500 ng/ml. Spiral-CT pulmonary arteriography(CTPA) was the main diagnostic method. Nineteen patients received thrombolytic therapy, Twenty-nine patients received anticoagulative therapy, 2 patients didn't receive any therapy because of bleeding. Five(10%) patients were cured, 43 (86%) patients got better, 2(4%) patients were dead. Conclusions The clinical manifestations of pulmonary embolism in the elderly were varied. CTPA is the main diagnostic method and ventilation-perfusion lung scan or pulmonary angiography can be done when it is necessary. Thrombolytic treatment can improve the survival rate of patients with acute pulmonary embolism.

As important adhesion molecule on cell surface,integrin is a kind of important receptor family. It′s im- portant role is to mediate intercellular adhesion and adhesion between cells and extracellular matrix,also regulate proliferation,differentiation,migration of cells and tissue remodeling,and plays an important role in maintaining cellular form and growth.The present article made a short review about research progress of integrin β1 effect in cardiovascular diseases in recent years.

Objective To investigate the characteristics of early changes in serum IL-17 and IFN-γ levels in elderly patients with acute myocardial infarction (AMI).Methods 70 hospitalized elderly patients with AMI and 35 healthy people were selected.Serum level of interleukin-17 (IL 17) and interferon-γ (IFN-γ) were assayed by enzyme linked immunosorbent assay (ELISA).Results Serum levels of IL17 and IFN-γ showed increasing trends in elderly patients with AMI as compared with that in control group,but there were no significant differences between the two group [(35.73 30.28) pg/ml vs.(28.70±17.12) pg/ml,(15.29±14.64) pg/ml vs.(11.38±10.10) pg/ml,t=0.144 and 0.138,P=0.365 and 0.377].There were correlations between serum IL-17 and IFN γ levels in patients with AMI and controls(r=0.936 and 0.989,both P=0.00).Serum levels of IL-17 or IFN-γhad no significant differences between AMI patients with well and poor prognosis [(35.43± 34.36) ng/L vs.(36.11±30.16) ng/L,(13.90±13.98) ng/L vs.(15.99±14.14) ng/L,U=0.266 and 0.166,P=0.687 and 0.668].Conclusions Serum IL-17 level has an increasing trend in AMI patients within 24h,but has no statistical significant.Serum IL 17 level has a significantly positively correlation with serum IFN γ level in the elderly,but serum levels of IL-17 or IFN γ have no significant correlations with short term prognosis in elderly patients with AMI.

Objective To evaluate the clinical effect of bandage contact lens on patients after the pterygium excision surgery.Methods 200 patients with pterygium excision were used retrospectively analysis and were divided into control group and observation group by random digital table method.Patients in observation group wore bandage contact lens while those in control group did not (traditional method).The degree of pain in both groups was evaluated at 8 hours,24 hours and 48 hours after surgery.In addition,patients were surveyed for the satisfaction of our treatment when discharge.Results The ocular foreign body sensation,eye burning,tears and eye open difficult were obviously decreased in observation group (t=5.643,10.315,P ＜ 0.01).The overall comfort level of the patients in observation group was obviously improved.pain feeling of the eye after surgery in 8h,24h and 48h markedly decreased in observation group (t=6.196-12.495,P ＜ 0.01).The degree of satisfaction scores of preoperative guidancein,postoperative pain control,emotion,mental nursing,self-care ability,comfort in observation group were (1.59-0.62) points,(1.83±0.70) points,(1.42±0.68) points,(1.42±0.68) points,(1.48±0.67) points,(1.29±0.77) points,(1.43-0.64)points which were obviously increased than (0.76±0.55) points,(0.83±0.64points,(0.82±0.56) points,(0.80±0.47) points,(0.79±0.61) points,(0.66±0.54) points (t=4.311-9.354,all P＜ 0.01).Conclusions Compared with traditional treatment,bandage contact lens can significantly reduce the discomforts after the pterygium excision surgery,and increase the postoperative comforts of patients.It proves that bandage contact lens have extensive clinical value.

