1. Immunomodulatory effect and its mechanism of triepoxide on regulatory dendritic cells

Objective To explore the influence of triepoxide (TPT) on the function of CD11clowCD45RBhigh dendritic cell (DC) subsets which produce IL-10. Methods Splenic CD11clowCD45RBhigh DCs and CD4+ T cells were harvested from C57BL/6 mice by magnetic bead sorting. CD11clowCD45RBhigh DCs were treated with various doses of TPT (0, 1, 10, 20ng/ml), and those without TPT as controls. Flow cytometry was used to determine the expression of CD11clowCD45RBhigh DC surface molecules including CD40, CD80, CD86, I-a/e, and IL-10 levels in CD11clowCD45RBhigh DC culture supernatants were determined by sandwich enzyme-linked immunosorbent assays (ELISA). The CD4+ T cells were divided into the following 5 groups: control group, untreated group (TPT-untreated CD11clowCD45RBhigh DC+CD4+ T cells), high-dose TPT-treated group (20ng/ml TPT-treated CD11clowCD45RBhigh DC+CD4+ T cells), high-dose TPT-treated + antibody I group (20ng/ml TPT-treated CD11clowCD45RBhigh DC+CD4+ T cells and IL-10 antibody) and high-dose TPT-treated + antibody II group (20ng/ml TPT-treated CD11clowCD45RBhigh DC+CD4+ T cells and IL-10 antibody isotype). Proliferative activity of CD4+ T lymphocytes was determined by MTT, whereas IL-4 and IFN-γ contents in CD4+ T cell culture supernatants were determined by flow cytometry. Results TPT markedly decreased the expression of CD40 and I-a/e while enhancing the expression of CD86 on CD11clowCD45RBhigh DC surface and IL-10 secretion. The enhancement of IL-10 secretion level hinged upon the rise of TPT concentration. The proliferative activity and IFN-γ secretion level in high-dose TPT-treated group and high-dose TPT-treated + antibody II group were markedly lower than those in the untreated group with significantly higher IL-4 secretion levels in TPT, whereas the proliferative activity and IFN-γ secretion level in high-dose TPT-treated group and high-dose TPT-treated + antibody I group were significantly higher than those in the untreated group with markedly lower IL-4 secretion level in TPT. Conclusions TPT could activate the secretion of IL-10 by CD11clowCD45RBhigh DCs, and further induce activation of immune function of T lymphocyte with shifting of Th1 to Th2 in vitro. TPT may down-regulate the immune response by promoting the phenotypic and functional maturation of CD11clowCD45RBhigh DC.
4.Expression and relationship of EBV LMP1, COX-2 and VEGF-C in nasopharyngeal carcinoma

Objective:To evaluate the expression of vascular endothelial growth factor C(VEGF-C),Epstein-Barr virus(EBV) latent membrane protein 1(LMP1),cyclooxygenase-2 (COX-2) in nasopharyngeal carcinoma(NPC).Method:LMP1, COX-2 and VEGF-C were detects by immunohistochemical staining for 57 case NPC tissue.Result:The positives rates of LMP1,COX-2 and VEGF-C detected by immunohistochemical staining were 49.1%(28/57), 75.4%(43/57)and 59.6%(34/57), respectively. The expression of LMP1, COX-2 and VEGF-C were correlated to each1 other in NPC(P<0.05).Conclusion:LMP1 and COX-2 may induce expression of VEGF-C directly or LMP1 induce expression of VEGF-C by induce COX-2 expression, may contribute to lymph metastasis and develop NPC.
5.Disinfectant and Antibiotic Resistance Genotypes in Escherichia coli Isolated from Urine

OBJECTIVE To investigate the disinfectant and antibiotic resistance genotypes in 60 strains of Escherichia coli isolated from urine.METHODS Sixty strains of E.coli isolated from inpatients′ urine were collected from Jan 2006 to Oct 2008.Antibiotic susceptibility tests for fifteen antibiotics were performed by Kirby-Bauer method.And three kinds of disinfectant and antibiotic resistance genes(qacE△1,tehA,merA)were analyzed by polymerase chain reaction(PCR) and DNA sequencing.RESULTS More than 70.0% of the sixty strains of E.coli were susceptible to imipenem,piperacillin/tazobactam,cefoxitin,amikacin and gentamicin,and less than 50.0% were susceptible to the other ten antibiotics.There were 42 strains with qacE△1 gene(70.0%),10 strains with merA gene(16.7%) and all strains with tehA gene.The sequence of the first strain was different from those reported in GenBank,so it was a new subtype.CONCLUSIONS There are 70% of E.coli strains isolated from urine samples with qacE△1 gene.And disinfectant resistance may be one of the main factors for hospital infection in the future.
6.Comparison of Prognosis of Acute Subarachnoid Hemorrhage within or without Clinical Standardized Pathway

ObjectiveTo compare the prognosis of patiens with acute(within 72 hours) subarachnoid hemorrhage (SAH) within or without the clinical standardized pathway(CSP).Methods123 acute SAH cases were collected before CSP established meanwhile another 146 cases after CSP established from 2005 to 2009 in neurological intensive care unit of our hospital. Information such as age, gender, Hunt-Hess and CT-Fisher grade, timing and result of digital subtraction arteriography, treating time of aneurysm, and Modified Rankin Scale(MRS) at different time were recorded. Rehaemorrhagia, complications, mortality, prognosis and average stay were compared between two groups.ResultsThere was a significant difference between two groups in rehaemorrhagia, vasospasm,hydrocephaly, mortality, prognosis and average stay.ConclusionCSP is helpful to improve the prognosis of aneurysm subarachnoid hemorrhage.
7.Effect of Rehabilitative Ankle-Foot Orthoses on walking function of acute stroke hemiplegic patients

