1.Preparation and Quality Evaluation of O/W-Type Nifedipine Transdermal Nanoemulsion

Objective:To optimize the formula of nifedipine nanoemulsion (NFNE) to lay foundation for the development of nifedipine transdermal preparation.Methods:The content of nifedipine as the index,the compositions in the formula were screened out by single factor experiments,and then the formula of NFNE was optimized by orthogonal tests with the ratio of emulsifier to co-emulsifier,ratio of mixed emulsifiers to oil phase and the amount of water as the influencing factors.N-Trimethyl chitosan (TMC) was used as the transdermal enhancer.The formula of TMC-NFNE was optimized by orthogonal tests with the degree of quaternization of TMC,the concentration of TMC solution and the amount of TMC solution as the influencing factors.The quality evaluation of TMC-NFME was determined including appearance,type identification,particle size and drug content.Results:The optimal formula was as follows:7% ethyl oleate,32% Tween 80,16% alcohol,45%water and the mass ratio of TMC of 0.45%.The type of the emulsion was O/W.The content of NFNE,TMC20-NFNE,TMC40-NFNE and TMC60-NFNE was (12.76±0.22),(14.76±0.16),(12.26±0.18) and (12.92±0.27) mg·ml-1,respectively.The particle size was 13.62,17.10,16.26 and 17.25 nm,respectively.The morphology of the nanoemulsion was spherical.Conclusion:The optimal formula and preparation process of TMC-NFNE are simple with high drug content,which is worthy of further studies.

4.Ultrasound-guided percutaneous microwave ablation of lung cancer

Objective To evaluate the efficacy and feasibility of ultrasound-guided percutaneous microwave ablation for peripheral lung cancer. Methods Seventeen patients with peripheral lung cancer (23 nodules) were treated with ultrasound-guided microwave ablation. The microwave energy was initially set at 60 W for 100s, and then at 30~40 W for 300~600 s. The treatment was conducted by using multiple simultaneously-powered antennas in lesions ≥3 cm in diameter. Results After the treatment, all the nodules were decreased in size and the blood flow signals inside the nodule disappeared (14 nodules) or weakened (9 nodules). Enhanced CT scanning revealed no enhancement in 16 nodules and partial enhancement in 7. Repeated biopsy in 4 patients showed a complete tumor necrosis. Clinical symptoms disappeared in 8 patients and subsided in 9 patients 1 month after treatment. Follow-up observations in 15 patients for 6~47 months (mean, 23 months) showed 9 survivors, without serious complications. Conclusions Ultrasound-guided percutaneous microwave ablation is an effective, safe, and feasible method for treating peripheral lung cancer.
5.Adverse thrombogenic events induced by discontinuing aspirin in patients during prostate operation: case report and review

Objective To determine the incidence of thrombogenic events during the prostate operation period in patients discontinuing aspirin.Methods Among a retrospective cohort of 342 patients admitted in our institution for benign prostatic hyperplasia (or prostate cancer),combined with acute coronary syndrome (or stroke),we studied 4 patients who had not been taking aspirin before thrombogenic vascular event.Data on age,sex,vascular disease risk factors,and clinical outcome were collected.Results The 4 patients' mean age was 78.8±5.9 years.Each patient had at least two following risk factors:atrial fibrillation,old cerebral infarction and type 2 diabetes.80% patients had a clinical history of hypertension.2 of the 4 patients stopped aspirin before a surgical procedure and developed acute ischemic stroke and acute myocardial infarction,separately.The other two patients developed acute ischemic stroke without aspirin prescription.The median time between admission and thrombogenic events was 15.5± 10.5 days.All patients were not given finasteride on admission.Conclusions This study should alert clinicians to know the risk of aspirin withdrawl perioperatively in patients at high risk of cardiovascular and cerebrovascular diseases.
6.The Percutaneous Permeability of Methoxypsoralen Liposomal Gel

OBJECTIVE:To study the percutaneous permeability of methoxypsoralen liposomal gel METHODS:The preparations were applied on the rat skin in vitro and in vivo with common gel and Mingbailing tincture as control,and the drug concentrations in the receiver,skin,blood and other organs were determined by RP-HPLC RESULTS:In vitro experiment,liposomal gel showed the lowest percutaneous permeation rate and the highest retention in skin In vivo studies illuminated that the skin depositions of drug encapsulated in liposome was(4 37?0 91) and (3 36?0 58) times those of the common gel and tincture respectively CONCLUSION:Methoxypsoralen liposomal gel showed an evident targeting effect on skin

