1.CT quantitative diagnosis in fatty liver: a clinical study

Objective To establish the CT criteria of quantitative diagnosis for liver steatosis by means of studying the CT features of fatty liver cases proven histologically. Methods Twenty-eight cases of fatty liver were underwent non-enhanced CT scan, and the attenuation of liver parenchyma was measured. To differentiate the degree of fatty liver, the mean CT value and the relative density of hepatic vessels were observed. The quantitative diagnosis was made according to the CT number threshold and the criteria of relative density of hepatic vessels, respectively. Results Among the 28 cases, there were 17 cases of mild steatosis with mean CT number of 46 HU (32-65 HU), 7 cases of middle degree fatty liver with mean CT number of 28 HU (15-38 HU), and 4 cases of sever fatty liver with mean CT number of 0.2 HU (-7-11 HU). For the relative density of hepatic vessels, 16 of the 17 cases of mild fatty liver had a appearance of hepatic vessels immersion and 1 mild case had reverse hepatic vessels display, 6 of 7 middle degree cases had reverse hepatic vessels display with 1 case having the appearance of hepatic vessels immersion, and all the 4 case of sever steatosis had the appearance of reverse hepatic vessels display with sharp contrast between vessels and the liver parenchyma. 2.Primary discussion of qualitative and quantitative recognition on deqi after acupuncture: a study report of West China School of Medicine.

OBJECTIVETo determine the recognition of healthy medical students on deqi after acupuncture, reveal the qualitative and quantitative rules of deqi and understand whether these rules are the factors of the clinical application of acupuncture therapy.
METHODSThe class questionnaires were used for the investigation study on the understanding of deqi after acupuncture at Hegu (LI 4) or Zusanli (ST 36) in 86 healthy students in the clinical medicine class.
RESULTS(1) Deqi was a kind of complicated compound feelings, with many sensation qualities such as distending pain, distension and pain. (2) Deqi was a kind of mild and moderate sensations. In 10-score credit sys tem of Massachusetts General Hospital acupuncture sensation scale (MASS), the scores of distending pain (4.69 +/- 2.83), distension (4.39 +/- 2.91) and soreness and distension (3.93 +/- 2.93) were around 5 (moderate degree), the scores of stabbing pain (1.89 +/- 2.02) were around 2 (mild degree). (3) The differences in the quantitative scores were significant for stabbing pain, distending pain, distention, soreness and distention and the others before and after treatment (all P<0.05).
CONCLUSIONDeqi of acupuncture is the mild and moderate complicated sensations manifested as distension, soreness, pain and numbness. As the invasive therapy, the filiform needle puncture will bring a certain psychological impacts on the receptors. The subjective sensation is possibly the factor for the patients' selection of acupuncture treatment.

3.Detection and Drug Resistance Analysis of Ureaplasma urealyticum and Mycoplasma hominis of Urogenital Tract

OBJECTIVE To find out the status about detection and drug resistance of Ureaplasma urealyticum(Uu) and Mycoplasma hominis(Mh) of urogenital tract infection in Shaoxing area of Zhejiang Province.METHODS The detections and drug sensitive tests of Uu and Mh adopting the reagent boxes produced by bioMerieux Company in France were made.RESULTS In 2678 samples of suspicious urogenital tract infection patients,980 samples cultured of mycoplasma were positive,and the positive rate was 36.6%.Among these cases,763 samples(28.5%) were Uu positive;116 samples(4.3%) were Mh positive.In the drug sensitive test,Uu and Mh were most sensitive to PRI,JOS,DOX and TET.To CFLX,OFL,ERY,CM and AM,they had the highest level of drug resistance.CONCLUSIONS The clinical therapy of mycoplasma infection ought to choose the appropriate antibiotics based on the results of the drug sensitive test of the laboratory.

5.Applications of digital blood collection quality training modular in outpatient blood collection room

Objective To discuss training of the nurses in outpatient blood collection room with the digital training modular of blood sample collection to improve the quality of blood sample collection. Methods Nurses were trained with the digital training modular by multimedia,group discussion to impmve the quality of blood sample collection continuously. Results The unqualified blood sample rate in the same season after training were statistically different compared with that before training. Conclu-sions Training the nurses in outpatient blood collection room with the digital traimg modular of blood sample collection have actual direction value to improve the quality of blood sample collection.

Sixteen compounds including daphnoretin (1), isofraxidin (2), scopoletin (3), kaempferol (4), quercetin (5), guaijaverin (6), astragalin (7), quercetin-3-O-β-D-glucopyranoside (8), naringenin-7-O-β-D-glucopyranoside (9), 5-O-methylapi- genin-7-O-β-D-glucopyranoside (10), methyl gallate (11), prionitiside A (12), (2S)-2,3-dihydroxypropyl-1,6,8-trihydroxy-3- methyl-9,10- dioxoanthracene-2-carboxylate (13), 3,3'-di-O-methyl ellagic acid (14), 3'-O-methyl-3,4-O,O-metheneellagic acid-4'-O-β-D- glucopyranoside (15) and 3,4-methylenedioxy-3'-O-methylellagic acid (16), were isolated from the 70% acetone extract of Euphorbia dracunculoides Lam. Among them, compounds 1-3, 6-9, 11, and 14 were isolated from E. dracunculoides for the first time, and compounds 10, 12, 13, 15, and 16 were firstly obtained from the genus Euphorbia. Their structures were elucidated by spectroscopic analysis, including 1H-NMR, 13C-NMR, and ESI-MS.

