1. Application of 64-slice spiral coronary CT angiography in diagnosis of restenosis in patients with coronary heart disease after stenting

Objective: To evaluate the clinical value of 64-slice spiral coronary CT angiography (CTA) in diagnosis of in-stent restenosis (ISR) after percutaneous coronary intervention (PCI) in patients with coronary artery disease. Methods: CTA was used to reconstruct and analyze the 345 segments (each stent was divided into three segments: proximal,middle and distal) of 115 stents in 60 patients with coronary heart disease after PCI. The results of selective coronary angiography (SCA) were taken as the golden standard to evaluate the sensitivity and specificity of CTA in diagnosis of ISR after PCI. Results: CTA clearly showed the location and length of the stents,the stenosis at stent and the characteristics of restenosis plaques. Thirty-nine in-stent restenosis lesions were found by CTA, including 8 calcified lesions and 31 non-calcified lesions. There were 25 lesions at the proximal end of the stent, 7 at the middle and 7 at the distal end. Forty-two lesions were found by CTA, including 9 calcified lesions and 33 non-calcified lesions, with 26 at the proximal end, 8 at the middle and 8 at the distal end. CTA correctly diagnosed 36 segments, missed 6, and misdiagnosed 3. The sensitivity, specificity,PPV,NPV and accuracy of CTA were 85.71%,99.01%,92.31%,98.04% and 97.39%, respectively, CTA had the highest sensitivity and specificity for diagnosis of the proximal ISR,being 96.15% and 100%, respectively. Proximal stent restenosis accounted for 64.10% of the total. Conclusion: CTA can clearly demonstrate the in-stent restenosis and has a high accuracy in diagnosing restenosis after PCI. CTA is a safe, simple and reliable noninvasive diagnostic method for diagnosis of in-stent restenosis after PCI.
2.Effects of Betaxolol Hydrochloride Cationic Liposomes on Intraocular Pressure, Blood Pressure and Heart Rate in Rabbits

Objective: To study the effects of betaxolol hydrochloride (BH) cationic liposomes on intraocular pressure (IOP), blood pressure ( BP) and heart rate ( HR) in rabbits to lay foundation for the development of new BH eye preparations. Methods:N-trimethyl chitosan (TMC60) was used as the coating material, the BH-loaded cationic liposomes (TMC60-BHL) were prepared. Total-ly 24 healthy rabbits were randomly divided into 4 groups using number method. Unilateral carotid artery intubation was carried out in the rabbits and connected a physiological recorder. TMC60-BHL, uncoated BHL, the marketed BH eye drops ( the positive control) and sterilized distilled water ( the negative control) with the volume of 50 μl was respectively used in one eye of the rabbits. IOP was detected by an ophthalmotonometer, and BP and HR were determined by the physiological recorder at the predetermined time intervals. Results:In the ipsilateral eyes, the maximum decrease in IOP for uncoated BHL and TMC60-BHL was 23. 00% and 26. 65%, respec-tively. In the contralateral eyes, the maximum decrease in IOP for uncoated BHL and TMC60-BHL was 18. 54% and 24. 33%, respec-tively. Compared with that of the positive control (19. 01%,15. 19%), the IOP decrease effect of uncoated BHL and TMC60-BHL was significant higher(P<0. 05), and that of TMC60-BHL was the highest(P<0. 05). BP and HR were both reduced in the various BH groups, while there was no notable difference when compared with those in the negative group(P>0. 05). Conclusion:TMC60-BHL shows significant IOP decrease effect without notable effect on BP and HR, which is valuable to be studied further.

4.Effects of Paired Box2,Proliferation Cell Nuclear Antigen and Cell Apoptosis in Nephrotic Syndrome with Steroid-Resistance

Objective To investigate the expression of paired box2(Pax2),proliferation cell nuclear antigen(PCNA) and cell apoptosis in steroid-sensitive and steroid-resistant groups with primary nephrotic syndrome(PNS) and to find out the action of Pax2 expression in PNS with steroid-resistance.Method The expressions of Pax2,PCNA were evaluated by immunohistochemistry and cell apoptosis by fluorescence micoscope.Results Pax2 expression in renal tubule had a positive correlation with PCNA expression in steroid-sensitive group.In steroid-resistant group,Pax2 expression had no correlation with PCNA.Pax2 had a negative correlation with cell apoptosis.Conclusions Pax2 proper expression facilitate PCNA expression and repair tubulointerstitial lesions in steroid-sensitive group.Renal tubular epithelial cell proliferation coordinated with cell apoptosis.Pax2 overexpression in steroid-resistant group lead to the decrease of cell proliferation and cell apoptosis and lead to the severe tubule lesions,which made to glucocorticoid resistance.
5.Effect of Molecular Weight and Substitution Degree of Chitosan-poly-arginine on in vitro Transdermal Pene-tration Enhancement