Objective To improve the cure rate of hepatobiliary lithiasis.Methods Two hundred and seventy-five patients with retained gallstones in the hepatic bile ducts after choledochostomy were treated with choledochofiberscope from August 1989 to August 2001 in this hospital.Results In 252 cases(91.6%), the residual gallstones were thoroughly removed with choledochoscope and in remaining 23 cases(8.4%), the stones were not completely removed. The factors affecting the results of the procedures include improper locating of T-tube, the exfoliation of T-tube and closure of fistula, the size and the incarceration of retained gallstones, the narrowed bile ducts, the performance of intraoperative choledochoscope, the method of operation.Conclusions Choledochofiberscope is of great value in surgical treatment of hepatobiliary lithiasis. However it can't completely replace to remove the damaged liver caused by the gallstones.

Objective To investigate the clinical value of the plasma N-terminal pro-brain natriuretic peptide levels in patients with acute cardiac dyspnea or noncardiac dyspnea.Methods 93 patients with acute dyspnea were divided into two groups:cardiac dyspnea group(61 cases)and noncardiac dyspnea group(32 cases).In cardiac dyspnea group,according to the results of echocardiography,the cases were divided into three groups:pure diastolic heart failure(25 cases),pure systolic heart failure(21 cases)as well as diastolic and systolic heart failure(15 cases).At the same time,heart function was classified according to NYHA.The plasma NT-proBNP level was measured by ELISA.The data was analyzed by SPSS12.0.Results The plasma NT-proBNP level of the cases with cardiac dyspnea and noncardiac dyspnea was(3.591 ?2.943)ng/L and(0.429 ?0.119)ng/L respectively.There was a significant difference between cardiac dyspnea cases and noncardiac dyspnea cases(P

Objective To investigate the prognostic value of National Early Warning Scores (NEWS) combined with arterial lactate level in elderly patients with critical illness in emergency department.Methods A total of 413 elderly patients with critical illness randomly (random number) selected from resuscitation room in emergency department from September 2013 to July 2015 were enrolled in this study.NEWS system was employed to determine the patients' condition.Arterial lactate level was detected.The patients were followed for the 30-day death by telephone calls and patients were divided into survival group and death group.Multivariate logistic regression analysis and receiver operating characteristic curve (ROC) were used to evaluate the prognostic values of NEWS score,arterial lactate level,and NEWS score combined with arterial lactate level in patients without survival.Results There were significant differences in NEWS score and arterial lactate level between the death group and the survival group.The results of multiple factor regression showed that NEWS score,arterial lactate level,and NEWS score combined with arterial lactate level were all independent predictors of 30-day death (P ＜ 0.05,OR ＞ 1).The AUCs of the three evaluation modalities were 0.721,0.746 and 0.762,respectively.There was a significant difference in the area under the ROC curve between NEWS score and NEWS score combined with arterial lactate level.The optimal cut-off point of NEWS score was 7,and arterial lactate level was 2.7 mmol/L.Conclusions NEWS score,arterial lactate level,and NEWS score combined with arterial lactate level were independent predictors of 30-day death in elderly patients with critical illness in emergency department.They all had some predictive value,and NEWS score combined with arterial lactate level had higher capability to predict 30-day death in elderly patients with critical illness in emergency department than NEWS score alone.

Objective To evaluate the efficacy of laparoscopic first-stage Fowler-Stephens for high cryptorchidism. Methods Seven children with high cryptorchidism (10 testes) were enrolled in this study. After detecting the testicles under a laparoscope, we dissected the spermatic cord vessels. Then, the spermatic duct was separated, and the gubernaculum testis was retained. Afterwards, the first-stage orchidopexy was performed laparoscopically. Results Nine of the 10 testes were successfully placed into the scrotum by first-stage Fowler-Stephens. In one patient who had bilateral cryptorchidism, the left testis was placed into the scrotum at the second stage operation. The seven patients were followed up for 6-24 months (mean, 14 months). None of them had retraction or atrophy of the testis. Conclusions Laparoscopic first-stage Fowler-Stephens is effective for high cryptorchidism. The procedure is worth being widely used.
