Objective To investigate the effect of baicalain on high expression of ECM and TGF-?1 in proximal tubular epithelial cells cultured in high glucose concentration. Methods LLC-PK1 cells were divided into six groups: (1)normal glucose group(NG, 5. 5 mmol/L D-glucose), (2)high glucose group(HG, 25 mmol/L D-glucose), (3) HG + PKC inhibitor(10?mol/L chelerythrine chloride), (4)HG + baicalein(50 ?mol/L), (5) HG + baicalein(100 ?mol/L), (6) HG + baicalein (200 ?mol/L) . PKC activity was detected. Expression of ColⅣ, FN and TGF-?1 was examined by in situ hybridization and immunocytochemistry(ABC) techniques. Through several thousand years’ inheritance and development by Chinese people, traditional Chinese medicine (TCM) has formed its unique theoretic system, whose efficacy has been widely accepted. However, because TCM theory focuses on the relationships among syndromes, treatment and efficacy, instead of the cause-and-effect relationship explored by modern natural science, the scientificity of TCM has always been questioned. In recent years, because virtual-world clinical research mode and the concept of “big data” were emphasized, increasing researchers began to put their research emphasis on the correlativity between intervening measures of diseases and outcome indicators. This change and the advancement of computer data mining and analysis technology, bring great opportunities for the further development of TCM theory and practice. This article concluded data mining technology used in TCM diagnosis and treatment in recent years, such as clustering analysis, decision tree, Bayesian network, neural network and multi-instance learning, which showed how to apply these methods to reveal rules of TCM diagnosis and treatment from a large number of TCM syndrome data, find knowledge hidden in data, and show TCM effectiveness supported by data.

7.Chemical constituents from endophyte Chaetomium globosum in Imperata cylindrical.

Isolation and purification of chemical constituents from solid culture of endophyte Chaetomium globosum in Imperata cylindrical was performed through silica gel column chromatography, gel filtration over Sephadex LH-20 and preparative HPLC. Nine compounds were obtained and their structures were determined as chaetoglobosin F(1), chaetoglobosin Fex(2), chaetoglobosin E(3) cytoglobosin A(4), penochalasin C(S), isochaetoglobosin D (6), N-benzoylphenylalaninyl-N-benzoyphenylalaninate(7), uracil(8) and 5-methyluracil(9), respectively, based on HR-MS and NMR data and comparison with literatures. Compound 7 was isolated from Chaeeomium sp. for the first time. In vitro cytotoxicity of compounds was evaluated using MTT mothed and 1,3,4 and 5 showed inhibition activity to the human cervical carcinoma cell HeLa with IC50 values of 99.43, 23.77, 97.92, 86.25 micromol x L(-1), while positive cotolocisnin Ad apno1ch alse IC50 24.33 micromol x L(-1).

9.Selective three-field lymphadenectomy for thoracic esophageal squamous carcinoma.

OBJECTIVETo investigate the lymph node metastasis and the rational lymphadenectomy in thoracic esophageal carcinoma.
METHODSEighty-seven patients with thoracic esophageal squamous carcinoma received esophagectomy plus two-field or three-field lymphadenectomy based on cervical ultrasonography.
RESULTSThirty-five patients (40.2% ) with enlarged cervical nodes revealed by cervical ultrasonography received cervical lymphadenectomy. The proportion of cervical lymphadenectomy was 66.7% (16/24) in upper thoracic esophageal carcinomas, significantly higher than 30.2% (19/63) in middle and lower esophageal carcinomas (P=0.002). Regional and cervical lymph node metastasis were found in 48(55.2% ) and 17(19.5% ) patients respectively. The regional lymph node metastatic rates were 37.5% (9/24), 62.3% (33/53) and 60.0% (6/10) respectively in the patients with upper, middle, and lower thoracic esophageal carcinoma. The cervical lymph node metastatic rates in the patients with or without regional lymph node metastasis were 31.3% (15/48) and 5.1% (2/39) respectively(P=0.002). The rates of upper, mid, lower mediastinal and upper abdominal lymph node metastasis were 25.3%, 23.0%, 5.7%, and 24.1% respectively. Cervical lymph node metastasis was significantly correlated with upper and mid mediastinal metastasis (both P< 0.01), but not with lower mediastinal and upper abdominal lymph node metastasis. The overall postoperative morbidity rate was significantly higher in three field lymphadenectomy group than that in two field group(60.0% vs. 34.6%, P=0.020).
CONCLUSIONSelective 3-field lymphadenectomy based on cervical ultrasonography should be performed in thoracic esophageal carcinoma, especially with upper and mid mediastinal lymph node metastasis.

10.Tragus cartilage tympanoplasty for treatment of adhesive otitis media.

OBJECTIVETo evaluate the efficacy of cartilage tympanoplasty in the treatment of adhesive otitis media.
METHODSFrom June to October, 2008, 18 patients with adhesive otitis media (18 ears) were treated with tragus cartilage tympanoplasty. The air-bone gap changes and the self-perceived symptomatic improvement were evaluated at 1 month and 1 year after the operation.
RESULTSAll the patients showed dry ear within 6 weeks after the operation. Tympanic membrane healing was achieved in 17 cases, and 1 case presented with a inferior-anterior fissure in the tympanic membrane. With the average preoperative air-bone gap (at 0.25, 0.5, 1.0, and 2.0 kHz) of 44.65 dB, the patients showed an obvious decrease of the air-bone gap by over 10 dB at 1 month after the operation and by over 25 dB at one year. Symptomatic improvements were achieved in these cases, including alleviated ear discomforts (3/15 cases), total tinnitus relief (1/11 cases), and alleviated tinnitus (10/11 cases). High-frequency tinnitus was noted in 1 case (1/7 cases), and the tympanic membrane appeared normal in 17 cases.
CONCLUSIONTympanic membrane reconstruction using the tragus cartilage can be feasible for treatment of secretory otitis media, but the surgical indications should be carefully controlled.