Objective:To study the effect of molecular weight and degree of substitution (DS) of chitosan-poly-arginine (CS-R9) on transdermal penetration enhancement in vitro. Methods:Low molecular CS, medium molecular CS or high molecular CS was respectively used to synthesize CS-R9 with different molecular weight (LCS-R9-1, MCS-R9 and HCS-R9). Low molecular CS was used to synthesize CS-R9 with various degree of substitution by changing the mole ratio between R9 and CS (LCS-R9-1, LCS-R9-2 and LCS-R9-3). The in vitro transdermal penetration enhancement of the different CS-R9 on tinidazole ( TNZ) was studied using Franz diffusion cells. Results:According to the results of FTIR and 1 H-NMR, a series of target CS-R9 were synthesized including LCS-R9-1 with the DS of 2. 30, MCS-R9 with the DS of 2. 17, HCS-R9 with the DS of 2. 20, LCS-R9-2 with the DS of 8. 05 and LCS-R9-3 with the DS of 15. 87. Compared with the blank control group, Azone group, LCS group, R9 group and LCS+R9 group, LCS-R9-1 could enhance the in vitro transdermal penetration of TNZ significantly (P<0. 05). When the DS was unchanged, LCS-R9-1 and HCS-R9 showed similar enhancement in the first 12h, and the effects were both higher than that of MCS-R9 (P<0. 05). The enhancement of HCS-R9 was decreased during 12-24h, while compared with that of LCS-R9-1, the difference was not notable (P>0. 05). When the molecular weight of CS was unchanged, the effect was increased with the rise of DS in the first 21h, however, after that, the effect was decreased with the rise of DS. Conclusion:Molecular weight and DS both have significant effect on the in vitro transdermal penetration enhancement of CS-R9, and it is valuable to further study the in vivo transdermal penetration enhancement of CS-R9 and underlying mechanisms.
7.NF-κB inhibitor reduces the expression of zincfinger protein A20 in rabbit lumbar intervertebral disc nucleus pulposuscells induced by lipopolysaccharide

Objective To observe the expression of zinc finger protein A20(A20), NF-κB and related inflammatory factors before and after lipopolysaccharide (LPS) stimulates degeneration of rabbit intervertebral disc nucleus pulposus cells.Methods The normal and degenerative nucleus pulposus cells were isolated and cultured, then divided into normal group,degenerative group,LPS stimulation group and NF-κB inhibition group.HE staining observe the morphological changes of nucleus pulposus and annulus fibrosus,immunohistochemistry was used to detect the expression of A20,NF-κB/p65 and COL-Ⅱ.Real-time PCR was employed to analyze the expression of A20,IL-1β,TNF-α,NF-κB and COL-Ⅱ,Western blot was used to observe the A20 protein,p65 and COL-Ⅱexpression in the four groups, and TNF-α, IL-1β in cell supernatant was determined by ELISA.Results The number of nucleuspulposus cells significantly decreased, aggregation occured in the degenerative group.COL-Ⅱ was obvious lower and A20, p65 significantly higher than that in normal group by immunohistochemical staining.Compared with the normal group,A20,TNF-α,IL-1β,p65 expression was significantly increased and COL-Ⅱ decreased in the mRNA and protein levels in degenerative group.Above indexes changed more significant in LPS stimulation group than in degenerative group.The expression of A20, TNF-α, IL-1β, p65 in the NF-κB inhibitor group was lower than that in the LPS group, and the expression of type Ⅱ collagen increased(P<0.05).Conclusions Intervertebral disc inflammatory response is closely related to the development of intervertebral disc degeneration, A20 may play an important role.
8.Analysis of Nosocomial Deep Mycosis in Our Hospital

OBJECTIVE:To prevent and reduce the occurrence of nosocomial deep mycosis.METHODS:The medical records of patients with nosocomial deep mycosis in the period from January2001to December2002in this hospital were ret?rospectively analysed.RESULTS:There were43patients with nosocomial deep mycosis,which accounted for10.54%of all nosocomial infections in the same period.The predilection sites were respiratory tract(34.88%)and digestive tract(30.23%). The main pathogen was Candida albicans(77.78%).All patients had serious underlying diseases and were treated with broad-spectrum antibiotics,and some of them had received adrenocortical steroids.CONCLUSION:Rational use of broad-spectrum antibiotics and adrenocortical steroids is the most important way to prevent deep mycosis and improve the prognosis.
