Objective To compare the diagnostic value of the Standardized Bedside Swallowing Assessment (SSA)and the Scottish Intercollegiate Guidelines(SIC)for bedside dysphagia screening of patients after acute cerebral infarction(ACI). Methods Eighty-six ACI patients within 24 h of stroke onset were enrolled in the study.Within 48 h after hospitalization,swallowing function assessments and dysphagia screenings were carried out with the SSA and SIG,as well as video fluroseopic swallowing studies(VFSSs).The SSA and SIG assessments generated sensitivities,specificities,positive predict values(PPVs),negative predict values(NPVs),positive likelihood ratios (PLRs)and negative likelihood ratios(NLRs).These were compared with the VFSS results. Results The SSA showed a sensitivity of 89.1%,specificity of 75%,PPV of 0.804,NPV of 0.833,PLR of 3.56,NLR of 0.15,and Kappa of 0.648 compared to the VFSS results.The SIG showed a sensitivity of 60.9%,specificity of 82.5%,PPV of 0.800,NPV of 0.647,PLR of 3.44,NLR of 0.47 and Kappa of 0.425 compared to the VFSS.The sensitivity of of the SIG.Both assessments correlated well with the VFSS results. Conclusions Both the SSA and the SIG are elfective in dysphagia screening.The SSA is more sensitive than the SIG and is more suitable for early screening.The SIG is helpful in guiding dysphagia management and evaluating curative effects.

Objective To study the hypoxia-inducible factor-1α (HIF-1α) expression in the col-lagen induced arthritis(CIA) model and its role in rheumatoid arthritis (RA) pathogenesis. Methods We established CIA model, and taked CIA model of the ankle joint to have them HE staining and HIF-1α immu-nohistochemistry staining, then the HIF-1α expression in the tissues of RA was observed. Results The ex-pression of CIA synovial membrane of rat and the lower synodal both was expressed in HIF-1α. The positive expression of HIF-1α reached the highest volume in the first 21 days. Then, with the disease progression, the expression gradually decreased, which was in significantly positive relation with the synovial pathology score, synovial hyperplasia and angingenesis rating score, however, the expression was no positively correla-ted with the inflammatory infiltration. Conclusion The expression of HIF-1α in RA tissue is positively re-lated with inflammation severity, It indicates that HIF-1α is closely related to the occurrence and develop-ment of RA.

OBJECTIVE To study the efficacyof the lateral vocal fold injection of autogenous fat for vocal fold insufficiency. METHODS Twenty nine patients with vocal fold insufficiency were treated in our department with fat injection. Deep lateral vocal fold injection was performed through direct laryngoscope under general anesthesia. Fat was injected into the lateral aspect of the vocal fold (paraglottic space) using a special high pressure Brunning injector. The perceptual acoustic,phonatory function,and video laryngostroboscopy were evaluated before and after operation. RESULTS All the patients were followed up for 10 to 18 months. There were 24 patients with excellent results,2 patients with improvement, and 3 patients with no improvement. Phonatory function showed significant improvement(P

Objective To investigate the association between arsenic(+3 oxidation state) methyltransferase (AS3MT) genetic polymorphism and susceptibility to endemic arsenism.Methods Polymerase chain reactionrestriction fragment length polymorphism-single strand conformation polymorphism(PCR-RFLP-SSCP) technology was performed to detect mutations of AS3MT gene intron 8 and exon 9 in genome DNA of the 79 cases and 110 controls.PCR products with abnormal band forms were further sequenced to find the types and sites of mutation.Chi-square test and multivariate Logistic analyses were conducted.Results The incidence of the 9149 base mutation(A→C) in AS3MT gene intron 8(AS3MT-9149) in case group(19.0％,15/79) was lower than that in control group (23.6％,26/110).The incidence of the codon 287 mutation(ATG→AT/CG) in AS3MT gene exon 9(AS3MT-287)in case group(10.1％,8/79) was lower than that in control group (11.8％,13/110).However,statistical analysis indicated no significant difference in both mutations between two groups[AS3MT-9149:odds ratio(OR) =0.59,95％ confidence interval(CI):0.26-1.31,P =0.195; AS3MT-287:OR =0.85,95％ CI:0.32-230,P =0.751].Conclusions There are no significant association between the genetic polymorphisms of AS3MT-9149,AS3MT-287 and the susceptibility to endemic arsenism.Similarly,due to small sample amount,we can not exclude the possibility that these gene polymorphisms are related to susceptibility to endemic arsenism.

AIM:To establish a method of optimizing the formulation of resveratrol solid lipid nanoparticles(SLN)quickly and accurately by uniform design in combination with central composite design and response surface.METHODS:Firstly,uniform design was used to optimize the main factors from the five influence factors such as entrapment efficiency,drug loading,average diameter,and PDI(Polydispersity Index).Then,on the ground of it,the main factors determined by unifrom design were optimized by central composite design with three evaluation parameters and response surfaces were nonlineated according to best-fit mathematic models,so optimized formulation was obtained.RESULTS:According to the optimal conditions,the entrapment efficiency,the drug loading,the average diameter and the PDI(Polydispersity Index)of the prepared Res-SLN were 56.77%,2.59%,167 nm and 0.176,respectively.CONCLUSION:The design of combination of uniform design with central composite design is a feasible and convenient method to optimize the prescription of Res-SLN.

ObjectiveTo observe the effect of rehabilitation on fractures around knee treated with external fixation.Methods48 cases with the fractures around knee who accepted early rehabilitation after external fixation had been observed for 0.5～4 years.ResultsAll the cases came up to clinical healing standards within 7 months. According to Kolmert's standard, the rate of fine and good was 89.6%.ConclusionThere was satisfactory outcome treating fractures around knee with external fixation. Early rehabilitation is important to avoid the knee joint rigor.

Objective To study the effects of morroniside on the expression of Wnt signaling-related transcription factors neurogenin 2 (Ngn2), Pax6 and Tbr2 in the ischemic ipsilateral cortex 7 days after cerebral ischemia-reperfusion in rats. Methods 15 male Sprague-Dawley rats were randomly divided into sham group (n=3), ischemia group (n=3), and morroniside groups (low, medium and high dosage groups, n=3). The middle cerebral artery were occluded for 30 min, and re-perfused. Morroniside was administered intragastrically once a day at dose of 30 mg/kg, 90 mg/kg and 270 mg/kg 3 hours after operation. The expression of Ngn2, Pax6 and Tbr2 in the ischemic ipsilateral cortex were detected with Western blotting analysis 7 days after operation. Results The expression of Ngn2 increased in the ischemia group compared with the sham group (P<0.05), and it further increased the morroniside groups of medium and high dosage compared with the ischemia group (P<0.01). There was no significant difference between the ischemia group and sham group in the expression of Pax6, while it increased the morroniside groups of medium and high dosage compared with the ischemia group (P<0.01). There was no significant difference among all the groups in the expression of Tbr2. Conclusion Morroniside could increase the expression of Ngn2 and Pax6 in the ischemic ipsilateral cortex 7 days after ischemia-reperfusion in rats, suggesting promoting the neurogenesis after ischemia.

Conclusion Morroniside could decrease the expression of MMP-2 and MMP-9 in the peri-infarct cortex 3 days after ischemia, suggesting protecting the function of blood-brain barrier from ischemia.
