1.The evaluation of educational measurement indexes from the Undergraduates Clinical Skills Competition in a medical university

Objective The purpose of this study was to analyze and evaluate the educational measurement indexes from the undergraduates clinical skills competition in a medical university.The rationality of the competition was discussed in the view of education survey.Methods 84 students' results which selected from the China Medical University 2nd Clinical Skills Competition were the research objects in this study.The indexes of the difficulty,discrimination,reliability and criterion-reated validity of the competition were evaluated based on the Education surveying principle.The difficulty was calculated by subjective topic difficulty computing formula,and other indexes were carried out with SPSS 18.0 except the difficulty index.Results The average difficulty of the questions of this university was 0.61,the average discrimination was 0.77 and the overall reliability was 0.926.The OSCE scores of the participating students were taken as the criterion in this study.The calculated criterion-related validity was 0.416.Conclusion The college students' clinical skills competition has good reliability,discrimination and effective criterion-related validity.For the selective competition,the difficulty can be further improved while considering the scope of the topic.
2.The associaion of serum uric acid level with prevalence of non-alcoholic fatty liver in Uyghur and Han ethnicities

Objective To explore the association of serum uric acid level with non-alcoholic fatty liver disease(NAFLD) in Han and Uyghur ethnic groups.Methods A cross-sectional study was performed in the population in 2011 in the First Affiliated Hospital of Xinjiang Medical University.The study included 2439 Uyghurs and 2285 Hans with a questionnaire survey,and body mass index (BMI),waist circumference,blood pressure,blood glucose,blood lipid,and serum uric acid (SUA) level were measured.The participants were divided into 4 groups according to the quartiles of the SUA levels within the normal range,and those with SUA levels above the normal range served as hyperuricemia group.The associations between serum uric acid level and prevalence of non-alcoholic fatty liver,hyperuricemia,and metabolic syndrome (MS,including each component of MS) were analyzed.Results Among Uyghurs and Hans,the detection rates of NAFLD were 26.7% and 23.6% respectively,the detection rates of hyperuricemia were 7.8% and 18.2%,and the level of serum uric acid in Uyghur group was lower than that in Han group (P<0.01).The detection rate of NAFLD in the hyperuricemia patients was significantly higher than that in nonhyperuricemia group.Among the Uyghurs and Hans,the detection rates of NAFLD in the hyperuricenia patients were 24.0% and 19.9%.The detection rate of NAFLD was positively associated with concentration of serum acid even that within the normal range.Conclusion The relationship between the level of serum uric acid and the prevalence of NAFLD is more evident in Uyghur than that in Han.
3.Association of physiological level of serum uric acid with reduced renal function in young and middle-aged healthy people

Objective To investigate the association of serum uric acid level with glomerular filtration rate and explore the predictive value of the physiological uric acid levels for reduced renal function.Methods A total of 6 532 subjects from Karemary community and 2 402 subjects from the First Affiliated Hospital of Xinjiang Medical University were enrolled.All subjects were surveyed with questionnaire and their serum biochemical indices and liver functions were measured.Meanwhile,a cohort study was conducted,752 subjects with normal renal function were selected from young and middle-aged healthy people in 2008.During the 3-year follow-up,the incidence of reduced renal function and the risk factors of estimated glomerular filtration rate (eGFR) were analyzed.Results Serum uric acid (SUA) was the independent risk factor of reduced GFR in cross-sectional analysis.The 2 groups were stratified by sex and age,except for female population over 45 years old in the first group,the level of physiological serum uric acid was the risk factor for decreased eGFR in the rest of groups.During 3-year follow-up,the incidence of reduced renal function was 15.40% and SUA was the independent risk factor of decreased eGFR.Conclusions The concentration of physiological serum uric acid was closely related with renal function,and the increase in SUA whthin normal range had an important early predictive value for decreased GFR.
4.Diagnostic and clinical value of low-dose volumetric high-resolution CT in children with bronchiectasis

Objective To investigate the value of low-dose volumetric high-resolution CT (vHRCT)in children with bronchiectasis.Methods Forty-nine children with bronchiectasis were diagnosed by vHRCT.The CT manifestations and follow-up changes were observed. Results Among the 49 cases,there were 43 cases(85.7%)with cylindrical bronchiectasis,2 cases with cystic bronchiectasis(5.7%) and 4 cases with varicose bronchiectasis(8.6%).Forty-four cases with bronchiectasis were complicated with pneumonic illness,among which 9 cases with atelectasis,5 cases with pulmonary fibrotic changes and 3 cases with airway foreign body.And 5 cases were complicated with endobronchial inflammation only.The CT signs of cylindrical bronchiectasis were as follows:bronchial wall thickening in 38(88.4%), double-track sign in 29(67.4%),signet ring sign in 35(81.4%),mucus plugging in 6(14.0%),air bronchogram in 9(20.9%),and bronchi appeared within 1 cm distance from the pleura in 18 cases(41.9%).Following-up 10 cases,bronchial dilatation improved significantly in 4 cases,progressed in 2 cases,and there were 4 cases with no significant change.Conclusion vHRCT is an effective method for early diagnosis of bronchiectasis in children,which is valuable for improvement of bronchiectasis by prompt and effective treatment.
5.Formula Optimization and Quality Evaluation of Testosterone Undecanoate Binary Ethosomes

