Objective: To explore the heat injury level of vocal cord by different power of the Diomed 25 semi conductor laser so as to provide scientific evidence for clinical laser therapy of larynx diseases. Methods: Canine was used. The power of laser was set on 5, 10 and 20 W respectively, and the time of laser exposure was fixed on 2 s.When canine vocal cord was hit by laser in vivo , the depth and width of the tissue heat injury were measured. Results: When laser hit with 5, 10 and 20 W for 2 s exposure time, the corresponding depth of the tissue heat injury were 0.2 0.4,0.4 0.6,0.8 1.0 mm respectively, the corresponding width of the tissue heat injury were 1.0 1.7,1.7 2.0,2.0 2.6 mm respectively. Objective To study the influence of theaflavin on the expressions of TLR4 and release of TNF-αand IL-6 in primari-ly cultured rat airway epithelial cells .Methods Lipopolysacchride(LPS) was adopted to establish the in vitro inflammatory injury model of rat airway epithelial cell ,the TNF-αand IL-6 secreted by the airway epithelial cells and the TLR4 protein level in the air-way epithelial cells were detected with ELISA and the expressions of TLR4 mRNA was detected with RT-PCR .Results LPS sig-nificantly induced the airway epithelial cells to secrete TNF-αand IL-6 ,and enhanced the TLR4 mRNA and TLR4 protein expres-sion(P<0 .01) .Theaflavin could inhibit LPS induced TNF-αand IL-6 secretion and TLR4 gene expression(P<0 .05) ,which was related with the theaflavin concentration (P<0 .05) .Conclusion In rat airway epithelial cell culture ,the anti-inflammatory effect of theaflavin may be decrease the release of inflammatory cytokines via inhibiting TLR 4 gene expression and LPS/TLR4 signal trans-duction pathway .

Objective To investigate the effect of video-based teaching in the training of nursing operation for newly-contracted nurses. Methods One hundred and twenty one nurses newly recruited in September 2011 to September 2012 were set as the control group, another 128 in October 2012 to October 2013 were set as the experiment group. The former were trained and assessed with traditional training method and the latter were trained for 1 year in video-based teaching methodology. After training, both groups were examined about their operation skills and meanwhile a survey on video-based teaching was conducted. Result After training, the results in operating skills evaluation in the experiment group were significantly better those of the control group (Z=2.82, P<0.05). Conclusion Compared with traditional training method, the video-based teaching can raise the level of nursing operation skills and significantly improve the quality of nursing, thus worthy of popularization and application.

Objective To compare the differences of liver function indexes in patients with acute left heart failure and congestive heart failure , and to provide the basis for the diagnosis of heart failure .Methods 142 cases of heart failure from March 2014 to December 2015 in our hospital were selected and divided into 68 cases of acute left heart failure group and 74 cases of congestive heart failure group.The differences in liver function tests were compared between the two groups by comparing the differences in the indexes of heart failure, liver blood biochemistry, general biochemical indexes and late follow-up data.Results Compared with the indexes of heart failure, there were no significant differences in ejection fraction, NT-proBNP, history of heart failure>one years, and the number of patients with right heart failure, the difference was statistically significant (P<0.05).Comparison of liver blood biochemistry: patients with acute left heart failure were alanine aminotransferase, aspartate amino transferase, albumin increased, the difference was statistically significant (P<0.05), and glutamyl aminotransferase, total bilirubin, direct bilirubin, indirect bilirubin decreased, the difference was statistically significant (P<0.05).There was no significant difference between the two groups of patients with alkaline phosphatase and total protein.Compared with the general biochemical indexes, increased triglyceride levels in patients with congestive heart failure group, the difference was statistically significant (P<0.05), there was no significant difference in other indexes.Conclusion AHF patients with ALT, AST increased, CHF with GGT, TBIL, DBIL, IBIL increased, ALB decreased mainly.

Purpose To study the clinicopathologic features and differential diagnosis of primary mucosal melanoma of the nasal cavity (PMMNC).Methods 17 cases of PMMNC diagnosed from January 2003 to September 2016 were studied by clinical pathological analysis and immunohistochemical staining,and relevant literatures were reviewed.Results 73％ of the PMMNC was characterized by unilateral nasal congestion and intermittent epistaxis and 61％ of the PMMNC occurred in the nasal septum and nasal side wall.Microscopically,the organizational structure and morphology were complex and diverse,which had several cell types including epithelioid cell type (6cases,35.3％),spindle cell type (3 cases,17.6％) and snall cell type (5 cases,29.4％),the other 3 cases (17.6％)were mixed cell type.Mitotic activity and tumor necrosis were more likely to be seen in PMMNC,among other clinicopathological features with a small amount of fibrous stroma and melanoma and rich blood vessels.The immunohistochemical study showed that the positive rate of S-100 and HMB-45 were both 93.8％(15 cases) and those of Melan-A and vimentin were both 87.5％ (14 cases),while CK and EMA were both negative (16 cases).Conclusion PMMNC is a rare disease and the phenotype of S-100,HMB-45,Melan-A and vimentin are useful for diagnosis of PMMNC.

OBJECTIVE To understand the pathogen distribution and drug resistance of pelvic inflammatory disease in women,then help the clinic to use drug rationally. METHODS Totally 342 bacteria were isolated from the cervical or pelvic secretion and given in vitro drug sensitivity test with Kirby-Bauer method. ESBLs detected by ESBLs affirm test and AmpC detected by cefoxitin three dimensional test in G-bacteria. RESULTS From the 342 strains isolated from this group,there were 215 G-(62.9%),the primary pathogenic bacteria were Escherichia coli,Klebsiella pneumoniae and Serratia marcescens,at the same time,there were 127 G+ (37.1%),the most common pathogens were coagulase-negative staphylococci (CNS) and Staphylococcus aureus. The detectable rate of ESBLs and AmpC together were 38.1% and 34.9% in G-,that of only ESBLs occupied 13.0%,that of only AmpC was 9.8%,that of ESBLs combined with high yield AmpC occupied 13.9%,and that of ESBLs combined with induced AmpC occupied 11.2%. Otherwise,the resistance rate to antibiotics was all higher than 48% except VAN,TEC,QDA and rifampin in G+. The rate of resistance to IPM,MEM and FEP were 5.58%,3.72%,and 26.0% respectively in G-,and the drug resistance rate of enzyme-prodncing G-were much more than that without enzyme-producing (P