7.Prevention and treatment of stricture after esophageal burns in 168 cases

Objective To summarize our experience in prevention and treatment of stricture after esopageal burns in the past thirty years.Methods There were 168 cases in this series.Of them,158 cases underwent surgical management in this study.Modified intraluminal stenting was used in 34 cases, colon interposition without resection of strictured esophagus in 77 cases,gastric transposion with resection of the stricture in 27,repair of cervical stricture with platysma myocutaneous flap in 22,and miscellane- ous operation in 12.Eleven cases experienced operation twice or more at our department.Results Twenty-nine cases recovered after treatment with intraluminal stenting,and 5 re-experienced stricture after stent removal.One of the 5 cases with failed stent responded to bougienage,and the remaining 4 cases re- quired esophageal reconstruction later.Of the 77 colon interpositions,5 cases died postoperatively,and complications of cervical anastomotic fistula occurred in 14 cases,anastomotic stenosis in 4,and abdomi- nal incision dehiscence in 2 cases.In the 27 cases with gastric transpositions,postoperative complications of anastomotic stricture occurred in 2 cases and empyema in 1 patient.There was a cervical leak in 3 ca- ses of the 22 cases treated with the repair of cervical esopageal or anastomotic stricture with a platysma myocutaneous flap.In the 12 cases treated with miscellaneous operation,one died of intestinal obstruc- tion.All the survivors had regular diet after discharge.Conclusions Intraluminal stenting can prevent the formation of caustic esophageal stricture.The location of the cicatricial esophagus dictates whether to perform concomitant esophagectomy during esophageal reconstruction.Platysma myocutaneous flap repair is an excellent method for the treatment of severe cervical esophageal or anastomotic stricture.
8.Analysis of main influence factors on coronary artery image quality with 64-multidetector row helical CT using a pulsating cardiac phantom

Objective To explore the main influence factors(heart rate,rotation speed,and reconstruction algorithm)on the image quality of coronary artery with 40 mm VCT(64-detector row helical CT)using a pulsating cardiac phantom.Methods An adjustable pulsating cardiac phantom(GE) containing predetermined simulated coronary arteries was scanned using a 40 mm VCT(GE LightSpeed CT) with cardiac pulsating rates of 40,45,50,55,60,65,70,75,80,85,90,95,100,105,110,and 115 beats per minute(bpm).The variable rotation speeds technique of 0.35 s,0.40 s,and 0.45 s were used, respectively.The raw data were reconstructed using both one-sector and multi-sector reconstruction algorithm at optimal window of the R-R interval.The image quality score(IQS)was evaluated by two radiologists according to the same evaluation standard of reformated image.The correlation between heart rate(HR), roation speed,reconstruction algorithm,and IQS were analyzed.The IQS as independent variable and the HR,rotation speed,reconstruction algorithm as dependent variables were analyzed by multiple linear regression analysis.Restllts The heart rate and the reconstruction algorithm had significant influence on IQS.The rotation speed(0.35s,0.40s,and 0.45s)didn't have significant influence on IQS.There was linear regression relationship between heart rate,reconstruction algorithm and IQS(P

Objective To investigate the safety and feasibility of intraluminal brachytherapy using iodine-125 seed strand for locally advanced pancreatic ductal adenocarcinoma with obstructive jaundice.Methods Clinical data of 17 consecutive patients,from January 2010 to February 2015,diagnosed with pancreatic ductal adenocarcinoma (4 cases of T4 N0 M0 and 13 of T4 N1M0) with obstructive jaundice and received intraluminal brachytherapy using iodine-125 seed strand were collected and analyzed retrospectively.Liver function was evaluated using paired-samples t test.The iodine-125 seed strand radiation doses were calculated using iodine-125 radiation field distribution calculation software (version 0.1,Institute of Radiation Medicine,Fudan University,Shanghai,China) based on the American Association of Physicists in Medicine TG43U1 brachytherapy formula.Obstruction free survival and overall survival were calculated using the Kaplan-Meier method.Complications were assessed according to the CTCAE 4.0 criteria.Results The estimated mean accumulating dose (r =5 mm,240 days) was 167.2Gy,from 164.19Gy to 170.05Gy.The mean and median obstruction free survival time were (9.62 ± 1.47) months (95%CI:6.73-12.50) and (7.26 ± 1.71) months (95 %CI:3.90-10.62).The mean and median overall survival time were (9.89 ± 1.59) months (95%CI:6.78-13.00) and (7.26 ± 1.71) months (95 % CI:3.90-10.62),retrospectively.Total bilirubin and conjugated bilirubin decreased significantly after the therapy.Two patients had adverse event of Grade 3,one of Grade 4.Stent dysfunction occurred in 1/17 (5.9 %) patients.Conclusions Intraluminal brachytherapy using iodine-125 seed strand might be considered as a safe treatment option for the locally advanced pancreatic duct adenocarcinoma complicated by obstructive jaundice.
10.Effects of enalapril on inflammation in kidney of diabetic rats and its possible mechanism

AIM: To investigate the effects and mechanism of enalapril on nephritis of diabetic mice. METHODS: Diabetes was induced by injection of streptozotocin after uninephrectomy. Rats were randomly divided into three groups: control, diabetes, diabetes treated with enalapril (10 mg?kg~-1 ?d~-1 by gavage). 8 weeks after STZ injection, urine albumin excretion rate (AER) were measured, and glomerular morphology were observed by light microscopy. The levels of malonyldialdehyde (MDA) in renal tissue and urine as well as activities of superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-PX) in renal tissue were determined. Immunohistochemistry for ED-1 (macrophage marker), monocyte chemoattractant protein-1 (MCP-1) and intercellular adhesion molecule-1 (ICAM-1) were performed by streptavidin-biotin complex (SABC) technique.
