1.Research on the relationship of recurrence of cerebral infarction with total sleeping time

Objective Cerebral infarction is a main disease threatening human health and the recurrence of cerebral infarction is more inclined to cause disability and death.The article aimed to explore the influence of total sleeping time on the recurrence of cerebral infarction.Methods According to the criteria, 70 cases of cerebral infarction patients who hospitalized in our hospital from March 2016 to November 2016 were selected to undergo polysomnography (PSG) and were divided into two groups according to medical history: original group and recurrence group.Another 20 simultaneous outpatients who had no significant diseases to undergo PSG were chosen as the control group.Data were collected on all the individual''s total sleeping time (TST), gender and age.Hypothesis testing and logistic related analysis were applied in data analysis.Results The TST of recurrence group was shorter than those of the original group and the control group (P<0.05), among which the TST of the original group is shorter than that of the control group (P<0.05), both representing significant difference.Multiple regression analysis showed that TST was an independent risk factor for the recurrence of cerebral infarction and that the risk of recurrence of cerebral infarction increased with the lack of TST.Conclusion The shortening of TST may increase the risk of the recurrence of cerebral infarction.
2.Reason analysis of catheter-related infections in ICU patients and the countermeasures

[Objective] To explore the reasons of catheter-related infections in ICU critical pa-tients and raise up the countermeasures.[Methods] The clinical data of 46 patients with catheter-related infections were surveyed and analyzed from May 2010 to March 2011 in our hospital.[Results] Elderly patients occupied the majority of ICU patients with catheter-related infections.The main infection sites included the respiratory system,urinary system and gastrointestinal system.Establishment of artificial airway was the main cause of infection,indwelling catheter came next.[Conclusions] ICU patients has a high incidence of catheter-related infections.We should handle the ICU management appropriately,strengthen the control and prevention of factors of catheter-related infections,strictly enforce disinfection and segregation regulation,cut off infection way,enhances the resistance of patients,limit antibiotic use and various invasive operation in order to reduce the incidence of catheter-related infections.
3.Modification of GABAA receptor subunit mRNA content in audiogenic seizure rat cortex and hippocampus following tolerance to flurazepam

Objective: To investigate the modification of GABAA receptor subunits in audiogenic seizure rat cortex and hippocampus when rendered tolerant to flurazepam. Methods: Rats were administrated with flurazepam for two weeks, which resulted in tolerance to flurazepam, in the absence of behavioral signs of withdrawal. GABAA receptor subunits α1, α3, α5, γ2L and γ2S were assayed using quantitative competitive RT-PCR in rat frontparietal motor(FrPaM) and hippocampus. Results: In FrPaM, the contents of mRNA encoding for α1, α3, γ2L and γ2S were all significantly decreased (24%,17%,35% and 45% respectively),whereas that of α5 was significantly increased (33%) compared with the control. In hippocampus, α1, γ2L, and γ2S mRNA contents were significantly decreased (33%, 35%and 27% respectively). Conclusion: The accomodated change in GABAA receptor subunits α1, α3, α5,γ2L and γ2S in FrPaM and hippocampus may be associated with the mechanism of flurazepam tolerance in audiogenic seizure rats.
4.Effects of esomeprazole and omeprazole on clopidogrel platelet inhibition following cardiac stent implantation

OBJECTIVE:To compare the effects of omeprazole and esomeprazole,two kinds of proton pump inhibitors,on clopidogrel platelet inhibition following cardiac stents implantation.METHODS:Totally 180 patients with coronary artery disease underwent percutaneous coronary intervention (PCI) at the Zhujiang Hospital of Southern Medical University from June 2008 to May 2009 were selected,including 83 males and 97 females.All patients were randomly divided into 3 groups,omeprazole + clopidogrel + aspirin group (OCA group,receiving omerprazole 20 mg/d),esomeprazole+clopidogrel+aspirin group (ECA group,receiving esomeprazole 10 mg/d),and control group (No proton pump inhibitor),with 60 patients in each group.In addition,all patients received a 300 mg clopidogrel and 0.1 g aspirin prior to PCI,and received 75 mg/d clopidogrel and 100 mg/d aspirin treatment for 1 week after PCI.Blood samples from patients were obtained from cubital vein before and at 1 week after adminstration,respectively.The vasodilator stimulated phosphoprotein phosphorylation state and platelet reactivity index (PRI) were calculated by flow cytometric assay.RESULTS:The PRI had no significant difference before administration (P>0.05),which was obviously decreased at 1 week after administration (P<0.05),especially lowest in the control and ECA groups (P<0.05).However,the PRI differences between the control and the ECA group had no significant (P>0.05).CONCLUSION:The administration of omeprazole rather than esomeprazole is associated with impaired clopidogrel platelet inhibition.Esomeprazole can be used as one of the preferred proton pump inhibitor in curing gastrointestinal bleeding caused by anti-platelet therapy following cardiac stents implantation.
5.Application of contrast-enhanced ultrasound in brain perfusion

Ultrasound brain perfusion imaging can evaluate brain perfusion in normal and pathological conditions. After basilar stent implantation, contrast-enhanced ultrasound (CEUS) can show normal visualization of color flow within the stent and provide essential functional information by evaluation of blood flow velocities. Furthermore, CEUS can estimate the volume of brain hemorrhage and readily identify residual tumor. CEUS can be used in thrombolysis, and will be applied in cerebral trauma. The value of CEUS in brain perfusion imaging was reviewed in this article.

CONCLUSION The accomodated change in GABA A receptor subunit ?1, ?5,?2L and ?2S in FrPaM and hippocampus may be associated with the mechanism for tolerance to clobazam in audiogenic seizure rat.

9.Endovascular treatment modalities in patients with ischemic stroke

Ischemic cerebrovascular disease is one of the major reasons of mortality and disability in adult populations.Ultra-early recanalization is by far the most positive treatment.Relative to intravenous thrombolysis,various endovascular treatment modalities can extend therapeutic time window and increase the recanalization rate.Furthermore,interventional therapy can also improve the clinical outcomes of patients by increasing the perfusion of colhteral circulation.This article reviews various endovascular treatment modalities and their application validated in previous clinical studies in China.
