1.Change of pre-ablative thyroid-stimulating hormone after thyroid hormone withdrawal and its response to 131I therapy in patients with low to intermediate risk differentiated thyroid cancer

Objective To investigate the relationship between the change of pre-ablative TSH after thyroid hormone withdrawal(THW) and the response of subsequent 131I therapy in patients with low to intermediate risk DTC after total or near total thyroidectomy.Methods A total of 120 DTC patients (38 males,82 females,age (40.8±10.9) years) were enrolled in this retrospective study.Serial TSH levels determined on the day of THW and on the day of receiving 131I ablative therapy were monitored,which were marked as TSH1 and TSH2 accordingly.The THW duration (t) was recorded,the change of TSH was defined as △TSH and the change rate of TSH was calculated (V=△TSH/t).The responses to 131I therapy were classified as excellent response (ER),indeterminate response (IDR),biochemical incomplete response (BIR) and structural incomplete response (SIR) according to ATA guideline.According to the TSH2(mU/L) levels,patients were divided into G1 group (30≤TSH2<60),G2 group (60≤TSH2<90),G3 group (90≤TSH2< 120),G4 group (120≤TSH2<150) and G5 group (TSH2 ≥ 150).Clinical and pathological features,THW duration,the change rate of TSH,residual thyroid,131 I dose and follow-up time were compared among these groups.In order to evaluate the relationship between response to 131I ablation and change rate of TSH,patients were divided into V1 group (V≤2.5),V2 group (2.5<V≤5.0) and V3 group (V>5.0),and their responses to 131I ablation were compared.Patients were classified into RI group (including ER and IDR)and R2 group (including BIR and SIR),the differences of clinical and pathological features,131I doses between the two groups were explored.Furthermore,logistic regression was performed to identify factors associated with BIR and SIR.Results Patients with male gender (x2=11.863),younger age (F =4.975),and faster TSH change rate (H =44.911) and lower thyroid residue (H =18.159) achieved a higher value of TSH2(all P<0.05).G3 group presented the highest rate of ER (83.8%,31/37).The percentage of ER + IDR in V2 group was higher than those in V1 group and V3 group,which was 92.4% (61/66),85.7% (18/21) and 5/7,respectively,but the difference was not significant (U=407.5,P>0.05).TSH2 level (OR=0.835) and pre-ablative Tg level (OR =1.196) were independent factors in predicting BIR and SIR (both P<0.05).Conclusions The changing rate of TSH before 131 I ablation may not be associated with the response to 131I therapy in patient with low to intermediate risk DTC,while the level of TSH2 does.Patients with TSH2 ranging from 90 to 120 mU/L could be of help in achieving a better clinical response.

3.Expression of caspase-3 and HAX-1 after cerebral contusion in rat.

OBJECTIVE:
To observe the expression pattern of caspase-3 and HCLS1-associated protein X-1 (HAX-1) at different time after cerebral contusion in rat, and explore the new method for estimating the injury interval.
METHODS:
The cerebral contusion model was established using adult SD male rats. Then the rats were randomly allocated into 8 groups: 2 h, 6 h, 12 h, 1 d, 3 d, and 7 d after cerebral contusion, sham-operation and normal control. Expression of caspase-3 and HAX-1 protein after cerebral contusion in rat was detected by Western blotting. Laser scanning confocal microscope was used to observe the number of HAX-1 positive cells and TUNEL-stained cells after cerebral contusion.
RESULTS:
The expression of caspase-3 increased parallelly with the time after cerebral contusion and reached the peak value on 3 d. The expression of caspase-3 decreased gradually and still maintained a high level expression on 7 d (P < 0.05). The expression of HAX-1 positive cell went up after injury, and reached the peak value at 6 h (P < 0.05), then turned down gradually after 12 h and went out of detection after 3 d. The number of TUNEL-stained cells increased obviously at 2 h and reached the peak value on 3 d. The number of TUNEL-stained apoptotic cells decreased gradually and still maintained a high level expression on 7 d (P < 0.05).
CONCLUSION
The expression of caspase-3 and HAX-1 after cerebral contusion has time sequential regularity, which may provide new evidence for forensic diagnosis of cerebral contusion interval.

