1.Analysis of CT Signs of Small Peripheral Lung Cancer

Objective To understand further the CT signs of small peripheral lung cancer and to improve its diagnostic rate by CT.Methods 63 patients with small peripheral lung cancer(diameter was approximately 3 cm)were examined with spiral CT scanning before operation.All cases were confirmed by surgery and pathology.And additional high resolution spiral CT scanning was done in 28,and enhanced CT scanning was done in 41 of them.The size,edge signs,internal texture and enhanced degree were analyzed.Results Of 63 cases,CT findings included the lobular sign in shape in 57,burr sign in 56; sign of hollowed pleura in 48; sign of blood vessel bundle in 36; sign of small bubbles in 15; sign of air bronchus in 6.Of 41 cases with additional enhanced CT scanning, the increasing range in CT value of 34 cases was over 30 HU.Conclusion Peripheral pulmonary nodules which are of more than 3 edge signs on CT are highly suspected to be peripheral pulmonary carcinoma.Enhanced CT scanning is very helpful in distinguishing benign and malignant pulmonary nodules.
2.Study on the evaluation of different levels of QRS-T angle in patients with acute myocardial infarction

Objective To investigate the evaluation of different levels of QRS-T angle in patients with acute myocardial infarction (AMI).Methods A total of 186 AMI patients from January 2010 to May 2016 in our hospital as the research object.The review method was used to analyze the clinical and followup data of the patients.The left ventricular ejection fraction (LVEF),plane QRS-T angle,white blood cell (WBC) and isoenzyme count and creatine (CK-MB) were evaluated and compared.Results (1) The age of the patients with heart failure was larger than that in the patients with normal heart function,and the proportion of myocardial infarction and hypertension and diabetes distributionthe in two groups had statistically significant difference (P < 0.05);(2) The WBC and CK-MB levels of patients with normal cardiac function were lower than those in patients with heart failure.The planar QRS-T angle < 50 degrees,between 90 degrees and 100 degrees and more than 100 degrees were statistically significant between normal heart function patients and failure heart function patients (P < 0.05),and with the decrease of LVEF the levels of QRS-T angle increased;(3) The sensitivity and specificity of 90 degrees ≤ QRS-T angle < 100 degrees and QRS-T angle greater than 100 degrees were 96.32% and 95.98%,95.02% and 94.68%,and those were different with the sensitivity and specificity of WBC,CK-MB and other QRS-T angle(P < 0.05).Conclusions Different planar QRS-T angle on the evaluation of cardiac function insufficiency has certain clinical value,but when the 90°≤ QRS-T angle < 100° and QRS-T angle ≥ 100° have better sensitivity and specificity with no difference between two QRS-T angles.The QRS-T angle≥ 90° can be used as one of the indicators for the evaluation of cardiac function insuffieiency.
3.The value of the combined detection of IMA, NT-proBNP and dynamic electrocardiogram in the diagnosis of silent myocardial ischemia

Objective:To evaluate the clinical value of combined detection of ischemia modified albumin(IMA), N-terminal pro brain natriuretic peptide(NT-proBNP) and dynamicelectrocardiogram(DEG) in the diagnosis of silent myocardial ischemia.Methods: IMA, NT-proBNP and DEG of 114 patients with silent myocardial ischemia who were suspected coronary heart disease were detected, and then these results were compared and analyzed with the results of coronary angiography(CAG).Results: In 114 patients with suspected coronary heart disease, 68 cases(59.65%)were positive as the results of CAG. And there were 60 cases were positive as the results of IMA detection, and the specificity and sensitivity of IMA compared with CAG were 82.61% and 88.24%, respectively. There were 63 cases were positive as the results of NT-proBNP, and the specificity and sensitivity of NT-proBNP compared with CAG were 80.43% and 92.64%, respectively. There were 57 cases were positive as the results of DEG, and the specificity and sensitivity of DEG compared with CAG were 86.96% and 83.82%, respectively. While there were 66 cases were positive as the combined detection of the three methods, and specificity and sensitivity of the combined detection compared with CAG were 95.65% and 97.06%, respectively. Therefore, the specificity and sensitivity of combined detection were significantly higher than that of alone detection of IMA, NT-proBNP and DEG (F=140.637,F=255.467,P<0.05).Conclusion: The comprehensive analysis for the combined detections of IMA, NT-proBNP and DEG can increase both of the specificity and sensitivity of diagnosis, and it can be used as a important screening method for earlier finding silent myocardial.
4. Promoter region methylation analysis of glutathione S-transferase p1 gene in prostate carcinoma

Objective: To investigate the promoter region CpG island methylation status of glutathione S-transferase p1 (GSTP1) gene in prostate carcinoma (PCa) tissues and to search for new molecular markers for the early diagnosis of PCa. Methods: The promoter region CpG island methylation status of 31 PCa, 18 benign prostatic hyperplasia tissues (BPH) and 3 normal prostate tissues (NP) were examined by nest methylation-specific PCR (NMSP) technique, cloning and sequencing. Results: NMSP showed that the methylation rates of PCa, BPH and NP tissues were 83%, 0% and 0%, respectively. The CG sites methylation rates of PCa, BPH and NP tissues were 96%, 34% and 37%, respectively as determined by cloning and sequencing (P<0.01), with no significant difference found between those of BPH and NP tissues(P>0.05). A combination of NMSP and prostate-specific antigen (PSA) was better than PSA alone in screen of PCa. Conclusion: CpG islands of promoter region of GSTP1 gene is hypermethylated in PCa tissues. NMSP is sensitive and specific in detection of GSTP1 methylation, which may serve as a new method for early screen of prostate carcinoma.
6.Research progress of RNF180 value to the diagnosis of gastric cancer

