated by Twist during breast tumorigenesis.The findings provide new insights into the mechanism by which Twist is involved in tumorigenesis.
8.Stratified analysis on correlation between abnormal blood lipid in senile people and coronary atherosclerotic cardiopathy

BACKGROUND: Hyperlipemia is one of the important risk factors of coronary atherosclerotic cardiopathy. Great attention has been paid on how to explain the correlation between blood lipid and coronary heart disease and the correlation of characteristics and level of blood lipid with coronary heart disease.OBJECTIVE: To probe into the correlation among the characteristics, the level of blood lipid and coronary heart disease in senile people.DESIGN: Case-control observation and stratified analysis.SETTING: Second Cadre Ward of General Hospital of Shenyang Military Area Command of Chinese PLA.PARTICIPANTS: Totally 105 inpatients were selected in coronary heart disease group, diagnosed as coronary heart disease, tallied with WHO Diagnostic Standard on Coronary Heart Diseases, in Second Cadre Ward of General Hospital of Shenyang Military Area Command of Chinese PLA from January 2001 to January 2002. Totally 99 inpatients, by checked, without coronary heart disease and cerebral vascular disorder were selected in the control in the same period.METHODS: 2 mL venous blood was collected in fasting in the morning and the serum was collected after centrifuged at 3 000 r/minute, for 5 minutes. Auto-biochemical analyzer and enzyme kit were used to determine cholesterol, triglycerin, high density lipoprotein cholesterol and low density lipoprotein cholesterol. Compared with the data in the control, the ranges of cholesterol, triglycerin and low density lipoprotein were calculated successively and divided by 10; at the class intervals of 0.53, 0. 39 and 0.29 mmonl/L, the frequency table was worked out. According to the frequency table, P25 (the 25th percentile), P50 (the 50th percentile), P75 (the 75th percentile) and P90 (the 90th percentile) of cholesterol, triglycerin and low density lipoprotein cholesterol were calculated successively. The stratified groups were divided based on P < 25, P25 to PS0, PS0 to P75, P75 to P90 and > P90, by which, the cases belonging to 5 groups were determined in coronary heart disease group and the control respectively. The exposure factor was determined by > P90, compared with other 4 groups, Menta-Haensecel method was used to calculate x2. If the exposure factor was related with coronary heart disease, the relevant coefficient and 95% confidence interval should be calculated. In accordance with the standards (Proposal on Prevention of Abnormal Blood Lipid and Targets of Abnormal Blood lipid on 2-grade Prevention of Coronary Heart Disease), the cases beyond the targets and morbidity were determined in two groups.MAIN OUTCOME MEASURES: Determination of blood lipid, abnormal cases and morbidity in two groups; cases beyond targets of blood lipid and morbidity; stratified comparisons of percentiles in two groups.RESULTS: According to intention measurement, 204 cases entered result and the control, there was no significant difference in cholesterol, triglycerin cases presented hypercholesterolemia, 34.3% cases hypertriglycerlipidemia and 11. 4% cases low density lipoprotein-hypercholesterolemia. It was indicated that the blood lipid was in the normal range among 64. 8% to 88.6% comparison between the cases with cholesterol ≥5.47 mmonl/L and the cases with cholesterol varied from 3.85 to 4.63 mmol/L, the morbidity of coronary heart disease in the former was 2.98 times as high as the latter (x2 = 6.29, 95% confidence interval was 1.27 -6.98) . In the comparison between the cases with cholesterol ≥ 5.47 mmol/L and the cases with cholesterol varied from 4.64 to 5.05 mmol/L, the correlation with coronary Lipid for 2nd Prevention of Coronary Heart Disease, among the patients with coronary heart disease, cholesterol ≥ 4.68 mmonl/L in 54. 2% cases,triglycerin ≥ 1.70 mmol/L in 34. 3% cases and low density lipoprotein ≥ 2.6 mmol/L in 52. 3% cases.CONCLUSION: For senile patients with coronary heart disease, the satisfactory level of cholesterol is 3.85- 4.63 mmol/L. For those with cholesterol ≥ 5.47 mmol/L, compared with those varied from 3.85 to 4.63 mmol/L, the morbidity of coronary heart disease in the former is possibly 2.98 times as same as the latter(x2 = 6.29, 95% confidence interval was 1.27 - 6.98).