BACKGROUND: Association of lumbar facet tropism and orientation with lumbar disc herniation is an issue of concern.OBJECTIVE: To overview the association of lumbar facet tropism and orientation with lumbar disc herniation and to clarify the existing problems.METHODS: The articles related to the association of lumbar facet tropism and orientation with lumbar disc herniation published between January 1967 and January 2017 were retrieved from CNKI and PubMed databases by computer. The keywords were "lumbar facet joints, lumbar disc herniation, facet orientation and tropism" in Chinese and English, respectively. Totally 42 articles were enrolled based on the inclusion and exclusion criteria.RESULTS AND CONCLUSION: (1) The association of lumbar facet tropism and orientation with lumbar disc herniation remains unclear. The relationship between the orientation of lumbar disc herniation and lumbar facet tropism is also controversial. (2) The main problems to drive controversy are: inappropriate control group and lacking normal lumbar facet tropism and orientation data; the large age range of subjects;the uncertain standard of measuring lumbar facet tropism.
5.Experimental research on treating hepatic carcinoma by tram-arterial delivery of p53 gene mediated by lipsome combined transferring

Objective To investigate the trans-arterial delivery of p53 gene transfection efficiency and therapy effect on hepatic carcinoma in combination with transferrin mediated by liposome. Methods Twenty-five VX2 experimental rabbits were randomly divided into five groups, and the different doses of transferrin combined with p53-LipofectAMINE complex were delivered into the hepatic arteries of the VX2 hepatic carcinoma models. The tissue protein of the carcinoma was extracted after 48 h and the transfection efficiency and expression rate of p53 gene were analyzed by western blot and immune histochemical techniques, to inspect the expression proportion of p53 with different doses transferring. Another ten VX2 were divided into two groups, recombinant plasmid p53-LipofectAMINE complex and transferrin-p53-LipofectAMINE complex were delivered into the hepatic arteries in two groups respectively. The liver function, size of the tumor and survival time of the animals was compared between the two groups, and results were analyzed statistically. Results Semiquantitative analysis by Western Blot showed that the transfection and expression efficiency of p53 gene combined with transferrin were higher than those without it. By immune histochemieal techniques, the p53 gene's positive rate of highly expression with various doses of transferrin were found to be different, and there was remarkable difference between the groups with and without transferring. They were 58. 33%, 69. 44%, 80. 00%, 83.33%, 81.67% respectively, there was remarkable difference between the groups with and without transferring ( Totality: x2 = 42. 37, P < 0. 01 ). The p53 gene's positive rate of expression increased gradually as the doses of transferrin increasing from 0 up to 200 μg, but the differences of positive rate had no statistical significance as the doses of transferrin increasing from 200 up to 400 μg ( x2 section : 3 groups as former x2 = 4. 82, P < 0. 05,3 groups as latter x2 =0. 67 ,P <0. 05). There was no statistical difference in the liver function at points of time between VX2 rabbits with and without transferring. But the tumors' sizes had significant difference at various points of time. Conclusion Liposome-mediated p53 gene on treating hepatic carcinoma by trans-arterial gene delivery combined with transferrin is safe, and it can markedly enhance transfection efficiency and improve the therapy effect of p53 gene.
6.An analysis of etiological and genetic factors of a patient with familial hemophagocytic lymphohistiocytosis