8.Approach to CDISC SDTM Implementation for Clinical Trials Data Submission

This study was aimed to discuss the ways of converting clinical data entered in the Case Report Form (CRF) to Study Data Tabulation Model (SDTM) database format in clinical research of traditional Chinese medicine (TCM). Currently, there were three approaches for implementing SDTM, which were the pure SDTM approach, submission-only approach and database-only approach. This article compared and analyzed advantages and disadvantages of three approaches and introduced experiences of the Clinical Evaluation Center of China Academy of Chinese Medical Sciences using database-only approach for SDTM implementation. The results showed that pure SDTM approach can maximally embrace SDTM standards. However, the current process and software system should be modified. Therefore, it was time-consuming and expensive. The submission-only approach was the most economic way in the application of SDTM standards. However, the data quality and traceability may not be guaranteed. The database-only approach built the study database based on the SDTM standard by writing transformed program before data entry while the data collection system was not SDTM-compatible. It was concluded that database-only approach for implementation SDTM was a suitable and practical way to TCM clinical research.
9.The associaion of serum uric acid level with prevalence of non-alcoholic fatty liver in Uyghur and Han ethnicities

Objective To explore the association of serum uric acid level with non-alcoholic fatty liver disease(NAFLD) in Han and Uyghur ethnic groups.Methods A cross-sectional study was performed in the population in 2011 in the First Affiliated Hospital of Xinjiang Medical University.The study included 2439 Uyghurs and 2285 Hans with a questionnaire survey,and body mass index (BMI),waist circumference,blood pressure,blood glucose,blood lipid,and serum uric acid (SUA) level were measured.The participants were divided into 4 groups according to the quartiles of the SUA levels within the normal range,and those with SUA levels above the normal range served as hyperuricemia group.The associations between serum uric acid level and prevalence of non-alcoholic fatty liver,hyperuricemia,and metabolic syndrome (MS,including each component of MS) were analyzed.Results Among Uyghurs and Hans,the detection rates of NAFLD were 26.7% and 23.6% respectively,the detection rates of hyperuricemia were 7.8% and 18.2%,and the level of serum uric acid in Uyghur group was lower than that in Han group (P<0.01).The detection rate of NAFLD in the hyperuricemia patients was significantly higher than that in nonhyperuricemia group.Among the Uyghurs and Hans,the detection rates of NAFLD in the hyperuricenia patients were 24.0% and 19.9%.The detection rate of NAFLD was positively associated with concentration of serum acid even that within the normal range.Conclusion The relationship between the level of serum uric acid and the prevalence of NAFLD is more evident in Uyghur than that in Han.
10.Correlation between the characteristic of intraoperative contrast enhanced ultrasound and expression of isocitrate dehydrogenase 1 in glioblastoma

Objective:To explore the correlation between the characteristics of contrast-enhanced sonography of intraoperative glioblastoma multiform (GBM) and molecular markers of isocitrate dehydrogenase-1(IDH1).Methods:A retrospective analysis were performed in 30 patients who underwent neurosurgery and pathologically confirmed to be GBM at Beijing Tiantan Hospital from May 2018 to April 2019. All neurosurgical glioblastoma patients after craniotomy underwent conventional ultrasound and contrast-enhanced ultrasound(CEUS) guided navigation. The characteristics of the ultrasound imaging (whether the tumor involves the structure of the corpus callosum, the clarity of the tumor boundary after enhanced ultrasound and whether the tumor has necrotic areas with enhanced ultrasound images) were analyzed. The ratio between tumor necrosis area and whole tumor area (N/W) was measured, and the correlation with IDH1 gene expression was analyzed.Results:There were statistical differences in clarity of tumor boundary after CEUS and tumor necrosis after CEUS between positive IDH1 and negative IDH1 groups(all P<0.05). The positive expression of IDH1 was negatively correlated with the N/W area of the contrast-enhanced ultrasound mode( r=-0.756, P<0.05), suggesting that the expression level of IDH1 gene was negatively correlated with the area of tumor necrosis. Conclusions:Ultrasound contrast agent examination can more accurately distinguish the active proliferation area, hemorrhagic necrosis area and peripheral edema area of glioblastoma. Accurately identifying the extent of tumor necrosis area through ultrasound contrast agent examination can predict expression of IDH1.