RNF180 is a novel membrane-bound E3 ubiquitin ligase that participates in cell development,proliferation and apoptosis.It is a tumor suppressor gene that inhibits cell proliferation and induces apoptosis and may inhibit gastric cancer cell lymph node metastasis.The study found that RNF180 gene methylation and gastric cancer is closely related to the occurrence and development.Therefore,RNF180 gene methylation is expected as a tumor marker of gastric cancer for early diagnosis and prognosis of gastric cancer.In this paper,RNF180 on the diagnosis of gastric cancer research progress made a review.
7. Analysis of Influential Parameters on Tacrolimus Dosage and Concentration One Year after Lung Transplantation

OBJECTIVE: To investigate the influence of patients' genetic and clinical parameters on tacrolimus dosage and concentration after lung transplantation. METHODS: A total of 50 patients who had received lung transplantation in our hospital from April 2017 to August 2018 were enrolled in this study. Information was collected 1 year after transplantation. Tacrolimus concentration was determined by chemiluminescent microparticle immunoassay (CMIA) and CYP3A5 genotype was detected by Sanger sequencing. Body weight adjusted dosage (D) and dosage adjusted concentration (C/D) were calculated. Influence of CYP3A5 genotype on D and C/D were analyzed. Multiple linear regression was performed to determine influential factors on tacrolimus dosage 1 year after lung transplantation. RESULTS: While maintaining tacrolimus concentration within therapeutic range, dosage requirement for CYP3A5*1/*1 and *1/*3 patients was higher than that of *3/*3 patients and C/D value was lower than that of *3/*3 patients (P<0.05). Multivariable linear regression results showed that CYP3A5 genotype, hemoglobin and age had significant influence on tacrolimus D, among which hemoglobin and age showed negative correlation (P<0.05). Prediction equation for tacrolimus D was obtained by multivariable linear regression. The correlation coefficient between predicted dose and actual dose was 0.824 (95% CI: 0.705-0.893). CONCLUSION: Tacrolimus dosage after lung transplantation is influenced by CYP3A5 genotype, hemoglobin and age. Using these factors for tacrolimus dosage prediction could possibly be instructive for individualized use of tacrolimus.
8.A report of 103 cases of breast intraductal papillomatosis (BIDP)

Objective To analyze the relationship between BIDP and breast cancer, and to evaluate the treatment of BIDP. Methods 103 cases of BIDP were studied retrospectively in respect of its clinical and pathological features, diagnosis, and treatment. Results Among the patients, there were 102 women and 1 man. The diagnosis was all verified by pathological examination. The rate of misdiagnosis was high(62.1%). Local excision was performed in 18 patients, regional lobular excision in 62, in nine patients modified radical mastectomy for malignant change in 9 patients, and simple excision for malignancy in 2 patients. The recurrent rate after local excision was 11.7% after 13 years, and the rate of malignant change was 6.8%. Conclusions To sum up the relationship and differentiate method between BIDP and Breast Cancer, and briefly discuss the methods of treatment.
9.Application of p504S、CK34?E12、p63 in the differential diagnosis of benign and malignant lesions of the prostate

Objective To evaluate the value of p504S,CK34?E12 and p63 immunostaining for the diagnosis of benign and malignant lesions of the prostate. Methods Expression of p504S、CK34?E12 and p63 in benign and malignant lesions of the prostate was observed by microwave SP immunohistochemical method. Results Continuous positive expression of CK34?E12 and p63 was found in the acini and ducts of most benign prostatic hyperplasia, and few cases showed interrupted expression, but p504S staining presented negative reaction; Interrupted expression of CK34?E12 and p63 was found in high prostatic intraepithelial neoplasia cases, and was weekly positive or negative for p504S; CK34?E12 and p63 staining was positive in five atypical adenomatous hyperplasia cases, one case presented focally positive reaction by p504S; CK34?E12 and p63 staining was negative in forty prostatic carcinoma, but positive by p504S;The gleason histologic grading of prostate carcinoma was negatively related to expression of p504S. Conclusion Combined detection of p504S、CK34?E12 and p63 can be most helpful for diagnosis of prostate carcinoma, especially in needle biopsy specimen.
10.Laparoscopic repair of abdominal wall incisional hernia:A report of 41 cases

Objective To investigate effects of laparoscopic repair of abdominal wall incisional hernia using polypropylene and expanded polytetrafluoroethylene(e-PTFE) composite mesh. Methods Forty-one patients with abdominal wall incisional hernia(4~25 cm in length and 3~18 cm in width) were treated in this hospital from October 2004 to August 2005. The patients received laparoscopic mesh herniorrhaphy after complete dissection of adhesion using an ultrasonic scalpel.A polypropylene and(e-PTFE) composite mesh(Bard Composite Mesh) was used and fixed using the Ethicon Endopath Multifeed Stapler(EMS).Results The laparoscopic mesh herniorrhaphy was successfully completed in all the 41 patients without conversions to open surgery.The operative time was 60~182 min(mean,85 min).Postoperatively,the patients felt slight pain and began to take food on the second day.The time to first passing flatus was 25~41 hours(mean,32 hours).The postoperative hospital stay was 5~7 days(mean,6 days).No recurrence occurred during a follow-up period of 6~16 months(mean,9 months) in the 41 patients. Conclusions The laparoscopic incisional hernia repair using the Bard Composite Mesh is a safe and effective method.