To screen the optimal formula and evaluate the quality of testosterone undecanoate ( TU) binary ethosomes to lay the foundation for the transdermal delivery system of TU. Methods:The mixture of ethanol and propylene glycol was used as the softeners, and TU binary ethosomes were prepared by ethanol injection method. The ratio of TU to lipids ( A) , the quality percentage of the mixture of ethanol and propylene glycol ( B) and the ratio of ethanol to propylene glycol ( C) as the influencing factors, and the entrapment efficiency as the index, an orthogonal test was used to optimize the formula of TU binary ethosomes. The morphology, size, zeta potential, in vitro drug release and stability of TU binary ethosomes were studied. Results:The optimal formula of TU binary etho-somes were as follows:the ratio of TU to lipids was 1∶15, the quality percentage of the mixture of ethanol and propylene glycol was 10% and the ratio of propylene glycol to ethanol was 6∶4. The optimal TU binary ethosomes were concentric circles under an optical microscope with uniform size, and the average size was (185. 5 ± 52. 8)nm, zeta potential was ( -15. 87 ± 0. 26)mV, and the entrap-ment efficiency was (79. 14 ± 0. 66)%. TU release from the binary ethosomes in vitro was fitted the first-order equation:Q=20. 79t-11. 01 (r2 =0. 998 4). Under the high temperature, the entrapment efficiency was decreased significantly, while under the other test conditions, all the indices of TU binary ethosomes showed no significant difference. Conclusion:The optimal TU binary ethosomes are easy to be prepared with promising quality and sustained release property in vitro, which are valuable to be studied further.
6.Chemical constituents from stems and leaves of Micromelum integerrimum.

A new benzene derivative microintegerrin C (1) and a new norsesquiterpenoid microintegerrin D (2), along with six known compounds (3-8), were isolated and identified from stems and leaves of Micromelum integerrimum by various chromatographies such as silica gel, Sephadex LH-20, RP-18 column chromatography and HPLC. Their structures were mainly identified based on the spectral data analysis such as 1D-, 2D-NMR and HR-EI-MS. All known compounds were isolated from this plant for the first time.

7.ITS sequences variation and phylogenetic analysis on 31 geographical populations of Notopterygium incisum.

In this study, 31 Notopterygium incisum populations were analyzed using ITS sequences to investigate the genetic structure. The results showed that: the ITS region ranged in size from 634 to 635 bp and base composition was with high G + C content of 57.8%. Thirty-one polymorphic sites were detected from 402 sequences of 31 populations of N. incisum, and the proportion of polymorphic sites was 4.88%, in which parsimony informative sites were up to 12. And 31 haplotypes were identified based on these polymorphic sites. Molecular variance analysis (AMOVA) indicated that high genetic differentiation (57%) existed among population, and gene flow was low (N(m) = 0.38) among populations. Phylogenetic relationships of 31 haplotypes were analyzed using NJ method with N. forbesiias an out-group. Phylogenetic analysis showed that 31 haplotypes from different populations mixed together and did not form distinct geographically separated clades.

8.New progress on three-dimensional movement measurement analysis of human spine.

Spinal biomechanics, especially the range of spine motion,has close connection with spinal surgery. The change of the range of motion (ROM) is an important indicator of diseases and injuries of spine, and the essential evaluating standards of effect of surgeries and therapies to spine. The analysis of ROM can be dated to the time of the invention of X-ray and even that before it. With the development of science and technology as well as the optimization of various types of calculation methods, diverse measuring methods have emerged, from imaging methods to non-imaging methods, from two-dimensional to three-dimensional, from measuring directly on the X-ray films to calculating automatically by computer. Analysis of ROM has made great progress, but there are some older methods cannot meet the needs of the times and disappear, some classical methods such as X-ray still have vitality. Combining different methods, three dimensions and more vivo spine research are the trend of analysis of ROM. And more and more researchers began to focus on vivo spine research. In this paper, the advantages and disadvantages of the methods utilized recently are presented through viewing recent literatures, providing reference and help for the movement analysis of spine.

One new dicyclopeptide cyclo-(L-N-methyl Glu-L-N-methyl Glu) (1), together with one new natural dicyclopeptide cyclo-(L-methyl Glu ester-L-methyl Glu ester) (2), and two known dicyclopeptides cyclo-(L-methyl Glu ester-L-Glu) (3), and cyclo-(L-Glu-L-Glu) (4), were isolated from the aerial parts of Dianthus chinensis L. Their structures were determined by spectroscopic analyses and chemical methods.
