1.Clinical effect of circumcision stapler in the treatment of phimosis and redundant prepuce.

OBJECTIVETo observe the clinical effect and safety of circumcision stapler in the treatment of phimosis and redundant prepuce.
METHODSWe treated 120 patients with redundant prepuce or phimosis using circumcision stapler and another 60 by conventional dorsal-incision circumcision. We observed intraoperative blood loss, operation time, postoperative pain, wound healing time, cosmetic appearance of the penis, and postoperative complications and compared them between the two groups of patients.
RESULTSStapler circumcision showed obvious advantages over the conventional method in intraoperative blood loss ([2. 3 ± 1. 3] vs [15.6 ± 2.9] ml), operation time ([7.1 ± 1.4] vs [22.6 ± 4.6] min), wound healing time ([12.0 ± 2.9] as [16.3 ± 3. 1] d), postoperative pain score (1. 9 ± 1. 3 vs 5. 2 ± 1. 7), incision edema, and cosmetic appearance of the penis (all P <0. 05). Besides, stapler circumcision exempted the patients from stitch-removal pain. However, the incidence rate of postoperative local ecchymosis was significantly higher in the circumcision stapler group than in the conventional circumcision group (20. 8% vs 8. 3% , P <0. 05).
CONCLUSIONCircumcision stapler, with its advantages of easier manipulation, shorter operation time, better cosmetic penile appearance, less pain, and fewer complications, is superior to conventional circumcision in the treatment of phimosis and redundant prepuce.

2.Detection rate of myocardial bridge in coronary angiography and its clinical significance.

OBJECTIVETo explore the detection rate of myocardial bridge (MB) in coronary angiography and its clinical significance.
METHODThe clinical data of 2 165 cases who had received selective coronary angiography were studied retrospectively.
RESULTSMB was detected in 66 patients (3. 0% ) , among whom 62 were found in left anterior descending artery. Among the 58 cases with isolated myocardial bridge, 24 had no remarkable symptoms, whereas 34 showed clinical symptoms such as angina. The arterial stenosis beneath MB was significantly correlated with clinical symptoms (P < 0. 05 ).
CONCLUSIONThe improvement of the detection rate of MB can effectively decrease misdiagnosis and benefit the treatment.

3.Middle & distal circum peeling with bleeding control technique for redundant prepuce.

OBJECTIVETo investigate the middle and distal circum peeling with bleeding control technique for redundant prepuce.
METHODSThe middle and distal part of prepuce was circum peeled with bleeding control and electrical scalpel. More preputial inner plate was reserved.
RESULTSFrom January 2004 to July 2008, 51 patients with redundant prepuce were treated with this procedure. The average blood loss was 1 to approximately 2 ml with minimal edema. The proportion of inner and outer plates was suitable.
CONCLUSIONSThis technique has the advantages of minimal blood loss and slight edema. The cosmetic result is also satisfactory.

4.Therapeutic effect of dense-packing autologous hair grafting technique for restoration of seborrheic alopecia

Objective To investigate the therapeutic effect of dense-packing auto hair grafting technique on the restoration of seborrheic alopecia. Methods With local anesthesia, a scalp strip was harvested from the back of the head. Under operating microscope (Various graft was created from the scalp strip, including micro-grafts with 1-2 hairs, mini-grafts with 3-4 hairs and sliver graft with 5-6 hairs. In the alopecia recipient area, micro slots were made with a small triangle-edged needle for the micro-grafts,mini slits were made with mini blade for the mini-grafts and foramen ovale were made with a slot punch. The grafts were then implanted into these holes. Results 32 cases of seborrheice alopecia were treated with the above mentioned technique from March 2007 to July 2009. Postoperative following up for 12-24 month showed that the grafted hairs were growing well with average 90 % survival of the hair. 81 % of the patients obtained satisfactory results with only one stage operation. Six patients needed the second operation to improve the appearance. All of the patients were satisfied with the appearance. Conclusions The dense-packing hair grafting technique with various grafts not only saves time of operation, but also obtains dense grafted hair and well appearance. The results are satisfactory to most patients with only one stage operation.
5.Meta-analysis of androgen deprivation therapy for prostate cancer related with diabetes

