2.Clinical study of holmium laser resection as well as instant intravesical Pirarubicin instillation for superficial bladder carcinoma

Objective To study the efficacy of transurethral holmium laser resection as well as instant intravesical Pirarubicin instillation for superficial bladder carcinoma.Methods 51 cases with superficial bladder cancer were divided into observation group(A) including 26 cases and control group(B) including 25 cases stochastically.The 26 cases of group A were received transurethral holmium laser resection as well as instant intravesical Pirarubicin instillation therapy whereas 25 cases of group B received transurethral resection therapy.All patients of both groups were treated with intravesical instillation of Pirarubicin therapy regularly at least 1 year and have been followed up for 15 to 39 months.Results Differences were not significant between the two groups in the mean operation time and bladder perforation rate(P >0.05 ),but indwelling period of urethral catheter was markedly decreased in group A than group B ( P < 0.05 ).There were 4 cases of recurrence in group A,recurrence rate was 15.4% (4/26).While there were 11 cases of recurrence in group B,recurrence rate was 44.0% (11/25 ).Significant difference in cancer recurrence rate was found between two groups ( P < 0.05 ).Conclusion Compared with transurethral resection therapy transurethral holmium laser resection as well as instant intravesical Pirarubicin instillation for superficial bladder carcinoma was more effective and quicker recovery,which has a good applied future in the clinical practice.

4.Synopsis of research on the role and scope of nongovernmental medkal services in China

Objective To find out about the development of nongovernmental medical services in China as well as the appraisals of such services by various quarters so as to provide basis for the formulation of policies for their management and development. Methods Cross-sectional studies were made in the provinces of Guangdong, Shanxi and Sichuan. By means of questionnaires and interviews, appraisals of nongovernmental medical services by residents, physicians, managers of medical institutions, investors of nongovernmental medical institutions and managers from the administrative departments of health were collected and differences in the appraisals made by people representing various interest groups were analyzed. Results Over 30% of the residents make use of nongovernmental medical services and appraisals of them surpass those of public medical services. Nongovernmental medical institutions have both advantages and disadvantages, with unfair social treatment being the major obstacle to their development. Conclusion Since nongovernmental medical services have already taken an important position in the setup of medical services of China, efforts should be made to coordinate relevant policies so as to bring their role into greater play.
6.Comparison between albendazole and triclabendazole against Fasciola gigan-tica in human

Objective To compare the anthelmintic effect of albendazole with that of triclabendazole against Fasciola giganti-ca. Methods Two patients infected with Fasciola gigantica were investigated,and one was administered with albendazole orally (200 mg,twice per day for 5 days)and another was administered with triclabendazole[10 mg/(kg?d)for 2 days]. Their total fe-ces were collected daily during the period of whole therapy,and the eggs of the parasite were collected by using the nylon bag method,and incubated at 28℃. Results The parasite eggs were detected from the first patient’s dejecta on the 1st,2nd and 5th day after the end of the treatment,and no miracidiums hatched out as well as no eggs developed during the period of 25 days of the incubation. Meanwhile,her body temperature fluctuated between 37.4℃and 38.3℃,and she still complained bellyache. For the other invalid,the eggs were not detected on the 2nd and 5th day after the end of the treatment. However,the eggs before and dur-ing the treatment developed the miracidiums of Fasciola gigantica on the 13th day after the incubation,but the eggs collected from the 1st day after the termination of the therapy did not develop and no miracidiums hatched out. At the same time,the signs and symptoms of the patient vanished after the 4th day of the end of the therapy. Conclusions Albendazole has no obvious insecticid-al activity on adult Fasciola gigantica in the short term,but may affect the development of eggs. Triclabendazole has the anthelmin-tic effect on the adults as well as eggs of Fasciola gigantica. In addition,triclabendazole has the characteristics of well curative ef-fect,short course of treatment,and no obvious side effects.
7.The effect of tissue factor gene silencing on human umbilical vein endothelial cell with placental abruption

Objective To silence the expression of tissue factor(TF) gene of human umbilical vein endothelial cell(HUVECs) of the newborns with placental abruption(PA) and normal newborns.Methods There were two groups in the experiment,normal group and PA group.Three different treatments were established in each group:(1) the blank; (2) the false-intervention ; (3) the TF gene silencing.There were three samples in each treatments.After these treatments,the changes of mRNA expression of the HUVECs were observed before and aftcr thc gene silencing and the changes of the immunofluorescence of the TF protein level.Results After amplificated,plasmid DNA were sequenced to show that the pENTRTM/U6-TF-shRNA was the positive clone.After the transfected,the levels of the mRNA of TF decreased from 0.657 ± 0.097 to 0.220 ± 0.030 and 1.323 ± 0.323 to 0.207 ± 0.150 in the normal and PA group respectively.Compared with the normal group,there were significant differences for the levels of TF mRNA in PA group with the blank,(1.323 ± 0.323 vs 0.657 ± 0.097,P =0.023) and the same result for the second management (1.057 ±0.178 vs 0.540 ± 0.079,P =0.01).But there was no significant difference between the normal and PA group after RNA interference gene silencing (0.220 ± 0.030 vs 0.207 ± 0.150,P > 0.05).Meanwhile,there were significant differences among the three managements in the themselves groups of normal and pathological ones(F =19.30,P =0.002 ;F =27.66,P =0.001).Conclusion The vectors are transfected into HUVECs and play the biological function.And they silence the expression of TF mRNA.PENTRTM/U6-TF-shRNA could inhibit the expression of TF mRNA of HUVECs in the PA newborn.
8.The Normal Anatomy and CT Study of HDL

Objective To study the normal anatomy and pathologic CT findings of HDL and compare the HDL dimension of two conditions.Methods 160 consecutive CT scans of the normal upper abdomen were reviewed and 19 cases of cadavers were used.Results There was no statistics differences between the cadavers and normal subjects on HDL.Conclusion The real dimension of HDL can be replaced by the measurement on CT image,CT is a accurate means of measuring HDL in vival of human being.
9.Diagnosis and Operative Treatment of Concealed Penis in Children

Objective To improve the knowledge,diagnostic level on concealed penis in children,and explore the operative treatment methods of concealed penis.Methods Ten children with concealed penis were reviewed retrospectively in the First Hospital of Jilin University from Jun.2005 to Jul.2007.Patients were 6-12 years old.Penis length was 1.2-2.6 cm.Patients growthing and micturition were normal,the results of androgne and estrogen examination were normal.The etiology was simple obesity in one case,fiber streak of penis sarcolemma in 9 cases.The clinical symptoms and signs,diagnosis and operative method were analyzed.Results The diagnosis of 10 children were correct,and all patients were treated by Devine's operation and 10 cases patients had healed in one stage.There was no vessel,nerve and urethra injury during the operation.The length of penis was 3.0-5.5 cm,average in 3.6 cm after operation.Followed up 3 months to 2 years,10 children's phimosis were removed,the micturition were normal;the appearance of penis was also satisfactory after operation.One patient with under-developed penis had a amelioration after treatment with HCG postoperatively.The penis contour was dissatisfactory in 1 obesity patient,but it was ameliorated gradually while growing up.Conclusions The correct diagnosis and differential diagnosis are very important for operative treatment depend on the different etiology and pathologic changes of concealed penis.Suitable operation can extend the length;moreover,ameliorate the symptom of micturition and the appearance of the penis.