Objective To analyze the etiological factor and genetic feature of a familial hemophagocytic lymphohistiocytosis patient with PRF1 mutation (FHL2) with human herpesvirus 7 (HHV7)infection and its family constellation. Methods Clinical characteristics, laboratory examinations of a FHL2 case with HHV7 infection were reported. HHV1-HHV8 virus DNA was screened by PCR; NK cell function was analyzed by flow cytometry; PRF1 gene mutations were analyzed by PCR and direct sequencing, structure of mutant PRF1 proteins were analyzed using ExPasy and I-TASSER server and genetics pedigree were analyzed. Results The patient's HHV7 viral was detected positive with DNA copy number of 350/106 peripheral nucleated cells. Flow cytometry analysis showed decrease both in proportion of perforin positive NK cells and perforin protein expression. Genetic testing showed PRF1 biallelic heterozygote mutations (c. 503G > A/p. S168N and c. 1177T > C/p. C393R) and pedigree analysis showed they were inherited. The patient was then treated with antivirus therapy, dexamethasone and VP16 therapy, but only achieved partial response. The patient was then followed by human leukocyte antigen 10/10 allele identical nonconsanguinity allogeneic hematopoietic stem cell transplantations (allo-HSCT) and soon the successful implantation of donor hematopoietic cells and persistent recovery was achieved. The patient was now surviving without recurrence for 9 months after allo-HSCT. Conclusions FHL is prone to be misdiagnosed as lymphoma. Genetic analysis of related gene mutation and herpes simplex virus detection will help in early and accurate diagnosis. Allo-HSCT is a fundamental treatment of FHL.
7.Study on inflammatory effect of toxic raphides from Pinellia ternate and its correlation with macrophages.

OBJECTIVETo study the toxic mechanism of toxic raphides from Pinellia ternata.
METHODMouse peritoneal macrophage in vitro culture model was adopted to study dose-dependent and time-dependent curves of toxic raphides, with TNF-alpha, IL-1beta and IL-6 in supernatant as indexes. Scanning electron microscopy was used to observe the changes in surface morphology of raphides-treated macrophages. Macrophages-neutrophils co-cultured the transport model to study the effect of toxic raphides' stimulation of macrophages on neutrophils migration.
RESULTToxic raphides' stimulation of macrophages could cause the increase in the levels of TNF-alpha, IL-1beta and IL-6 released, and showed dose dependence and time dependence. Scanning electron microscopy showed that toxic raphides were swallowed by macrophages, with notable cell membrane creases, increase in the number of pseudopods and decrease in integrity of cell membranes, and could significantly induce migration of neutrophils.
CONCLUSIONThe inflammatory process induced by toxic raphides is mainly mediated by macrophages. The toxic mechanism of toxic raphides from P. ternata is that toxic raphides penetrate into tissues to activate resident macrophages, release phagocytic and inflammatory cytokines, and cause migration of neutrophils, which finally results in acute inflammatory response.

8.Experimental comparison study of the tissue characteristics in transjugular intrahepalic portosystemic shunt and vascular stent

Objective To investigate the tissue characteristics within vascular stent and transjugular intrahepatic portosystemic shunt(TIPS)on swine and to provide more information for the understanding and prevention of vascular stent and TIPS restenosis.Methods Animal models for TIPS were built in 6 swine and vascular stents were implanted in iliac veins simultaneouly.14-28 days after the operation.the 6 swine were killed to remove the TIPS and vascular stent and the pathological examinations were performed on the tissues within the shunt and stent.The similarities and difierences of the tissues within the shunt and stent were analvzed with Kruskal Wallis test. Results Restenosis of TIPS occurred in 4 models and complete occlusion were seen in 2,while all vascular stents were patent and coated with a thin layer of intimal tissueElectron micmscopic results showed that the tissues in restenotic TIPS were loose and with more extra matrix and fibers.and less smooth muscle,fibroblastic and myofibroblastic cells with different and irregular shape and rich secretory granules.The tissues in patent,TIPS contained more extra fibers,smooth muscle and fibmblastic cells with normal organdie.The intimal tissues in vascular stent contained more fibers and fibroblasts cells.less smooth muscle cells.On immunohistoehemical staining,the tissues in restenotlc and Datent TIPS as well as the intimal tissues in vascular stent had strong positive expression for anti-SMCactin-α.the expression were gradually weakened for PCNA,the intimal tissues in vascular stent had a strong positive expression for vimentin,while the expression of the tissues in restenotic and patent TIPS were weakened gradually.For myoglobulin,the tissues in restenotic TIPS had weakly positive expression,the expression in patent TIPS and vascular stent were almost negative. Western blot results for TGF-β showed that the absorbance ratios of the intima tissues in vascular stent,normal vascular tissues,normal liver tissues.the tissues in restenotic and patent TIPS were 0.23,0,0,0.57 and 0.30 respectively,and they were significantiy different (H=27.8,P<0.01).Conclusions The tissues in restenotic TIPS mainly contain proliferated SMCs which have positive expression for anti-SMC-actin-α,strong proliferation and movement but unstability.The tissues in patent TIPS and intimal tissues in Vascular stent mainly contain fibroblastie cells which have positive expression for vimentin and stability.
9.Dynamic finite element analysis of three ferrule designs in post crown under impact loading.