Objective To examine whether androgen deprivation therapy (ADT) is associated with increased risk of diabetes in men with prostate cancer (Pca).Methods The study data were systematically searched from Medline,Embase,and Cochrane Library Central Register.Studies comparing ADT vs control aimed at treating Pca,reporting diabetes as outcome were included.Results Eight studies met inclusion criteria with a total of 65 695 ADT users and 91 893 non-ADT users investigating the relationship between ADT and diabetes.The incidence of diabetes was 39% higher in ADT groups,and significant association was observed in overall analysis [RR =1.39,95%CI(1.27-1.53),P < 0.01].In subgroup-analyses stratified by ADT types,diabetes was found to be significantly associated with gonadotropin-releasing hormone (GnRH) alone [RR =1.45,95%CI(1.36-1.54),P < 0.01),GnRH plus oral anti-androgen (AA) [RR =1.40,95%CI(1.01-1.93),P < 0.01] and Orchiectomy [RR =1.34,95%CI(1.20-1.50),P < 0.01],but not with AA alone [RR =1.33,95%CI(0.75-2.36),P =0.33].Conclusions ADT,especially GnRH alone,GnRH plus AA and orchiectomy can increase the incidence of diabetes in patients with Pca.

Searching for new antidiabetic lead compound, 4-(1-aryl-3-oxo-5-phenylpentylamino) benzenesulfonamides were designed and synthesized directly by three component one-pot condensation of 4-phenyl-2-butanone and sulfanilamide with some aromatic aldehydes at an yield of 23%-97%. The chemical structures of the twelve new Mannich bases were confirmed by 1H NMR, 13C NMR, FTIR, ESI-MS and HR-MS. The screening results of antidiabetic activity indicated that most of these title compounds possess alpha-glucosidase inhibitory activity, among which compound le is the strongest one. And compound 11 possesses good peroxisome proliferator-activated receptor response element (PPRE) agonist activity. The structure-activity relationship of these new beta-amino ketones containing benzenesulfonamide unit was also discussed preliminarily.

8.The inherent liability to infection of various tissue for urethra reconstruction.

OBJECTIVEObserving the long-term results of reconstructed urethra to reduce the postop infection rate of hypospadias. To explore the standard of selecting tissue for urethra reconstruction by comparing the different inherent liability to infection in use of different kinds of tissue for urethra reconstruction.
METHODSNeo-urethra inner wall and normal urethra were observed through urography and urethroscope. The tissue section of neo-urethra, normal urethra and tissue for urethra reconstruction were observed through light microscope, scanning electron microscope and transmission electron microscope.
RESULTSThe structure and liability to infection of neo-urethra made of bladder mucosa, buccal mucosa or skin never changed after long time. Metaplasia never took place in neo-urethra.
CONCLUSIONSThe structure and liability to infection of neo-urethra were the same as those of the congeneric tissue. Bladder or buccal mucosa is the best tissue for urethra reconstruction considering urethra microenvironment. Neo-urethra made of skin is more liable to infection by nature. The best opportunity of hypospadias operation should be before puberty.

9.Bacteriology study of hypospadias.

OBJECTIVEThis clinical study was to explore why the neo-urethra is liable to be infected after hypospadias operation, find the source and the common floras of infection, and accordingly, improve the peroperation procedures so as to reduce postoperative infection rate.
METHODSThe pathogenic floras were examined and analyzed by germiculture and karyotype analysis.
RESULTSThe bacteria in the neo-urethra mostly came from the orifice and the reconstruction material of the urethra. The most common floras that caused infection were gram-positive coccus. The most sensitive antibiotics for hypospadias infection were demethylvancomycin.
CONCLUSIONThe postoperative infection of hypospadias is incisional, not the urinary system infection. Because the microenvironment of the neo-urethra is more suitable for infection than that of the skin or mucosa, the reconstructed urethra is likely to be infected.

10.Hand-assisted laparoscopic surgery in radical resection of rectal cancer.

OBJECTIVETo investigate the application experience and clinical effects of hand-assisted laparoscopic radical resection for rectal cancer.
METHODSThe clinical data of 156 patients with rectal cancer treated with hand-assisted laparoscopic surgery between August 2009 and April 2011 were analyzed retrospectively.
RESULTSThe operative procedures of 156 patients were completed successfully and 1 case was converted to laparotomy (0.6%). The mean operation time was (125 ± 35) minutes; the mean intraoperative blood loss was (118 ± 60) ml; the mean time of bowel function recovery was (60 ± 8) hours; the median postoperative hospital stay was (9.5 ± 2.2) days. The mean number of lymph nodes dissection was (14 ± 5). Five patients (3.2%) had postoperative complications. All the patients were followed up. There had been no local recurrence or trocar site implantation metastasis.
CONCLUSIONHand-assisted laparoscopic surgery approach for the curative resection of rectal cancer is safe and effective and has broad prospects in clinical application.