ObjectiveTo study the effect of Rehabilitative Ankle-Foot Orthoses training program on walking function of hemiplegic patients after stroke .Methods95 patients were randomly divided into two groups: observed group (49 cases) and control group (46 cases). The patients of control group were trained by routine rehabilitation training program, the patients of observed group were trained by Rehabilitative Ankle-Foot Orthoses and routine rehabilitation training program.ResultsAfter training, either observed group or control group showed significant improvement at walking function and ADL(P<0.01), but the improvement on observed group were more marked than coutrol group(P<0.01 and P<0.05). ConclusionsThe ability of walking in acute stroke patients were obviously improved, and the degree of the disability of them was decreased by Rehabilitative Ankle-Foot Orthoses.
8.Correlation of bone metabolic index with osteoarthritis and postmenopausal osteoporosis

BACKGROUND:Increasingly studies report that the normal balance of bone metabolism may be destroyed in the case of postmenopausal osteoporosis or osteoarthritis. The concrete metabolic process of bone turnover could be revealed sensitively by measuring the bone turnover markers in the serum or urine.
OBJECTIVE:To study the bone density and bone metabolic index of knee osteoarthritis (KOA) and postmenopausal osteoporosis (PMO), and to discuss the characteristics of bone density and bone metabolic index in KOA and PMO.
METHODS:A total of 248 postmenopausal women were detected for bone mineral density and knee X-ray. Final y 180 patients were included in this study and were divided into three groups:KOA group, PMO group, and control group. The levels of bone turnover markers (bone alkaline phosphatase, bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b) in serum from the participants were measured. The correlation between bone turnover markers and the disease progression was analyzed by Logistic regression analysis.
RESULTS AND CONCLUSION:The bone mineral density in the KOA group was higher than the control group but col agen type I cross-linked C-telopeptide was lower. The levels of bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b in serum from PMO group were higher than the control group. The decrease of col agen type I cross-linked C-telopeptide was associated with the incidence of KOA, and the increases of bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b were associated with the incidence of PMO. The lower bone absorption can be seen in postmenopausal women with KOA. PMO patients showed a higher bone turnover rate. The difference of bone metabolism between patients with KOA and PMO led to negative relationship of bone mineral density. The serum levels of bone gla protein, col agen type I cross-linked C-telopeptide, and tartrate-resistant acid phosphatase 5b can assist clinical diagnose and therapeutic effect detection of both KOA and PMO.
9.Analysis of 59 Anaphylactic Death Cases.

OBJECTIVE:
To analyze the cases of anaphylactic death cases and explore the standards of judicial expertise of anaphylactic death for providing evidence for judicial expertise.
METHODS:
Fifty-nine cases death due to allergic reaction in Shanghai were collected. And details of medical history, clinical manifestation of anaphylactic reaction and postmortem examination findings were reviewed for all cases.
RESULTS:
In the 59 cases, there were 58 cases died from drug allergy, including 77.6% of them were antibiotics. The rates of treating in standard hospital and illegal clinic were 37.3% and 61.0%, respectively. The allergic symptoms were dyspnea and facial cyanosis. The time from contacting allergens to death ranged from 1 min to 3 d. The concentration of total serum IgE ranged from 50 to 576.92 IU/mL. The results of clinical manifestation and pathological anatomy had obviously changes.
CONCLUSION
Based on the exclusion of all other cause of death and synthetically analysis of details of cases, medical history, clinical manifestation and anatomy, the conclusion of anaphylactic death can reached. The details of cases including clinical history, exposure to allergens, and clinical manifestation play an important role in diagnosis of anaphylactic death.

10.The predictive value of vasoactive-inotropic score at different time points in the risk of death in patients with septic shock

Objective:To evaluate the value of vasoactive-inotropic Score (VIS) at different time points in predicting the 28-day mortality of patients with septic shock, so as to reduce the risk of death and improve the prognosis of patients.Methods:This experiment was a single-center retrospective cohort study. The clinical data of 275 adult patients with septic shock who were treated with vasoactive drugs in the intensive care unit of the Affiliated Hospital of Xuzhou Medical University from February 2016 to February 2020 were collected. According to the 28-day survival condition, all recruited patients were divided into the death group and the survival group, and the maximum vasoactive-inotropic score of all patients at the first 24 h and the second 24 h were calculated, which were expressed as VIS max24 and VIS max48. Multivariate logistic regression analysis was used to find the independent risk factors that influencing the prognosis. The receiver operating characteristic curve was used to analyze the predictive value of VIS. Results:There was no significant difference between the death group and the survival group in the characteristics including age, sex, weight, infection sites, blood culture results, cardiac arrest, hormone use, and 24 h rehydration volume ( P>0.05). APACHE II score, basic lactic acid, and lactic acid after 24 h of treatment were increased significantly in the death group ( P<0.05). VIS max24 could accurately predict the 28-day mortality (AUC=0.953, 95% CI: 0.924-0.982), which were more efficent compared to VIS max48 (AUC=0.919, 95% CI: 0.881-0.957), basic lactic acid (AUC=0.937, 95% CI: 0.900-0.966) and APACHEⅡ score (AUC=0.865, 95% CI: 0.818-0.913). Conclusion:VIS max24 can more accurate predict the 28-day mortality in patients with septic shock.