OBJECTIVETo investigate the stress distribution in three ferrule designs in a maxillary central incisor restoration using PFM crown with post, and evaluate the biomechanical mechanism of the ferrule effect in the post crown by 3D finite element dynamic analysis.
METHODSThe 3D finite element model of a maxillary central incisor restored with post and PFM crown was established. By simulating three types of ferrule effect [crown wrapping dentine (A), core collar wrapping (B), and contrabevel (C)] under dynamic loading, the dentin stress was analyzed.
RESULTSDuring dynamic loading, the stress distribution tended to increase from the cervical part to root middle and radical part of the tooth, and greater high stress area was found around the apex of the post, where the peak stress was observed value. The stress of the labial dentin of the root inferior segment increased obviously. The high stress areas were invariable at every loading step during dynamic loading. The peak stress was sigma(vonA) CONCLUSIONSWhen the ferrule is 2 mm, the ferrule effect maintained by crown wrapping cervical dentine (A) and core collar wrapping (B) is greater than that maintained by contrabevel (C),but there is no significant difference between A and B. The post with higher elastic modules may cause restore failure due to the high stress at the post apex and labial dentin.

10.Longqingpian combined with levofloxacin highly effective for type IIIA prostatitis.

OBJECTIVETo study the therapeutic efficacy of Longqingpian combined with Levofloxacin on Type IIIA prostatitis.
METHODSEighty patients with Type IIIA prostatitis were equally randomized into a Longqingpian group and a Puleanpian group, both treated for 12 weeks. The former were given Longqingpian (once 6 tablets, twice a day) combined with Levofloxacin (once 0.2 g, twice a day) during the first 4 weeks, followed by Longqingpian alone (once 6 tablets, twice a day) for 8 weeks. The latter received Puleanpian (once 4 tablets, 3 times a day) combined with Levofloxacin (once 0.2 g, twice a day) in the initial 4 weeks, followed by Puleanpian alone (once 4 tablets, 3 times a day) for 8 weeks. Before and 4, 8, and 12 weeks after the treatment, all the patients were evaluated by The National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) and EPS examination.
RESULTSAfter 4-week, 8-week and 12-week treatment, total score of NIH-CPSI, and the score of pain, voiding symptoms and the quality of life dropped significantly in both groups compared with pre-treatment (P < 0.05 or 0.01), so did the leukocyte count in EPS (P < 0.05). Compared with the 4-week treatment, total score of NIH-CPSI, and the score of pain, voiding symptoms and the quality of life were shown to be significantly different (P < 0.05) in the Longqingpian group from the 8-week and 12-week treatment, except the leukocyte count in EPS. However, there was no significant difference between the 8-week and 12-week treatment in any of the indexes (P > 0.05). Longqingpian effected significant differences in the above indexes (P < 0.05), while Puleanpian did not after the treatment (P > 0.05). And the lecithin Amato body analysis revealed no significant changes in EPS after the treatment in either group (P > 0.05).
CONCLUSIONLongqingpian combined with Levofloxacin is highly effective for Type IIIA prostatitis, by relieving pain and voiding symptoms, decreasing the leukocyte count in EPS and improving the life quality of the patients.
